Aunque los espirómetros modernos pueden ofrecer resultados de multitud de variables, en realidad, para poder interpretarla, es suficiente con tres parámetros básicos: la capacidad vital forzada (FVC), el volumen espiratorio forzado en el primer segundo (FEV1) y la relación entre ambos (FEV1/ FVC).
Tipos de curvas espirométricas
El primer paso para la correcta interpretación de la espirometría es revisar las curvas generadas durante la maniobra.
Estas curvas son de dos tipos: curvas de volumen-tiempo (V-T) y curvas de flujo-volumen (F-V). La revisión de dichas curvas nos permitirá decidir si la maniobra realizada ha sido correcta y se puede valorar como aceptable, o si por el contrario han existido fallos importantes que obliguen a repetir la prueba.
Curva volumen-tiempo: El volumen se mide en litros, en el eje de ordenadas (vertical), y el tiempo, en segundos, en el eje de abscisas (horizontal). Una curva de V-T normal tiene un ascenso rápido, ya que se expulsa más del 70% de todo el aire en el primer segundo; posteriormente, la pendiente se va haciendo cada vez menos pronunciada hasta que se aplana y llega al máximo volumen (FVC). El volumen de aire expulsado en el primer segundo es el FEV1 (figura 1).
Curva flujo-volumen: En este tipo de curva, el flujo se mide en litros/segundo, en el eje de ordenadas (vertical) y el volumen, en litros, en el eje de abscisas (horizontal). La curva de F-V normal tiene un ascenso muy rápido, cercano al eje de ordenadas (flujo) hasta alcanzar un pico (PEF). A partir de ahí, desciende en línea más o menos recta, con una pendiente menos pronunciada que el ascenso, hasta acabar contactando, de forma asintótica, con el eje de abscisas (volumen), marcando así la FVC.
Prueba broncodilatadora
La Prueba Broncodilatadora (PBD) permite estudiar la dilatación que puede producirse en los bronquios del paciente tras administrarle un broncodilatador (generalmente salbutamol), y si esa broncodilatación (BD) llega a la reversibilidad completa (normalización de un patrón previamente obstructivo).
Para realizar la PBD, se hace primero una espirometría basal (preBD). A continuación, se le administran al paciente 4 disparos de salbutamol en cámara de inhalación. Se esperan 20 minutos, y se repite la espirometría (postBD). Se compara el FEV1 preBD con el FEV1 postBD. Si aumenta un 12% o más y ≥ 200 ml, se considera la PBD positiva.
Hay que mirar también el patrón preBD y el patrón postBD. Si un patrón es obstructivo en la preBD, pero se normaliza en la postBD, se habla de reversibilidad completa de la obstrucción.

Although modern spirometers can yield multiple-variable results, three basic parameters are enough to interpret them: the forced vital capacity (FVC), the forced expiratory volume in the first second (FEV1), and their ratio (FEV1/FVC).
Types of spirometric curves
The first step to a correct interpretation of a spirometry test is to check the curves generated during the manoeuvre.
These curves belong to two types: volume-time curves (V-T) and flow-volume curves (F-V). The review of such curves will determine whether the manoeuvre has been performed correctly and can be considered as acceptable, or if otherwise there have been significant errors which require a repetition of the test.
Volume-time curve: Volume is measured in litres, on the ordinate axis (vertical), while time is measured in seconds, on the abscissa axis (horizontal). A normal V-T curve has a steep rise, given that over 70% of all the air is expelled in the first second. Following this, the slope becomes gradually smoother until flattening out, when it reaches peak volume (FVC). The volume of expelled air in the first second is the FEV1 (graphic 1).
Flow-volume curve: In this type of curve, flow is measured in litres per second, on the ordinate axis (vertical), and the volume is measured in litres, on the abscissa axis (horizontal). The normal F-V curve has a very steep rise, near the ordinate axis (flow) until reaching a peak (PEF). From there, it decreases in a somewhat straight line, with a less pronounced slope, until it finishes by asymptotically reaching the abscissa axis (volume), showing thus the FVC.
Bronchodilator test
The Bronchodilator Test (PBD) is used to study the dilation that can occur in the patient’s bronchi after being administered a bronchodilator (usually Salbutamol), and whether that bronchodilation (BD) reaches full reversibility (normalisation of a previously obstructive pattern).
In order to conduct a PBD, a baseline spirometry must be first made (preBD). Following this, the patient is administered 4 Salbutamol puffs through an inhaler with a spacer. After 20 minutes, the spirometry is repeated (postBD). The FEV1 preBD and the FEV1 postBD are then compared. If there is a 12% increase and it is ≥ 200 ml, the PBD is considered to be positive.
The preBD and postBD patterns must be considered as well. If a pattern is obstructive at the preBD, but normalises at the postBD, this is referred to as complete reversibility of the obstruction.
