By jim Hutchison The heart is a powerful muscle, weighing between 200 and 425 grams, that ceaselessly pumps blood around the body, delivering oxygen and nutrients and removing carbon dioxide and waste products. It beats about once a second, drawing a constant source of oxygen-rich blood through a complex network of coronary arteries. Over time, bad habits such as smoking, poor diet and lack of exercise, as well as genetic factors may lead to a buildup of cholesterol and fatty deposits, or plaque, inside the artery walls. Known as atherosclerosis or coronary-artery disease, the plaque slowly accumulates, narrowing the arteries. Through a biological mechanism not fully understood, the surface of the plaque can become inflamed and rupture, forming a blood clot. If this coronary thrombosis completely obstructs the artery, the area of heart muscle it feeds is suddenly starved of blood, triggering a heart attack. Within minutes cardiac muscle cells begin to die. Most heart attacks are not fatal, and their severity depends on many factors, including which part of the heart is affected, and for how long and how large an area of cardiac muscle is left ischemic (without blood supply). Symptoms can vary from mild - a quarter of all heart attacks are "silent," without symptoms, occurring most commonly in diabetics - to severe, but usually the heart sends out such distress signals as chest pain, which sometimes radiates out to the shoulders, arms and jaw; shortness of breath; and nausea (indigestion or vomiting) and sweating. An ischemic heart muscle, especially if it is a large area, may be unable to conduct the electrical impulses that originate in the sinus node and orchestrate the heart's rhythmic pumping action. The resulting arrhythmia (irregular heartbeat) can set the scene for a deadly chain of events. Instead of beating and contracting uniformly, the heart may fall into a quivering, chaotic state that results in cardiac arrest. With the heart no longer beating, blood pressure plummets, and the victim collapses. Breathing and blood circulation cease, and without swift intervention - CPR to maintain some pumping action and to keep oxygen and blood flowing to the heart and brain, and electric shock from a defibrillator to restore rhythmic pumping action -tissue and vital organs are deprived of oxygen and begin to die within minutes. For every minute of delay in defibrillation, the survival rate decreases by seven to ten percent. More than 80 percent of heart-attack victims who reach hospital survive. Half of those who die from a heart attack do so within an hour of the first symptoms, and before reaching the hospital. A person is considered clinically dead when his or her heart stops, but they may be resuscitated. This may be achieved through cardiopulmonary resuscitation (CPR), which consists of two basic life-support skills: Artificial respiration provides oxygen to the lungs while chest compressions keep blood circulating through the body. The first thing you should do if you suspect a heart attack is to send or go for medical help. Then begin CPR. Here's how: 1 Protecting the head and neck, place the person face up. Open the airway by tilting the head and lifting the jaw. Check for normal breathing for up to ten seconds. Place your ear just above the person's nose and mouth, listen and feel for breath, and watch for chest movement. 2 If the person is not breathing normally, seal your mouth around his or hers and, while pinching the nose, breathe into the person twice. Release the person's nose and mouth to allow air to escape. Each breath should take one second. Use enough air to make the chest rise. 3 Make sure the person is on a firm, flat surface and kneel to his or her side. Position your shoulders directly over your hands with elbows locked. Position your hands mid-chest. Push hard with the heels of your hands 30 times, then give two more breaths. Source text - English A SIMPLE IDEA A vaccine for cervical cancer? It took Ian Frazer to make it happen By Louise WIlliams Flying back into Australia recently, it suddenly hit Professor Ian Frazer that his life really had changed. Here, perhaps, was a chance to bask in a moment of hard-earned glory after decades of solid backroom work. So he put his characteristic humility to one side. And instead of filling in "medical researcher" under profession on his landing card, he carefully wrote out "Australian of the Year." "No-one even noticed," he laughs, recalling how an immigration clerk processed his documents and nodded him through without a second glance. Such is the modest public profile of a man now standing on the brink of medical history. Frazer's cervical cancer vaccine was approved in June last year for use in Australia and the US. Other countries have followed suit and, as a result, the next generation of women around the world may well dismiss the human papillomavirus (HPV) as a scourge from a bygone age, just as we take for granted the end of polio. Until now, this common sexually transmitted virus – which can go on to cause cervical cancer – has killed some 250,000 women a year. Cervical cancer is the only human cancer yet proven to be caused entirely by a virus. And Frazer's vaccine, developed with his late research partner, Dr Jian Zhou, makes it possible for us to wipe it out. Professor Peter Doherty, an Australian Nobel laureate who made one of last century's most significant medical discoveries in physiology and immunology, says that in a world of daily headline claims and hype, Frazer's work truly can be called a breakthrough – one that surprised even the scientific community, because it works so well. "Ian has made an incredible contribution to human well-being," he says. Growing up in the cold, smoggy cities of Edinburgh and Aberdeen, Scotland, in the late 1950s, Ian Frazer plus a chemistry set equalled an explosive combination. His father was a professor of medicine; his mother had a PhD in science. Little wonder that by early primary school, Frazer had decided he wanted to be a physicist. Then, when he was about nine, he remembers lining up with schoolmates for their polio vaccinations. "The needle got my attention," he says – which is where most kids are happy to leave it. "Then, I realised there were [crippled] kids around who had had polio. That got me interested in how the body fights infection." Frazer studied medicine at university, graduating in 1977. From there, the young Scot with a brilliant eye for detail could have walked into a research job at Cambridge University. Instead, he had figured that much of the best work in immunology was coming out of the Walter and Eliza Hall Institute of Medical Research in Melbourne. He'd spent several months as an intern there in 1974 and liked the laid-back Aussie lifestyle, so he emigrated. "In retrospect, it was a risky strategy," says the immunologist, now 53. But he also had a deliberate plan. A strange new illness was brewing. Frazer began working on liver diseases linked to hepatitis B in gay men. And when a US researcher dropped by the institute and mentioned a mysterious immunity problem among gay men in the US, Frazer realised his patients had a similar problem, later recognised as HIV-AIDS. Frazer also noticed his patients were