Spanish to English Medical Medical (general) Cataracts
In a medical history report. The full entry is "CIE-9 XXX.XX HEMORRAGIA VITREA+CATARATA OD (H.POLV)" I have the rest up to OD (right eye) but can't find "H.POLV.

agree asking the customer I agree with Stephen to ask the client. We merely looked at what the CIE-9 code says about cataracts but the only thing that would make sense would be 'hypermature'. We could not find anything for POLV except for polar under V classification.

Sí, la referencia a un hospital (Hospital Polv...) donde se realiza la intervención por hemorragia más catarata parece la opción más razonable.

Otra -mucho menos probable- sería alusión a "humor vítreo con imagen de polvo de tabaco", pero ciertamente nunca he visto este acrónimo y parece una extraña forma de expresarlo.

Ask the customer? Maybe this is the name, abbreviated, of the hospital where the cataract surgery was done.

El Hospital de Alta Resolución El Toyo dispone de la denominada Unidad Polivalente. Se trata de una alternativa a la hospitalización convencional, en la que se desarrolla la atención a pacientes en régimen de hospital de día médico, hospital de día quirúrgico, observación de urgencias y hospitalización de corta estancia.

La Unidad Polivalente es un mecanismo de soporte estructural y funcional. Su objetivo es lograr una gestión eficiente y flexible de los recursos destinados a la atención de los usuarios. En el caso de El Toyo, se cuenta con 22 módulos, de uso individual, además de un área central con 10 puestos de sillones, usados como soporte para la actividad quirúrgica. Yeah, it was just an idea if it was on a different line or something or a hashed up PDF conversion...you never know.
agree Samuel Sebastian Holden Bramah : In this case I think it means where the diagnosis was performed, since if that hospital is not the "Hospital Polivalente" [which does exist] then it could indicate that they don't have the full history of that diagnosis or action taken.
