The present invention relates generally to the field of the production and selection of binding and catalytic polypeptides by the methods of molecular biology, using both combinatorial chemistry and recombinant DNA. The invention specifically relates to the generation of both nucleic acid and polypeptide libraries derived therefrom encoding the molecular scaffolding of Fibronectin Type III (Fn3) modified in one or more of its loop regions. The invention also relates to the "artificial mini-antibodies" or "monobodies", i.e., the polypeptides comprising an Fn3 scaffold onto which loop regions capable of binding to a variety of different molecular structures (such as antibody binding sites) have been grafted.
BACKGROUND OF THE INVENTION
Antibody Structure
A standard antibody (Ab) is a tetrameric structure consisting of two identical immunoglobulin (Ig) heavy chains and two identical light chains. The heavy and light chains of an Ab consist of different domains. Each light chain has one variable domain (VL) and one constant domain (CL), while each heavy chain has one variable domain (VH) and three or four constant domains (CH) (Alzari et al., 1988). Each domain, consisting of ~110 amino acid residues, is folded into a characteristic β-sandwich structure formed from two β-sheets packed against each other, the immunoglobulin fold. The VH and VL domains each have three complementarity determining regions (CDR1 3) that are loops, or turns, connecting β-strands at one end of the domains (Figures 1A, C). The variable regions of both the light and heavy chains generally contribute to antigen specificity, although the contribution of the individual chains to specificity is not always equal. Antibody molecules have evolved to bind to a large number of molecules by using six randomized loops (CDRs). However, the size of the antibodies and the complexity of six loops represents a major design hurdle if the end result is to be a relatively small peptide ligand.

[0001] The present invention relates to compositions and their use for reducing the immune response in an animal,
such as a human or another mammal.
[0002] In one embodiment, the invention relates to compositions and their use for reducing the adhesion of dendritic
cells to T cells.
[0003] More specifically, this embodiment of the invention relates to compositions and their use for reducing the
adhesion of C- type lectin receptors on the surface of dendritic cells to the ICAM- receptors on the surface of T cells. By
modulating this adhesion, both dendritic cell- T cell interactions, such as cluster formation and antigen presentation, as
well as for instance primary T cell responses dependant thereon, can be influenced, resulting in a modulation of the
immune response.
[0004] The compositions of the invention can therefore be used to alter immune responses to specific antigens as
well as immune responses caused by disorders of the immune system, such as may occur in auto- immune diseases or
in allergy.
[0005] In a further embodiment, the method of the invention can further be used in the treatment of HIV -infections
and similar disorders of the immune system, as well as to modulate the immune response to grafts or after transplant.
[0006] The invention is based on the surprising discovery that the adhesion of dendritic cells to T cells is mediated by
a C- type lectin receptor on the surface of the dendritic cells. It has also been found that this C- type lectin binds to the
ICAM receptors on the surface of T cells. With the term "ICAM receptor (s)" both the ICAM- 2 and ICAM- 3 receptor are
meant, and in particular the ICAM- 3 receptor.
[0007] The invention is further based on the finding that the inhibition of this C- type lectin receptor on the dendritic
cells by specific antibodies directed against the C- type lectin receptor, can modulate, and more specifically reduce, the
adhesion of T cells to dendritic cells, and can thereby influence the immune response, in particular the initial stages of
the immune response.
[0008] WO 96/23882 describes a murine and human receptor with C- type lectins domains that is abundantly expressed
on the surface of dendritic cells and thymic epithelial cells. The murine receptor -named "DEC- 205"- is described as a
295 kDa protein with an isoelectric point of about 7,5 that contains 10 C- type lectin domains and that is homologous to
the macrophage mannose receptor (MMR).
[0009] WO 96/23882 further describes monoclonal and polyclonal antibodies against DEC- 205. However, these antibodies
were not able to block dendritic cell function. In particular, monoclonal and polyclonal anti- DEC- 205 antibodies
were unable to inhibit the interaction between dendritic cells and helper T cells, both in vitro (as determined by the
inability of anti- DEC- 205 to inhibit allogenic T cell proliferation in a one way mixed leucocyte reaction) and in vivo (as
determined by the inability of anti- DEC- 205 to inhibit an in vivo response, i.e. in a local graft- versus- host (GVH) reaction).
These results suggest that the DEC- 205 receptor is not involved in dendritic cell- T cell interaction (i.e. adhesion) and
that the anti- DEC- 205 antibodies cannot be used to modulate the immune response.
[0010] Curtis et al.. in Proc.Natl.Acad.Sci. USA, 89 (1992), p. 8356-8360, as well as in WO 93/O 1820. describe a
non- CD4 gp 120 receptor isolated and cloned from human placenta tissue. This gp 120 receptor is expressed on
mammalian cells which do not exhibit high levels of CD4, such as placenta, skeleton muscle, brain, neural and mucosal
cells, as well as other tissues and cells including colon, thymus, heart, T cells, B cells and macrophages (but not in the
liver or the kidney). The amino acid sequence of the C- type lectin gp120 receptor disclosed in SEQ ID’s no. 1 and 2 of
WO 93/01820 has a high degree of sequence homology (>98%) with the C- type lectins that are now found to be present
on dendritic cells.
[0011] Curtis and WO 93/01820 further discuss the role this C- type lectin receptor plays in the infection of the aforementioned
cells/ tissues with HIV, i.e. by binding the major HIV envelope glycoprotein gp 120 prior to internalization of
the virion into the cell. It was found that inhibition of the C- type lectin gp120 receptor can reduce or inhibit HIV infection
of these cells/ tissues. As suitable inhibitors, WO 93/01820 discloses mannose carbohydrates, fucose carbohydrates,
plant lectins such as concanavalin A, specific antibiotics such as pradimicin A, and sugars such as N- acetyl- D- glucosamine
and galactose (which however are described as less potent). These compounds and compositions containing
them are used either in vitro or in vivo to inhibit the binding of HIV to the cell surface.
[0012] WO 93/01820 further discloses that binding of HIV to COS- 7 cells can be inhibited by pre- incubation of gp 120
with an anti- gp 120 monoclonal antibody (named "antibody 110.1 "). However, this antibody is not directed against the
C- type lectins, but against the gp120 protein.
[0013] However, neither Curtis nor WO 93/01820 mentions or suggests the presence of such a C- type lectin on
dendritic cells, nor do these references mention or suggest their role in dendritic cell - T cell interaction during the initial
stages of an immune response.
[0014] WO 95/32734 describes FcgRII (CD32) bridging (or crosslinking) compositions and their use in modulating the
immune response to specific antigens. This reference is based upon the finding that the bridging of FcgRII (CD32)
molecules on antigen presenting cells (APCs) impaires the expression of the essential co- stimulatory molecules B7-1/2

(i.e. prevents their up- regulation) and causes thereby impaires the expression of (i.e. causes the down- modulation of)
the adhesion molecule ICAM- 3, with the functional consequence of an impaired capacity of the monocytes to co- stimulate
the activation of antigen- specific T cells (i.e. resulting in the modulation of antigen- specific T cell unresponsiveness).
The bridging agent is chosen from aggregrated human IgG molecules or Fc- fragments thereof; bi- or multivalent monoclonal
antibodies to FcgRII or fragments thereof, or a fusion of two or more humane IgG Fc parts.
[0015] WO 95/32734 is therefore directed towards modulating (i.e. inhibiting) the co- stimulation signal required for T
cell activation (i.e. besides the primary signal of TcR/CD3 interaction), in particular to induce proliferation and maturation
into effector cells. WO 95/32734 is not directed towards modulating dendritic cell - T cell adhesion, nor does it disclose
or suggest either the presence of C- type lectins on (the surface of) dendritic cells or their interaction with the ICAM- 3
receptors on T cells.
[0016] WO 98/02456 discloses a group II human C- type lectin isolated from a stimulated human macrophage library.
WO 98/49306 discloses a group IV C- type lectin present in human pancreatitis- associated protein ("PAP"). WO 98/41633
discloses a group V human C- type lectin isolated from a human tumor clone.

these C- type lectins.
[0018] However, none of these references relates to C- type lectins on dendritic cells; the C- type lectins disclosed in
these references differ from the C- type lectins described therein in origin, in biological function, and in structure.
[0019] Dendritic cells (DC) are professional antigen- presenting cells that capture antigens in the peripheral tissues
and migrate via lymph or blood to the T cell area of draining lymph nodes and spleen. Here they present processed
antigens to naive T cells, initiating antigen- specific primary T cell responses.
[0020] Due to their position in the body surface as immunosurveillance cells, it is likely that DC are the first cells
infected with HIV- 1 after mucosal exposure and are therefore implicated to play an important role in the immunopathogenesis
of HIV. It is now generally believed that HIV converts the normal trafficking process of DC to gain entry into
lymph nodes and access to CD4’’ T cells, as was demonstrated in vivo using primary simian immunodeficiency virus
infection of macaque as a model system (Spira et al., 1996) (Joag et al., 1997). Productive infection of DC with HIV- 1
has been reported by several investigators (Granelli- Piperno et al., J Virol 72 (4), 2733-7, 1998) (Blauvelt et al., Nat Med
3 (12), 1369-75. 1997.) and substantial evidence indicates that DC pulsed with HIV- 1 in vitro can induce a vigorous
infection when co- cultured with T cells (Cameron et al., Science 257 (5068), 383-7, 1992). Although it is still unclear
whether a similar scenario occurs in HIV infected individuals, HIV- 1 transmission from DC to T cells could contribute to
the CD4
T cell depletion observed in AIDS. Studying HIV- DC interactions should contribute to the understanding of
early events of HIV infection and will hopefully lead to strategies aimed at blocking early events in transmission. For a
further discussion, reference is also made to WO 95/32734 and WO 96/23882.
[0021] DC are unique in their ability to interact with and activate resting T cells. However, prior to the present invention,
it was largely unknown how DC- T cell contact is initiated and regulated. Herein, the role of ICAM- 3 in DC- T cell interactions
is investigated. It is demonstrated that although DC strongly adhere to ICAM- 3, this adhesion is not mediated by LFA1,
aD or any other integrin. In the search for this novel ICAM- 3 receptor on DC a C- type lectin receptor was cloned,
designated DC- SIGN, that is preferentially expressed by DC. Besides its prominent role in DC- T cell clustering and
initiation of T cell responses we discovered that DC- SIGN is a major HIV- 1 receptor involved in infection of DC and
subsequent HIV- 1 transmission to T cells. Thus HIV- 1 and resting T cells exploit a similar highly expressed receptor to
interact with DC.
[0022] In a first aspect, the invention relates to the use of an antibody that binds to the receptor designated DC- SIGN,
on the surface of a dendritic cell and represented as seq ID NO: 2 or a natural variant or fragment thereof, in the preparation
of a medicament for reducing an immune response in an animal, wherein said antibody inhibits the interaction between
a dendritic cell and a T cell.
[0023] In particular, the antibody may reduce the adhesion between a dendritic cell and a T cell.
[0024] Especially, the antibody may reduce the adhesion between DC- SIGN on the surface of a dendritic cell and an
ICAM receptor on the surface of a T cell.
[0025] The amino acid sequence of one C- type lectin designated DC- SIGN that was found to be involved in the binding
of the dendritic cells to the T- cells is shown in SEQ ID no. 1 and Figure 9. This C- type lection receptor is essentially
similar to to the C- type lectin gp 120 receptor described by Curtis et al. in Proc.Natl.Acad.Sci. USA, 89 (1992), p.
8356-8360 and in WO 93/01820. In particular, it has a high degree of homology (< 98%) to the amino acid sequence
given in SEQ ID no. 1 of WO 93/01820. It is a group II C- type lectin of 404 amino acids; with an apparant Mr of about 44
kDa; and with a first domain (Met 1 to Ala 76) comprising a cytoplasmic amino terminus, a second domain (Ile 77 to Val
249) comprising tandem repeats, and a third domain (Cys 253 to Ala 404) with a high degree of homology to other known
C- type lectins which are type II membrane proteins. Further characterisation is given below.
[0026] In the invention, this C- type lectin on dendritic cells was found to bind to ICAM receptors on the surface of T -cells.
[0027] Accordingly, the compositions of the invention can be used to modulate (i.e. alter and/or modify), and more
specifically inhibit (i.e. reduce and/or down- tune), the interaction (s) between dendritic cells and T cells.

[0028] Such interactions include the adhesion of T- cells to dendritic cells, for instance in dendritic cell - T cell clustering,
T- cell activation and further include all interactions that rely on contact between dendritic cells and T- cells, by which is
meant either direct cell- to- cell contact or close proximity of dendritic cells and T cells.
[0029] Such further interactions include, but are not limited to, processes involved in generating an immune response,
in particular during the initial stages of such a response, such as primary sensitation/ activation of T- lymphocytes, (i.e.
presentation of antigen and/or MHC- bound peptides to T- cells) and co- stimulation of T cells; as well as processes such
as chemical signalling, endocytosis and trans- epithelial transport. For a discussion of dendritic cell- T cell interactions in
general, all of which may be influenced by the compositions of the invention, reference is made to the discussion below
as well as to WO 95/32734 and WO 96/23882.
[0030] The compositions of the invention can therefore be used to influence the immunomodulatory ability of dendritic
cells; to modulate, and in particular reduce, dendritic cell- mediated (primary) T cell responses, and/or generally to
influence, and in particular inhibit, the immune system.
[0031] Some specific applications include preventing or inhibiting immune responses to specific antigens; inducing
tolerance; immunotherapy; immunosuppression, for instance to prevent transplant rejection; the treatment of auto- immune
diseases such as thyroiditis, rheumatoid arthritis, systemic lupus erythematosus (SLE), multiple sclerosis and
auto- immune diabetes; and the prevention or treatment of allergies.
[0032] The compositions of the invention may also modulate the activation of other receptors on T cells which are
dependant upon the adhesion or close proximity of dendritic cells to T cells. Furthermore, the finding of the invention
that a C- type lectin on dendritic cells binds to the ICAM receptors on T cells may open up new strategies or possibilities
for influencing the interaction between dendritic cells and T cells, and thereby for modulating the immune system in
general.
[0033] Furthermore, the compositions of the invention can be used to prevent or reduce the transfer of matter from
dendritic cells to T cells, such as chemicals, signalling factors such as chemokines and/or interleukines, etc., and in
particular of viral particles such as HIV. In this way, by using the compositions of the invention, not only can the initial
adhesion of NV to dendritic cells be inhibited, but also the spread of HIV infection from dendritic cells to T cells.
[0034] This finding is of particular importance as it is thought that dendritic cells may serve as a "reservoir" of HIV, in
particular during the initial stages of MV infection. The compositions of the invention can therefore not only be used to
prevent HIV infection of dendritic cells, but also to reduce the spread of HIV infection to T cells after the dendritic cells
have been infected, thereby slowing down the disease process.
[0035] Also, it is known that activation of T cells -i.e. in the lymph glands- plays an important role in the development
of AIDS in a HIV- infected patient. It is believed that the compositions of the invention may be used to prevent, inhibit or
at least delay said T- cell activation and thereby slow the onset and/or the progress of the disease.
[0036] Therefore, in a further aspect, the invention further relates to the use of an antibody that binds to the receptor
designated DC- SIGN, on the surface of a dendritic cell and represented as seq ID NO: 2 or a natural variant or fragment
thereof, in the preparation of a medicament for inhibiting the adhesion of HIV surface protein (i.e. gp120) to the surface
of a dendritic cell and thereby the entry of HIV into said dendritic cell.
[0037] Compounds that can be used as inhibitors for the C- type lectins are known per se, including those described
in WO 93/01820 as mentioned above.
[0038] In general, these are compounds that can bind or adhere to (e.g. in a reversible manner), or that can serve as
a ligand for, the C- type lectins, in particular the C- type lectin of SEQ ID no. 1/Figure 9 or natural variants or equivalents
thereof. Examples are mannose carbohydrates such as mannan and D- mannose; fucose carbohydrates such as Lfucose;
plant lectins such as concanavalin A; antibiotics such as pradimicin A; sugars such as N- acetyl- D- glucosamine
and galactose (which however are described as less potent); as well as suitable peptidomimetic compounds and small
drug molecules, which can for instance be identified using phage display techniques. Furthermore, proteins such as
gp120 and analogs or fragments thereof or similar proteins with binding capacity to C- type lectins on dendritic cells may
be used, as well as isolated ICAM- receptors and analogs thereof, including part or fragments thereof. Such parts or
fragments should then still be such that they can bind to the C- type lectins on the surface of dendritic cells.
[0039] However, the use of carbohydrates is usually less desired from a therapeutic point of view, as such they can
be rapidly metabolized in vivo. Also, the use of plant lectins such as concanavalin A and pradimicin antibiotics can have
disadvantages in a therapeutic setting, in particular when treating patients with auto- immune disorders and/or HNinfections.
[0040] One or more physiological tolerable and/or pharmaceutically acceptable compounds may be used, such as
defined in WO 93/01820. For instance, the use of gp120 or derivatives thereof may cause undesired side effects, in
particular on the nervous system (vide WO 93/01820).
[0041] In contrast, according to the invention, an antibody directed against DC- SIGN as present/ expressed on the
surface of a dendritic cell, or a part, Segment or epitope thereof, is used. As used herein, the term antibodies includes
inter alia polyclonal, monoclonal, chimeric and single chain antibodies, as well as fragments (Fab, Fv, Fa) and an Fab