commonly afflicted with genital warts caused by the human papillomavirus. HPV wasn't just a nuisance; the warts seemed to be associated with abnormal cells that were on their way to becoming cancer. To link a virus to cancer was contentious. Only one other scientist, German virologist Harald zur Hausen, had linked HPV in women to cervical cancer. But Frazer was convinced this needed further investigation. Research grants were easier to find in Queensland, and with his wife Caroline, whom he had met while at university in Scotland, and their two children (with a third on the way), he moved to Brisbane and was soon running his own lab at the Princess Alexandra Hospital, the University of Queensland's teaching hospital. The relaxed, friendly environment was some compensation for the long hours Frazer spent on his medical research. Then, in 1989, he was offered the chance to go and spend some time in Cambridge on a sabbatical. It was an academic's dream, and once more Frazer and his family were unpacking suitcases in a new location. It wasn't long, however, before the Frazers realised their Australian dollar savings converted to too few British pounds. In just such a precarious financial situation were a Chinese couple, Dr Jian Zhou and his wife and assistant Dr Xiao-Yi Sun, toiling long hours in the lab next door. "Jian's English was limited and his wife's was nonexistent, but somehow we managed to talk," Frazer says. And as they talked, they connected – a Scottish immigrant and a young Chinese molecular biologist who'd survived the proletariat farm and factory labour of the Cultural Revolution. "We just traded off each other. I could see what he was trying to do, and what I was trying to do was very similar," says Frazer of their common interest in the papillomavirus. But for Zhou, a talented virologist, the interest was in the behaviour and characteristics of the virus itself. As an immunologist, Frazer was working on how the human body responded to it. It was a perfect scientific match. Zhou and Sun explained to Frazer how they weren't keen to go back to China. "We wanted the opportunity to advance our careers and work with leading international scientists, whether that be in the UK, US, Germany or Australia," says Sun. In the end, it took nine months for Frazer to get the papers necessary to get the two researchers to Brisbane. In March 1991, six months after they began working together in Brisbane, the team was stunned by the results of an experiment. Unlike most other viruses, the papillomavirus cannot be grown in a test tube; it grows only on intact, living skin. But to create a vaccine, they needed something that so closely resembled the real virus that the body would be tricked into recognising it and – as with all vaccines – trigger a mild immune response that it could quickly draw on if it ever had to deal with a serious attack. When the group examined the electron microscope photographs of their tests to combine two proteins, they spotted virus-like particles and they realised they'd managed to mimic the "coat" of the real virus; it was the building block they needed to create their revolutionary vaccine. "We'd cracked it. I don't think any of us doubted for a moment that we had done it. We got very excited, but then we weren't sure we should tell other people," says Frazer. It wasn't, as he explains, "exactly a champagne moment." They realised there was so much work still to be done. As it turned out, it was two more years before the vaccine was turned over to the Melbourne-based biopharmaceutical company CSL Limited, and another 13 years before the general public would see the benefit of the team's work. The biggest regret Frazer has now is that Zhou's sudden death in 1999, from a complication to a routine medical problem during a trip to China, means he can't share the success with his partner of so many years. But the work is continuing. With a carbon copy of the papillomavirus in a test tube, Frazer has been able to conduct previously impossible tests on how it works. And now his lab is chasing down the next vaccine: one that can be used to treat someone who already has the virus in her system. What advice does Frazer have to other medical researchers to keep believing in what they're doing? "The art in science is to make sure you are trying to answer answerable questions," he says. "I tell my students, don't go into science if you want to be famous." And indeed it's been a very long, uncertain road, 20 years at least since the first germ of an idea to a vaccine. But now Frazer's discovery is the stuff of history, not mere celebrity. And that's far more durable than a brief glance of recognition at the immigration counter. Box: Making Cervical Cancer History How will the vaccine change our lives? By the time you finish reading this article, a woman somewhere in Asia will have died of cervical cancer. Over half of the 500,000 cases of cervical cancer diagnosed worldwide each year occur in the region. But in the not-too-distant future, girls are likely to be vaccinated against cervical cancer as infants, and the HPV vaccine will be just another of the cocktails protecting our children. Here's what you need to know now: THE VIRUS According to the World Health Organization, HPV is the most common sexually transmitted viral infection with approximately 630 million people infected worldwide. About 30 per cent of women will get a high-risk HPV during their lifetime, usually between the ages of 18 and 25. In most women, their immune system kicks in to fight it and it disappears. About one in 50 women remain chronically infected and that may progress to cancer. "Many women who come for a Pap test have no idea what HPV is," says Professor Jatupol Srisomboon, chairman of the Department of Obstetrics and Gynaecology at Chiang Mai University in Thailand. In Singapore, the Obstetrical and Gynaecological Society and the Association of Women Doctors Singapore recently started their Empowered to Protect campaign (empoweredtoprotect.com) to teach women about the disease and how to take action against it. A similar campaign in Malaysia also spreads the word about the dangers of HPV (tellsomeone.com.my). THE CANCER Once a woman is infected, the papillomavirus gets inside a cell and triggers it to multiply, usually causing harmless warts. However, the cell multiplication can go wrong, says Professor Ian Frazer, causing a cancerous growth that overwhelms the virus, killing its host. Across Asia, some 143,000 women die each year from the disease. According to the Department of Public Health in Thailand, over 6000 cases of cervical cancer are diagnosed each year. And in the Philippines, two in three women diagnosed with cervical cancer are already in the advanced stage, and half of them die within five years. THE JAB It's best if girls get the vaccine before they are sexually active, but it is never too late: PRE-PUBERTY The recent approval of the vaccine in most countries in Asia means that girls as young as nine can receive the vaccine. Although parents might balk at immunizing young girls against a sexually transmitted disease, Professor Margaret Stanley, director of research at the Department of Pathology, University of Cambridge, points out that the objective