MUCH OF the world’s hope for fending off the coronavirus depends on the vaccine developed by AstraZeneca and the University of Oxford known as Covishield. This two-dose vaccine is based on a traditional viral vector, can be stored at normal refrigerator temperatures and is inexpensive compared with the others. It serves as a mainstay of the Covax Facility, an international effort to help vaccinate the poorest populations. But recent events have highlighted the fragility of the campaign to vaccinate the world.
Although the United States is making headway, a wave of infections is sweeping Central Europe, setting horrific daily records in Brazil and suddenly taking off again in populous India. The vaccine campaign in Europe has suffered supply interruptions and worries about side effects from the AstraZeneca shot, and now the deteriorating situation in India threatens to delay export and delivery of the vaccine to millions of people in poorer countries because India needs the doses at home.
India’s daily case count has exploded. In January, daily new cases dropped below 10,000, but on March 24 largest producers of the AstraZeneca vaccine, was making 2.4 million doses every day and exported more than 60 million doses to more than 70 countries. But in recent weeks, as the government has struggled to cope with the changed landscape at home, it has sharply curtailed the outflow.
This has crimped the Covax Facility. So far, Covax has been supplied with 28 million AstraZeneca doses, and it expected an additional 40 million in March and up to 50 million doses in April. Now it has notified recipients of delays because of the India crisis; some nations that had expected first deliveries in March may not see them until April. On Friday, World Health Organization Director-General Tedros Adhanom Ghebreyesus appealed to other nations for urgent donations of 10 million doses for the Covax Facility. “Covax is ready to deliver but we can’t deliver vaccines we don’t have,” he said.
India appeared to have made progress against the pandemic in January, when tests showed that a large proportion of people, including in New Delhi, had already been exposed and had antibodies to fight the virus. But infections began to climb last month, particularly in the western state of Maharashtra, home to Mumbai. Researchers say the new surge is driven in part by a decision to lift restrictions when cases declined.
The AstraZeneca vaccine has suffered a difficult rollout, plagued by reports of rare blood-clot side effects that shook confidence in Europe, and by a challenge from an independent regulatory panel in the United States to the company’s clinical trial data. Neither should obscure the fact that this is a safe, efficacious vaccine that will prevent hospitalization and death for millions of people, once they can get it.

إن أكثر آمال العالم لمكافحة فيروس كورونا معلقة على لقاح طورته شركة آسترازينيكا وجامعة أوكسفورد، يسمى بـ كوفيشيلد. وهذا اللقاح ذو الجرعتين مبني على ناقل فيروسي تقليدي ويمكن ان يتم وضعه في درجة الحرارة العادية للثلاجات ورخيص بمقارنة مع اللقاحات الأخرى. وهو يشكل عمودا فقريا لمبادرة عالمية تدعى/ مبادرة توفير لقاحات الكوفيد-19 للجميع على مستوى العالم وذلك لتطعيم السكان الأكثر فقرا ولكن الأحداث الأخيرة كشفت النقاب عن هشاشة حملة تطعيم سكان العالم. ولو أن الولايات المتحدة تتقدم الى الأمام ولكن موجة الاصابات بفيروس كورونا تكتسح أوربا الوسطى حيث تسجل البرازيل ازديادا مرعبا في الإصابات بالكورونا وبدأت الإصابات تزداد في الهند المزدحمة بالسكان. وقد عانت حملة التطعيم في أوربا من توقف إمدادات اللقاحات والقلق من التأثيرات الجانبية النابعة من جرعة لقاح أسترازينيكا والآن يهدد الوضع المتدهور في الهند بالتأخير في تصدير وتسليم اللقاح الى ملايين الناس في بلدان أفقر وذلك لأن الهند تحتاج الى جرعاته في داخلها.
تفجر عدد حالات الإصابة اليومية بفيروس كورونا في الهند ففي شهر يناير كانت انخفضت حالات الإصابة بفيروس كورونا اليومية الى أدنى من 10000 ألف اصابة ولكن في 24 من شهر مارس، قد سجلت الهند 53476 حالة الإصابة بفيروس كورونا، العدد الأعلى منذ شهر اكتوبر. ومعهد سيروم الهندي، الواحد من أكبر منتجي لقاح استرازينيكا في العالم، كان ينتج 2.4 مليون جرعة للقاح كل يوم وقام بتصدير أكثر من 60 مليون جرعة إلى أكر من 70 بلدا من بلدان العالم. ولكن في الأسابيع الأخيرة، بما أن حكومة الهند عانت من الوضع المتدهور لإصابة السكان بفيروس كورونا فإنها قللت بشدة تصدير اللقاحات الى الخارج.
قد حال دون تسهيل مبادرة كوفاكس وحتى الحين تم تزويد مبادرة كوفاكس بـ 28 مليون جرعة للقاح الذي طورته شركة آسترازينيكا والتي توقعت الحصول على الإمدادات الإضافية بقدر 40 مليون جرعة في شهر مارس و حوالي 50 مليون جرعة في شهر ابريل ولكنها أبلغت المتلقين او المستلمين عن التأخير بسبب الأزمة الهندية، فإن بعض البلدان التي كانت تنتظر لاستلام أول دفعة في شهر مارس، قد تضطر على الانتظار لها الى شهر ابريل. ويوم الجمعة قد أهاب المدير العام لمنظمة الصحة العالمية السيد تيدروس آذانوم غيبرييسوس بالبلدان الأخرى لتبرع 10 ملايين جرعة بشكل عاجل لمرفق كوفاكس. وكوفاكس مستعدة للتوصيل ولكن لا يمكن لنا ان نقوم بتوصيل اللقاحات التي لا تتوفر لدينا.
بدا أن الهند كانت تقدمت في مكافحة الوباء في شهر يناير حينما أظهرت الفحوص أن نسبة كبيرة من الناس في أرجاء البلاد بما فيها دهلي، كانوا اصيبوا بها مما ولدت فيهم أجساما مقاومة لمكافحة الفيروس ولكن بدأت حالات الإٍصابة بالفيروس تزداد في الشهر الماضي وبصفة خاصة في إقليم مهاراشترا في غرب البلاد والتي تكون عاصمتها هي مدينة مومباي. وقال الباحثون إن الموجة الجديدة للإصابة بالفيروس نتجت جزئيا عن قرار رفع القيود حينما انخفضت حالات الاصابة بالوباء.
إن لقاح شركة استرازينكا قد عانى من عملية البيع الصعبة وذلك بسبب التقارير الواردة عن التأثيرات الجانبية التي تتسبب في التجلط الدموي/التخثر الدموي/الجلطة الدموية مما زعزع ثقة الناس في اوربا وأيضا بسبب التحدي الذي شكلته لجنة الرقابة المستقلة في الولايات المتحدة لبيانات التجارب الطبية للشركة ولم ينكر أحد أن هذا اللقاح آمن وفعال يمنع ملايين الناس من دخول المستشفى للمعالجة والموت، لو يتلقون ذلك.
