disagree  Ildiko Santana: Witness Statement (UK) / Witness Deposition (US) / Tanúvallomás (HU) is a statement summarising the oral evidence that a witness will give at trial. That is not the term in question, "Ténytanúsító jegyzőkönyv" (which I believe is "Certificate of Facts").
