This Policy to which this Endorsement is attached and made a part thereof, has been made and accepted by, between, and among all the parties and is thereby made subject to the following provisions and stipulations:

Hola Eugenio. Creo que en este caso es indispensable más contexto. Además, el término respecto al cual pides ayuda no es el mismo que ofreces en contexto. Más aún, parecen discrepar (el término "subscription" no figura en la frase en contexto). Mi pregunta es: ¿Cuál es el término exacto que tienes que traducir?.
