It's odd to think that our first date is going to be at his Christmas party. Due to all the preparations that need to be made, we're not even arriving together. Parker has spent most of the day in the ballroom of the Marriott Rivercenter, and I'm just now getting to the room that he booked for us. I'm kind of glad he's not going to get to see me in my dress beforehand. I want it to be a surprise. I bought it specifically for this date. Specifically for him. He wanted me to be the most beautiful girl in the room, and I don't want to disappoint him.
I wasn't exactly sure what to wear, so I decided to go extravagant, charging a gorgeous gold and black lace-up evening dress to my credit card along with a matching clutch and elegant black heels. It's more than I can afford, but I'll be getting paid in another week, so I'm not going to worry about it now.
I'm honestly not sure what I'm more nervous about, being seen in public with him or spending our first night together alone. It's all I've dreamed about since he told me he wanted to share a room with me after the party. This is going to put our relationship on a whole new level.
I wait until it's time for the party to start, then I leave the room to take the elevator downstairs. Parker said he'd meet me in the ballroom. It's kind of weird that he didn't offer to come up to the room and retrieve me beforehand, but I don't question it. He's probably busy greeting guests now.
To my surprise, when I reach the open double doors that lead into the ballroom, it's already crowded inside. I'm relieved to see that I'm not overdressed. There are a lot of other women who are wearing gowns similar to mine, though not as pretty. As for the women themselves, a lot of them are gorgeous. Some of them I recognize from the office. Most of them I don't. He was right, it seems like the guest list is an equal mix of business associates and friends. At least, that's who I assume these other people are.
I scan the crowd for Parker, making a detour at a table where champagne glasses have been lined up and filled for the guests. I've only been in the room for a few minutes, and my nerves are already shot. Where in the hell is he?
“You're new.” A female voice startles me from behind.
I turn and come face to face with a beautiful brunette wearing a gown encrusted with jewels. My status as belle of the ball has been spirited away in the fleeting second it took for me to soak in her dress. Everything about this woman speaks of sophistication and wealth, from her perfect updo to the diamond necklace ringing her delicate throat.
“I am.” I nod before taking a long swig from the champagne glass. Any poise I once held melts away with the uncouth gesture. “I just started working for Enkidu Industries two weeks ago.”
“I can see why he invited you.” She looks me up and down, assessing me.
“I'm Parker Bernier's date.” I'm not sure why I feel the need to inform her of that, but I do. I'm not even sure if I'm allowed to tell her that, but it's too late now.
“I was his date last year.” She extends a gloved hand to grab a glass of champagne from the table, keeping her nose up to me all the while. A tremor of jealousy burns through me as I realize he's probably fucked her too.
“Good on you.” Bitterness shines through in my voice. I wish she would just go away, but she doesn't look like she has any intention of leaving. While I had thought that having some company would be nice, hers is most unwelcome.
“Don't expect that he'll bring you next year.” She stares out into the crowd. “He brings a different employee with him every year.” She points towards a woman in a red shift dress. “See her? That's Nancy Joyner. She was his date the year before me.” She gestures to another woman in a blue mermaid dress. “And that's Cynthia Roger. She was his date the prior year.”
Learning that he brings a different employee to the party as his date every year diminishes my experience. The fact that I'm not special makes my hopes of his genuine interest in me sink like a lead brick to the bottom of the ocean. This isn't actually a date. He's just following a pattern.
“Ah. There's our host.” Her face brightens as Parker steps up onto a small stage at the far end of the room. He looks as dashing as ever in a classic tuxedo, though my attraction for him is tempered by my bad mood.
Someone hands him a microphone, and he looks out over the crowd, his eyes skimming the faces of his friends and employees. “Welcome, everyone, to my fifth annual Christmas party.” He pauses for a moment to make sure he has everyone's attention. “Thank you all for joining me tonight. As always, I want everyone to enjoy themselves and have fun. Tonight is a bit more special than usual though, because my best friend has flown all the way from San Francisco to share the evening with me.” The smile on his face broadens as he prepares to introduce his friend. It's another personal side of Parker Bernier I'm not used to seeing. He looks genuinely happy… and like he's completely forgotten about me. “I'd like everyone to put their hands together for my mentor and business partner, Mister Asher Shepard.”
No! This can't be happening. I reach up to touch my chest as if it will still my rapidly beating heart. The look on my face is of utter horror. Best friend. Business partner. San Francisco. It can't be the same man. It can't be the same Asher.
A wave of nausea hits me like a freight train as my smiling asshole of an ex-boyfriend takes the stage and promptly hugs my now lover. To make things worse, he does what it seems like Parker couldn't do to save his life. He zeroes in on me almost the second that his eyes leave Parker's cheerful face.