[0042] Such antibodies against DC- SIGN of the invention can be obtained as described hereinbelow or in any other
manner known per se, such as those described in WO 95/32734, WO 96/23882, WO 98102456, WO 98/41633 and/or
WO 98/49306.
[0043] For instance, polyclonal antibodies can be obtained by immunizing a suitable host such as a goats rabbit,
sheep, rat, pig or mouse with DC- SIGN or an immunogenic portion, fragment or fusion thereof, optionally with the use
of an immunogenic carrier (such as bovine serum albumin or keyhole limpet hemocyanin) and/or an adjuvant such as
Freund’s, saponin, ISCOM’s, aluminium hydroxide or a similar mineral gel, or keyhole limpet hemocyanin or a similar
surface active substance. After an immuneresponse against the DC- SIGN has been raised (usually within 1-7 days),
the antibodies can be isolated from blood or serum taken from the immunized animal in a manner known per se, which
optionally may involve a step of screening for an antibody with desired properties (i.e. specificity) using known immunoassay
techniques, for which reference is againt made to for instance WO 96/23882.
[0044] Monoclonals may be produced using continuous cell lines in culture, including hybridoma and similar techniques,
again essentially as described in the above cited references.
[0045] Fab- fragments such as F (ab) 2 . Fab’ and Fab fragments may be obtained by digestion of an antibody with
pepsin or another protease, reducing disulfide- linkages and treatment with papain and a reducing agent, respectively.
Fab- expression libraries may for instance be obtained by the method of Huse et al., 1989, Science 245: 1275-1281.
[0046] Preferably, a monoclonal antibody against DC- SIGN on dendritic cells is used, more specifically against the
peptide with the amino acid sequence shown in/ encoded for by SEQ ID no’s 1 and 2 and Figure 9, or (an antigenic) part
thereof; and such monoclonals are a further aspect of the invention. Hereinbelow, the invention will be illustrated by
means of two such monoclonals, herein referred to as AZN- D1 and AZN- D2, although similar monoclonals with comparable
specificity for DC- SIGN may also be used.
[0047] In a further aspect, the invention provides a cell line such as a hybridoma that produces antibodies, preferably
monoclonal antibodies, against DC- SIGN on dendritic cells. more specifically against the peptide with the amino acid
sequence shown in/ encoded for by SEQ ID no’s 1 and 2 and Figure 9 or (an antigenic) part thereof. Hybridomas that
produce the abovementioned monoclonals AZN- 1 and AZN- 2 of the invention were deposited on April 8. 1999 with the
European Collection of Cell Cultures under (provisional) EGACC accesion numbers 990400818 and 99040819, respectively.
[0048] The invention also relates to a method for producing an antibody, preferably a monoclonal antibody, against
DC- SIGN on dendritic cells, more specifically against the peptide with the amino acid sequence shown in (or encoded
for) by SEQ ID no’s 1 and 2 and Figure 9 or (an antigenic) part thereof, said method comprising cultivating a cell or a
cell line that produces said antibody and harvesting/ isolating the antibody from the cell culture.
[0049] Neither (monoclonal) antibodies against DC- SIGN on dendritic cells, nor cells or cell lines that produce such
antibodies, have to date been described in the art, and it is envisaged that the novel antibodies of the invention will have
broad applicability (i.e. besides the pharmaceutical/ therapeutic uses disclosed herein). Some of these application -which
form yet another aspect of the invention- will be clear to the skilled person from the disclosure herein.
[0050] For instance, the antibodies of the invention can be used to detect the presence of (and thereby determine the
expression of) DC- SIGN in or on tissues or whole cells, as well as the detect the presence of DC- SIGN in other biological
samples such as cell fragments or in cell preparations. The information thus obtained can then (also) be used to determine
whether the method or compostions of the invention can be applied to such tissues or cells. The antibodies of the
invention could also be used to detect (qualitatively and/or quantitatively), isolate, purify and/or produce dendritic cells,
for instance in/ from biological samples, including biological fluids such as blood, plasma or lymph fluid; tissue samples
or cell samples such as bone marrow, skin tissue, tumor tissues, etc; or cell cultures or cultivating media.

Experimental
[0070] Dendritic cells (DC) capture antigens and migrate to secondary lymphoid tissues where they present antigens
to naive T cells. HIV- 1 subverts this unique capacity to gain access to CD4
T cells. In the invention, a DC specific Ctype
lectin was cloned, designated DC- SIGN, that not only binds to ICAM- 2 and/or ICAM- 3 with high affinity but is also
able to bind HIV- 1. Also, anti- DC- SIGN antibodies were developed that not only inhibit transient DC- T cell interactions
and DC induced T cell proliferation but also effectively inhibit HIV- 1 infection of DC. These findings not only have important
consequences for the understanding on CD4- independent HIV entry into DC but also shed new light on the role of DC-
SIGN in initiating primary immune responses.
Example 1: Adhesion of DC to ICAM- 3 is not mediated by integrins
[0071] The role of ICAM- 3 mediated adhesion in first DC- T cell contact was investigated. Exploiting a novel flowcytrometric
adhesion assay involving ICAM- 3- Fc chimera coated fluorescent beads (Geijtenbeek et al.), the capacity of DC,
resting peripheral blood lymphocytes (PBL) and monocytes to bind to this integrin ligand was tested. Immature DC,
obtained after culturing of monocytes for 7 days in the presence of IL- 4 and GM- CSF, strongly bind ICAM- 3 without prior
activation of b2 integrins (72%, Figure 1A). Figure 1 demonstrates that the adhesion of DC to ICAM- 3 is Ca 2
-dependent
and integrin- independent: in Figs. 1A, B and C one representative experiment of at least 3 is shown (SD

