MH would be used at the end of a complete course of study (3 or 4 years), while SB could be used at the end of each academic year.

I would go for either of these two options.

I believe you cannot translate "matrícula de honor" as "first class honours" or "with honours", because an MH is given at the end of each term or year, where as the honours are given when you graduate (after 3 or 4 years of study, as Tully mentioned in his answer).

A matrícula de honor is a top mark in a subject at university with the right to free registration the following year, and is awarded to only 1 or 2 students per year (it must be less than 5% of the students registered in that year). Because there is no such thing at British universities, it is hard to find an equivalent term.

Another option could be something like "Distinction with Fee Waiver" - although it sounds clunky, it is less clunky than leaving untranslated with an explanation in parenthesis.

PS. I know all this because I've been to university both in Britain and in Spain!
