period/time coursing with/suffering from disease

Explanation:
time suffering from disease/UTI.
period coursing with disease/UTI.

"tiempo de evolución del padecimiento" [or simply "tiempo padeciendo"] = for how long the patient had or has had the UTI.
This is the key, we don't from the Spanish if it refers to a completed period (because the UTI has been resolved) or an ongoing period (patient still has the UTI).
The use of "time suffering from UTI", or similar, covers both eventualities.

agree  Wyman Borts Jr: But remember in English there is a nuanced distinction between "progression" ("continuing to grow/spread) and "duration" (may or many not be growing or continuing to spread), both for which "evolución" is used in Spanish.

disease duration [RUTI duration]

Explanation:
I'd say "disease duration" for "tiempo de evolución". In your specific case, RUTI duration (recurrent urinary tract infection duration; period of time in which the patient suffered or has been suffering recurrent infections).

Participants reported an average of 52 lifetime UTIs and an average RUTI duration of nine years.

-> Thanks a lot, Wyman. And yes, I am aware of that. The original might well refer to progression here, but lacking more specific context "duration" sounds like a safer and quite reasonable option -I believe.

neutral  Neil Ashby: The problem with using "time since onset" is that we are assuming the UTI is still coursing// I don't doubt it, it's a standard phrase, but that's not 100% what the source says; that's the problem. As I said before, it involves an assumption.

-> ~Thing is that I can find loads of examples for time since onset:)

agree  Wyman Borts Jr: Neither Carina nor Chema are wrong, but I like this one best. In response to Neil's comment, remember that it's not a UTI but Recurring UTIs (ITUR is infección del tracto urinario recurrente (o de repetición)) , which is an ongoing issue.