Descrizione
La presente invenzione riguarda composizioni e il loro uso per ridurre la risposta immune in un animale, come un essere umano o altro mammifero.
In una forma esecutiva, l'invenzione riguarda composizioni e il loro uso per ridurre l'adesione delle cellule dendritiche alle cellule T.
Più nello specifico, questa forma esecutiva dell'invenzione riguarda composizioni e il loro uso per ridurre l'adesione di recettori di lectina tipo C sulla superficie delle cellule dendritiche, ai recettori ICAM sulla superficie delle cellule T. Modulando questa adesione, è possibile influenzare sia le interazioni cellule dendritiche-cellule T, come la formazione di agglomerati e la presentazione dell'antigene, che, ad esempio, le risposte primarie delle cellule T da esse dipendenti, con il risultato di ottenere una modulazione della risposta immune.
Le composizioni dell'invenzione possono dunque essere utilizzate per alterare le risposte immuni a specifici antigeni, nonché le risposte immuni causate da disordini del sistema immune, come possono avvenire nelle malattie autoimmuni o in un'allergia.
In un'ulteriore forma esecutiva, il metodo dell'invenzione può essere ulteriormente usato nel trattamento di infezioni da HIV e di disordini simili del sistema immune, oltre che per modulare la risposta immune ad impianti o la risposta immune post-trapianto.
L'invenzione è basata sulla sorprendente scoperta che l'adesione delle cellule dendritiche alle cellule T è mediata da un recettore di lectina tipo C sulla superficie delle cellule dendritiche. È stato inoltre scoperto che questa lectina tipo C si lega ai recettori ICAM sulla superficie delle cellule T. Con il termine "recettore/i ICAM" si intendono sia il recettore ICAM-2 che il recettore ICAM-3, e in particolare il recettore ICAM-3.
L'invenzione è ulteriormente basata sulla scoperta che l'inibizione di questo recettore di lectina tipo C sulle cellule dendritiche, attraverso specifici anticorpi diretti contro il recettore di lectina tipo C, è in grado di modulare, e più specificatamente di ridurre, l'adesione delle cellule T alle cellule dendritiche, e può dunque influenzare la risposta immune, in particolare le fasi iniziali della risposta immune.
WO 96/23882 descrive un recettore murino e umano con domini di lectine tipo C che è espresso abbondantemente sulla superficie di cellule dendritiche e cellule epiteliali timiche. Il recettore murino - denominato "DEC-205" – è descritto come proteina di 295 kDa con un punto isoelettrico di circa 7,5 che contiene 10 domini di lectina di tipo C, e che risulta omologo al recettore per il mannosio dei macrofagi (MMR, macrophage mannose receptor).
WO 96/23882 inoltre descrive anticorpi monoclonali e policlonali contro DEC-205. Tuttavia, questi anticorpi non erano in grado di bloccare la funzione delle cellule dendritiche. In particolare, gli anticorpi monoclonali e policlonali anti-DEC-205 non erano in grado di inibire l’interazione tra cellule dendritiche e cellule T helper, sia in vitro (come determinato dall’incapacità di anti-DEC-205 di inibire la proliferazione di cellule T allogeniche in una reazione leucocitaria mista), sia in vivo (come determinato dall’incapacità di anti-DEC-205 di inibire una risposta in vivo, ovvero in una reazione locale impianto-contro-ospite (GVH, graft-versus-host)). Questi risultati suggeriscono che il recettore DEC-205 non è coinvolto nell’interazione cellula dendritica - cellula T (ovvero l’adesione) e che gli anticorpi anti-DEC-205 non possono essere usati per modulare la risposta immune.
Curtis et al., in Proc.Natl.Acad.Sci. USA, 89 (1992), pagg. 8356-8360, come pure in WO 93/O 1820, descrivono un recettore di gp 120 non-CD4 isolato e clonato da tessuto di placenta umana. Questo recettore di gp 120 è espresso su cellule di mammifero che non esibiscono alti livelli di CD4, come placenta, muscolo scheletrico, cervello, cellule neurali e mucose, come pure altri tessuti e cellule tra cui colon, timo, cuore, cellule T, cellule B e macrofagi (ma non nel fegato o rene). La sequenza di amminoacidi del recettore di gp120 lectina tipo C, riportata nelle SEQ ID no. 1 e 2 di WO 93/01820, presenta un alto grado di omologia di sequenza (>98%) con la lectina tipo C, ora nota per essere presente sulle cellule dendritiche.
Curtis e WO 93/01820 discutono ulteriormente il ruolo che questo recettore di lectina tipo C ricopre nell’infezione delle cellule/tessuti sopramenzionati da parte di HIV, ovvero andando a legare la glicoproteina maggiore d’involucro di HIV, gp 120, prima dell’internalizzazione del virione nella cellula. È stato trovato che l’inibizione del recettore di gp120 lectina tipo C può ridurre o inibire l’infezione di HIV di tali cellule/tessuti. Come inibitori adatti, WO 93/01820 divulga carboidrati di mannosio, carboidrati di fucosio, lectine vegetali come concanavalina A, antibiotici specifici come pradimicina A, e zuccheri come N-acetil-D-glucosammina e galattosio (che sono comunque descritti come meno potenti). Questi composti e composizioni che li contengono sono usati in vitro o in vivo per inibire il legame di HIV alla superficie della cellula.
WO 93/01820 inoltre descrive che il legame di HIV su cellule COS-7 può essere inibito mediante preincubazione di gp 120 con un anticorpo monoclonale anti-gp 120 (denominato "anticorpo 110.1 "). Questo anticorpo, tuttavia, non è diretto contro le lectine di tipo C, ma contro la proteina gp120.
Tuttavia, né Curtis né WO 93/01820 fanno menzione o suggeriscono la presenza di tale lectina tipo C sulle cellule dendritiche, e tali rimandi non fanno neppure menzione o suggeriscono il suo ruolo nell’interazione cellula dendritica–cellula T durante gli stadi iniziale di una risposta immune.
WO 95/32734 descrive composizioni con legame a ponte (o a legame crociato) di FcγRII (CD32) e il loro uso nella modulazione del sistema immune verso specifici antigeni. Questo rimando si basa sulla scoperta che il legame a ponte di molecole FcγRII (CD32) su cellule presentanti l’antigene (APC, antigen presenting cell) non permette l’espressione di molecole co-stimolatorie essenziali B7-1/2 (ovvero previene la loro sovra-regolazione) e causa quindi l’inadeguata espressione (ovvero la sotto-regolazione) della molecola di adesione ICAM-3, con la conseguenza funzionale di una compromessa capacità dei monociti di co-stimolare l’attivazione di cellule T antigene-specifiche (il che porta, in altri termini, alla modulazione di una mancata responsività delle cellule T antigene-specifiche). L’agente di legame a ponte è selezionato tra molecole IgG umane aggregate o loro frammenti Fc; anticorpi monoclonali bi- o multi-valenti per FcγRII, o loro frammenti, oppure una fusione di due o più porzioni di Fc di IgG umane.
WO 95/32734 è inoltre diretto alla modulazione (ovvero l’inibizione) del segnale di costimolazione richiesto per l’attivazione di cellule T (ovvero oltre al segnale primario dell’interazione TcR/CD3), in particolare per indurre la proliferazione e maturazione in cellule effettrici. WO 95/32734 non è diretto alla modulazione dell’adesione cellula dendritica–cellula T, né descrive o suggerisce la presenza di lectine tipo C su (sulla superficie di) cellule dendritiche o la loro interazione con recettori ICAM-3 sulle cellule T.
WO 98/02456 descrive una lectina tipo C umana di gruppo II, isolata da una libreria di macrofagi umani stimolati. WO 98/49306 descrive una lectina tipo C di gruppo IV C presente nella proteina umana associata alla pancreatite ("PAP, human pancreatitis-associated protein"). WO 98/41633 descrive una lectina tipo C umana di gruppo V, isolata da un clone di tumore umano.
WO 98/02456, WO 98/49306 e WO 98/41633 inoltre descrivono metodi per produrre anticorpi contro queste lectine tipo C.
Tuttavia, nessuno di questi rimandi fa riferimento a lectine tipo C sulle cellule dendritiche; le lectine tipo C descritte in questi rimandi differiscono dalle lectine tipo C descritte in questa sede, in termini di origine, funzione biologica e struttura.
Le cellule dendritiche (DC, dendritic cell) sono cellule presentati l’antigene professionali che catturano antigeni nei tessuti periferici e migrano attraverso la linfa o il sangue verso l’area delle cellule T nei linfonodi di drenaggio e milza. Qui presentano gli antigeni elaborati alle cellule T naïve, innescando risposte primarie antigene-specifiche delle cellule T.
A causa della loro posizione nella superficie corporea in qualità di cellule di immunosorveglianza, è probabile che le DC siano le prime cellule infettate da HIV-1 dopo l'esposizione mucosa e, quindi, che siano coinvolte in un ruolo importante nell'immunopatogenesi di HIV. Attualmente si ritiene generalmente che HIV converta il normale processo di traffico delle DC per ottenere l’ingresso nei linfonodi e accedere alle cellule T CD4'', come è stato dimostrato in vivo usando come sistema modello il virus principale dell’immunodeficienza di scimmia di macaco (Spira et al., 1996)(Joag et al., 1997). Un’infezione produttiva di DC con HIV-1 è stata riportata da numerosi ricercatori (Granelli-Piperno et al., J Virol 72(4), 2733-7, 1998) (Blauvelt et al., Nat Med 3(12), 1369-75. 1997.) e una prova fondata indica che DC stimolate in vitro con HIV-1 possono indurre una vigorosa infezione quando co-coltivate con cellule T (Cameron et al., Science 257(5068), 383-7, 1992). Benché non sia ancora chiaro se un simile scenario insorga negli individui infettati da HIV, la trasmissione di HIV-1 da DC alle cellule T potrebbe contribuire all’impoverimento delle cellule T CD4 osservata nell’AIDS. Lo studio delle interazioni HIV-DC potrebbe contribuire alla comprensione degli eventi precoci dell’infezione da HIV e porterà sperabilmente a strategie dedicate al blocco degli eventi precoci della trasmissione. Per un’ulteriore discussione, è fatto rimando anche ai documenti WO 95/32734 e WO 96/23882.
Le DC sono uniche nella loro capacità di interagire con e attivare le cellule T quiescenti. Tuttavia, prima della presente invenzione, era in gran parte sconosciuto la modalità con cui il contatto DC-cellule T fosse innescato e regolato. Nel presente contesto, è studiato il ruolo di ICAM-3 nelle interazioni DC-cellula T. È stato dimostrato che, benché le DC aderiscano fortemente ad ICAM-3, tale adesione non è mediata da LFA-1, αD o da qualsiasi altra integrina. Nella ricerca di questo nuovo recettore per ICAM-3 sulle DC, è stato clonato un nuovo recettore di lectina tipo C, denominato DC-SIGN, che risulta essere espresso di preferenza sulle DC. Assieme al suo ruolo principale nell’agglomerazione e innesco di DC-cellule T nelle risposte mediate dalle cellule T, abbiamo scoperto che DC-SIGN è un importante recettore di HIV-1 implicato nell’infezione delle DC e nella successiva trasmissione di HIV-1 alle cellule T. Per interagire con le DC, HIV-1 e le cellule T quiescenti adoperano quindi un recettore simile altamente espresso.
In un primo aspetto, l'invenzione riguarda l'uso di un anticorpo che si lega al recettore denominato DC-SIGN sulla superficie di una cellula dendritica, e rappresentato come seq ID NO:2, o di una sua variante o frammento naturale, nella preparazione di un medicinale per ridurre una risposta immune in un animale, in cui detto anticorpo inibisce l'interazione tra una cellula dendritica e una cellula T.
In particolare, l'anticorpo può ridurre l'adesione tra una cellula dendritica e una cellula T.
In special modo, l'anticorpo può ridurre l'adesione tra un DC-SIGN sulla superficie di una cellula dendritica e un recettore ICAM sulla superficie di una cellula T.
La sequenza di amminoacidi di un recettore di lectina tipo C, denominato DC-SIGN, che è stato rilevato essere coinvolto nel legame delle cellule dendritiche alle cellule T, è riportata nella SEQ ID no. 1 e nella figura 9. Questo recettore di lectina tipo C è essenzialmente simile al recettore di gp 120 lectina tipo C descritto da Curtis et al. in Proc. Natl. Acad. Sci. USA, 89 (1992), p. 8356-8360 e in WO 93/01820. In particolare, esso presenta un alto grado di omologia (< 98%) con la sequenza di amminoacidi fornita nella SEQ ID no.1 di WO 93/01820. Questa è una lectina tipo C di gruppo II di 404 amminoacidi; con un Mr apparante do circa 44 kDa; e con un primo dominio (da Met 1 a Ala 76) comprendente un ammino-terminale citoplasmatico, un secondo dominio (da Ile 77 a Val 249) comprendente ripetizioni in tandem, e un terzo dominio (da Cys 253 ad Ala 404) con un alto grado di omologia con altre lectine tipo C note, che sono proteine di membrana di tipo II. A seguire è fornita un'ulteriore caratterizzazione.
Nell'invenzione è stato scoperto che questa lectina tipo C sulle cellule dendritiche si lega a recettori ICAM sulla superficie delle cellule T.
Pertanto, le composizioni dell'invenzione possono essere utilizzate per modulare (ovvero alterare e/o modificare), e più specificatamente per inibire (ovvero ridurre e/o sottoregolare), la/e interazione/i tra cellule dendritiche e cellule T.
Tali interazioni includono l'adesione di cellule T a cellule dendritiche, ad esempio nell'agglomerazione tra cellule dendritiche e cellule T, nell'attivazione delle cellule T, e comprendono inoltre tutte le interazioni che fanno affidamento sul contatto tra cellule dendritiche e cellule T, con il quale si intende sia un contatto diretto cellula-cellula, sia una stretta prossimità tra cellule dendritiche e cellule T.
Tali interazioni inoltre comprendono, non essendo tuttavia limitate a, processi implicati nella generazione di una risposta immune, in particolare durante gli stadi iniziali di tale risposta, come una sensibilizzazione/attivazione primaria di linfociti T, (ovvero, presentazione alle cellule T dell’antigene e/o di peptidi legati a MHC) e co-stimolazione di cellule T; come pure processi quali la segnalazione chimica, endocitosi e trasporto trans-epiteliale. Per una discussione delle interazioni tra cellula dendritica-cellula T in generale, le quali possono essere tutte influenzate dalle composizioni dell’invenzione, è fatto rimando alla discussione che segue nonché a WO 95/32734 e WO 96/23882.
Le composizioni dell'invenzione possono dunque essere usate per influenzare la capacità immunomodulatoria delle cellule dendritiche; per modulare, e in particolare ridurre, le risposte (primarie) delle cellule T mediate dalle cellule dendritiche, e/o per influenzare in generale, e inibire in particolare, il sistema immune.
Alcune specifiche applicazioni comprendono la prevenzione o inibizione di risposte immuni per specifici antigeni; tra cui la tolleranza; immunoterapia; immunosoppressione, ad esempio per prevenire un rigetto di trapianto; il trattamento di malattie auto-immuni come tiroidite, artrite reumatoide, lupus eritematoso sistemico (SLE, systemic lupus erythematosus), sclerosi multipla e diabete auto-immune; e la prevenzione o trattamento di allergie.
Le composizioni dell'invenzione possono anche modulare l'attivazione di altri recettori su cellule T che dipendono dall'adesione o dalla stretta prossimità delle cellule dendritiche alle cellule T. Inoltre, la scoperta dell’invenzione che una lectina tipo C su cellule dendritiche si lega a recettori ICAM presenti su cellule T, può dare avvio a nuove strategie o possibilità per influenzare l’interazione tra cellule dendritiche e cellule T, e quindi per la modulazione del sistema immune in generale.
Inoltre, le composizioni dell'invenzione possono essere utilizzate per impedire o ridurre il trasferimento di materia dalle cellule dendritiche alle cellule T, come sostanze chimiche, fattori di segnalazione quali chemochine e/o interleuchine, ecc., e in particolare di particelle virali come HIV. In questo modo, usando le composizioni dell'invenzione, non solo è possibile inibire l'adesione iniziale di NV alle cellule dendritiche, ma anche la diffusione dell'infezione HIV dalle cellule dendritiche alle cellule T.
Questa scoperta è di particolare rilevanza, dal momento che si ritiene che le cellule dendritiche possano agire da "serbatoio" per HIV, in particolare durante le fasi iniziali dell'infezione da MV. Le composizioni dell'invenzione possono quindi essere usate non solo per impedire un'infezione da HIV di cellule dendritiche, ma anche per ridurre la diffusione dell'infezione da HIV alle cellule T una volta che le cellule dendritiche sono state infettate, rallentando così il processo patologico.
Inoltre, è noto che l’attivazione delle cellule –ovvero nelle ghiandole linfatiche - ricopre un ruolo nello sviluppo dell’AIDS in un paziente infettato da HIV. Si ritiene che le composizioni dell’invenzione possano essere usate per prevenire, inibire o almeno ritardare detta attivazione delle cellule T, abbassando quindi l’insorgenza e/o il progredire della malattia.
Inoltre, in un ulteriore aspetto, l'invenzione riguarda anche l'uso di un anticorpo che si lega al recettore denominato DC-SIGN sulla superficie di una cellula dendritica, e rappresentato come seq ID NO:2, o di una sua variante o frammento naturale, nella preparazione di un medicinale per inibire l'adesione di una proteina di superficie di HIV (ovvero gp120) alla superficie di una cellula dendritica, e quindi l'ingresso di HIV in detta cellule dendritica.
Composti che possono essere usati come inibitori per le lectine tipo C sono di per sé noti, tra i quali quelli descritti in WO 93/01820, come sopra menzionato.
In generale, questi sono composti che possono legarsi o aderire a (ad esempio in una maniera reversibile) oppure che possono fungere da ligando per le lectine tipo C, in particolare la lectina tipo C della SEQ ID no.1/figura 9 o loro varianti o equivalenti naturali. Esempi sono carboidrati di mannosio come mannano e D-mannosio; carboidrati di fucosio come L-fucosio; lectine vegetali come concanavalina A; antibiotici come pradimicina A; zuccheri come N-acetil-D-glucosammina e galattosio (che, tuttavia, sono descritti come meno potenti); come pure composti peptidomimetici adatti e piccole molecole farmaceutiche, che possono ad esempio essere identificati usando tecniche di visualizzazione su fago. Inoltre, è possibile usare proteine come gp120 e suoi analoghi o frammenti, o proteine simili, dotati di capacità di legame alle lectine tipo C sulle cellule dendritiche, come pure recettori ICAM isolati e suoi analoghi, sue porzioni o frammenti inclusi. Tali porzioni o frammenti dovrebbero essere quindi ancora tali da essere in grado di legarsi alle lectine tipo C sulla superficie di cellule dendritiche.
Tuttavia, l’uso di carboidrati è solitamente meno desiderabile da un punto di vista terapeutico, poiché essi possono essere rapidamente metabolizzati in vivo. Inoltre, l’uso di lectine vegetali, come concanavalina A e antibiotici come pradimicina, possono avere svantaggi in un approccio terapeutico, in particolare quando si trattano pazienti con disordini auto-immuni e/o infezioni HN.
È possibile usare uno o più composti fisiologicamente tollerabili e/o farmaceuticamente accettabili, come delineato in WO 93/01820. Ad esempio, l’uso di gp120 o suoi derivati può causare effetti collaterali indesiderati, in particolare sul sistema nervoso (vedere WO 93/01820).
Per contro, secondo l’invenzione, si usa un anticorpo diretto contro DC-SIGN come presente/espresso sulla superficie di una cellula dendritica, o come sua porzione, segmento o epitopo. Nel presente contesto, il termine anticorpi è usato per comprendere, inter alia, anticorpi policlonali, monoclonali, chimerici e a singola catena, come pure frammenti (Fab, Fv, Fa) e una libreria di espressione di Fab. Inoltre è possibile usare anticorpi "umanizzati", ad esempio come descritto in WO 98/49306.
Tali anticorpi contro DC-SIGN dell’invenzione possono essere ottenuti come descritto a seguire nel presente contesto o in qualsiasi altra maniera di per sé nota, come quelle descritte in WO 95/32734, WO 96/23882, WO 98102456, WO 98/41633 e/o WO 98/49306.
È possibile ad esempio ottenere anticorpi policlonali immunizzando un ospite adatto, come capre, conigli, ratti, maiali o topi, con DC-SIGN o una sua porzione, frammento o fusione immunogenica, opzionalmente con l’uso di un trasportatore immunogenico (come albumina di siero bovino o emocianina di fissurella) e/o un adiuvante come saponina di Freund, ISCOM, idrossido di alluminio, o un simile gel minerale, oppure emocianina di fissurella o una simile sostanza tensioattiva. Dopo avere suscitato una risposta immune contro DC-SIGN (di solito, entro 1-7 giorni), gli anticorpi possono essere isolati da sangue o siero prelevato dall'animale immunizzato in una maniera di per sé nota, che opzionalmente può implicare un passaggio di screening per un anticorpo dotato delle proprietà desiderate (ovvero, una specificità desiderata) usando tecniche note di immunosaggio, per cui si fa ancora riferimento ad esempio a WO 96/23882.
I monoclonali possono essere prodotti usando linee cellulari continue in coltura, comprendenti tecniche di ibridoma e similari, ancora come descritto essenzialmente nei succitati rimandi.
Frammenti Fab, come frammenti F(ab)2. Fab' e Fab, possono essere ottenuti tramite digestione di un anticorpo con pepsina o altra proteasi, riducendo i legami disolfuro e, rispettivamente, mediante trattamento con papaina e un agente riducente. Librerie di espressione di Fab possono ad esempio essere ottenute con il metodo di Huse et al., 1989, Science 245:1275-1281.
Si usa preferibilmente un anticorpo contro DC-SIGN su cellule dendritiche, più specificamente contro il peptide con la sequenza di amminoacidi nota in/codificata dalla SEQ ID no 1 e 2 e nella figura 9, oppure una sua porzione (antigenica); e tali monoclonali come un ulteriore aspetto dell’invenzione. A seguire nel presente contesto, l’invenzione sarà illustrata per mezzo di due di questi monoclonali, nel presente contesto indicati AZN-D1 e AZN-D2, sebbene possano essere anche usati simili monoclonali dotati di specificità paragonabile per DC-SIGN.
In un altro aspetto, l’invenzione fornisce una linea cellulare, come un ibridoma, che produce anticorpi, preferibilmente anticorpi monoclonali, contro DC-SIGN presente su cellule dendritiche, più nello specifico contro il peptide con la sequenza di amminoacidi riportata in/codificata dalle SEQ ID no 1 e 2 e nella figure 9 o una sua porzione (antigenica). Ibridomi che producono i sopramenzionati monoclonali AZN-1 e AZN-2 dell’invenzione, sono stati depositati in data 8 aprile 1999 presso la European Collection of Cell Cultures rispettivamente sotto i numeri d’ingresso (provvisori) EGACC 990400818 e 99040819.
L'invenzione riguarda anche un metodo per produrre un anticorpo, preferibilmente un anticorpo monoclonale, contro DC-SIGN su cellule dendritiche, più nello specifico contro il peptide con la sequenza di amminoacidi mostrata in (codificata dalle) SEQ ID no 1 e 2 e nella figura 9 o una sua porzione (antigenica), detto metodo comprendente i passaggi di coltivare una cellula o una linea cellulare che produce detto anticorpo e raccogliere/isolare l’anticorpo dalla coltura cellulare.
Né gli anticorpi (monoclonali) contro DC-SIGN presente su cellule dendritiche, né cellule o linee cellulari che producono tali anticorpi, sono stati attualmente descritti nel ramo, e si considera che i nuovi anticorpi dell’invenzione avranno un’ampia applicabilità (ovvero, oltre agli usi farmaceutici/terapeutici descritti in questa sede). Alcune di queste applicazioni – che formano ancora un altro aspetto dell’invenzione – risulteranno chiare alla persona esperta dietro lettura della presente descrizione.
Ad esempio, gli anticorpi dell’invenzione possono essere usati per rilevare la presenza (e quindi determinare l’espressione) di DC-SIGN, in o su tessuti o cellule intere, nonché per il rilevamento della presenza di DC-SIGN in altri campioni biologici, come frammenti cellulari o in preparazione cellulari. Le informazioni così ottenute possono quindi (anche) essere usate per determinare se il metodo o le composizioni dell'invenzione possano essere applicate a tali tessuti o cellule. Gli anticorpi dell’invenzione potrebbero anche essere usati per rilevare (per via qualitativa e/o quantitativa), isolare, purificare e/o produrre cellule dendritiche, ad esempio in/da tessuti biologici, tra cui fluidi biologici come sangue, plasma o fluido linfatico; campioni tissutali o campioni cellulari, come midollo osseo, tessuto cutaneo, tessuto tumorale, ecc; oppure colture cellulari o mezzi di coltura.
Ad esempio, i pochi metodi attualmente disponibili per isolare/produrre cellule dendritiche da campioni biologici – un tale metodo è descritto in US-A-5.643.786, comprendente leucoferesi seguita da separazione cellulare attivata per fluorescenza – sono procedure a più passaggi notevolmente disagevoli che forniscono soltanto basse rese e campioni eterogenei. Quindi, la limitata disponibilità di cellule dendritiche ha severamente ostacolato la ricerca in questa classe di cellule.
Usando i monoclonali dell’invenzione, sarebbe possibile isolare e produrre cellule dendritiche con resa elevata e maggiore, e con elevata specificità. In tale metodo, i monoclonali potrebbero essere usati in una maniera di per sé nota per la raccolta, isolamento e/o purificazione di cellule da fluidi biologici con l’uso di anticorpi.
Ad esempio, i monoclonali potrebbero essere fissati a una colonna o matrice, a sferette (para)magnetiche o a un supporto solido simile, che potrebbe essere quindi messo in contatto con un campione biologico o mezzo di coltura contenente cellule dendritiche. Le cellule che non si attaccano al trasportatore sono quindi separate o rimosse - ad esempio con un lavaggio, dopo di che le cellule dendritiche sono separate dal trasportatore e isolate in una maniera di per sé nota.
Inoltre, i monoclonali dell’invenzione potrebbero essere usati per rilevare/determinare la presenza di cellule dendritiche (cellule e/o DC-SIGN) e/o l’espressione di loro geni codificanti in campioni biologici, in cui gli anticorpi potrebbero essere ancora usati in una maniera di per sé nota per analisi che si avvalgono di anticorpi, come saggi di inibizione per competizione o immunosaggi di tipo ELISA. Ad esempio, gli anticorpi potrebbero essere usati in combinazione con tecniche di microscopia, tecniche di separazione cellulare, tra cui citometria di flusso e FACS, tecniche basate su supporti solidi e/o etichette o marcatori rilevabili (che possono essere fissati sugli anticorpi), tecniche basate su sferette (para)magentiche o qualsiasi altra tecnica di rilevamento o saggio di per sé nota, in cui è possibile usare anticorpi. Tali saggi e kit – che oltre agli anticorpi dell’invenzione possono contenere tutti gli ulteriori componenti di per sé noti per i saggi basati su anticorpi, nonché manuali ecc - formano un ulteriore aspetto dell’invenzione.
I monoclonali dell’invenzione costituiscono quindi uno strumento diagnostico e di ricerca molto utile, sia per un uso vitro che in vivo. Campi non limitanti possibili comprendono lo studio di cellule dendritiche e la loro funzione e interazioni; lo studio del sistema immune; il rilevamento di cellule dendritiche e/o DC-SIGN in cellule, tessuti, fluidi biologici, come tessuto sinoviale e tessuto cutaneo/cellule della cute (cellule dendritiche del derma); nonché lo studio del ruolo svolto dalle cellule dendritiche nei processi biologici o meccanismi patologici, quali tumore (cellule dendritiche sono usate infatti in prove cliniche in vivo per eradicare la formazione e lo sviluppo di tumori) e malattie auto-immuni (comprendenti, ad esempio, l’artrite reumatoide).

