HARDWARE INSTALLATION GUIDE

2.1 Setting Up

1. Place the case upright on a flat, stable surface with the rear of the case facing you.
2. Remove the thumbscrews from the right side panel. Grip the panel at the top and bottom and slide it toward you to open the case.
3. Remove the screws from the left side panel. Grip the panel at the top and bottom and slide it toward you to remove the left side panel.

Note: Do not use your fingernails to pry or lift the panels.

Inside the case you should see two separate chambers: the upper chamber for the motherboard, externally accessible drives and hard drives; and the lower chamber for the power supply and hard drives. You will also find some wiring with marked connectors (USB, PWR, etc.), the installed I/O panel, a box containing one set each of drive rails for a 5.25” drive and a floppy disk drive, and a tool box attached to the upper HDD cage containing more hardware (screws, brass standoffs, spare silicone grommets, etc.).

2.2 Power Supply Installation

Most power cables can be routed behind the motherboard through the cable management compartment. If some cables can’t reach, run them through the passage that goes straight into the motherboard compartment.

1. Remove both side panels.
2. If you are installing an Antec CP series power supply, you will need to remove the PSU adapter plate.
3. Slide the power supply into the case. If not routing cables through the cable management compartment behind the motherboard, route cables through the covered opening. This will save a few inches of cable travel and help improve airflow in the power supply compartment.

Note: Power supplies with fans on the bottom will need to be mounted so that the fan is facing the top of the case. There are mounting holes for power supplies to be installed upside up or upside down.

4. Secure the power supply by fastening it to the back of the case with the provided screws.

PHASE I/II STUDY OF MLN8237 IN COMBINATION WITH IRINOTECAN AND TEMOZOLOMIDE FOR PATIENTS WITH RELAPSED OR REFRACTORY NEUROBLASTOMA

What is this study about

This study is a clinical trial, a type of research study. Clinical trials include only patients who choose to take part. Please take your time to make your decision about participating. You may discuss your decision with your friends, family, and health care team. If you have any questions, you may ask your study doctor.

You are invited to participate in this study because you have been diagnosed with neuroblastoma. Your cancer has either grown back (relapsed) or has never gone away (persistent tumor) after standard treatment. Standard treatment may have included chemotherapy, surgery, radiation therapy and/or high-dose chemotherapy with a stem cell transplant.

Why is this study being done?

The purposes of this study are:

• To find the highest doses of MLN8237, that can be given with irinotecan, and temozolomide without causing severe side effects.
• To find out the side effects seen by giving MLN8237 at different dose levels with irinotecan, and temozolomide.
• To measure the levels of MLN8237 and irinotecan in the blood at different dose levels
• To determine if your tumor gets smaller after treatment with MLN8237, irinotecan, and temozolomide
• To determine if specific gene changes makes you more prone to side effects from and/or response to the combination of MLN8237, irinotecan, and temozolomide
• To determine if the amount of something in your tumor called MYCN or Aurora A makes you more likely to have a good response to the combination of MLN8237, irinotecan, and temozolomide

The research is being done because:

• Currently there is no known effective treatment for your type of cancer.

This study will combine an oral drug called MLN8237 with two chemotherapy medicines called irinotecan and temozolomide. MLN8237 is an investigational drug that is not approved by the FDA. MLN8237 blocks the function of a protein that is important in the growth of cancer cells. This drug has been tested as a single-agent in children with relapsed solid tumors, including patients with neuroblastoma. In the laboratory, MLN8237 appears to make neuroblastoma tumors smaller. This effect is even greater when MLN8237 is combined with the chemotherapy drugs, irinotecan and temozolomide.

What is radiotherapy?

Radiotherapy is a treatment which involves precisely targeting high energy X-rays (ionizing radiation) at a specific area with the aim of destroying any cancer cells there. In the same way that having a normal X-ray does not hurt, you will not see or feel anything whilst you are having radiotherapy. In certain situations some patients may benefit from having a form of radiotherapy known as Intensity Modulated Radiotherapy (IMRT). This technique permits the better shaping of dose around the tumour and helps to reduce the dose to healthy tissue. For example, this tends to be appropriate when we need to treat both sides of the head and neck yet want to try to reduce the dose to certain tissues within that area in order to minimize long term side effects, such as a dry mouth.

