I would say "suspect drug" but "suspected drug reaction/interaction/effect".
But no one is going to be castigated for saying "suspected" or "suspect"(but never "suspicious"). English seems to be less restrictive than Spanish in this sense.

Interesting point about "personal preference"...I am not dictating to anybody, but have found myself that if I do not use the "exact" medical term, used by those "in the know", then I am criticised or penalised for it!! Cheers! The problem, in general, with medical terminology, is that if we don't get the "right" word we could be making a serious mistake. OK "suspect/suspected" is not life-threatening,....

Would it be better to post this as a reference? On the issue itself, one can find documents to support each of the options. The final preference probably comes down to personal experience.

I think "suspect drug" would probably be OK, in fact, but I think I'd prefer "medication" (in the sense of a medicinal substance).

This is a very crude measure that proves nothing, but for what it's worth, if you Google "adverse drug reaction" plus various possible translations of fármaco sospechoso you get the following results:
"adverse drug reaction" + "suspected drug": 7970
"adverse drug reaction" + "suspect drug": 4090
"adverse drug reaction" + "suspected medication": 430
"adverse drug reaction" + "suspect medication": 360
So on this measure, my choice comes in last! Well, there you go...