Male: "Immunology", 2a Ed., Churchill Livingstone (1994); tutti incorporati in questa sede tramite rimando. Si fa particolare rimando agli usi generali di anticorpi e tecniche coinvolte in questa sede, come menzionato nelle sezioni da 2.7 a 2.17 del lavoro di riferimento generale di Janeway-Travers: "Immunobiology, the immune system in health and disease", terza edizione.
Una composizione dell’invenzione può contenere due o più dei sopra menzionati composti attivi, oppure tali composti possono essere usati in combinazione. Ad esempio, un anticorpo anti-DC-SIGN può essere formulato con carboidrati di mannosio e fucosio, lectine e/o antibiotici come pridamicina A, attraverso cui è possibile ottenere un effetto sinergico.
Le composizioni dell'invenzione possono anche contenere o essere usate in combinazione con composti co-inibitori noti, come anti-LF3A; come pure altri principi attivi, di per sé noti, in subordine alla condizione che deve essere trattata. Le composizioni dell’invenzione possono essere ad esempio formulate o usate in combinazione con immunosoppressivi (ovvero, per prevenire il rigetto di un trapianto), immunomodulanti, antibiotici, auto-antigeni o allergeni (ad esempio come descritto in WO 95/3234 o WO 96/23882), fattore di necrosi tumorale (TNF), e agenti antivirali, come agenti anti-HIV e inibitori di CD4, tra cui anticorpi diretti su CD4, ad esempio Leu-3A, attraverso cui è possibile ottenere ancora un effetto sinergico.
Le composizioni dell’invenzione possono essere inoltre formulate usando nuovi trasportatori e/o adiuvanti per fornire una forma farmeceutica di per sé nota, come una compressa, capsula, polvere, preparazione essiccata per congelamento, soluzione da iniezione, ecc, preferibilmente in una forma di dosaggio unitaria. Tali forme farmaceutiche, loro uso e somministrazione (con forma di dosaggio unitaria o multipla), come pure trasportatori, eccipienti, adiuvanti e/o formulanti, da usare al loro interno, sono generalmente noti nel ramo e sono ad esempio descritti in WO 93/01820, WO 95/32734, WO 96/23882, WO 98/02456, WO98/41633 e/o WO 98/49306, questi ultimi essendo tutti incorporati in queste sede tramite rimando. La formulazione può essere inoltre nella forma di un liposoma, come descritto in WO 93/01820.
Formulazioni farmaceutiche e anticorpi, loro somministrazione e uso, sono descritti in generale in WO 95/32734, WO 96/23882, WO 98/02456, WO 98/41633 e/o WO 98/49306.
Le composizioni dell’invenzione possono essere inoltre confezionate, ad esempio in fiale, bottiglie, buste, blister, ecc; opzionalmente con bugiardini informativi pertinenti per il paziente e/o istruzioni per l’uso.
Le composizioni dell’invenzione possono essere usate per modulare la risposta immune in un animale, nella fattispecie un essere umano o altro animale, mediante somministrazione a detto animale di un anticorpo che si lega o che può legarsi a un DC-SIGN sulla superficie di una cellula dendritica, preferibilmente nella forma di una composizione come sopra descritta, in quantità sufficiente tale da alterare o modificare una risposta immune.
L'anticorpo o la composizione è in particolare somministrata in una quantità tale da alterare o modificare le una o più interazioni tra cellule dendritiche e cellule T, più in particolare in una quantità tale da ridurre l’adesione delle cellule dendritiche alle cellule T.
Questo metodo può essere usato per prevenire e/o trattare disordini del sistema immune, come pure per prevenire un rigetto di trapianto, come sopra descritto.
La composizione dell’invenzione può essere usata per la prevenzione o trattamento di infezioni da HIV, mediante somministrazione a un paziente infettato da HIV, o una persona che corre il rischio di essere infettata da HIV, di un anticorpo che può legarsi o che si lega a DC-SIGN sulla superficie di una cellula dendritica, in una quantità tale da inibire l’adesione di HIV sulle cellule dendritiche, e in particolare dell'adesione tra la proteina d’involucro di HIV gp120 e DC-SIGN sulla superficie delle cellule dendritiche.
In tutti i metodi e forme esecutive precedenti, i composti/composizioni usati saranno somministrati in una quantità terapeuticamente efficace, e per quest’ultimo termine si fa generale rimando a WO 93/01820, WO 95/32734 e/o WO 96/23882. La somministrazione può essere in una dose singola, tuttavia fa preferibilmente parte di un regime di somministrazione multidose, svolto per uno o più giorni, settimane o mesi.
Tutti i termini usati in questa sede hanno l’usuale significato nel ramo, per cui è possibile fare rimando, inter alia, alle definizioni fornite in WO 93/01820, WO 95/32734, WO 96/23882, WO 98/02456, WO98/41633 e/o WO 98/49306, applicati in maniera analoga.
Inoltre, benché l’invenzione sia descritta in questa sede con riferimento allo specifico recettore di lectina tipo C di 44kDa, denominato DC-SIGN, divulgato in questa sede, non si esclude che altre lectine tipo C, generalmente simili tra loro, tra cui varianti naturali della sequenza SEQ ID no.1 e figura 9, possano anche essere presenti su cellule dendritiche e/o possano essere implicate nell’interazione cellula dendritica–cellula T. Tali varianti avranno solitamente un alto grado di omologia di amminoacidi (da più di 80% a più di 90%) con, e/o essendo funzionalmente equivalenti alla specifica lectina tipo C divulgata in questa sede. Inoltre, qualsivoglia di tale recettore in genere esibirà proprietà simili a quelle descritti in questa sede; in particolare, tali che l’inibizione del recettore, tramite inibitori carboidratici o anticorpi specifici, porterà a un'alterazione dell’interazione cellula dendritica/cellula T. Qualsivoglia di tali varianti di recettore dovrebbe essere tuttavia distinta dal recettore di lectina tipo C divulgato in WO 96/23882, la cui inibizione non comporta l’inibizione dell’interazione tra cellule dendritiche e cellule T.
L’invenzione sarà ulteriormente illustrata mediante la sezione sperimentale, fornita di seguito, nonché dalle figure in cui:
- Le figure 1A - 1C sono grafici che mostrano: la spontanea adesione di leucociti a ICAM-1 e ICAM-3 (fig. 1A); l’adesione di leucociti a ICAM-3 dopo attivazione delle integrine β2 (fig. 1B); l’adesione di DC a ICAM-3 in presenza di anticorpi bloccanti (20µg/ml) contro le integrine β2 (NKI-L19), integrina β1 (AIIB2), ICAM-3 (CBR-IC3/1, CBR-IC3/2) o in presenza di EDTA (5mM) o EGTA (5mM) (fig. 1C).
- Le figure 2A - 2C sono grafici che mostrano che gli anticorpi AZN-D1 e AZN-D2 inibiscono l’adesione di DC a ICAM-3 e riconoscono un antigene che è specificamente espresso dalle DC.
- Le figure 3A e 3B mostrano che DC-SIGN è identico alla lectina tipo C, legante gp 120 di HIV, di placenta umana, come è possibile osservare in SDS-PAGE (fig. 3A) e nella rappresentazione schematica di DC-SIGN isolato da DC umane (3B).
- Le figure 4A e 4B mostrano che DC-SIGN, sovraespresso in cellule COS7, è riconosciuto dall’anticorpo anti-DC-SIGN, AZN-D 1, e che si lega a ICAM-3.
- La figura 5 mostra la distribuzione tissutale di DC-SIGN come illustrato mediante analisi immunoistochimica dell’espressione di DC-SIGN in sezioni di tonsille (A e B) e linfonodo (C e D) (OMx100).
- Le figure 6A - 6D mostrano che l’adesione mediata da DC-SIGN di DC e ICAM-3 è implicata nell'interazione DC-linfocita T, come dimostrato dall’adesione mediata da DC-SIGN di DC a ICAM-3 (fig. 6A); agglomerazione eterotipica di cellule DC con cellule K562-ICAM-3 (fig. 6B); agglomerazione dinamica di cellule DC con PBL quiescenti (fig. 6C); e il ruolo che ricopre l’interazione DC-SIGN-ICAM-3 nella proliferazione di cellule T indotta da DC (fig. 6D).
- La figura 7 mostra che DC SIGN è un recettore di HIV-1 su DC.
- La figura 8 mostra che DC SIGN si lega a cellule K562 esprimenti sia ICAM-3 che ICAM-3.
- la figura 9 mostra le sequenza di DC-SIGN.
Sezione sperimentale
Le cellule dendritiche (DC) catturano antigeni e migrano verso tessuti linfoidi secondari, dove presentano gli antigeni a cellule T naïve. HIV-1 approfitta di questa esclusiva capacità, per ottenere l’ingresso nelle cellule T CD4 . Nell’invenzione, è stata clonata una specifica lectina tipo C, denominata DC-SIGN, che non soltanto si lega a ICAM-2 e/o ICAM-3 con elevata affinità, ma che è inoltre in grado di legare HIV-1. Inoltre, sono stati sviluppati anticorpi anti-DC-SIGN che non solo inibiscono le interazioni transienti DC-cellula T e la proliferazione di cellule T indotta da DC, ma che inibiscono anche in modo efficace l’infezione di HIV-1 delle DC. Queste scoperte, oltre ad avere importanti conseguenze per la comprensione dell’ingresso CD4-independente di HIV nelle DC, gettano anche nuova luce sul ruolo di DC-SIGN nella promozione di risposte immuni primarie.
Esempio 1: L’adesione di DC a ICAM-3 non è mediata da integrine

[0001] The present invention discloses an element for
the segmental distalisation of the canine- to- molar posterior
maxillary sector for orthodontic treatments which affords
significant advantages over the currently known elements
for this purpose.
[0002] Auxiliary elements for segmental distalisation
are known in this art, for example those corresponding
to Spanish patent application No. 200102210 in the name
of the same applicant.
[0003] With the auxiliary element for segmental distalisation
according to the aforementioned patent in the
name of the same applicant there was disclosed the formation
of said auxiliary element in two parts, namely a
median segment equipped with an enlargement intended
to be fixed to the canine by means of adhesive and an
arcuate arm ending with a ball which is introduced into a
small arcuate receiving shoe fixed to the upper molar by
adhesive.
[0004] Although the invention according to the aforementioned patent represented a significant advance over
formerly known devices, it had specific problems owing
to its production in two parts due to the relative production
costs and the relatively awkward handling of the two- part
assembly.
[0005] The inventor carried out research to obtain novel
improvements to segmental distalisation devices and
arrived at the present invention.
[0006] As the fruit of the investigations and tests, the
inventor has developed the present distalisation device
of which a specific characteristic is the single- part construction, the ease of adaptation thereof owing to inherent
design characteristics and the production thereof from a
metallic material which is preferably of the type known
as super- resilient or a thermoelastic material which has
the necessary characteristics of adequate toughness
and resilience.
[0007] Basically, the new segmental distalisation element
is characterised in that it comprises a single- part
elongate body having, at one end, a terminal which can
be adapted to the external surface of the canine and
which has an attachment for the resilient means, while
the other end has, with interposition of a zone of greater
flexibility, a shoe which can be coupled to the molar of
the upper portion, preferably by an adhesive or other
means.
[0008] In a preferred solution, the zone of greater resilience
will consist of a simultaneous double elbow with
a constriction of the material cross- section.
[0009] The portion with a double elbow and constriction
is characterised in that it has two successive angle
folds and at the same time a constriction of the cross
section of the material between the central portion of the
distalisation element and the end or terminal for coupling
to the molar. The first of the angles, in other words the
angle which connects the intermediate elbow to the main
element or arcuate bridge of the distalisation element will
preferably be an obtuse angle while the second angle,
in other words the angle between the intermediate bridge
of smaller cross- section and the end shoe for application
to the molar, can be a slightly obtuse or right angle. The
angle between the end of the central arm of the device
and the longitudinal axis of the shoe for fixing to the canine
will also be of interest for defining the structure of
the distalisation element according to the present invention.
[0010] The zone equipped with a constriction and
which has two successive angles will preferably have a
significant constriction and can similarly be formed by
two fine parallel arms which define an intermediate aperture
or other similar form.
[0011] However, the zone of greater flexibility preceding
the element which can be adapted to the molar may
be produced in a wide variety of forms and not necessarily
with the double elbow provided for the preferred embodiment.
Thus, for example, instead of a double elbow, a
smooth curved zone, an omega like or similarly shaped
zone with an open or closed shape, or a zone with one
or more helical turns in an arrangement resembling a
helical spring may be provided.
[0012] The accompanying drawings of a preferred embodiment
of the invention, given as non- limiting examples,
will assist the understanding thereof.

ement according to the present invention comprises, in
a single part, a smoothly arcuate main body 1 of substantial
length which terminates at one end with a head
2 for fixing it to the canine and at the other end with a
head 3 for fixing it to the molar on the same side, having
respective curved internal zones 4 and 5 for better adaptation
to the canine and molar respectively and a variable
number of reliefs such as the ribs 6 and 7, grating
profiles, grids or other types of roughness intended to
improve the fixing to the aforementioned tooth parts using
an adhesive.
[0014] The distalisation element according to the
present invention has the basic characteristic of comprising a more flexible zone preferably in the form of a curved,
constricted zone in various dispositions. In a preferred
embodiment, said more flexible zone will adopt the form
of a double- elbow connecting bridge 8 between the central
arm 1 and the end head 3 for adaptation to the molar.
Said double- elbow- shaped bridge or zone will also have
a significant reduction in the material cross- section, resulting
in significant flexibility in the end zone 3 which will
allow better adaptation thereof and will therefore allow
the production of the distalisation segment in a single
part.
[0015] The double elbow will be produced by means
of a first, preferably obtuse, angle 9 and a second slightly
obtuse or else right angle 10. In addition to said angles,
of which the first is indicated by the letter a, the angle b
will also be important, this angle being formed between
the axis 1 of the end 12 of the arm 1 and the axis 13
which substantially corresponds to the longitudinal axis
of the head 3 which preferably adopts the form of a small
shoe 14 of rectangular or oval construction.
[0016] It will be appreciated that, despite the aforementioned
quantities relating to the angles of the double elbow,
the values of the angles can vary within wide limits,
according to the prerequisites of the distalisation device.
[0017] As mentioned, the zone of the double elbow,
which is indicated by reference numeral 15, has a significant
constriction with respect to the central portion 1,
which can either be achieved by a constriction in crosssection
as shown in Fig. 1 and 2 to form a single connecting
bridge between the central element or arm 1 and
the end head 14 or be formed by means of a constricted
zone 17, Fig. 6 and 7, which will also have a central aperture
18 determining parallel arms 19 and 20 of which
the precise shape can vary greatly, Fig. 8 showing three
versions in which the first one 21 has two arms with a
circular cross- section, the second, indicated by reference
numeral 22, has two arms with a rectangular cross- section
and the third 23 has two arms with an oval crosssection,
or, as will be appreciated, the with form of said
arms varying greatly.
[0018] Also in the case of the constriction in the zone
16 shown in Fig. 1 to 4, the cross- section can assume
various specific shapes such as those designated by reference
numerals 24 for a half- rod- shaped section, 25 for
a circular section, 26 for a substantially rectangular sec-
tion or 27 for an oval section, or else the shape of said
cross- section can vary within wide limits.
[0019] The various views show the upper face of the
end head 14 with a flat shape, taking into account the
fact that said shape is intended to allow the possible joining
of a molar tube to allow complementary actions.
[0020] Fig. 9 to 17 show various additional versions of
the flexible zone between the portion of the central element
of the device and the zone for adaptation to the
molar. Thus, for example, Fig. 9 shows the arrangement,
between said central zone 1 and the end zone 3 which
can be adapted to the molar, of a singly curved zone 28
which can have a sinusoidal, omega or similar shape.
[0021] Fig. 10 shows a version in which the zone of
greater flexibility consists of a closed curve 29 in the form
of an omega or another form.

