Source text - Spanish El autor llama la atención sobre la necesidad de un manejo cuidadoso de la vía área, aunque en nuestra experiencia este no ha sido un problema especialmente complejo. En cuanto a la pérdida sanguínea, también subrayada, no hemos tenido que transfundir a ningún niño durante los procedimientos de remodelación endoscópicos (6) y solo 2 precisaron hemoderivados en el postoperatorio inmediato. Sin embargo, nosotros consideramos que la colocación del paciente en la mesa quirúrgica si es un punto de máximo interés. En nuestra experiencia, nuestra principal preocupación es una óptima colocación del paciente en la mesa quirúrgica. (Figura 1) en función del abordaje y patología a tratar. Es clave una estrecha colaboración entre neurocirujanos y anestesiólogos que permita la máxima exposición craneal con el menor compromiso del drenaje venoso cerebral posible, en un ambiente tecnológico cada vez más complejo. Esta colaboración facilita y acorta el tiempo quirúrgico, disminuye el sangrado desde los senos durales y ofrece la máxima seguridad en la vía área. Al igual que Amr E. Abouleish, consideramos que este tipo de cirugía cada vez más frecuente en edades tempranas es un reto que requiere una especial consideración y planificación para los anestesiólogos.
Translation - English The author draws attention to challenges in airway management, although in our experience we have not found this a particularly complex problem. With regard to blood loss, also mentioned, we have not had to transfuse any patient during endoscopic remodeling procedures (6) and only 2 required blood derivatives in the immediate post-operative period. However, we do consider that patient positioning on the surgical table is of the utmost importance. In our experience, our main concern is optimum positioning of the patient on the surgical table (Fig. 1), depending on the pathology to be treated and the approach to be taken. Close collaboration between neurosurgeons and anesthesiologists is of primary importance to give maximum cranial exposure combined with the least possible compromising of venous drainage, in an increasingly complex technical environment. This collaboration facilitates access, reduces surgery time, reduces bleeding from the dural sinus and offers maximum airway security. Like Dr Abouleish, we consider that the increasing frequency of surgery of this type at earlier ages is a challenge that requires particular consideration and planning by anesthesiologists.