There are several different machines that can give radiotherapy and IMRT. These include a linear accelerator which is a type of treatment machine that rotates around you and delivers a number of different beams from a variety of angles. Your treatment will be delivered by radiographers. A linear accelerator has an attachment which means that the radiographers can take low dose X-ray or CT images which they can then check and assess to ensure you are in the correct position. Another type of treatment machine is a TomoTherapy unit which is a combination of a CT scanner and a radiotherapy treatment machine. This machine works by performing a CT scan first to check that you are in the correct position and then delivers treatment. During the treatment the bed that you are lying on will move slowly through the machine. CT scans taken during treatment involve a very small additional dose of X-rays. However these scans are important to ensure accurate treatment and overall serve to reduce the risk of side effects. These scans cannot be used to confirm how well the treatment is working.

It is extremely important that you are not pregnant or become pregnant during your course of radiotherapy. Even a small amount of radiation may damage an unborn foetus so it is very important to let the radiographers know immediately if you think there is even a possibility that you may be pregnant before any radiation exposures are given on the CT scanner or treatment machine.

Why do I need to have radiotherapy?

The team of doctors involved in treating cancers of the head and neck have recommended that you receive a course of radiotherapy. Sometimes people who have radiotherapy will also have chemotherapy and your doctor will have discussed whether this is appropriate in your case.

¿Qué es la radioterapia?

La radioterapia es un tratamiento que consiste en disparar rayos X de alta energía (radiación ionizante) en forma precisa a un área específica con el fin de destruir cualquier célula cancerosa que se encuentre en ella. Al igual como sucede cuando se toma una radiografía normal, usted no verá ni sentirá nada mientras recibe la radioterapia. En ciertas situaciones, para algunos pacientes puede ser beneficiosa una forma de radioterapia denominada «radioterapia de intensidad modulada» (IMRT). Esta técnica permite conformar mejor la dosis alrededor del tumor y disminuir la dosis sobre los tejidos sanos. Este tipo de radioterapia suele ser apropiada, por ejemplo, cuando se necesita tratar ambos lados de la cabeza y el cuello, pero al mismo tiempo se quiere disminuir la dosis sobre algunos tejidos de dicha área a fin de minimizar los efectos secundarios a largo plazo, tales como la sequedad en la boca.

Existen varios tipos de máquinas que pueden administrar radioterapia e IMRT. Algunos ejemplos son los aceleradores lineales, que son un tipo de máquina de tratamiento que gira alrededor del paciente y emite varios haces desde diversos ángulos. El tratamiento es administrado por radiólogos. Los aceleradores lineales tienen un aditamento con el que los radiólogos pueden tomar radiografías o tomografías de dosis baja, las que pueden revisar y evaluar para asegurarse de que usted está en la posición correcta. Otro tipo de máquina de tratamiento es la unidad de tomoterapia, que combina un aparato de TAC con una máquina de radioterapia. INFORMATION FOR PATIENTS UNDERGOING ANTIBIOTIC TREATMENT

The aim of this leaflet is to provide you as a patient or carer with important information to help enable you to use antibiotics as safely and effectively as possible.

General advice

If you have been prescribed antibiotics, you should take them according to the instructions you have been given and complete the course.

Remember to seek advice if you suffer from the side effects of your antibiotic.

What are antibiotics?

Antibiotics are prescription medications used to treat or prevent infections caused by bacteria.

Antibiotics do not work against viruses that cause common colds, flu, coughs and sore throats. Most viral infections are self-limiting and will clear up without treatment.

When should antibiotics be used?

Treatment

Your doctor will only prescribe antibiotics when you need them. The dose and duration of your treatment will depend on the type of infection, its severity and the antibiotic used. You should complete the course of antibiotics which has been prescribed for you.

Not taking your antibiotic in the way it was prescribed can lead to treatment failure and development of resistance to standard antibiotic treatment. This is the reason why it is important to finish the course of antibiotic even if you feel better.

Prophylaxis

If you are having an operation, you may be given antibiotics to prevent an infection. This is known as surgical prophylaxis and is common before surgery.

If your immune system is impaired, you may be given long term antibiotic prophylaxis to protect you from infection (e.g. after a splenectomy).

How to take antibiotics?

Before taking your antibiotic, tell your doctor or pharmacist if you:

• Have any history of medication allergies
• Have any liver or kidney problems
• Are pregnant or breast feeding

Some antibiotics may interact with alcohol, food or some medications. Inform your doctor or pharmacist of any regular medications that you take including medicines your buy. Always follow the instructions from your doctor or pharmacist and information provided for you on the label and patient information leaflet from the medication package.