31 in the form of loops which are directed toward the
upper portion and lower portion respectively.

a spring, directed upwardly and downwardly respectively.

plan view of an embodiment in which the zone of greater
flexibility 35 is formed by a loop or the like situated in a
disposition which can be described as lateral, based on
the convention employed in the previous figures, in which
the loops which, simultaneously with the reduction in
cross- section, impart the greatest flexibility are disposed
in a plane perpendicular to that of the embodiments
shown in the earlier figures. It is obvious that the disposition
shown in Fig. 16 and 17 could be found on either
of the sides determined by the longitudinal plane of symmetry.
Similarly, although not shown, other planes which
are intermediate to those shown could be used for said
loops and similar elements.
[0026] Although not shown in the figures, the invention
will similarly allow the production of the more flexible connection
zone, between the central portion of the distalisation
segment and the head for fixing to the molar, from
a metal part made of a more highly adaptable metal such
as, for example, stainless steel. In this case, the constriction in said zone of greater flexibility may be omitted since
the adaptation can be achieved partially by direct bending
of said metal part.
[0027] The materials for producing the distalisation element
according to the present invention can be varied,
providing that the mechanical characteristics, in particular adequate toughness and resilience, necessary for the
operation thereof are achieved. In particular, a metal material of the type known as super- resilient, steel, thermoe-

lastic materials or a plastics material, for example a polymer,
may be provided.
[0028] Super- resilient materials include those known
as Ni- Ti such as Cu, Ni, Ti and Cr alloys.
Claims
1. Segmental distalisation element for orthodontic
treatments, comprising an elongate single- part element
having a smoothly arcuate central body (1)
equipped, at one end, with a head (2) for adaptation
to the canine and, at the other end, with a head (3)
for adaptation to the molar, characterized in that
the arcuate body (1) is connected to the end head
(3) via a zone of greater flexibility (8) with a smaller
cross- section than the remainder of the element, to
increase the resilience of the distalisation element
in said zone and achieve easy, direct adaptation.
2. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that the zone of reduced cross- section has two successive
angles (9, 10), the first angle (9) being between
the central arcuate portion of the distalisation
element and the reduced- section intermediate
bridge for connection to the head for adaptation to
the molar and the second angle (10) being in the
actual intermediate bridge before its connection to
said head (3) for adaptation to the molar.
3. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that the reduction in the cross- section of the bridge
for connection to the end head (3) for coupling to the
molar is obtained by providing a single bridge of reduced
cross- section.
4. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that the constricted zone of the bridge for connection
to the head (3) for attachment to the molar is obtained
by providing an aperture or apertures to make the
connecting bridge more flexible.
5. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that the zone of greater flexibility (8) consists of a
segment which is singly curved with smooth curvature
(32).
6. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that the zone of greater flexibility (8) consists of a
closed loop (29, 30, 31, 35).
7. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that the zone of greater flexibility (8) consists of one
or more turns (33, 34).
8. Segmental distalisation element for orthodontic
treatments according to claim 2, characterised in
that the angles (9, 10) of the double elbow are obtuse
angles.
9. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that it is produced from a super- resilient material.
10. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that it is produced from a plastics material.
11. Segmental distalisation element for orthodontic
treatments according to claim 1, characterised in
that it is produced from thermoelastic material.
Patentansprüche
1. Segmentdistalisationselement für kieferorthopädische
Behandlungen, umfassend ein längliches einteiliges
Element mit einem leicht bogenförmigen
zentralen Körper (1), der an einem Ende mit einem
Kopf (2) zur Anpassung an den Eckzahn und am
anderen Ende mit einem Kopf (3) zur Anpassung an
den Molaren ausgestattet ist, dadurch gekennzeichnet,
dass der bogenförmige Körper (1) mit
dem Endkopf (3) über eine Zone (8) größerer Flexibilität
mit einem kleineren Querschnitt als der Rest
des Elements verbunden ist, um die Nachgiebigkeit
des Distalisationselements in dieser Zone zu erhöhen
und einfache, direkte Anpassung zu erreichen.
2. Segmentdistalisationselement für kieferorthopädische
Behandlungen nach Anspruch 1, dadurch gekennzeichnet,
dass die Zone reduzierten Querschnitts
zwei aufeinander folgende Winkel (9, 10)
aufweist, wobei sich der erste Winkel (9) zwischen
dem zentralen bogenförmigen Abschnitt des Distalisationselements
und der querschnittsreduzierten
Zwischenbrücke für die Verbindung mit dem Kopf
zur Anpassung an den Molaren befindet und sich
der zweite Winkel (10) in der eigentlichen Zwischenbrücke
vor ihrer Verbindung mit dem Kopf (3) zur
Anpassung an den Molaren befindet.
3. Segmentdistalisationselement für kieferorthopädische
Behandlungen nach Anspruch 1, dadurch gekennzeichnet,
dass die Reduzierung im Querschnitt
der Brücke zur Verbindung mit dem Endkopf
(3) zum Ankoppeln an den Molaren durch Bereitstellen
einer einzelnen Brücke reduzierten Querschnitts
erreicht wird.

Revendications
1. Elément de distalisation segmentaire pour des traitements
orthodontiques, comprenant un élément
oblong d’un seul tenant avec un corps central arqué
de façon uniforme (1) muni, sur une extrémité, d’une
tête (2) pour l’adaptation sur la canine et, sur l’autre
extrémité, d’une tête (3) pour l’adaptation sur la molaire,
caractérisé en ce que le corps arqué (1) est
raccordé à la tête d’extrémité (3) par l’intermédiaire
d’une zone de plus grande souplesse (8) ayant une
section transversale plus petite que le reste de l’élé-
ment, pour augmenter l’élasticité de l’élément de distalisation
dans ladite zone et permettre une adaptation
facile et directe.
2. Elément de distalisation segmentaire pour des traitements orthodontiques selon la revendication 1, caractérisé
en ce que la zone de section transversale
réduite présente deux angles successifs (9, 10), le
premier angle (9) se situant entre la portion arquée
centrale de l’élément de distalisation et le pont intermédiaire
à section réduite pour le raccordement sur
la tête destiné à l’adaptation sur la molaire, et le second angle (10) se situant dans le pont intermédiaire
proprement dit avant son raccordement sur ladite

en ce que la zone étranglée du pont pour
le raccordement sur la tête (3) destiné à la fixation
sur la molaire s’obtient en prévoyant une ouverture
ou des ouvertures pour rendre le pont de raccordement
plus souple.

en ce que la zone de plus grande souplesse
(8) consiste en un segment qui est uniquement
incurvé avec une courbure douce (32).

Descrizione
La presente invenzione divulga un elemento per la distalizzazione segmentale di un settore mascellare posteriore canino-molare per trattamenti ortodontici, che fornisce vantaggi significativi rispetto agli elementi attualmente noti per questo scopo.
Sono noti nel ramo elementi ausiliari per la distalizzazione segmentale, ad esempio quelli che corrispondono alla domanda di brevetto spagnolo n. 200102210 al nome dello stesso richiedente.
Con l’elemento ausiliare per la distalizzazione segmentale secondo il brevetto sopra menzionato, al nome dello stesso richiedente, è stata divulgata la formazione bipezzo di detto elemento ausiliare, ovvero un segmento mediano munito di un allargamento inteso per essere fissato al canino tramite un adesivo, e un’estremità di braccio arcuato con una sfera, che viene introdotta in una piccola fessura arcuata ricevente, fissata sul molare superiore tramite un adesivo.
Benché l’invenzione secondo il sopra menzionato brevetto rappresenti un significativo avanzamento rispetto ai dispositivi abitualmente noti, esso presenta specifici problemi relativi alla sua produzione bipezzo, a causa dei costi relativi di produzione e della scomoda manipolazione dell’assemblaggio bipezzo.
L’inventore ha condotto una ricerca per conferire migliorie innovative ai dispositivi di distalizzazione segmentale, il che ha condotto alla presente invenzione.
Come risultato delle ricerche e dei test, l’inventore ha sviluppato il presente dispositivo di distalizzazione, di cui una caratteristica specifica è la costruzione monopezzo, la sua facilità di adattamento, relativa alle sue intrinseche caratteristiche di disegno, e la produzione da un materiale metallico che è preferibilmente del tipo noto come super-elastico o da un materiale termoelastico che esibisce le necessarie caratteristiche di durezza e resilienza opportune.
In generale, il nuovo elemento di distalizzazione segmentale è caratterizzato dal fatto di comprendere un corpo allungato monopezzo, provvisto, a un’estremità, di una porzione terminale che può essere adattata alla superficie esterna del canino e che ha un struttura di fissaggio per i mezzi elastici, mentre l’altra estremità, dopo interposizione di una zona di maggiore flessibilità, possiede una fessura che può essere accoppiata al molare della porzione superiore, preferibilmente tramite un adesivo o altri mezzi.
In una soluzione preferita, la zona di maggiore resilienza sarà costituita da un doppio gomito simultaneo, con un restringimento della sezione trasversale del materiale.
La porzione con doppio gomito e restringimento è caratterizzata dal fatto di possedere due successivi piegature angolari e, allo stesso tempo, un restringimento della sezione trasversale del materiale tra la porzione centrale dell’elemento di distalizzazione e l’estremità o il terminale per l’accoppiamento al molare. Il primo degli angoli, ovvero l’angolo che collega il gomito intermedio all’elemento principale o al ponte arcuato dell'elemento di distalizzazione, sarà preferibilmente un angolo ottuso, mentre il secondo angolo, ovvero l'angolo tra il ponte intermedio di sezione trasversale più piccola e la fessura terminale per l’applicazione al molare, può essere un angolo leggermente ottuso o retto. Sarà inoltre d'interesse per la definizione della struttura dell'elemento di distalizzazione secondo la presente invenzione l’angolo tra l’estremità del braccio centrale del dispositivo e l'asse longitudinale della fessura per il fissaggio al canino.
La zona munita di un restringimento e che presenta due successivi angoli avrà preferibilmente un restringimento significativo, potendo essere formata in modo simile tramite due bracci paralleli sottili, che delineano un'apertura intermedia o una forma similare.
La zona di maggiore flessibilità, che precede l’elemento che può essere adattato al molare, può essere tuttavia prodotto in un’ampia gamma di forme, non necessariamente con un doppio angolo provvisto per la forma esecutiva preferita. Così, ad esempio, al posto di un doppio gomito, è possibile provvedere una zona a dolce curvatura, una zona ad omega, o di forma similare, con una forma aperta o chiusa, oppure una zona con una o più spire in una disposizione simile a una molla elicoidale.

Come mostrato nei disegni, l'elemento di distalizzazione secondo la presente invenzione comprende, monopezzo, un corpo principale 1 dolcemente arcuato di lunghezza sostanziale, che termina ad una estremità con una testa 2 per fissarlo al canino, e all'altra estremità con una testa 3 per fissarlo al molare sullo stesso lato, avente rispettive zone interne curve 4 e 5 per un migliore adattamento al canino e al molare, rispettivamente, e un numero variabile di rilievi come le nervature 6 e 7, profili reticolari, griglie o altri tipi di scabrosità, destinati a migliorare il fissaggio alle succitate parti di dente usando un adesivo.
L’elemento di distalizzazione secondo la presente invenzione ha la caratteristica generale di comprendere preferibilmente una zona più flessibile nella forma di una zona curvata che ha un restringimento, in varie disposizioni. In una forma esecutiva preferita, detta zona più flessibile adotterà la forma di un ponte di collegamento a doppio gomito 8 tra il braccio centrale 1 e la testa d’estremità 3 per l’adattamento al molare. Detto ponte o zona a forma di doppio gomito avrà inoltre una significativa riduzione nella sezione trasversale del materiale, che impartisce una significativa flessibilità alla zona terminale 3, che consentirà un suo migliore adattamento e che permetterà quindi la produzione del segmento di distalizzazione monopezzo.
Il doppio gomito sarà prodotto tramite un primo angolo, preferibilmente ottuso, 9, e un secondo angolo leggermente ottuso o altrimenti retto 10. Oltre a detti angoli, di cui il primo è indicato con la lettera α, sarà inoltre importante l’angolo β, essendo tale angolo formato tra l’asse 1 dell’estremità 12 del braccio 1 e l’asse 13, che corrisponde sostanzialmente all’asse longitudinale della testa 3, che preferibilmente adotta la forma di piccola fessura 14 di costruzione rettangolare od ovale.
Sarà apprezzato che, in luogo delle sopra menzionate quantità relative agli angoli del doppio gomito, i valori degli angoli possono variare entro ampi limiti, secondo i prerequisiti del dispositivo di distalizzazione.
Come menzionato, la zona del doppio gomito, che è indicata con il numerale di riferimento 15, presenta un significativo restringimento, non presente nella porzione centrale 1, che può essere ottenuto mediante restringimento della sezione trasversale, come mostrato nelle Fig. 1 e 2, formando un singolo ponte di collegamento tra l’elemento centrale, o braccio 1, e la testa d’estremità 14, oppure può essere formato tramite una zona di restringimento 17, Fig. 6 e 7, che inoltre presenterà un’apertura centrale 18 che determina i bracci paralleli 19 e 20, la cui forma precisa può variare ampiamente, Fig. 8, che mostra tre versioni in cui la prima, 21, ha due bracci con una sezione trasversale circolare, la seconda, indicata con il numerale di riferimento 22, ha due bracci con una sezione trasversale rettangolare, e la terza, 23, ha due bracci con una sezione trasversale ovale oppure, come sarà apprezzato, in cui la forma associata può variare ampiamente.
Inoltre, nel caso del restringimento nella zona 16, mostrata dalla Fig. 1 a 4, la sezione trasversale può assumere varie forme specifiche, come quelle indicate con i numerali di riferimento 24 per una sezione emibastoncellare, 25 per una sezione circolare, 26 per una sezione sostanzialmente rettangolare o 27 per una sezione ovale, oppure la forma di detta sezione trasversale può altrimenti variare entro ampi limiti.
Le varie viste mostrano la faccia superiore della testa d’estremità 14 con una forma piatta, prendendo in considerazione il fatto che detta forma è intesa per consentire il possibile collegamento di un tubo molare che permette azioni complementari.
Le Fig. da 9 a 17 mostrano varie versioni aggiuntive della zona flessibile tra la porzione dell'elemento centrale del dispositivo e la zona per l'adattamento al molare. Quindi, la Fig. 9 mostra ad esempio la disposizione, tra detta zona centrale 1 e detta zona terminale 3, che può essere adattata al molare, di una zona a curva singola 28 che può avere una forma sinusoidale ad omega o similare.
La Fig. 10 mostra una versione in cui la zona di maggiore flessibilità è costituita da una curva chiusa 29 nella forma ad omega o altra forma.
Le Fig. 1 e 12 mostrano versioni rispettive 30 e 31 nella forma di anse che sono dirette rispettivamente verso la porzione superiore e la porzione inferiore.
Fig. 13 mostra una versione in cui una zona di curva dolce 32 è disposta singolarmente tra la porzione centrale 1 del dispositivo e la testa d’estremità per l’applicazione al molare.
Le Fig. 14 e 15 mostrano rispettive versioni in cui le zone di maggiore flessibilità 33 e 34 sono prodotte tramite rispettivi elementi di forma elicoidale con una pluralità di spire, ovvero nella forma di una molla, rispettivamente dirette verso l'alto e il basso.
Le Fig. 16 e 17 mostrano un’elevazione e una vista in pianta di una forma esecutiva, in cui la zona di maggiore flessibilità 35 è formata da un’ansa, o similare, situata in una disposizione che può essere descritta come laterale in base della convenzione usata nelle precedenti figure, in cui le anse che contemporaneamente con la riduzione della sezione trasversale conferiscono la maggiore flessibilità, sono disposte in un piano perpendicolare a quello delle forme esecutive mostrate nelle precedenti figure. È ovvio notare che la disposizione mostrata nelle Fig. 16 e 17 potrebbe essere presente su ambedue i lati determinati dal piano longitudinale di simmetria. In modo simile, anche se non è mostrato, altri piani che sono intermedi rispetto quello mostrati, potrebbero essere usati per dette anse ed elementi similari.
Sebbene non mostrato nelle figure, l’invenzione consente, allo stesso tempo, la produzione della zona di collegamento flessibile, tra la porzione centrale del segmento di distalizzazione e la testa per il fissaggio al molare, da una parte in metallo costituita di una metallo ampiamente adattabile, come ad esempio acciaio inossidabile. In questo caso, la restringimento in detta zona di maggiore flessibilità può essere omessa, poiché l'adattamento può essere ottenuto tramite legame diretto di detta parte in metallo.
I materiali destinati alla produzione dell’elemento di distalizzazione della presente invenzione possono variare, posto l’ottenimento delle caratteristiche meccaniche, in particolare l'adeguata durezza e resilienza, necessarie per la sua fase operativa. In particolare, può essere fornito un materiale metallico del tipo noto come super-elastico, acciaio, materiali termoplastici o un materiali plastico, ad esempio un polimero.
Materiali super-elastici comprendono quelli noti come Ni-Ti, ad esempio leghe di Cu, Ni, e and Cr.
Rivendicazioni
1. Elemento segmentato di distalizzazione per trattamenti ortodontici, comprendente un elemento monopezzo allungato avente un corpo centrale (1) dolcemente arcuato provvisto, ad una estremità, di una testa (2) per l'adattamento al canino e, all'altra estremità, di una testa (3) per l'adattamento al molare, caratterizzato dal fatto che il corpo arcuato (1) è collegato alla testa d'estremità (3) attraverso una zona di maggiore flessibilità (8) con una sezione trasversale più piccola del resto dell'elemento, così da aumentare la resilienza dell'elemento di distalizzazione in detta zona, e da ottenere un adattamento facile e diretto.
2. Elemento segmentato di distalizzazione per trattamenti ortodontici secondo la rivendicazione 1, caratterizzato dal fatto che la zona di sezione trasversale ridotta ha due angoli successivi (9, 10), il primo angolo (9) essendo tra la porzione arcuata centrale dell'elemento di distalizzazione e il ponte intermedio a sezione ridotta di collegamento alla testa per l'adattamento al molare, e il secondo angolo (10) essendo nel ponte intermedio vero e proprio, prima del suo collegamento a detta testa (3) per l'adattamento al molare.

