Law/Patents - Medical: Dentistry / Health Hazard Evaluations
English term or phrase: vigilance report
All,

I am translating a document relating to Health Hazard Evaluations of medical devices.
The document is a questionnaire to be filled in case of incidents.
There are the following questions:

" - Adverse events, complaints and problems or incidents that may be related to the device defect
- Number of Complaints
- Vigilance Reports
- Injuries Reported"

There are enough Google hits to see what a vigilance report is, but how could I express it in German?

Thank you.

Peter
