I definitely wouldn't use "contractor" for "contratante" here, in the context of medical insurance, especially since as translators, we're normally "independent contractors" ourselves, i.e., those providing the service, not receiving it.

In the case of medical insurance, the "contratante" is normally the person buying/receiving the service of the insurer.

El contratante o tomador del seguro, que puede coincidir o no con el asegurado, por su parte, se obliga a efectuar el pago de esa prima, a cambio de la cobertura otorgada por el asegurador

So "contratar un seguro" is the usual phrasing in Spanish, but "buying/purchasing insurance" is typically how we say that in English.

However, in US English, rather than use "buyer", the conventional term is "subscriber".

Subscriber: The individual who signs and is responsible for a contract with a health insurance plan. The subscriber is the person subcribing [sic] to the insurance plan for the patient case. The subscriber is different from the enrollee, who is defined as anyone covered under the contract.
