Es una planilla en la cual el operario anota las observaciones pertinentes para el procedimiento productivo que ha realizado.
Esta planilla se supone que se debe llenar rápidamente, en no más de 5 minutos. Normalmente las preguntas ya están redactadas y solo hay que ingresar SI//NO y si es necesario algún comentario.
La planilla la recoge el supervisor que hace un relevamiento primario para verificar si se ha apartado del procedimiento.