The champagne glass I'm holding falls from my hand and crashes to the floor, snapping in half at the stem and sending champagne splattering all over the bottom of Miss Sophisticated's expensive dress. She takes a small hop away from me, but it's too late.
“I'm sorry.” My head snaps to the side to apologize. I am sorry. I never meant to ruin her dress. The shock was just too much for me to bear, and I'm so unnerved that I'm shaking. The accident allows me a moment's reprieve to drop to my knees and out of sight while I desperately wipe off the bottom of her dress with a napkin before picking up the pieces of the broken glass.
“Don't worry about it,” she huffs, stomping off into the crowd, probably to find a towel or change or who really gives a shit.
I stay on the floor meticulously picking up each tiny piece of broken glass while I formulate my escape plan. If I wasn't supposed to go find Parker, I'd crawl under the table next to me and hide there until the night was over. I can't believe that Asher Shepard and I are in the same room together.

In the urinary tract, all roads lead to the urethra. An understanding of its anatomy, in both the male and female, is important for the successful and safe removal of bladder calculi and for the safe passage of both cystoscopes and ureteroscopes.
The male urethra spans approx. 20 cm from the distal meatus to the bladder neck. The urethral meatus can vary greatly in its diameter but in general is the tightest portion of the entire urethra. Occasionally it is necessary to manually dilate the urethral meatus or perform a meatotomy to pass an instrument or to extract a distally lodged stone.
For practical purposes the male urethra is divided into three sections: the pendulous, membranous, and prostatic urethra. The pendulous urethra encompasses the distal penile and more proximal bulbar portions of the urethra, which lie within the corpus spongiosum. The urethra will normally accommodate instruments up to 28 French. As the penile urethra runs directly into the bulbar urethra, its course curves anteriorly and enters the membranous urethra. The membranous urethra is intimately surrounded by the external (voluntary) sphincter. This point of anterior deflection and muscular based occlusion is a potential site of trauma where false passages can be made posterior; understandably it is the most common point of stricture formation. It is important for the endoscopist to realize that if a false passage has been made with a catheter or rigid endoscope, that the flexible endoscope may be most helpful as it can be manipulated such that its tip will hug the anterior portion of the urethra and facilitate passage of a guide wire or the endoscope itself into the true urethral lumen. When stone passage is inhibited by short structures along the course of the urethra, direct vision urethrotomy with retrograde manipulation of the stone back into the bladder can usually be performed by cystolitholapaxy. More significant strictures that cannot be easily dilated may require transvesical extraction of a bladder stone followed by urethroplasty (1,2).
The prostatic urethra extends from the male external sphincter in the membranous urethra to the bladder neck (internal sphincter). Here, the urethral lumen is usually at its widest. Under certain pathologic circumstances that cause obstruction of the bladder neck such as prostatic hyperplasia and bladder neck contracture or bladder neck hypertrophy, there can be difficulty passing stones out of the bladder or passing an endoscope into the bladder. Obstruction at this level may require transurethral incision of the prostate or bladder neck.
In females, the urethra is typically 4 cm in length. The female urethra can usually be dilated to 32 Fr to accommodate endoscopes of varying size and for the removal of bladder stones up to 9 mm in diameter. In addition, stones can form inside urethral diverticuli. These are best treated with diverticuletomy and primary repair.
BLADDER ANATOMY
The normal functioning bladder is able to store urine at low pressure and then, in a coordinated fashion with the urethral sphincters, empty to completion. The majority of the bladder wall is composed of large bundles of interconnecting smooth muscle fibers that form a powerful meshwork capable of expansion and contraction depending on the stage of the voiding cycle. These fibers react primarily to cholinergic input. In contrast, the smooth muscle configuration of the trigone and bladder neck is more organized and performs several key functions. The trigonal muscle provides a thick supportive backing for the ureters, which enter the bladder base posterolaterally and travel medially for 1.5-2.5 cm to open into the bladder lumen and trigone. As the ureter passes through the bladder wall it is compressed and narrowed, which can prevent passage of a stone antegrade or a ureteroscope retrograde. Difficulties with ureteral access can be encountered following ureteroneocystotomy (discussed later in this chapter).
The bladder neck is intimately related to the trigone and the two as a unit act to funnel urine into the urethra during the active phase of micturition. Difficulty with bladder emptying caused by a neurogenic problem, an obstructive process, or poor detrusor function may lead to urinary stasis and increase the risk of infection. Stasis and infection are the two main risk factors for bladder stone formation.
Several other factors can lead to bladder stones. In particular, bladder diverticuli can be a source of urinary stasis and thereby promote stone formation. Foreign bodies such as iatrogenically placed sutures, indwelling Foley catheters, and indwelling ureteral stents can each act as a nidus for vesicolithiasis. Last, in patients who have undergone bladder augmentation or continent urinary diversion, mucous formation and stasis can create a lithogenic environment (3).