of all vaccines is to protect before exposure to a virus. "Nine to 12 year olds have the best immune response to the vaccine," says Stanley. "Your immune response goes downhill after puberty." Adult WOMEN Associate Professor Tay Eng Hseon, chairman of KK Women's and Children's Hospital medical board in Singapore and a principal investigator for the vaccine trial in Asia, says sexually active adults can still be vaccinated. Gardasil by Merck protects against four strains of HPV – type 16 and 18, which causes 70 per cent of cervical cancer, and type six and 11, which causes approximately 90 per cent of genital warts. Experts at the WHO say there are some 32 million cases of genital warts worldwide. "If a woman has not been exposed to any of the four HPV types, she will benefit from the vaccine," says Tay. HPV INFECTED For those already exposed to HPV, the best defence is still regular Pap smears to pick up abnormal cells while they can still be treated. Gynaecologists recommend the Pap smear test within three years of having sexual intercourse for the first time or by the time a woman is 25 years of age. Regular testing can prevent cervical cancer in up to 90 per cent of cases. If you are HPV positive, you may be able to receive the vaccine two years after the infection has been properly treated and eradicated. This will protect you against other strains of HPV. While there is no cure for HPV, maintaining a healthy lifestyle, practising safe sex and being in a monogamous relationship will definitely help prevent further infection. AFTER VACCINATION So then, do women still need the Pap smear test even after being vaccinated? "Definitely!" says Tay. "Until a vaccine is developed to protect a woman from all cancer-causing types of HPVs or when the Pap test is no longer cost-effective, Pap smears are still helpful." Sin embargo, los Frazer no tardaron en darse cuenta de que sus ahorros en dólares australianos no eran gran cosa una vez convertidos a libras esterlinas. En igual situación económica estaba una pareja china: el doctor Jian Zhou y su esposa y asistente, la doctora Xiao-Yi Sun, que trabajaban extenuantes jornadas en el laboratorio de al lado. “Jian hablaba poco el inglés, y su mujer nada, pero nos las ingeniamos para charlar”, cuenta Frazer. Y la conversación los hizo grandes amigos: un inmigrante escocés y un joven biólogo molecular chino que había sobrevivido los campos de trabajo agrícola y las fábricas de la Revolución Cultural. “Nuestra amistad nos enriqueció a los dos. Teníamos objetivos de investigación muy parecidos”, dice Frazer del interés que compartían por el VPH. Sólo que a Zhou, virólogo eminente, le interesaban las características y el comportamiento del virus en sí, mientras que Frazer, como inmunólogo, investigaba la reacción que producía en el cuerpo. Eran la pareja científica perfecta. Zhou y Sun le explicaron a Frazer que no querían volver a China. “Preferíamos aprovechar la oportunidad de desarrollo profesional y de colaboración con científicos de talla internacional, ya fuera en el Reino Unido, Estados Unidos, Alemania o Australia”, cuenta Sun. Frazer tardó nueve meses en conseguir los papeles para llevar a los dos investigadores a Brisbane. En marzo de 1991, a los seis meses de iniciada su investigación conjunta en Australia, el equipo quedó sorprendido por los resultados de un experimento. Cuando examinaron las microfotografías electrónicas de sus ensayos para combinar dos proteínas, observaron partículas semejantes a virus y comprendieron que habían logrado fabricar una réplica de la capa externa del VPH; era el bloque constructivo que necesitaban para crear su revolucionaria vacuna. “Creo que no dudamos ni por un instante que habíamos descifrado el enigma. Estábamos muy emocionados, pero no sabíamos si divulgar el hallazgo; no podíamos cantar victoria”, explica Frazer. Después de todo, aún faltaba mucho que hacer. Tuvieron que pasar dos años más antes de que pusieran su invento en manos de los laboratorios biofarmacéuticos CSL Limited, y 13 para que los beneficios de la vacuna llegaran al gran público. Lo que más lamenta Frazer es que la súbita muerte de Zhou en 1999, de una enfermedad que sufrió en un viaje a China, le impidiera participar del éxito junto con su colaborador de hacía tantos años. Pero la labor continúa. Source text - English Hunting Britain’s Mr. Cocaine Detective Sergeant Adrian Hodgetts suspects one man is behind the flow of ‘crack’ flooding the streets. Now the net is closing By tim bouquetAs the wedge shape of the Eurostar to Paris flashes through the Kent countryside, a well-built man leans his close-cropped head on to the backrest behind him and shuts his eyes. After 20 years hunting major criminals, Adrian Hodgetts, a detective sergeant in the Specialist Crime Directorate, is not easily excited. But today he has a meeting with drug investigators at the Ministry of the Interior in Paris. They have picked up a string of drug “mules” travelling from the French territory on the island of St Martin in the West Indies, all heading to the UK. It’s the best lead yet in a battle to halt a growing wave of super-addictive “crack” cocaine spreading through the streets of Britain. Hodgetts eases himself back into the upholstered seat and tries to relax as the train speeds quietly into the Channel Tunnel. “Bienvenue à Paris...et merci pour votre...” Hodgetts picks up his briefcase as the train enters the Gare du Nord. Within minutes he is greeting members of the French National Drug Squad at Place Beauvau. What he hears is breathtaking in its scale. Sixteen couriers have been picked up carrying up to ten kilos of cocaine each, altogether some 160 kilos. But police estimate that three times that has escaped detection: that’s nearly half a ton of cocaine on its way to Britain. Most of the mules are female, in their late teens or early twenties. Up to three travel on a flight, but their paymasters need only one to get through to make a fortune. The others are left to their fate. The drugs are hidden in books and suitcases. Liquid cocaine is in sealed gift-boxed rum bottles. It’s clearly a massive, well-organised operation. There’s no point arresting a few mules. Hodgetts knows he must find the spider at the centre of the web without giving any warning that police are on to him—or he will simply vanish. “Next time your intelligence guys discover a mule is on the way,” says Hodgetts, “let her run.” Seventeen-year-old Chantal Joly* grips her suitcase tightly as she steps from customs into the arrivals hall at Charles de Gaulle airport, her heart still pounding despite the light security on what is considered a domestic flight. She stops to scan the massed faces and searching eyes, then sees a girl holding a handwritten sign with her name on it, exactly as she’d been promised. The girl, called Tasha, travels with her on the express train into Paris. Tasha checks Chantal into a hotel, they have lunch and for a while they catch the sights of the city. Next day, April 20, 2004, they arrive at the International Coach Station in Bagnolet. Chantal, with four gift-boxed bottles of rum still safely in her luggage, boards an overnight coach to London. At his office at New Scotland Yard Adrian Hodgetts reaches out to an- swer