Yeah. I definitively think it is the laboratory because a few lines below, it says:

Sobre el considerando referido al MEDI-570, producto en investigación del presente estudio, el administrado señala, a través de su recurso de reconsideración, que....

and the laboratory (name's laboratory XXXX) is the one filing the appeal for reconsideration, as mentioned earlier in the doc.

Possibly but my answer is so general because it's hard to say who this entity is exactly. I imagine there's no further context that can help clear this up, but I would have thought it would be the "investigator" of the "producto en investigación"

That was my first thought but on reflection, it seems to be the entity under the control or supervision of the DIGEMID, which is obliged to provide the information, especially considering that it say's "proporcionada POR el administrado", and it would seem odd that a patient would provide such information.