Antibiotics must be stored out of the reach of children. Do not give your antibiotics to friends, family or pets and do not keep left-over antibiotics. If you have received more antibiotics than were prescribed for you, return the remaining to your local pharmacy for safe disposal.

Profilaxis

Si tiene el sistema inmunitario debilitado, puede que le administren un tratamiento profiláctico prolongado con antibióticos para protegerlo de las infecciones (p. ej., después de una esplenectomía).

¿Cómo se deben tomar los antibióticos?

Antes de tomar el antibiótico, dígale a su médico o farmacéutico si usted

• tiene antecedentes de alergias a medicamentos,
• tiene problemas hepáticos o renales, o
• está embarazada o amamantando.

Algunos antibióticos pueden interactuar con el alcohol, las comidas u otros medicamentos. Dígale al médico o farmacéutico si toma algún medicamento en forma regular, incluidos medicamentos que compre por su cuenta. Each tablet contains 250 mg phenoxymethylpenicillin (as phenoxymethylpenicillin potassium).

4. Clinical particulars

4.1 Therapeutic indications

For use in the treatment of mild to moderately severe infections caused by penicillin sensitive organisms.

Consideration should be given to official guidance on the appropriate use of antibacterial agents.

4.2 Posology and method of administration

Posology

Adults: The dosage is 250-500 mg every six hours.

Elderly: The dosage is as for adults. The dosage should be reduced if renal function is markedly impaired.

Prophylactic Use: The dosage is 250 mg daily for long term prophylaxis of rheumatic fever.

Paediatric population

Children 1-5 years: 125 mg every six hours

6-12 years: 250 mg every six hours

To avoid late complications (rheumatic fever), infections with β-haemolytic streptococci should be treated for 10 days.

The treatment of acute otitis media with penicillin V should be limited to 5 days. However, 5-10 days treatment may be recommended in patients with potential for complications.

Method of administration

Penicillin VK Tablets 250 mg/Phenoxymethylpenicillin 250 mg Film-Coated Tablets are for oral use.

Each tablet should be swallowed whole with water, at least 30 minutes before food, as ingestion of phenoxymethylpenicillin with meals slightly reduces the absorption of the drug.

4.3 Contraindications

Phenoxymethylpenicillin is contraindicated in patients with known penicillin hypersensitivity.

Attention should be paid to possible cross-sensitivity with other beta-lactam antibiotics e.g. cephalosporins. Severe acute infections should not be treated with phenoxymethylpenicillin.

4.4 Special warnings and precautions for use

Phenoxymethylpenicillin should be given with caution to patients with a history of allergy, especially to other drugs. Phenoxymethylpenicillin should also be given cautiously to cephalosporin-sensitive patients, as there is some evidence of partial cross-allergenicity between the cephalosporins and penicillins. Patients have had severe reactions (including anaphylaxis) to both drugs. If the patient experiences an allergic reaction phenoxymethylpenicillin should be discontinued and treatment with the appropriate agents initiated (e.g. adrenaline and other pressor amines, antihistamines and other corticosteroids).

Particular caution should be exercised in prescribing phenoxymethylpenicillin to patients with an allergic diathesis or with bronchial asthma

Oral penicillins are not indicated in patients with severe illness or with a gastrointestinal disease that causes persistent nausea, vomiting gastric dilation, cardiospasm, intestinal hypermotility or diarrhoea because absorption may be reduced. Occasionally, patients do not absorb therapeutic amounts of orally administered penicillin.

Streptococcal infections should be treated for a minimum of 10 days and post-therapy cultures should be performed to confirm the eradication of the organisms.

In patients undergoing long-term phenoxymethylpenicillin treatment the complete and differential blood count, as well as the liver and kidney function, should be monitored.

During long-term treatment attention should also be paid to the potential overgrowth of resistant organisms including Pseudomonas or Candida. If super-infection occurs, appropriate measures should be taken.

Caution should be used when treating patients with a history of antibiotic-associated colitis.

Each tablet of Penicillin VK Tablets 250 mg/Phenoxymethylpenicillin 250 mg Film-Coated Tablets contains 28 mg of potassium, which may be harmful to people on low potassium diets and may cause stomach upset, diarrhoea and hyperkalaemia. High doses should be used with caution in patients receiving potassium-containing drugs or potassium sparing-diuretics.

In renal impairment the safe dosage may be lower than usually recommended.

During treatment with phenoxymethylpenicillin non-enzymatic glucose tests may be false-positive.