his phone. He hears the voice of a French colleague: “She’s on her way.” In a covered driveway at Victoria Coach Station the air vibrates with the steady roar of diesel engines. Chantal steps down from the Paris coach and stops to claim her suitcase from the hold. A man greets her and leads her to a red Citroën Xsara in the street. As the Xsara slips into the traffic, Detective Constable Nicholas Potts and other cars with police officers follow. At nearby Belgravia police station Hodgetts is in touch by radio. Traffic is very light as they move towards east London. The police alternate tails and hang back as far as they dare, but it’s difficult to hide the fact that they are following. Suddenly, as the Xsara reaches Stoke Newington in London’s north-east, the driver slams into a U-turn and floors the accelerator. He’s spotted the tail. As the car races away in the opposite direction, it’s Hodgetts’s call: move in and blow their cover or risk ten kilos of cocaine reaching the streets. Hodgetts thumbs his radio: “Put in the hit!” Police vehicles swoop, blocking the Xsara front and back. The driver sits motionless, but Chantal jumps out and runs with the rum bottles. She doesn’t get far. For Hodgetts and his team, it’s a devastating setback. The only hope now is that when the spider hears of the arrests, he’ll think it’s just a one-off. Meanwhile the scent has gone cold. Maybe Chantal and the driver will yield something under interrogation. At Belgravia police station Chantal is clearly terrified and shaking. She is also beginning to show signs of drug withdrawal. “The man in the car said, ‘Get out with the box, it’s got drugs in it,’ ” she says. “He said he’d shoot me if I said anything.” She claims to have had no idea what’s in the bottles. “This man at the airport says if I drop off the bottles he’ll pay for my ticket.” “Which man?” “His name is Terrence. He is from St Martin.” A search of Chantal’s handbag reveals another lead, the name Tasha with a number: 6094 95367. The driver is Clyde Benjamin, aged 29, born in Guyana. “I went to collect someone that my friend had asked me to get,” he states. “I do not know what was in her suitcase or anything else in her possession.” That is all he reveals. He has no criminal record. Hodgetts and his team find two more precious leads—a mobile phone on Benjamin and a second one hidden in the car. Hodgetts knows that mobiles can be crucial to a criminal gang’s organisation, the strands that hold the web together. Police begin checking records for both phones. They soon strike gold: Benjamin made five calls in a single day to Clare Jones, a 38-year-old single mother recently arrested by Customs along with a passenger who had £1 million of cocaine hidden in four vases. There are more calls to the number Chantal had written next to the name Tasha. Benjamin’s second mobile has just one name and number stored, at position 1, where users normally log their own details. The name was “Curel”. The next lead is the car itself. Hodgetts turned to the DVLA to trace the red Xsara’s owner. It is registered to a 33-year-old Guyanese woman, Candy Blackman. She turns out to be Benjamin’s sister. She earns £194 a week as a care worker, yet when police examine her diary, it details expenditure of more than £334,000. But Hodgetts decides not to arrest her for the time being, to reinforce the impression that the arrest of Benjamin and Joly was a one-off. Maybe, thinks Hodgetts, we can trap the spider after all. As Hodgetts’s team digs still deeper into mobile phone records, new names and numbers surface. One belongs to Bernard Clarke, 31, who lives with his girlfriend Lisa Bennett, 36, in a plush suburban house in Romford, Essex. Both have a taste for expensive watches, a Cartier for her, a Rolex for him. Surveillance teams report they spend a lot of time shopping, returning laden with expensive items. But suddenly their routine changes. “They’re visiting chemists,” Detective Constable Paul Allen tells Hodgetts. Detectives trace them to Bream Gardens, a quiet cul-de-sac in Newham, east London, where they let themselves into No 6, a terraced house. Later, after the couple have gone, police discover empty containers of bicarbonate of soda discarded outside. “It’s got to be a crack factory,” Hodgetts tells colleagues. He knows that bicarb mixed with water and boiled in a saucepan reduces the acid content of cocaine and it crystallises into crack. There is no point in raiding the house without Bennett and Clarke there. They wait for the couple to reappear. A search of Clyde Benjamin’s girlfriend’s house produces an address book with more mobile numbers, a set of accounting figures and next to them the name “Cofy”. On other mobile phones the name comes up as “Kofi”; then, on one, the same number has the name “BC”. “Kofi”, it seems, is Bernard Clarke. Hodgett is beginning to see a pattern to the web. The nearer the centre—or more important—a person is, the more things happen when they make a call. One name, in particular, stands out. When he calls, money moves, couriers fly and crack is made. Over a period of two and half months he has called “Kofi” at the factory 330 times. On other phones he has been calling France, St Martin, Guyana and the US. The name is stored in many phones in several guises—“Bow” or “BoFt” or “Foot”. But most often it is “Bowfoot”. The next morning as Clarke and Bennett return to Bream Gardens, Hodgetts gives the order to storm the building. The policeman is stunned by what they find. “I have never seen anything on this scale.” Freshly cooked crack in half-inch-thick pizza-sized slabs covers every work surface in the kitchen. Together they weigh 9.5 kilos, with a street value of more than £1 million. At Clarke and Bennett’s house, wardrobes overflow with Prada and Versace, diamonds and jewellery. Rooms are filled with oversized white leather sofas and marble statues. One receipt shows the pair have paid £109,500 in cash for a cabinet to display their spoils. Within two weeks the police have rounded up 17 members of the gang. But the biggest prize of all is still at large. Hodgetts takes a gamble. Twenty-year-old Amy Anne Farrow of Wallington, Surrey, had been observed leaving the country for Barbados. When she returns to Gatwick Airport she is, as police had suspected, smuggling 1.8 kilos of cocaine. Hodgetts hopes that for £150,000 “Bowfoot”, with his intermediaries gone, will risk meeting the girl himself. Clearing customs, as Hodgetts intends, Amy walks to a payphone. There is no reply to her call. She returns to her suitcase and waits. If Bowfoot is in the terminal he must have smelled a rat. As police arrest the girl, Hodgetts races to her house, hoping for any lead before the trail goes cold. As he searches, there is a knock on her door. On the doorstep is a slim, middle-aged man with dreadlocks. Hodgetts recognises him from surveillance photos. He can’t believe his luck. Deadpan, Hodgetts says: “So what brings you here?” “I’m just doing a favour for a mate,” the man tells Hodgetts. But he can’t explain why he has Amy Farrow’s flight details in his pocket, or how Lisa Bennett had a business card with his and her fingerprints, or why “Kofi” Clarke—a man he says he doesn’t know—has called him 220 times on