Elemento segmentato di distalizzazione per trattamenti ortodontici secondo la rivendicazione 1, caratterizzato dal fatto che la riduzione nella sezione trasversale del ponte di collegamento alla testa d'estremità (3) per l'accoppiamento al molare è ottenuta provvedendo un singolo ponte di sezione trasversale ridotta.
4. Elemento segmentato di distalizzazione per trattamenti ortodontici secondo la rivendicazione 1, caratterizzato dal fatto che la zona ristretta del ponte di collegamento alla testa (3) per il fissaggio al molare è ottenuta provvedendo una apertura o più aperture allo scopo di rendere più flessibile il ponte di collegamento.
5. Elemento segmentato di distalizzazione per trattamenti ortodontici secondo la rivendicazione 1, caratterizzato dal fatto che la zona di maggiore flessibilità (8) è costituita da un segmento a curva singola di curvatura dolce (32).
6. Elemento segmentato di distalizzazione per trattamenti ortodontici secondo la rivendicazione 1, caratterizzato dal fatto che la zona di maggiore flessibilità (8) è costituita da un'ansa chiusa (29, 30, 31, 35).

Description
FIELD OF THE INVENTION
[0001] This invention relates to glucagon- like peptide 2 (GLP- 2) derivatives. In particular, this invention relates to GLP2
peptide derivatives having an extended in vivo half-life, for the treatment or prevention of gastrointestinal disorders or
diseases such as inflammatory bowel disease and other gastrointestinal functions, from any segment of the gastrointestinal
tract, from the oesophagus to the anus.
BACKGROUND OF THE INVENTION
[0002] GLP- 2 is a 33 amino acid peptide expressed in a tissue- specific manner from the pleiotropic proglucagon gene,
and thus part of the glucagon super- family of peptide hormones. Alternative post- translational processing of proglucagon
occurs in pancreas, intestine and brain. Enzymatic cleavages in proglucagon produce numerous multifunctional peptide
hormones involved in nutrient metabolism. The major bioactive hormones derived from proglucagon are in the pancreatic
a- cells, and GLP- 1 and GLP- 2 in the intestinal L- cells and brain. It was first discovered to possess potent intestinotropic
properties by Drucker et al. (see Proc. Natl. Acad. Sci., 1996. 93, (115), 7911-7916). GLP- 2, as a natural intestinalderived
peptide, has been demonstrated to have a significant reparative activity for the mucosal epithelium of the small
and large intestine. It has also been demonstrated to increase the ability of the intestine to digest and absorb nutrients,
suggesting a potential therapeutic role in the treatment of intestinal insufficiency. Indeed, several studies have now
confirmed that GLP- 2 administration reduces or prevents intestinal damage in rodent models of colitis, enteritis, total
parenteral nutrition and massive resection. Very recently, phase 2 clinical trials of GLP- 2 have also been reported, in
which patients with short bowel syndrome were demonstrated to exhibit an enhanced ability to absorb enteral nutrients
after 30 days of GLP- 2 administration, with apparently no undesirable side effects.
[0003] The principal metabolic pathway for GLP- 2 clearance is through enzymatic degradation. GLP- 2 has been shown
to be rapidly degraded through the removal of its two N- terminal amino acids by dipeptidylpeptidase- IV (DPP- IV), which
represents a major limitation because it leads to the complete inactivation of the peptide. As a result, the half- life of GLP2
is thus quite short, and current GLP- 2 treatment necessitates infusion or frequent injections. Renal clearance has also
been shown to be involved in the clearance of GLP- 2. The major action of GLP- 2 involves stimulation of cell growth,
and the mechanism coupling GLP- 2 receptor activation, directly or indirectly, to cell proliferation has not been examined.
[0004] It has been shown that peptide analogs of native GLP- 2 possess enhanced trophic activity at the small intestine
as GLP- 2 receptor agonists (see for example US 5,990,077).
[0005] Although very useful, a critical disadvantage of GLP- 2 peptides and analogs, as stated above, is their very
short half- lives in vivo, which is typically not more than 2 minutes.
[0006] Inflammatory Bowel Disease (IBD) is a group of chronic disorders that cause inflammation or ulceration in the
small and large intestines. It may even be life threatening, and there is currently no known cure. IBD commonly refers
to ulcerative colitis (UC), limited to the colon and Crohn’s disease (CD) which can involve the entire gastrointestinal
tract, resulting in a chronic cycle of remissions and flares. Many pharmaceutical products are known to treat IBD, for
example to suppress inflammation, to prevent flare- ups, to control symptoms such as pain or diarrhea, or to replace or
supplement essential nutrients that are poorly absorbed because of extensive disease or surgery. Current treatment
options include a wide variety of pharmaceutical products like aminosalicylates, corticosteroids, immune modulators,
and anti- TNF- a agent, and are designed to reduce inflammation and relieve symptoms in addition to replacing lost fluids
and nutrients. Most of these products however have a limited use in IBD because of their undesirable side effects on
the body in general.
[0007] Approximately one million patients are treated for IBD every year in the United States and Europe, most of
them generally suffering from either Crohn’s disease or ulcerative colitis, In fact, it is not uncommon for subjects suffering
from IBD to undergo radical surgery involving the removal of major parts of the intestine. Direct annual healthcare costs
of IBD are approximately $US 700 million, and the total economic impact of both direct and indirect costs approximate
between $2 and $3 billion a year worldwide. Existing treatments, while often providing relief, have some shortcomings.
[0008] US 5,789,379 teaches GLP- 2 analogs among which one has been developed as a long- acting compound (ALX600™) and is currently in clinical trials. However, even if DPP- IV degradation of GLP- 2 appears to be somehow prevented,
its half- life remains limited by renal clearance, and does not exceed 2 minutes, as stated above.
[0009] A chimeric antibody (Remicade™ has been developed to bind specifically to human tumor necrosis factor alpha
(TNF- a) for the short- term treatment of Crohn’s. This antibody is indicated for the reduction of the symptoms of moderate
to severe Crohn’s disease in patients who have had an inadequate response to conventional therapy with corticosteroids,
other immunosuppressants and/or antibiotics. Nevertheless, serious side effects are observed with such treatment. For
example, it has been associated with hypersensitivity reaction, serious infections including sepsis, as well as fatal
infections. Its administration could further predispose patients to infections through TNF- blocking.

[0010] With the prevalence of IBD increasing in recent years, it would therefore be highly desirable to develop GLP2 peptide derivatives or analogs capable of substantially maintaining the same level of activity, low toxicity and therapeutic
advantages as GLP- 2, but with a much longer in vivo half-life, thus avoiding the necessity for continuous administration
thereof in the treatment of various diseases such as inflammatory bowel disease, Crohn’s disease and ulcerative colitis
representing the two major inflammatory bowel diseases.
[0011] WO 00/69900 describes protection of endogenous therapeutic peptides from peptidase activity through conjugation
to blood components.
SUMMARY OF THE INVENTION
[0012] In accordance with the present invention, there is now provided a GLP- 2 gastrointestinal tissue growth promoter
derivative having an extended in vivo half-life when compared with the corresponding unmodified GLP-2 gastrointestinal
growth promoter. The present invention provides a gastrointestinal tissue growth promoter derivative selected from the
group consisting of:

where MPA is maleimidopropionic acid and AEEA is 2-[2-(2- amino) ethoxy] ethoxy acetic acid. The GLP- 2 derivative
comprises a reactive entity (MPA) coupled thereto and capable of reacting with available functionalities on a blood
component, either in vivo or ex vivo, to form a stable covalent bond. The covalent bonding formed between the GLP-2
derivative and the blood component prevents undesirable cleavage of the GLP- 2 by enzymes such as dipeptidylpeptidase
IV, thereby extending its in vivo half-life and activity.
[0013] Preferred blood components comprise proteins such as immunoglobulins, including IgG and IgM, serum albumin,
ferritin, steroid binding proteins, transferrin, thyroxin binding protein, a- 2- macroglobulin, haptoglobin etc., serum
albumin and IgG being more preferred, and serum albumin being the most preferred.
[0014] The reactive entity, maleimidopropionic acid (MPA), is capable of forming a covalent bond with the blood
component by reacting with thiol groups present thereon, either in vivo or in vitro (or ex vivo).
[0015] In another aspect of the invention, there is provided a pharmaceutical composition comprising the present GLP2
gastrointestinal tissue growth promoter derivative in combination with a pharmaceutically acceptable carrier. Such
composition is useful for the treatment or prevention of bowel disorders or diseases such as inflammatory bowel disease
and other gastrointestinal functions. The composition may also be used for gene therapy to induce cells to endogenously
produce the gastrointestinal tissue growth promoter peptide derivative that may then be implanted in a subject to produce
the desired biological effect. Finally, the composition may also be used for manufacturing pharmaceutical or veterinary
compositions for the enhancement of large intestine tissue growth.
[0016] In a further embodiment of the present invention, there is provided the present GLP- 2 gastrointestinal tissue
growth promoter derivative or a conjugate thereof for use in the treatment or prevention of bowel disorders or diseases
such as inflammatory bowel disease, and gastrointestinal functions in a subject, preferably a mammal, animal or human.
[0017] In a further aspect of the present invention, there is provided a conjugate comprising the present GLP- 2 gastrointestinal
tissue growth promoter derivative covalently bonded to a blood component.
[0018] A method is described for extending the in vivo half-life of a GLP-2 gastrointestinal tissue growth promoter in
a subject, the method comprising covalently bonding the GLP- 2 gastrointestinal tissue growth promoter derivative to a
blood component. The covalent bonding may take place in vivo or in vitro.
[0019] The gastrointestinal tissue growth promoter compounds are peptides which are GLP- 2 analogs which possesses
gastrointestinal tissue growth promoting activity. Details of the sequences of these analogs are illustrated below.
[0020] A further use of the present compound derivative may be the determination of the intestinotrophic activity of a
hormone when used in combination with the present compound derivative, and particularly when the compound is a
GLP- 2 analog or a GLP- 2 fragment. Such method comprises the steps of: (a) coadministering the hormone with an
intestinotrophic amount of the GLP- 2 derivative to a test subject; (2) assessing the subsequent growth of small and large
intestine tissue in the test subject; and (3) determining whether the growth of small and/or large intestine tissue in the
test subject is enhanced relative to control subjects treated with unmodified GLP- 2 analog or GLP- 2 derivative.

DETAILED DESCRIPTION OF THE INVENTION
[0023] In vivo bioconjugation is the process of covalently bonding a molecule, such as the present gastrointestinal
tissue growth promoter compound derivative, within the body, to target blood components, preferably proteins, in a
manner that permits the substantial retention, or increase in some instances, of the biological activity of the original
unmodified GLP- 2 gastrointestinal tissue growth promoter peptide therein, while providing an extended duration of the
biological activity though giving the GLP- 2 derivative the biophysical parameters of the target blood component,
[0024] For the purposes of the present invention, the terms "analog" or "fragment" are meant to include amino acid
sequences comprising peptides with different amino acid sequences from the native sequence, such as the GLP- 2
sequence, but with similar or comparable activity. Such analogs preferably have an amino acid sequence at least 60%,
and more preferably at least 80%, and most preferably at least 95% the same as that of either GLP- 2 or a fragment of
GLP- 2 having the same number of amino acid residues.
[0025] The present gastrointestinal tissue growth promoter peptide derivative comprise a GLP- 2 gastrointestinal tissue
growth promoter peptide that has been modified by coupling thereto a reactive entity, either directly or via a linking group,
the reactive entity being capable of forming a covalent bond with a blood component, preferably blood proteins. The
reactive entity is stable in an aqueous environment, and is MPA. The covalent bond is formed between the reactive
entity and a thiol group on the blood component. The preferred blood component comprises mobile blood components
like serum albumin, immunoglobulins, or combinations thereof. In a most preferred embodiment, the blood component
is serum albumin.
[0026] The blood components are preferably mobile, which means that they do not have a fixed situs for any extended
period of time, generally not exceeding 5 minutes, and more usually one minute. These blood components are not
membrane- associated and are present in the blood for extended periods of time in a minimum concentration of at least
0.1 Pg/mL Preferred mobile blood components include serum albumin, transferrin, ferritin and immunoglobulins such
as IgM and IgG. The half- life of mobile blood components is at least about 12 hours.

in the field of peptide synthesis, and can be generically described as chemical moieties capable of protecting the peptide
derivative from reacting with other functional groups. Various protective groups are available commercially, and examples
thereof can be found in US 5,493,007 which is hereby incorporated by reference. Typical examples of suitable protective
groups include acetyl, fluorenylmethyloxycarbonyl (FMOC), t- butyloxycarbonyl (BOC), benzyloxycarbonyl (CBZ), etc.

[0034] The effect on growth elicited by the present GLP- 2 derivatives manifests as an increase in small bowel weight,
relative to a mock- treated control. In particular, the present GLP- 2 derivatives are considered to have "intestinotrophic"
activity if, when assessed in the murine model exemplified herein, the analog mediates an increase in small bowel weight
of at least 10% relative to a control animal receiving vehicle alone. Particularly suitable for therapeutic use are those
derivatives which mediate an increase of at least 20% in small bowel weight, while those more preferred mediate an
increase in small bowel weight of 50% or more. Intestinotrophic activity is noted most significantly in relation to the
jejunum, including the distal jejunum and particularly the proximal jejunum, and is also noted in the ileum.
[0035] In addition to exhibiting intestinotrophic activity, the present GLP- 2 derivatives incorporate an amino acid substitution
within a GLP- 2 peptide "backbone", which is a variant of a mammalian GLP- 2 species in which the C- terminus
has been altered by addition of a basic residue. Thus, the present peptide derivatives incorporate a lysine residue added
at the C- terminal of the GLP- 2 peptide sequence.

