Lima: progress notes: patient with astrocytoma: "Paciente se ha perdido de vista sin reevaluación de enfermedad por lo que no podemos asegurar progresión a temozolamida." AND "Se explica pronóstico de enfermedad, no se puede asegurar que exista Progresión a temozolamida por lo regular del tto, sin embargo familiar insiste en continuar dicho fármaco, refiere que traerá feches durante las cuales tomó el medicamento." Many thanks for an explanation of "progresión a temozolamida"...

I have seen this expression many times and that why I insist with the phrase I proposed. Once again, I think the source text is pretty dodgy and words like "regular" instead of "irregular" tell me that this is one of those cases where the best thing you can do is translating based strictly on what you see and making notes to the client saying that it doesn't make much sense but as translators we can't imagine how it should read.
In any case, could you please share the answer you finally chose? It would be very interesting to see how you or the client solved this riddle.

I think the first quoted sentence may mean "we cannot be certain of his reponse to temozolamide," and the second means "we cannot be certain that there has been a good response [i.e., progress] to temozolamide due to the irregularity of treatment." LJ

But surely the meaning is not the same, brainfloss... a patient cannot progress to temozolamide, in the sense you described, if he's already taking it. I'm thinking the sense of "progresión a temozolomida" must be something like "continuation of..." or - as Libe suggested - that the doctors cannot assure that patient will progress on temozolomide (or assure that it is having the desired effect) if he takes it so irregularly and doesn't come in for follow-ups. In any case, I don't think it can be "progress to" because he's already been taking it. LJ

Definitely! I did forget to mention that, it should have been "no se puede asegurar que exista "progresión a temozolamida por lo IRREGULAR del tto" from the beginning. Well, in my eyes the meaning is still the same, my answer doesn't change though the sentences read weird.

shedding light... Context I should have mentioned for the sentences I quoted: pt was somehow getting and taking the Temodal even though not coming in for checkups. I think there's a transcription error in the one sentence -- I think it should say "lo irregular del tto" because the problem was that he was taking the drug but they didn't know exactly when (so his mother was going to provide dates). Given this, any new thoughts on the meaning of "no se puede asegurar que exista..." ? Thanks.

Cuidado El original aunque no sea de la mejor calidad claramente dice "progresión A temozolamida" y no "progresión CON temozolamida" lo cual resulta en una enorme diferencia ya que evidentemente habla del tratamiento y no de la evolución clínica del paciente.

Los astrocitomas son un grupo de neoplasias intracraneales primarios del sistema nervioso central que aparecen en el parenquima cerebral y rara vez produce metástasis a otros tejidos. El tratamiento de los gliomas es únicamente paliativo y sólo consigue alargar algunos meses la supervivencia. La temozolamida ha sido aprobada en la Unión Europea para el tratamiento de pacientes con glioma maligno, La temozolamida es un agente alquilante quyo mecanismo de acción consiste en inhibir la replicación del DNA.Se ha demostrado que el fármaco es activo frente a los gliomas malignos humanos cuando se administra por vía oral a la dosis de 200 mg/m2 una vez

En tus notas eso de que "el paciente se ha perdido de vista sin reevaluación de enfermedad" hace pensar que no ha vuelto por la consulta médica y no pueden evaluar la evolución del tratamiento (progresión) con la temozolamida y que hay un familiar que quiere que siga haciendo el tratamiento ¿por qué no aparece el paciente?

Hello Lesley:
To answer your question "progresión a temozolamida" means that you have a patient currently receiving a specific treatment for a particular condition, and at some point in the future temozolomide will be added to the therapy usually because any other medication that they have been using did not help as expected to control the condition. It is almost like moving from tier 1 to tier 2 when temozolamida is going to replace the other drug. As you see it's easier to explain the meaning than trying to come up with a short sentece to convey the idea, sorta. I hope this helps