just one of his eight mobile phones, all at times when drugs or money are on the move. Handcuffs are snapped on to Bowfoot’s wrists. The trap has sprung. The police operation jailed a total of 65 people in the UK, Guyana, France and the US. In two years the Bling Bling Gang, named after their love of flashy lifestyle items, had imported £50 million of cocaine into Britain, which they turned into crack. Bowfoot, aka Ian Dundas-Jones, was born Leon Dundas in the US in 1970. Unlike Clarke and Bennett, Dundas-Jones lived a life with no bling, instead buying properties in Guyana. This February, Ian Dundas-Jones, condemned by the judge for bringing “ruination and despair on others”, was jailed for 27 years, Bernard Clarke and Lisa Bennett for 18 years each and Clyde Benjamin 12 years. In France, those jailed included Rochelle Barnwell—better known as “Tasha”. BOX COCAINE IN UK In Colombia one kilo of coca leaves costs a shade over £1. Turned into cocaine it costs £1,000 a kilo. Source text - English Report Card on Russia 2006 By David Satter Fifteen years after the fall of the Soviet Union, Russia has changed dramatically. In July 2006 it hosted the G8 summit for the first time. But how real is its evolution toward democracy? In 1990, David Satter filed a report card on Russia for Reader's Digest as it prepared to free itself from communism. We recently sent him back to update his findings. Personal Freedom: Just before New Year student Aliona Maximtsova, 26, prepared to board her flight from Moscow's Sheremetyevo-2 airport to Amsterdam, where she was joining her boyfriend to see in 2006. Thousands of young Russians like her, some already celebrating, were waiting in long lines for flights all over the world. The crowded departure lounge was full of women in long mink coats. “They wear them for beauty,” Aliona said, “and to show that they have a man who can buy them such a coat.” Aliona barely remembers the Soviet Union. She is accustomed to going abroad for holidays and plans a career as a tour guide. But Bella Minikova, a Moscow schoolteacher in her sixties, is amazed at the changes that have occurred in her country. “Children are now without complexes,” she said. “They make more of an effort to learn foreign languages. Foreign travel expands their world view and they see Russia with new eyes when they return.” The new freedom is reflected in other ways. In January 2006, Russian television began broadcasting a serial based on Alexander Solzhenitsyn's once banned novel, The First Circle. Thirty years ago, the mere possession of a copy could get you into serious trouble with the authorities. Moscow bookstores are filled with works critical of contemporary Russia and President Vladmir Putin. In Pushkin Square, where dissident gatherings used to be dispersed by the police, political demonstrations are a regular event. Yet this freedom is not complete. When I talked with her in 1990, Rita Luchkov, a Moscow housewife, was still stunned by the first stirrings of political freedom. Now, she has a more nuanced view of what Russia has achieved. “People can travel and read whatever they want and they don't fear that the authorities are listening to the telephone,” she reflected, “but there is no protection, particularly for people who don't have money. The bureaucracy is on the same level-rude, brutal and dishonest. If a person falls upon hard times, if he is a victim of a medical mistake or police abuse, there is no help for him.” Grade: A minus. The Press: When Islamic terrorists seized a school full of children in Beslan, southern Russia, in 2004 the newspaper Izvestiya claimed the authorities were lying about the number of hostages. Instead of 354 being held, as officials claimed, there were actually 1,200. Then the paper ran graphic full-page photographs of the attack that ended the siege. Editor-in-chief Raf Shakirov says that the Kremlin told the heads of Profmedia, which owns Izvestiya, that the paper's coverage was unacceptable. “The authorities were demanding blood,” says Shakirov, “and the only person whose blood they could have was mine.” The major newspapers, with few exceptions, are subject to direct or indirect control by the presidential administration. Alexander Podrabinek, to whom I talked 15 years ago when he was editing Ekspress Khronika, an unofficial weekly newspaper that campaigned against Soviet censorship, believes that many journalists are still afraid of the authorities. “If Putin wrinkles his brow,” he said, “the majority begin to shake in their boots.” Podrabinek went on to found Prima-News, an Internet news service specializing in human rights abuses in the former Soviet bloc. He travels every year to Cuba with computers, tape recorders and literature for dissidents. “Cuba is a Soviet product and I feel obliged,” he said. “This is the way Westerners helped us.” Three years ago, the independent management of NTV (Independent Television) was eliminated, leaving all national television networks under the control of the Kremlin. Press coverage during elections supports Kremlin-supported candidates. In the last presidential elections, Putin was omnipresent on television and always depicted positively. Communist candidate Nikolai Kharitonov was rarely mentioned and then only negatively. Journalists who seek to expose crime or corruption are often killed. According to the Committee for the Protection of Journalists (CPJ), at least 12 journalists have been murdered since Putin took office on Dec 31, 1999, making Russia the world's fourth most dangerous country for journalists. Not one person has been brought to justice. Grade: C. The Rule of Law: Transparency International, which monitors corruption worldwide, ranks Russia 126th in the world in terms of honesty alongside Niger, Albania and Sierra Leone, according to its 2005 index. The rule of law is absent throughout Russian society. Massive official corruption entangles business owners in a web of crime. Bribes amount to $316 billion a year, more than two-and-a-half times Russia's annual federal revenues, according to a 2005 report by Indem, an anti-corruption monitoring group. Business owners pay bribes to the police and government agencies and the weight of constant payoffs leads them to cheat on their taxes. Konstantin Gagarin, a wholesale clothing importer, said that businesses do not show their real turnover. “You need to show only what is necessary for salary, rent and expenses. Among small and medium-sized businesses, this is practically universal. If they did not do this, their business would be unprofitable. There would be too much money for the government.” Importers have to deal with the Russian customs. “Customs officials can place one code or another on your goods,” Gagarin said. “In one case, you pay 10 percent of your revenue, in the other 40 or 50 per cent. You pay a broker to get your goods through customs and part of what you pay him goes to the customs official.” Property rights are insecure. An entire industry has grown up dedicated to stealing companies from their rightful owners and selling them to the highest bidder. A company buys shares in a factory and then takes legal action against the management in a court thousands of miles away. The court rules in favor of the