4. Información clínica

4.1 Indicaciones terapéuticas

Para el tratamiento de infecciones leves a medianamente severas causadas por microorganismos susceptibles a la penicilina.

Se deben seguir las recomendaciones oficiales referentes al uso apropiado de antibióticos.

4.2 Dosis y método de administración

Dosis

Adultos: La dosis es de 250-500 mg cada seis horas.

Ancianos: La dosis es la misma que para los adultos. Debe disminuirse si la función renal se ve afectada de manera apreciable.

Uso profiláctico: La dosis es de 250 mg al día para el tratamiento preventivo prolongado de la fiebre reumática.

Niños y adolescentes

1 a 5 años: 125 mg cada seis horas

6 a 12 años: 250 mg cada seis horas

Para evitar complicaciones tardías (fiebre reumática), las infecciones por estreptococos β-hemolíticos deben tratarse durante 10 días.

El tratamiento de la otitis aguda del oído medio con penicilina V debe durar 5 días como máximo. No obstante, se recomienda un tratamiento de 5 a 10 días en pacientes que corren el riesgo de sufrir complicaciones.

Vía y forma de administración

Los comprimidos de penicilina VK 250 mg / comprimidos recubiertos de fenoximetilpenicilina 250 mg son para uso oral.

Cada comprimido debe ingerirse con agua por lo menos 30 minutos antes de las comidas, ya que ingerir fenoximetilpenicilina con las comidas disminuye levemente la absorción del fármaco.

4.3 Contraindicaciones

Se contraindica el uso de fenoximetilpenicilina en pacientes alérgicos a la penicilina.

Se debe estar atento a una posible sensibilidad cruzada con otros antibióticos betalactámicos, como las cefalosporinas. Las infecciones agudas severas no deben tratarse con fenoximetilpenicilina.

4.4 Advertencias especiales y precauciones de uso

La fenoximetilpenicilina debe usarse con precaución en pacientes con antecedentes de alergias, especialmente a otros medicamentos. También debe administrarse con cautela en pacientes alérgicos a las cefalosporinas, ya que hay indicios de que las cefalosporinas y las penicilinas tienen cierto grado de alergenicidad cruzada. Algunos pacientes han presentado reacciones graves (entre ellas choques anafilácticos) a ambos tipos de fármacos. Si el paciente sufre una reacción alérgica, se debe suspender el uso de fenoximetilpenicilina e iniciar un tratamiento con medicamentos apropiados (p. ej., adrenalina y otras aminas vasotensoras, antihistamínicos y otros corticoesteroides).

Se debe tener especial precaución al prescribir fenoximetilpenicilina a pacientes con diátesis alérgica o asma bronquial.

Se contraindica el uso de penicilinas para administración oral en pacientes con enfermedades graves o con afecciones gastrointestinales que les causen náuseas permanentes, vómito, dilatación gástrica, espasmos cardiacos, hipermovilidad intestinal o diarrea, ya que la absorción del medicamento podría disminuir. Ocasionalmente, los pacientes no pueden absorber las dosis terapéuticas de la penicilina de uso oral.

Las infecciones por estreptococos deben tratarse durante 10 días como mínimo, y después del tratamiento deben realizarse estudios de cultivos para confirmar que se haya erradicado a los estreptococos.

En los pacientes sometidos a tratamiento prolongado con fenoximetilpenicilina se debe monitorear la fórmula sanguínea completa y la fórmula leucocítica, además de la función hepática y renal.

Durante un tratamiento prolongado, se debe estar atento a la posible proliferación de microorganismos resistentes, como los de los géneros Pseudomonas o Candida. Si ocurre una sobreinfección, se deben tomar las medidas correspondientes.

Se debe tener precaución al tratar a pacientes con antecedentes de colitis posantibiótica.

Cada comprimido de penicilina VK 250 mg / comprimido recubierto de fenoximetilpenicilina 250 mg contiene 28 mg de potasio, lo cual puede ser perjudicial para las personas que llevan dietas bajas en potasio y causar malestar estomacal, diarrea e hiperpotasiemia. Se debe tener cautela al administrar dosis altas a pacientes que toman medicamentos con potasio o diuréticos preservadores de potasio.

En caso de insuficiencia renal, la dosis segura podría ser inferior a la que se suele recomendar.

Durante el tratamiento con fenoximetilpenicilina, los análisis no enzimáticos de la glucemia podrían dar falsos positivos.