URETERAL ANATOMY
The ureter originates from the renal pelvis and transports urine to the bladder in peristaltic waves. The length of the ureter varies from 22 to 30 cm (4). Its course runs from the ureteropelvic junction medially where it travels along the medial border of the psoas muscle just lateral to the transverse processes of the lumbar vertebrae. In the supine position, this is an uphill climb as the ureter travels over the sacral promontory. At the L3-L4 level the ureter is crossed anteriorly by the gonadal vessels. As the ureter passes over the pelvic brim it crosses directly over the bifurcation of the common iliac artery and then proceeds downhill hugging the contour of the pelvis just medial to the internal iliac artery. Once the artery branches, the ureter courses between the inferior vesical artery inferomedially and the superior vesical artery superolaterally. Just before its oblique insertion into the bladder base (the ureterovesical junction), the ureter is coursed anteriorly in males by the vas deferens and in females by the round ligament and then slightly more caudal, laterally by the obliterated umbilical artery (i.e., the medial umbilical ligament).
Stones tend to lodge at four common points of narrowing along the course of the ureter: the ureteropelvic junction, the pelvic brim as the ureter courses over the iliac vessels, the ureterovesical junction, and the ureteral orifice. Among these, the ureterovesical junction is often the narrowest, with a diameter of 2 mm. The combination of these areas of narrowing and the relatively serpiginous course that the ureter takes from the kidney to the bladder, can make passage of guide wires or endoscopes withi the ureteral lumen quite challenging. For example, it is sometimes necessary to balloon dilate the intramural portion of the ureter or the ureterovesical junction in order to allow passage of an ureteroscope. In addition, when passing a ureteroscope up the ureter over a guide wire in a supine patient, one must visualize the three dimensional course of the ureter coursing laterally and downward through the bladder wall and then taking a sharp turn upward (anteriorly) and medially as the instrument need to be guided anterior out of the pelvic and over the iliac vessels. The pulsations of the common iliac artery are clearly transmitted to the ureter at this point. On occasion, it may be difficult to pass a semi rigid ureteroscope over the pelvic brim, especially in the male patient. This is a potential cause of ureteral and/or endoscope damage and can be avoided by changing to a flexible endoscope. Alternatively, this angulation can be modified in the male by manually pushing the suspensory ligament of the phallus posterior thereby helping with the alignment of the urethra toward the ureteral orifice.
¬The upper ureter can, on occasion, J hook making the passage of a guide wire difficult. This is more common in elderly patients with ptotic kidneys. A variety of endourological methods of overcoming this problem have been described; however, the simplest approach is to place the patient in a steep head down position and proceed to manually push the kidney upward by pressing upward along the upper quadrant of the abdomen subcostally (Mertz maneuver) (5).
After summiting the sacral promontory it is a slightly downhill course along the mid and upper ureter toward the renal pelvis, which usually sits at the level of the second lumbar vertebral body. However, to traverse the ureteropelvic junction, the endoscope usually needs to be guided even more posterior.
The anterior-posterior course of the ureter as described above reversed if the patient is in the prone position during flexible ureteroscopy. In this position, it is most helpful to first pass a ureteral access sheath to facilitate passage of the flexible ureteroscope.

La mayor parte de la pared de la vejiga está formada por grandes conjuntos de fibras musculares lisas interconectadas que forman una malla capaz de expandirse o contraerse dependiendo del ciclo de la micción. Estas fibras reaccionan principalmente a los impulsos colinérgicos. Por otro lado, la configuración de músculo liso del trígono vesical y del cuello vesical es más organizada y desempeña varias funciones claves. El músculo trigonal provee un consistente apoyo para los uréteres, que entran a la base de la vejiga de manera posterolateral y viajan de forma medial por 1,5 – 2,5 cm para abrirse en el lumen vesical y el trígono. Mientras el uréter atraviesa la pared de la vejiga, ésta se comprime y estrecha, lo que puede impedir el pasaje anterógrado del cálculo o una ureteroscopía retrógrada. Se pueden encontrar dificultades con el acceso ureteral luego de una ureteroneocistostomía (a discutirse más adelante en este capítulo).
El cuello vesical está íntimamente relacionado al trígono y ambos, como una unidad, actúan para canalizar la orina dentro de la uretra durante la fase activa de la micción. Ciertas dificultades en el vaciado de la vejiga, causadas por un problema neurogénico, un proceso obstructivo o un deficiente funcionamiento del detrusor, pueden desarrollar estasis urinaria e incrementar el riesgo de infección. La estasis y la infección son los dos factores de riesgos principales para la formación de cálculos vesicales.