plaintiff in return for a bribe and the new shareholder begins an audit of the enterprise. Hired thugs escort the new “director” onto the grounds and the new management rewrites the register of stockholders, crossing out the names of the old. In Samara, about 1000 km southeast of Moscow, lies Togliatti-Azot, a giant chemical factory and the world's largest producer of ammonia. In 2005, the Renova group, run by Viktor Wechselberg, an oligarch close to Putin, and another holding company, Evrokhim, jointly formed a company, Synttech. It bought a ten percent interest in Togliatti-Azot and attempted to put its representative on the board of directors. The attempt failed but the Synttech shareholders immediately complained about “irregularities” at the factory. This led to searches by federal agents and the seizure of documents. In June 2005, the director of the factory, Vladimir Makhlai, who owns the controlling packet of shares, was charged with tax evasion and a warrant was issued for his arrest. He disappeared and, according to a factory spokesman, was receiving medical treatment at an undisclosed location. Renova announced it was ready to negotiate the purchase of his shares. Anatoly Ivanov, a deputy in the State Duma (Parliament), who is trying to block the takeover, said: “In 2002, the tax service did a check of Togliatti-Azot and did not find any violations. Now they find them. This is confirmation that the basic goal is not to get the taxes but to seize the enterprise.” Police corruption is notorious. Officers routinely shake down citizens during identity checks and traffic stops. Film-maker Andrei Nekrasov was stopped in St. Petersburg one night after he mistakenly turned down a one-way street. The officer who halted him should have written out a ticket to be paid at the state bank. Instead, he demanded 80 rubles. Nekrasov, who had just returned from England, had only a British £20 note (1,000 rubles). The policeman began to lecture him on traffic safety. Finally, he said, “I don't even have a car, you have a car, you should pay.” Nekrasov handed over the £20 note. “This how they earn money,” he commented. Grade: D. The Economy: Flush with Russia's oil profits, Moscow reeks of prosperity. It has more billionaires than any other city in the world. Roman Abramovich is Russia's richest man. Following the death of his parents, he was raised by an uncle and owns castles in France and Scotland, Chelsea Football Club in London, his own Boeing 767 jet, and four luxury yachts, each reportedly equipped with a swimming pool, helicopter landing pad and mini-submarine. Many of Russia's new capitalists live in the village of Zhukovka, eight kilometers outside Moscow. Protected by armed guards and sequestered behind high walls, they enjoy a lifestyle that would have been unimaginable during the Soviet era. In the Zhukovka-Plaza shopping centre, you can buy a gold mobile telephone with gold keys for 86,000 Euros or a phone made out of platinum with embedded diamonds for 46,000 Euros. There are Krone pens on sale, including a gold pen with a fragment of the last lipstick used by Marilyn Monroe embedded in the top for $14,000. The jewellery includes a necklace produced in 1945 by the Swiss firm Faerber with 20 24-carat diamonds (price $63,000) and a star sapphire brooch surrounded by diamonds for $49,000. “Only Russians buy these,” said a salesgirl. “Foreigners come, look, but never buy.” But small businessmen can also succeed in Russia today. Konstantin Vasiliev, the owner of Stensis, a furniture manufacturing company, worked as a dealer for a Finnish furniture firm until the Russian financial crash of 1998. With the sharp fall in the value of the ruble, the Finnish firm's products became too expensive for Russians so Vasiliev began to produce furniture himself. Stensis now makes office furniture and kitchen cabinets for the middle class market (Moscow's middle class alone is estimated at more than a million people). “You don't need connections to start a business,” says Vasiliev. “You just have to be ready to accept responsibility.” Russia is still plagued by gross inequalities in wealth. According to a 2004 report in Parlamentskaya Gazeta, the State Duma's official bulletin, the wealthiest 5 percent of the population possess 75 percent of the country's savings whereas 71 percent of the population has only 3 percent. “There are really two countries,” said Albert Speransky, a free trade union activist in Moscow. “All the power is in the hands of the officials and the elite, which lives in its own secluded world. There is no noticeable improvement in the standard of living in the provinces.” The energy sector is booming. High world prices have encouraged investment and produced windfall profits. If the price of oil were to drop, however, today's relative economic stability would be threatened. Grade: C minus. Health: In the post-communist era Russia has witnessed a catastrophic deterioration in public health. According to the World Bank, between 1988 and 1994, life expectancy of males fell by seven years to 58 years. Overall life expectancy in Russia today (65 years) ranks it 112 in the world, behind Ukraine and Indonesia. In 1990, overall life expectancy in the Russian part of the Soviet Union was 68.9 years. In the period 1992-95, deaths exceeded births by two million, a demographic catastrophe. Among the reasons for this decline are the failure to finance the public health system and the removal of restrictions on the sale of alcohol. Russia is also faced with a mounting Aids crisis. Figures from the Federal Center for Fighting Aids show the number of registered cases is rising by 30 percent each year. According to the World Bank's 2002 data (the latest available), Russia spent $12.5 per person per month on health care, compared to $439 in the US, placing Russia at 82 in the world healthcare rankings. Despite its large number of well-trained doctors, Russia is not caring for the health of the population. Grade: D Tighter State Control: On September 13, 2004, in the wake of the Beslan massacre, President Putin announced that the fight against terrorism demanded a “deep reshaping of policy.” His proposal: to eliminate the direct election of provincial governors. But the scheme had been on the agenda for months. Putin had used the tragedy to justify a change that had been planned well in advance. The government already controls the State Duma and the judiciary. The appointment of governors will help eliminate what little political pluralism still exists. A new law on foreign non-governmental organizations (NGOs) or those financed from abroad, perhaps the only bodies still capable of offering political opposition to the regime, was recently passed. It allows the government to refuse registration for foreign NGOs if they harm “national interests” or fail to comply with the Constitution or laws. Although NGOs have the right to challenge the decision, the vagueness of these criteria means that the government can close them at will. With this degree of control, there are deep misgivings as to whether Putin and his entourage will permit truly free elections in 2008. There have already been widespread abuses in provincial elections, including vote stuffing and intimidation. The