"primario" is giving me problems here.....especially as I am unable to find this phrase on any Sp medical site.

Due to the insistence of the other answerers in the meaning of primary, I've continued searching, but really, without further context it's difficult to reach a conclusion (you'll have found that there are no hits for the article in Spanish).
However, it does seem that another possibility might be:
Primary risk factors for vascular disease (but you have to add disease/disorder/event, or something, in English).