Muchos otros factores pueden llevar a la formación de cálculos vesicales. En particular, los divertículos vesicales pueden ser una fuente de estasis urinaria y, por lo tanto, promover la formación de piedras. Cuerpos extraños, como las suturas iatrogénicas, los catéteres para drenaje de Foley y los stents ureterales, pueden actuar como foco de infección para la vesicolitiasis. Por último, en pacientes que se han sometido a aumentos de la vejiga o a derivaciones urinarias continentes, las formaciones mucosas y la estasis pueden crear un ambiente litogénico (3).
ANATOMÍA DEL URÉTER
El uréter se origina en la pelvis renal y transporta orina a la vejiga en ondas peristálticas. La longitud del uréter varía de 22 a 33 cm. Su trayectoria se extiende desde la zona media de la unión ureteropélvica, donde viaja por el borde medial del músculo psoas apenas lateral a los procesos transversos de las vértebras lumbares. En la posición supina, el uréter viaja de forma ascendente por el promontorio sacro. En el nivel L3-L4 el uréter se cruza en sentido anterior con las venas gonadales. Cuando el uréter pasa por la cresta ilíaca, cruza directamente por la bifurcación de la arteria ilíaca común y luego sigue cuesta abajo abrazando el contorno de la pelvis medial hasta la arteria ilíaca anterior. Una vez que la arteria se bifurca, el uréter pasa entre la arteria vesical inferior en sentido inferomedial y la arteria vesical superior en sentido superolateral. Apenas antes de su inserción oblicua en la base de la vejiga (la unión ureterovesical), el uréter cursa anteriormente en los hombres por el conducto deferente y en las mujeres por el ligamento redondo y luego lateralmente por la arteria umbilical obliterada (es decir, el ligamento umbilical medio).
Los cálculos tienden a alojarse en 4 puntos de contracción comunes a lo largo de la trayectoria del uréter: la unión ureteropélvica, la cresta ilíaca cuando el uréter cursa sobre las venas ilíacas, la unión ureterovesical y el orificio ureteral. Entre éstos, la unión ureterovesical es generalmente la más estrecha, con un díametro de 2 mm. La combinación de estas áreas de contracción y la trayectoria relativamente serpiginosa que el uréter toma desde el riñón hasta la vejiga, pueden hacer que el paso de alambres guía o endoscopios al lumen ureteral sea una tarea bastante desafiante. Por ejemplo, en ocasiones es necesario utilizar dilatación con balón en la porción intramural del uréter o en la unión ureterovesical para permitir el paso de un ureteroscopio. Además, cuando se pasa un ureteroscopio hacia arriba por el uréter sobre un alambre guía en un paciente supino, se deben visualizar las tres trayectorias dimensionales del uréter cursando de manera lateral y descendente por la pared vesical, luego por medio de un giro hacia arriba (anteriormente) y medialmente al guiar al instrumento fuera de la pelvis y sobre las venas ilíacas. En este punto, las pulsaciones de la arteria ilíaca común se transmiten claramente al uréter. En ocasiones, puede ser difícil dar paso a un ureteroscopio semirrígido sobre la cresta ilíaca, especialmente en el caso de los pacientes masculinos. Ésta es una causa potencial de lesiones del uréter y/o daño del endoscopio y puede evitarse mediante el uso de un endoscopio flexible. De manera alternativa, tal angulación puede modificarse en el hombre por medio del empuje manual del ligamento suspensor de falo posterior, contribuyendo a la alineación de la uretra hacia el orificio ureteral.
El uréter superior puede, en ocasiones, tomar la forma de un gancho J, dificultando el paso de un alambre guía. Esto es muy común en pacientes mayores con riñones ptósicos. Para resolver este problema, se han descripto una gran variedad de métodos endourológicos. Sin embargo, el abordaje más simple consiste en colocar al paciente en posición empinada con la cabeza hacia abajo y manualmente empujar el riñón hacia arriba presionando a lo largo del cuadrante superior del abdomen subcostalmente (Maniobra de Mertz) (5).
Después de colocar en el punto máximo al promontorio sacro se continúa una leve trayectoria hacia abajo a lo largo del uréter medio y superior hacia la pelvis renal, que generalmente se encuentra al nivel del segundo cuerpo vertebral lumbar. Sin embargo, para atravesar la unión ureteropélvica, el endoscopio generalmente necesita ser guiado hacia un punto aún más posterior.
La trayectoria anterior-posterior del uréter, como fue descripto anteriormente, se revierte si el paciente se encuentra en posición prona durante una ureteroscopía flexible. En esta posición, es más conveniente pasar primero una funda de acceso ureteral para facilitar el paso del ureteroscopio flexible.