Organisation for Security and Co-operation in Europe (OSCE) reported that the 2004 presidential elections were marred by irregularities. Russia is becoming steadily more authoritarian again. Grade: D plus. For the most part, Russians are better off. They now have at least a degree of freedom and can make choices about their lives. But there is still a huge gap between the rulers and the ruled and, all too often, the ruled are viewed simply as material for exploitation. Communism conceived of the individual as a cog in a machine, without rights or feelings. Today's Russian rulers frequently see ordinary people in the same way. To complete the journey to democracy, the country needs justice, the rule of law and respect for the individual. Russia has come a long way but there is still a long way to go. En enero de 2006 la televisión rusa empezó a emitir una serie basada en El primer círculo, novela antes prohibida de Aleksandr Solzhenitsin. Hace 30 años, bastaba poseer un ejemplar para meterse en un lío grave con las autoridades. Las librerías moscovitas están a reventar de libros críticos de la Rusia contemporánea y el presidente Vladimir Putin. En la Plaza Pushkin, donde antes la policía dispersaba las reuniones de disidentes, hoy abundan las manifestaciones políticas. Pero la libertad no es completa. Cuando en 1990 hablé con Rita Luchkov, ama de casa de Moscú, ella aún no salía de su asombro ante los primeros signos de reforma política. Hay que contratar a un corredor para que saque la mercancía de la aduana, y el agente aduanal se queda con parte de sus honorarios”. El derecho de propiedad no está garantizado. Ha surgido toda una industria dedicada a robar compañías a sus dueños legítimos y venderlas al mejor postor. Una empresa compra acciones de una fábrica y luego demanda a la administración en un juzgado a miles de kilómetros de distancia. El juzgado falla en su favor a cambio de un soborno, y el nuevo accionista audita la empresa. Source text - English Taking a Bullet SOME FEEL A SHARP STING, SOME FEEL NOTHING AT ALL Describing what it’s like to be shot, a Toronto man who was wounded when gunmen shot him and killed the driver of a car in which he was a passenger, uses this analogy: You know rebar, the steel bars they use in construction? Imagine someone puts rebar in a fire and makes it redhot, then stabs you with that. It’s a stabbing that enters and exits and enters and exits again. Bam-bam-bam-bambambam- bam-bam-bam. It was loud, repeatedly, and then they sped off. An innocent victim, he was hit nine times. His assailants are facing trial, and he asked that his name not be used. Other gunshot victims have described it as a sharp slap with a stick, or a bee sting or a slight burning. The speed of the bullet varies, depending on the gun, and it is this velocity that is a factor in determining the damage to body tissue. “Remember your high-school physics?” asks Dr. Michael Pollanen. “Energy equals one half mass times velocity squared. The wounding power is determined by velocity, not the mass of the bullet.” Pollanen, chief SOME FEEL A SHARP STING, SOME FEEL NOTHING AT ALL witness in court cases, says even if someone is shot in the heart, “death is by no means immediate. People can still have ten to 15 more seconds of purposeful action because the brain still has enough oxygen to function.” Despite the popular image of a victim being hurled backward by the force of a gunshot, it’s not true, he adds. “This is probably the biggest fallacy. The human body does not get knocked backward.” I still have four bullets in my left lung. I was at church on Sunday, and one of my church brothers said, “Let’s run,” and he grabbed my hand to run off with me. I had to stop after four steps. I felt the pressure in my lung. If the bullet is in the middle of muscle mass in the leg or buttock, a surgeon likely won’t take it out—probing would likely do more damage than that done by the bullet. Then they came around and shot me in the head. It felt like a piece of steel. But it only grazed the top of my skull, leaving an indent. “Ninety percent of gunshot wounds to the head are fatal,” says Brenneman. Sunnybrook sees up to 80 gunshot wounds a year, a number that is steadily rising. In 1999, by comparison, there were 38. “If the person makes it alive to our hospital, they have an 87-percent chance of surviving,” says Brenneman. The most common cause of death from a gunshot wound is internal bleeding. I looked at my wounds. The entrance wound was like a little circle, about a half inch wide. The exit wound was like a tear, more than one inch wide, like a half-moon shape. “I hate it when people use the word gunplay,” says Brenneman. “When someone gets shot, this is not playful, not something to take lightly. The consequences of a gunshot wound are enormous. The impact on a family is huge. The worst part of my job is to tell a mother and father their 18-year-old son is dead from a gunshot wound.” I’m not the person I used to be. I was a person with a lot of trust, friendly and outgoing. Since then I can’t trust outsiders. Being a person of colour, a black man, there is a stigma to being shot— people say, “You had to be involved.” Now I don’t go out a lot. I go to the lawyer, to doctors’ appointments and to therapy. ■ forensic pathologist for the province of Ontario, saw three gunshot victims between Sunday and Wednesday one recent week. In 2005 there were 333 shootings in Toronto. If a bullet leaves the muzzle of a gun at about 1,000 feet per second, it may slow down 200 feet per second as it passes through a car windshield. A bullet has to travel between 170 and 210 feet per second to penetrate skin. Often the entry wound is small, round, giving little indication of the wreckage it might leave inside the body. The glass in the passenger window shattered. I couldn’t feel my left arm or the left side of my chest. My arm was jumping, jumping, but I couldn’t feel anything. When they took the bullets out of my left side [two months later], a piece of glass came out with the bullets. As the bullet leaves the firearm, hot gases, unburned gunpowder and soot go with it, says Pollanen. If fired at contact or close range to the skin, these substances enter the body with the bullet. When the bullets entered, I felt like something was expanding in me. As the bullet tears through tissue, it can become deformed and change its shape, says Pollanen. It can also change direction on rare occasions. As it does so, it starts to divide and tear open tissue, creating a zone of destruction. With this effect, known as mushrooming, the diameter of the bullet can double in size. As the bullet travels through the body, it creates a zone of injury larger than the actual bullet hole. The force of the bullet causes the bullet hole to temporarily expand outward, known as the cavitation effect, sometimes creating a space as large as an orange. “They are like shock waves around the bullet path and can be more damaging than the bullet,” says Dr. Fred Brenneman, chief of the trauma program at Sunnybrook Health Sciences Centre. “Sometimes the zone of injury can be much wider than the bullet itself. If you’re shot in the liver, it can be huge—ten centimetres or more.” If the bullet hits bone, the