You usually say "one up the spout" or "weapons hot". It means you have caused the rifle mechanism to pick a cartridge from the magazine and feed it into the chamber. Remember that the cartridge is an assembly consisting of a) casing with primer b)propellant (powder) and c) bullet. Upon hitting the primer with the firing pin the powder is lit generating gases that drive the bullet out of the barrel. The empty case is then ejected from the weapon.

Habitualmente se trata de una redundancia: al decir que un arma está cargada, eso indica que su sistema de disparo ha sido alimentado con un cartucho y está lista para disparar. El "bala en boca" es simplemente un énfasis localista, puesto que en ocasiones se habla de colocar el cargador lleno pero no alimentar la recámara. En práctica de tiro, la indicación "¡Cargue!" indica la colocación del almacén de munición (cargador), colocar munición en los alveolos (revólveres) o cartuchos en recámara (escopetas, armas monotiro), accionar el mecanismo de disparo (cerrojo, corredera, cierre, etc.), con lo que el arma queda lista para hacer fuego a continuación. El hecho que se de la orden de colocar o no seguro es independiente de la carga, y depende de muchos factores externos que afectan el uso inmediato, por ejemplo. ¡Suerte!

Porto a diario una CZ83 (aunque mi nivel de riesgo es mínimo), el arma tiene solo seguro para simple acción, así que no lo uso, la porto con bala en boca y sin seguro, martillo desmontado (condición 2), creo que "es lo más seguro para mi", sin bala en boca "es más seguro para el delincuente", considero que ese modelo en particular tiene 2 formas de dispararse.