kept between 6.5 and 7.4. At pH 7.0, the rate of reaction of maleimido groups with sulfhydryls is 1000- fold faster than

cleaved under physiological conditions.
[0038] The gastrointestinal tissue growth promoter derivatives of the invention provide for specific labeling of blood
components. Such specific labeling, particularly with a maleimide, offers several advantages. Free thiol groups are less
abundant in vivo than amino groups, and as a result, maleimide derivatives covalently bond to fewer proteins. For
example, in serum albumin, there is only one free thiol group per molecule. Thus, a GLP- 2 - maleimide - albumin conjugate
will tend to comprise a 1: 1 molar ratio of peptide to albumin. In addition to albumin, IgG molecules (class II) also have
free thiols. Since IgG molecules and serum albumin make up the majority of soluble proteins in the blood, i.e., 99%,
they also make up the majority of the free thiol groups available to covalently bond to a maleimide- substituted GLP- 2.
[0039] Further, even among free thiol- containing blood proteins, specific labeling with a maleimide leads to the preferential
formation of peptide maleimide- albumin conjugates, - due to the unique characteristics of albumin itself. The
single free thiol group of albumin, highly conserved among species, is located at amino acid residue Cys 34 . It has been
demonstrated recently that the Cys 34
of albumin has an increased reactivity relative to free thiols on other free thiolcontaining
proteins. This is due in part to the unusual pK value of 5.5 for the Cys 34
of albumin. This is much lower than
typical pK values for cysteines residues in general, which are typically about 8. Due to this low pK, under normal
physiological conditions, Cys 34
of albumin is predominantly in the anionic form, which dramatically increases its reactivity.
In addition to the low pK value of Cys 34 , another factor which enhances the reactivity of Cys 34
is its location, which is in
a hydrophobic pocket close to the surface of one loop of region V of albumin. This location makes Cys 34
accessible to
ligands of all kinds, and is an important factor in Cys 34
biological role as free radical trap and free thiol scavenger. As a
result, the reaction rate acceleration can be as much as 1000- fold relative to rates of reaction of peptide- maleimides
with other free- thiol containing proteins.
[0040] Another advantage of peptide- maleimide- albumin conjugates is the reproducibility associated with the 1: 1
loading of peptide to albumin specifically at Cys 34 . Conventional activation techniques, such as with glutaraldehyde,
DCC, EDC and other chemical activators of, for example, free amines, lack this selectivity. For example, albumin contains
52 lysine residues, 25-30 of which are located on the surface of albumin and accessible for conjugation. Activating these
lysine residues, or alternatively modifying a peptide to couple through these lysine residues, results in a heterogeneous
population of conjugates. Even if an equimolar ratio peptide: albumin (i.e., 1: 1) is employed, the end result is the production
of random conjugation products, some containing an indefinite number of peptides linked to each molecule of albumin,
and each conjugate having peptides randomly coupled at any one of the 25-30 available lysine sites. Consequently,
characterization of the exact composition is virtually impossible, not to mention the absence of reproducibility. Additionally,
while it would seem that conjugation through lysine residues of albumin would at least have the advantage of delivering
more therapeutic agent per albumin molecule, studies have shown that a 1: 1 ratio of therapeutic agent to albumin is
preferred. In an article by Stehle, et al. in Anti- Cancer Drugs, 1997, 8, 677-685, it is reported that a 1: 1 ratio of the anticancer
methotrexate to albumin conjugated via glutaraldehyde gave the most promising results. These conjugates were

taken up by tumor cells, whereas conjugates bearing 5: 1 to 20: 1 methotrexate molecules had altered HPLC profiles and
were quickly taken up by the liver in vivo. It would therefore seems that at higher ratios, the effectiveness of albumin as
a carrier for a therapeutic agent is diminished.
[0041] Through controlled administration of the present GLP- 2 peptide derivative, and particularly a GLP- 2 peptide
comprising a maleimide reactive entity, specific in vivo labeling or bonding of albumin and IgG can be controlled. In
typical administrations, it has been shown that 80-90% of the administered peptide derivative bonds to albumin and less
than 5% bonds to IgG. Trace bonding of free thiols present, such as glutathione, also occurs. Such specific bonding is
preferred for in vivo use as it permits an accurate calculation of the estimated half-life of the therapeutic agent administered.
[0042] Maleimide- substituted GLP- 2 peptides are generally quite stable in the presence of aqueous solutions and in
the presence of free amines. Because maleimide- substituted GLP- 2 peptides react with free thiols, protective groups
are not necessary to prevent them from reacting with themselves.
[0043] As stated above, the desired conjugates of GLP- 2 derivatives to blood components may be prepared in vivo
by administration of the derivatives directly to the subject, which may be an animal or a human. The administration may
be done in the form of a bolus, or introduced slowly over time by infusion using metered flow or the like.
[0044] Alternately, the conjugate may also be prepared ex vivo by combining blood or commercially available purified
blood components with the present GLP- 2 derivative, allowing covalent bonding of the GLP- 2 derivative to the functionalities
on blood components, and then returning or administering the conjugated blood or conjugated purified blood
component to the host. Moreover, the above may also be accomplished by first purifying an individual blood component
or limited number of components, such as red blood cells, immunoglobulins, serum albumin, or the like, and combining
the component or components ex vivo with the present compound derivative. The labeled blood or blood component
may then be returned to the subject to provide in vivo the therapeutically effective conjugates. The blood also may be
treated to prevent coagulation during handling ex vivo.
Peptide Synthesis
[0045] GLP- 2 peptides may be synthesized by standard methods of solid phase peptide chemistry well known to any
one of ordinary skill in the art. For example, the peptide may be synthesized by solid phase chemistry techniques following
the procedures described by Steward et al. in Solid Phase Peptide Synthesis, 2nd Ed., Pierce Chemical Company,
Rockford, Ill., (1984.) using a Rainin PTI Symphony™ synthesizer. Similarly, peptides fragments may be synthesized
and subsequently combined or linked together to form a larger peptide (segment condensation). These synthetic peptide
fragments can also be made with amino acid substitutions and/or deletion at specific locations.

to an inert solid support or utilized in solution by adding the next amino acid in the sequence having the complimentary
(amino or carboxyl) group suitably protected and under conditions suitable for forming the amide linkage. The protecting
group is then removed from this newly added amino acid residue and the next amino acid (suitably protected) is added,
and so forth.

CAMPO DELL'INVENZIONE
Questa invenzione riguarda derivati del peptide glucagone-simile 2 (GLP-2). In particolare, questa invenzione riguarda derivati del peptide GLP-2 aventi un’emivita in vivo prolungata, per il trattamento o la prevenzione di disordini e malattie gastrointestinali, come la malattia infiammatoria delle viscere e altre funzioni gastrointestinali, per qualsiasi segmento del tratto gastrointestinale, dall'esofago all'ano.
SFONDO DELL'INVENZIONE
GLP-2 è un peptide di 33 amminoacidi espresso in modo tessuto-specifico dal gene pleiotropico proglucagone, e quindi appartenente alla super-famiglia di ormoni peptidici del glucagone. Un’elaborazione post-traduzionale alternativa del proglucagone avviene nel pancreas, intestino e cervello. Scissioni enzimatiche del proglucagone producono numerosi ormoni peptidici multifunzionali, implicati nel metabolismo dei nutrienti. Gli ormoni bioattivi principali derivati proglucagone sono nelle cellule α pancreatiche, e GLP-1 e GLP-2 nelle cellule L intestinali e nel cervello. Per primi Drucker et al hanno scoperto che il proglucagone possiede potenti proprietà intestinotropiche. (vedere Proc. Natl. Acad. Sci., 1996. 93, (115), 7911-7916). GLP-2, in quando peptide naturale di derivazione intestinale, è stato dimostrato avere una significativa attività riparativa nei confronti dell’epitelio mucoso dell’intestino tenue e crasso. È stato inoltre dimostrato incrementare la capacità dell’intestino di digerire e assorbire nutrienti, suggerendo un potenziale ruolo terapeutico nel trattamento di insufficienza intestinale. Infatti, numerosi studi hanno ora confermato che GLP-2 riduce o previene una lesione intestinale in modelli di colite, enterite, nutrizione parenterale totale e resezione massiva nei roditori. Molto recentemente sono state riportate prove cliniche di fase 2 di GLP-2, in cui è stato dimostrato che pazienti con sindrome da intestino corto esibiscono una potenziata capacità di assorbire nutrienti enterici dopo 30 giorni di somministrazione di GLP-2, apparentemente senza effetti collaterali indesiderati.
La principale via metabolica di smaltimento di GLP-2 avviene attraverso degradazione enzimatica. È stato riportato che GLP-2 è rapidamente degradato attraverso la rimozione dei suoi due amminoacidi N-terminali da parte della dipeptidilpeptidasi-IV (DPP-IV), questo evento rappresenta una grande limitazione perché porta alla completa inattivazione del peptide. Di conseguenza, l'emivita di GLP-2 risulta essere quindi piuttosto corta, e l'attuale trattamento con GLP-2 rende necessaria l'infusione o frequenti iniezioni. La clearance renale è stata inoltre riportata per essere implicata nello smaltimento di GLP-2. È stato mostrato che analoghi peptidici di GLP-2 nativo possiedono una potenziata attività trofica nell’intestino tenue sotto forma di antagonisti del recettore di GLP-2 (vedere ad esempio US 5.990.077).
Nonostante la loro notevole utilità, uno svantaggio critico dei peptidi e analoghi di GLP-2, come sopra menzionato, consiste nelle loro brevi emivite in vivo, che sono tipicamente non superiori a 2 minuti.
La malattia infiammatoria delle viscere (IBD, inflammatory Bowel Disease) rappresenta un gruppo di disordini cronici che causano l’infiammazione e l’ulceramento dell’intestino tenue e crasso. Può anche rappresentare una minaccia per la vita e, attualmente, non è nota alcuna cura. Comunemente, IBD fa riferimento alla colite ulcerosa (UC), limitata al colon e al morbo di (CD, Crohn's disease), che può coinvolgere l’intero tratto gastrointestinale portando a un ciclo cronico di remissioni e infiammazioni. Sono noti molti prodotti farmaceutici nel trattamento di IBD, ad esempio per sopprimere l’infiammazione, prevenire irritazioni, controllare sintomi come il dolore o la diarrea oppure sostituire o integrare nutrienti essenziali che sono scarsamente assorbiti a causa di una malattia avanzata o di un intervento chirurgico. Attuali opzioni di trattamento comprendono un'ampia varietà di prodotti farmaceutici quali amminosalicilati, corticosteroidi, immunomodulatori e agenti anti-TNF-α, e sono concepiti per ridurre l’infiammazione e alleviare i sintomi, oltre alla sostituzione di fluidi e nutrienti persi. La maggior parte di questi prodotti trovano un uso limitato in IBD considerati i loro effetti collaterali indesiderabili sul corpo in generale.
Circa un milione di persone vengono trattate per IBD ogni anno negli Stati Uniti e in Europa, la maggior parte della quali affette da morbo di Crohn o colite ulcerosa. Infatti, è comune per i soggetti affetti da IBD di essere sottoposti a un radicale intervento chirurgico implicante la rimozione di parti importanti dell'intestino. I costi sanitari diretti annuali di IBD sono circa 700 milioni dollari, e l’impatto economico totale, sia dei costi diretti che di quelli indiretti, si avvicinano ai 2-3 miliardi di dollari all’anno in tutto il mondo. I trattamenti esistenti, se da una parte forniscono un sollievo, presentano alcuni inconvenienti.
US 5.789.379 descrive analoghi di GLP-2, tra cui è stato sviluppato un analogo come composto di lunga durata (ALX-600™) che è attualmente in fase di prove cliniche. Comunque, anche se la degradazione mediata da DPP-IV di GLP-2 sembra essere in un certo modo evitata, la sua emivita rimane limitata dalla clearance renale, e non supera i 2 minuti, come sopra riportato.
Un anticorpo chimerico (Remicade™) è stato sviluppato per legarsi specificamente al fattore di necrosi tumorale alfa umano (TNF-α) per il trattamento a breve termine del morbo di Crohn. L’anticorpo è indicato per la riduzione dei sintomi di un morbo di Crohn, da moderato a severo, in pazienti che hanno avuto una inadeguata risposta alla terapia convenzionale con corticosteroidi, altri immunosoppressivi e/o antibiotici. Tuttavia con tale trattamento si sono osservati seri effetti collaterali. È stato ad esempio associato con reazioni di ipersensibilità, serie infezioni, sepsi inclusa, come pure infezioni fatali. La sua somministrazione potrebbe inoltre predisporre i pazienti ad infezioni ascrivibili al blocco di TNF.
Con una prevalenza di IBD, in aumento negli ultimi anni, sarebbe pertanto altamente desiderabile sviluppare derivati o analoghi del peptide GLP-2 in grado di mantenere sostanzialmente lo stesso livello di attività, bassa tossicità e vantaggi terapeutici di GLP-2, tuttavia con una emivita in vivo maggiore, evitando quindi la necessità di una sua continua somministrazione nel trattamento di varie malattie come malattia infiammatoria delle viscere, morbo di Crohn e colite ulcerosa, che rappresentano la due principali malattie infiammatorie delle viscere.
WO 00/69900 descrive una protezione di peptidi terapeutici endogeni da un’attività peptidasica attraverso la coniugazione a componenti ematici.
RIEPILOGO DELL'INVENZIONE
Secondo la presente invenzione, è ora fornito un derivato del promotore di crescita del tessuto gastrointestinale GLP-2, avente un'emivita in vivo prolungata rispetto al corrispondente promotore di crescita gastrointestinale GLP-2 non modificato. La presente invenzione fornisce un derivato di promotore di crescita del tessuto gastrointestinale, selezionato dal gruppo costituito da:

dove MPA è acido maleimmidopropionico ed AEEA è acido 2-[2-(2-ammino)etossi]etossi acetico. In un’altra forma esecutiva della presente invenzione, è fornito il presente derivato di promotore di crescita del tessuto gastrointestinale GLP-2, o un suo coniugato, da usare nel trattamento o prevenzione di malattie intestinali o malattie come la malattia infiammatoria delle viscere, e funzioni gastrointestinali in un soggetto, preferibilmente un mammifero, animale o essere umano.
In un altro aspetto della presente invenzione, è fornito un coniugato comprendente il presente derivato di promotore di crescita del tessuto gastrointestinale GLP-2 legato in modo covalente a un componente ematico.
È descritto un metodo per estendere l’emivita in vivo di un promotore di crescita del tessuto gastrointestinale GLP-2 in un soggetto, il metodo comprendente il passaggio di legare in modo covalente il derivato di promotore di crescita del tessuto gastrointestinale GLP-2 a un componente ematico. Il legame covalente può avvenire in vivo o in vitro.
I composti promotori di crescita del tessuto gastrointestinale sono peptidi, che sono analoghi di GLP-2, che possiedono attività di promozione della crescita del tessuto gastrointestinale. Dettagli delle sequenze di questi analoghi sono illustrati a seguire.
Un altro uso del presente derivato di composto può essere la determinazione dell’attività intestinotrofica di un ormone quando usato in combinazione con il presente derivato di composto e, in particolare, quando il composto è un analogo di GLP-2 o un frammento di GLP-2. Tale metodo comprende i passaggi di: (a) cosomministrare l’ormone con una quantità intestinotrofica del derivato GLP-2 a un soggetto di prova; (2) valutare la seguente crescita di tessuto dell’intestino tenue o crasso nel soggetto di prova; e (3) determinare se la crescita dell’intestino tenue e/o crasso nel soggetto di prova viene potenziata, rispetto a soggetti di controllo trattati con un analogo di GLP-2 o un derivato di GLP-2 non modificato.
Se è presente un gruppo legante, esso è acido 2-[2-(2-ammino)etossi]etossi acetico, AEEA.
NEI DISEGNI
La figura 1 illustra le variazioni di peso umido dell’intestino tenue in topi trattati con soluzione salina o 5 µg dei composti degli esempi 1-8, due volte al giorno per 10 giorni (n = 10/gruppo). I risultati sono espressi come media della variazione del peso rispetto al controllo ± SEM.
La figura 2 illustra variazioni di peso umido dell’intestino tenue e dell’intestino crasso in topi trattati con soluzione salina o 5, 25 o 50 µg g dei composti degli esempi 5, 7 e 8, due volte al giorno per 10 giorni (n = 10/gruppo). I risultati sono espressi come peso medio ± SEM.
La figura 3 illustra le concentrazioni plasmatiche medie per i composti degli esempi 5 e 8 in ratti Sprague-Dawley dopo una singola dose intravenosa o una singola dose sottocutanea di 500 nmoli/kg (n = 4 / gruppo).
La figura 4 illustra il rilevamento del composto dell’esempio 8 coniugato a proteine plasmatiche di ratto con l’uso di un anticorpo policlonale, anticorpo anti-GLP-2, e un confronto con il modello ottenuto con un anticorpo anti-albumina di ratto.
DESCRIZIONE DETTAGLIATA DELL'INVENZIONE
La bioconiugazione in vivo è il processo attraverso il quale una molecola, come il presente derivato di composto promotore di crescita del tessuto gastrointestinale, si lega in modo covalente all’interno del corpo a componenti ematici bersaglio, preferibilmente proteine, in una maniera tale da permettere una sostanziale conservazione o, i alcune circostanze, un sostanziale aumento dell'attività biologica del peptide di promotore di crescita del tessuto gastrointestinale GLP-2 originale, fornendo al contempo una durata estesa dell’attività biologica conferendo al derivato di GLP-2 i parametri biofisici del componente ematico bersaglio.
Per gli scopi della presente invenzione i termini "analogo" o "frammento" sono intesi per comprende sequenze di amminoacidi, tra cui peptidi, con sequenze di amminoacidi differenti dalla sequenza nativa, come la sequenza di GLP-2, che siano tuttavia dotati di un’attività simile o paragonabile. Tali analoghi hanno preferibilmente una sequenza di amminoacidi almeno 60% e più preferibilmente almeno 80%, e con preferenza assoluta almeno 95% uguale a quella di GLP-2 o di un frammento di GLP-2 avente lo stesso numero di residui di amminoacido.
Il presente derivato del peptide promotore di crescita del tessuto gastrointestinale comprende un peptide di promotore di crescita del tessuto gastrointestinale GLP-2, che è stato modificato accoppiandolo con un'entità reattiva, in modo diretto o tramite un gruppo legante, l’entità reattiva essendo in grado di formare un legame covalente con un componente ematico, preferibilmente proteine ematiche. L’entità reattiva è stabile in un ambiente acquoso, ed è MPA. Il legame covalente è formato tra l’entità reattiva e un gruppo tiolo sul componente ematico. Il componente ematico preferito comprende componenti ematici mobili, quali albumina di siero, immunoglobuline o loro combinazioni. In una forma esecutiva preferita, il componente ematico è albumina di siero.
I componenti ematici sono preferibilmente mobili, il che intende che non possiedono un sito fisso per qualsivoglia periodo di tempo prolungato, in genere non superiore ai 5 minuti, e più usualmente un minuto. Questi componenti ematici non sono associati alla membrana e sono presenti nel sangue per periodi prolungati di tempo in una concentrazione minima di almeno 0,1 µg/mL. Componenti ematici mobili preferiti comprendono albumina di siero, transferrina, ferritina e immunoglobuline, come IgM e IgG. L’emivita dei componenti ematici mobili è almeno 12 ore circa.
Il presente derivato di promotore di crescita del tessuto gastrointestinale è un peptide GLP-2, e suoi gruppi protettivi possono essere richiesti durante il processo di sintesi del derivato di GLP-2. Nel campo tali gruppi protettivi sono di uso convenzionale e possono essere generalmente descritti come porzioni chimiche in grado di proteggere il derivato peptidico dalla reazione con altri gruppi funzionali. Vari gruppi protettivi sono reperibili in commercio, e loro esempi possono essere ritrovati in US 5.493.007, che è incorporato in questa sede tramite rimando. Esempi tipici di gruppi protettivi adatti comprendono acetile, fluorenilmetilossicarbonile (FMOC), t-butilossicarbonile (BOC), benzilossicarbonile (CBZ), ecc. La tabella 1 fornisce le abbreviazioni per gli amminoacidi sia a tre lettere che a una lettera.