bone may fracture and create secondary missiles that scatter fragments within the body. Source text - English UNFORGETTABLE B.B. KING Blues Brothers These solid-gold friendships mean the world to me. BY B.B. K I N G Iguess I've earned the right to make a few observations about life, since I am now an old man of 81. And one thing I know for sure: Age has nothing to do with friendship. Neither does race. Looking back over my long career as a bluesman, I know that three of my friendships—with guitarists Eric Clapton, Stevie Ray Vaughan, and Jimi Hendrix—stand out for me. Each of these men has given me so much more than I ever gave them. I first met Eric in 1967 at Café Au Go Go in Manhattan, when he was a youngster of 22 performing with Cream. He saw me in the audience and pointed me out. After the show, we got up onstage and made some music together. Eric pulls people in, and he’s been a good friend to me ever since. We’ve never talked about this, but I believe he and I bonded because neither of us had what you would call a normal upbringing. I grew up very, very poor on a plantation in Kilmichael, Mississippi. While Eric had a lot of advantages I didn’t, he was raised in England thinking his grandparents were his parents, and that his real mom was his older sister. I knew my mother, but she died when I was nine. Then I lived alone until I was 13 and rejoined my father. I can’t really say how I survived. I worked for a white family that was very good to me. I milked 20 cows a day. After I finished, I could go to school. I had to walk five miles to the one-room schoolhouse (the white kids had buses). I guess you never miss what you never had. But it all feeds into playing the blues. Eric told the newspapers in England that the one thing he really enjoyed about visiting the U.S. was meeting B.B. King. He told them, “If you like blues, you should go out and see him.” That was a big thing for me. It was before I ever traveled to Great Britain. Eric appeared on my first music DVD, B.B. King and Friends: A Night of Red Hot Blues, which we recorded live in Los Angeles in 1987. Straight from his tour, he chartered a plane in order to get there one night, though his fingers were sore from playing. I thought that was tremendous. He could have said, “I’m tired. I just finished work,” but he didn’t. Others were invited who didn’t come, but Eric’s a man of his word. His heart’s as big as I am. He plays better blues than most of us and may be the best living rock’n’roll guitarist there is. I loved working with him in the studio; he always had good ideas. In 1999 we recorded Riding with the King, which became my first platinum CD and introduced me to a new generation of fans. We won a Grammy for it in 2001, and all I could say to Eric was thanks. No way would it have happened without him. Even though there’s a 20-year difference in our ages, I don’t consider Eric a son. But I did think of Stevie Ray and Jimi as being just as much my kids as my own 15 biological children. Stevie used to come to me just like my sons did and ask about music. As young and handsome as he was, I thought he’d want to talk about girls. But he and Jimi talked to me about chords and how to make certain sounds on the guitar. I first met Jimi Hendrix in the early ’60s when he was playing for Little Richard. We toured for a few weeks together, and whenever his group had a break, he’d come by my dressing room and talk. Same thing with Stevie. He’d play something and say, “What do you think of this, B.? How does it sound?” He would ask for pointers. It made me feel good, like a teacher feels when he sees a student doing well. Stevie was very fast on the guitar. I’d tell him, “If I could play that well, I’d probably play as fast as I could too. But since I don’t play so well, I play slower to try to make my music more precise.” He would reply, “I just play what I feel.” Stevie was on tour with Eric when he died in that plane crash in 1990. When I first got the news on the radio, I heard it was Eric who had died. Later I found out it was really Stevie. I hurt just as bad. Same thing about Jimi’s death in 1970 from drugs—I heard it on the radio. I wish I’d gotten to talk to Jimi about the dangers of drugs. But when I knew him he wasn’t a superstar yet, and I don’t know if he was using then. He didn’t really look to me for personal advice, but now I tell younger musicians, “Get high off your music, not drugs or liquor.” Stevie was different. He hung on my every word. Whenever he’d come to see me, he would sit down, lean against my legs and talk. He had a thing about him that just made him lovable. I’ve been going back to Indianola, Mississippi, for 40 years now, playing free concerts in the park for children. As recently as 15 years ago, you would see just little black kids there. But in the last 10 years you’d begin to see a rainbow of color. It makes me so proud it seems my buttons are gonna pop off my shirt. Like Stevie, the children sit down around me, this old white-haired black guy. Kids who are 10, 12, 14, they ask questions like, “Hey, B.B., what about So-and-So?” Their faces light up when I start to talk; they get close and take in every word. You might say I have thousands of children— Eric, Stevie and Jimi have just been the most popular. One other thing about Eric Clapton and me. Being from Mississippi, I’ve got a pretty bad mouth. I swear a lot. Eric never does. He’s a real English gentleman. He’ll do anything he can to help people. He even opened a rehab center in the Caribbean for folks suffering from addiction. No por eso me dolió menos. Tampoco cuando Jimi murió en 1970 de una sobredosis. Me enteré por la radio. Quisiera haber podido advertirle los peligros de las drogas, pero cuando lo conocí aún no era una superestrella y no sé si entonces se drogaba. Source text - English Sleight of Hand Lisa Donafee This artist's illusions are one of a kind. Guido Daniele loves a challenge. When the Italian artist was hired by an advertising agency to create body paintings of animals, he took matters into his own hands—or those of his children, to be precise. "I loved the idea," says the Milan-based illustrator, painter and photographer. "I researched each animal in depth to see how I could transfer it to a hand, and then set about bringing it to life." Daniele's daughter Ginevra, 22, and son Michael James, 15, proved to be his best canvases. "If you're spending hours on end holding someone's hand, I'd rather it be the hand of someone I love. There's nothing worse than working with a nervous, unfamiliar model whose hands are shaking." His ulterior motive: to use the time to catch up with his children. The cheetah, Daniele's first painting, is still his favorite. "It turned out perfectly the first time and gave me the courage to complete the rest of the set." He now has grand designs for a horse, a panda and a snake, all for his personal portfolio. Daniele's paintings take an average of three to four hours to complete, although the first time he painted the eagle with outstretched wings it took him ten. The hardest part of his job is watching his creations disappear down the drain after they're photographed. "I'm getting used to it," he shrugs. "At least I get to start each day with a fresh canvas."
