A journal. It includes personal and professional targets, which help to motivate me. It is especially helpful when I have a long job and I want to be sure to work on it every day - I even set daily quotas. The focus shifts, depending on my own focus at the time, and sometimes I let it lapse when I'm really into the moment of what I'm doing or thinking.