Il derivato di GLP-2 forma un peptide stabile alle peptidasi dopo coniugazione ad un componente ematico, vale a dire un’albumina. È inoltre contemplato che uno o più amminoacidi possono essere aggiunti o sostituiti nel peptide prima dell’aggiunta dell’entità reattiva, facilitando il suo accoppiamento al peptide. Tale aggiunta o sostituzione può essere condotta all'N-terminale, il C-terminale, o tra i due estremi. Il derivato peptidico, così ottenuto, può essere somministrato a un soggetto, animale o essere umano, in modo tale che la coniugazione con i componenti ematici avvenga in vivo, oppure esso può essere prima coniugato ai componenti ematici del soggetto, animale o essere umano, in vitro, e il risultante coniugato o peptide stabilizzato alle peptidasi, come definito a seguire, può essere somministrato al soggetto.
Qualsiasi amminoacido, presente, sostituito o aggiunto alla sequenza di GLP-2, possono essere D-amminaocidi o L-amminoacidi, oppure loro combinazioni. In genere si preferiscono gli L-amminoacidi. I derivati della presente invenzione hanno una sostituzione di glicina nella posizione 2 della sequenza nativa di GLP-2, poiché conferisce all’analogo una maggiore resistenza all’enzima DPP-IV.
Un derivato di GLP-2 stabilizzato alle peptidasi è più stabile in presenza di peptidasi in vivo rispetto al corrispondente analogo di GLP-2 non stabilizzato. La stabilità alle peptidasi è determinata confrontando l'emivita dell’analogo di GLP-2 nativo, in siero o sangue, con l’emivita del corrispondente derivato contenente l’entità reattiva in siero o sangue. L’emivita viene determinata campionando il siero o sangue dopo somministrazione del derivato o del peptide non modificato, e determinando l'attività di ciascun composto.
In maggiore dettaglio, la presente invenzione è diretta alla modifica di GLP-2, e suoi analoghi e frammenti, potenziando la sua biodisponibilità, prolungando la sua emivita in vivo e la sua distribuzione attraverso la selettiva coniugazione a un componente ematico mantenendo al contempo in modo sostanziale o potenziando le sue rimarcabili proprietà terapeutiche.
È noto che il GLP-2 umano ha la seguente sequenza:
His-Ala-Asp-Gly-Ser-Phe-Ser-Asp-Glu-Met-Asn-Thr-Ile-Leu-Asp-Asn-Leu-Ala-Ala-Arg-Asp-Phe-Ile-Asn-Trp-Leu-Ile-Gln-Tbr-Lys-Ile-Thr-Asp.
L’invenzione riguarda usi terapeutici, e usi correlati, di derivati di GLP-2, aventi una prolungato emivita in vivo, con particolare riferimento a
- promuove la crescita del tessuto dell’intestino tenue e/o crasso;
- aumentare i livelli ematici di un derivato di GLP-2;
- ripristinare o mantenere la funzione gastrointestinale;
- promuovere la guarigione o ricrescita di una mucosa intestinale lesionata o ulcerata/infiammata;
- ridurre il rischio di una malattia enterica;
- potenziare lo stato nutrizionale;
- trattare o prevenire disordini o malattie nutrizionali o gastrointestinali;
- ridurre la perdita di peso;
- ridurre l’espressione di interleuchina-1;
- aumento della lunghezza del colon, sia dell’area mucosa che dell’integrità del colon, e della profondità delle cripte;
- promuovere la crescita del villi nel soggetto affetto da una malattia come morbo ciliaco, atrofia dei villi post-infettiva e sindromi da intestino corto.
- promuovere la proliferazione dell’intestino tenue e crasso in un soggetto sano, consentendo ad esempio un maggiore assorbimento di nutrienti nel bestiame, che permette un precoce svezzamento o quantità maggiori di latte o della produzione di carni; ecc.
L’effetto sulla crescita suscitata dai presenti derivati di GLP-2 manifesta un aumento del peso dell'intestino tenue rispetto a un controllo trattato in modo fittizio. In particolare, si considera che i presenti derivati di GLP-2 presentano un’attività "intestinotrofica" se, quando valutati nel modello murino esemplificato in questa sede, l’analogo media un aumento del peso dell’intestino tenue di almeno 10% rispetto a un controllo animale che riceve il solo veicolo. Particolarmente adatti per l’uso terapeutico sono quei derivati che mediano un aumento di almeno 20% del peso dell’intestino tenue, mentre quelli più preferiti mediano un aumento del peso dell'intestino tenue del 50% o più. L’attività intestinotrofica si nota in maniera in assoluto più significativa in corrispondenza del digiuno, comprendente il digiuno distale e in particolare il digiuno prossimale, e si nota anche nell’ileo.
Oltre ad esibire l’attività intestinotrofica, i presenti derivati di GLP-2 incorporano una sostituzione di amminoacidi all’interno della “struttura portante” del peptide GLP-2, che è una variante di una specie di GLP-2 di mammifero, in cui il C-terminale è stato alterato mediante aggiunta di un residuo basico. I presenti derivati peptidici incorporano un residuo di lisina aggiunto al C-terminale della sequenza del peptide GLP-2.
La funzionalità presente sulla proteina sarà un gruppo tiolo e l’entità reattiva sarà un gruppo contenente maleimmido, acido maleimmidopropionico (MPA) o acido 2-[2-(2-ammino)etossi)] etossi acetico (AEEA)-MPA.
Gruppi maleimmide sono molto più selettivi per i gruppi solfidrile sui peptidi quando il pH della miscela di reazione è mantenuta tra 6,5 e 7,4. A pH 7,0, la velocità della reazione dei gruppi maleimmido con i solfidrili è di un fattore 1000 più veloce rispetto alle ammine. Si forma un legame tiotere stabile tra il gruppo maleimmido e il solfidrile che non può essere scisso sotto condizioni fisiologiche.
I derivati di promotore di crescita del tessuto gastrointestinale dell'invenzione forniscono la marcatura specifica di componenti ematici. Tale specifica marcatura, in particolar modo con una maleimmide, offre numerosi vantaggi. Gruppi tioli liberi sono meno abbondanti in vivo che i gruppi ammino e, conseguentemente, derivati di maleimmide si legano in modo covalente a meno proteine. Ad esempio, nell’albumina di siero è presente soltanto un gruppo tiolo libero in ciascuna molecola. Un coniugato GLP-2-maleimmide-albumina tenderà a comprendere un rapporto molare di 1:1 tra peptide e albumina. Oltre all’albumina, anche le molecole IgG (classe II) hanno tioli liberi. Poiché le molecole di IgG e l’albumina di siero costituiscono la maggior parte delle molecole solubili nel sangue, ovvero il 99%, essi costituiscono la maggior parte del gruppi tiolo liberi disponibili per legarsi in modo covalente a un GLP-2 sostituito con maleimmide.
Inoltre, anche tra le proteine ematiche contenenti tiolo liberi, la marcatura specifica con una maleimmide conduce alla formazione preferenziale di coniugati peptide maleimmide-albumina, - considerando le caratteristiche esclusive dell’albumina stessa. Il singolo gruppo tiolo dell'albumina, altamente conservato tra le specie, è localizzato nel residuo di amminoacido Cys34. È stato recentemente dimostrato che la Cys34 dell’albumina esibisce una maggiore reattività rispetto ai tioli liberi presenti su altre proteine contenenti tioli liberi. Questo in parte è dovuto al valore insolito di PK di 5,5 per la Cys34 dell'albumina. Tale valore è molto inferiore rispetto ai tipici valori di pK dei residui di cisteina in generale, che sono tipicamente circa 8. Grazie a questo basso pK, sotto condizioni fisiologiche la Cys34 dell’albumina è in modo predominante nella forma anionica, la quale aumenta drammaticamente la sua reattività. Oltre al basso valore di pK della Cys34, un altro fattore che potenzia la reattività della Cys34 è la sua posizione, ovvero in una tasca idrofoba prossima alla superficie di un’ansa della regione V dell’albumina. Questa posizione rende la Cys34 accessibile a ligandi di tutti i tipi, e risulta essere un importante fattore nel ruolo biologico della Cys34 in qualità di trappola per radicali liberi e smaltitore di tioli liberi. L’accelerazione della velocità di reazione può essere pertanto nell’ordine di un fattore 1000 rispetto a velocità di reazione di peptide-maleimmidi con altre proteine contenenti tioli liberi.
Un altro vantaggio dei coniugati peptide-maleimmide-albumina è la riproducibilità associata al caricamento 1:1 tra peptide e albumina, specificamente alla Cys34. Tecniche convenzionali di attivazione, come con glutaraldeide, DCC, EDC e altri attivatori chimici, ad esempio di ammine libere, non possiedono tale selettività. Ad esempio, l’albumina contiene 52 residui di lisina, di cui 25-30 sono localizzati sulla superficie dell’albumina e accessibili per la coniugazione. L’attivazione di questi residui di lisina, o alternativamente la modifica di un peptide che si accoppia attraverso questi residui di lisina, comporta una popolazione eterogenea di coniugati. Anche se si impiega un rapporto equimolare peptide:albumina (ovvero 1:1), il risultato finale è la produzione di prodotti causali di coniugazione, alcuni contenenti un numero indefinito di peptidi legati a ciascuna molecola di albumina, e ogni coniugato avente peptidi accoppiati in modo causale in corrispondenza di qualsiasi dei 25-30 siti di lisina disponibili. Quindi, una caratterizzazione dell’esatta composizione è virtualmente impossibile, per non parlare dell’assenza di riproducibilità. Inoltre, sebbene la coniugazione attraverso residui di lisina di albumina sembrerebbe avere almeno il vantaggio di distribuire una quantità maggiore di agente terapeutico per molecola di albumina, studi hanno mostrato che è preferibile un rapporto 1:1 tra agente terapeutico e albumina. In un articolo di Stehle, et al. in Anti-Cancer Drugs, 1997, 8, 677-685, è stato riportato che un rapporto di 1:1 tra antitumorale metotrexato e albumina coniugata via glutaraldeide, forniva i risultati in assoluto più promettenti. Questi coniugati venivano assunti dalle cellule tumorali, mentre i coniugati provvisti di molecole di metotrexato comprese tra 5:1 e 20:1 presentavano profili HPLC alterati, e venivano assorbiti velocemente dal fegato in vivo. Sembrerebbe quindi che in corrispondenza di rapporti maggiori, l’efficacia dell’albumina come trasportatore per un’agente terapeutico è diminuita.
Attraverso una somministrazione controllata del presente derivato peptidico di GLP-2, e in special modo un peptide GLP-2 comprendente un’entità di maleimmide, è possibile controllare la specifica marcatura in vivo o il legame all’albumina e IgG. In tipiche somministrazioni, è stato mostrato che 80-90% del derivato peptidico somministrato si lega all’albumina, e meno del 5% si lega alle IgG. Avvenivano anche tracce di legame di tioli liberi presenti, ad esempio nel glutatione. Tale legame specifico è preferibile per un uso in vivo, perché permette un calcolo preciso dell'emivita stimata dell'agente terapeutico somministrato.
In generale, i peptidi GLP-2 sostituiti con maleimmide sono abbastanza stabili in presenza di soluzioni acquose e in presenza di ammine libere. Poiché i peptidi GLP-2 sostituiti con maleimmide reagiscono con i tioli liberi, non sono necessari gruppi protettivi per impedire una reazione incrociata tra loro.
Come affermato sopra, i coniugati desiderati tra derivati di GLP-2 e componenti ematici possono essere preparati in vivo, mediante somministrazione diretta dei derivati al soggetto, che può essere un animale o una persona. La somministrazione può essere effettuata sotto forma di un bolo, o può essere introdotta lentamente nel tempo per infusione, usando un flusso dosato o similare.
In alternativa, il coniugato può anche essere preparato ex vivo, combinando del sangue o componenti ematici purificati disponibili in commercio con il presente derivato di GLP-2, permettendo il legame covalente del derivato di GLP-2 alle funzionalità sui componenti ematici, e quindi restituendo o somministrando il sangue coniugato, o il componente ematico purificato coniugato, all'ospite. Inoltre, quanto sopra può anche essere condotto prima purificando un singolo componente ematico o un numero limitato di componenti, come eritrociti, immunoglobuline, albumina di siero o similare, e combinando il componente o i componenti ex vivo con il presente composto derivato. Il sangue o componente ematico marcato possono quindi essere restituito al soggetto, così da fornire in vivo i coniugati terapeuticamente efficaci. Il sangue può anche essere trattato in modo da impedire una sua coagulazione durante la manipolazione ex vivo.
Sintesi peptidica
I peptidi GLP-2 possono essere sintetizzati attraverso metodi standard di chimica dei peptidi in fase solida, ampiamente noti a qualsiasi persona avente una conoscenza ordinaria del ramo. Il peptide può ad esempio essere sintetizzato, attraverso tecniche di chimica in fase solida, osservando le procedure descritte in Steward et al. in Solid Phase Peptide Synthesis, 2a Ed., Pierce Chemical Company, Rockford, Ill., (1984), adoperando un sintetizzatore Rainin PTI Symphony™. In modo simile, è possibile sintetizzare frammenti peptidici, e quindi combinarli o legarli insieme per formare un peptide più grande (condensazione di segmenti). Questi frammenti peptidici sintetici possono anche essere realizzati con sostituzioni e/o delezioni di amminoacido in specifiche posizioni.
Per la sintesi di peptidi in fase solida, un riepilogo delle numerose tecniche può essere trovato in Stewart et al. in "Solid Phase Peptide Synthesis", W.H. Freeman Co. (San Francisco), 1963, e in Meienhofer, Hormonal Proteins and Peptides, 1973, 2, 46. Per la classica sintesi in soluzione, vedere ad esempio Schroder et al. in "The Peptides", volume 1, Acacemic Press (New York). In generale, tale metodo comprende l'aggiunta sequenziale di uno o più amminoacidi, o di amminoacidi opportunamente protetti, ad una catena peptidica in crescita su un polimero. Il gruppo ammino o carbossile del primo amminoacido viene normalmente protetto con un adatto gruppo protettivo. L'amminoacido protetto e/o funzionalizzato viene quindi o fissato ad un supporto solido inerte, o utilizzato in soluzione, aggiungendo l'amminoacido successivo nella sequenza, avente il gruppo complementare (ammino o carbossile) opportunamente protetto, e sotto condizioni adatte alla formazione di un legame ammidico. Da questo residuo di amminoacido appena aggiunto viene poi rimosso il gruppo protettivo, viene aggiunto l'amminoacido successivo (opportunamente protetto), e così via.
Una volta legati tutti gli amminoacidi desiderati nella sequenza appropriata, tutti i gruppi protettivi rimanenti (e tutto il supporto solido) vengono tagliati in sequenza o simultaneamente, generando il peptide finale. Attraverso una semplice modifica a questa procedura generale, è possibile aggiungere più di un amminoacido per volta ad una catena in crescita, ad esempio accoppiando (sotto condizioni che non racemizzano i centri chirali) un tripeptide protetto con un dipeptide opportunamente protetto, così da formare, in seguito alla deprotezione, un pentapeptide (condensazione di segmenti).
Un metodo particolarmente preferito per preparare i presenti derivati di GLP-2 coinvolge una sintesi di peptidi in fase solida in cui l'amminoacido α-N-terminale viene protetto con un gruppo sensibile agli acidi o alle basi. Tali gruppi protettivi dovrebbero avere le proprietà di essere stabili alle condizioni di formazione dei legami peptidici, essendo allo stesso tempo facili da rimuovere senza distruggere la catena peptidica in crescita, o senza racemizzare alcuno dei centri chirali contenuti al suo interno.
