This little one shows up at the top of a hospitalized patient's discharge report, after the medical record number, date of birth, admission/discharge dates, etc. as follows:
D./a. Quesada Ruiz, Juan [not the names in the actual report]
C/. Jardin de la Duquesa, 3 4D [Jardin de la Duquesa is real]
Pobl. Sevilla
Cód. P. 12345
What does D./a. stand for? (a side note about C/. would help, too, if it doesn't mean "address"). Pobl = Población, Cod.P = código postal.

It indeed means Don/Doña but Ms is more appropriate as Mrs only includes married women while Ms refers to any woman irrespective of marital status.
C/ means calle = street but address is better in translation.
