When evaluating the efficacy of an orally administered drug, what is the appropriate unit of measure?
Is the patient or the eye (ear) the unit of measure?
How should cure be defined?
One way to define cure is to dichotomize the outcome and consider that the patient is cured only when both eyes (ears) show no sign of disease.
Another approach would be to summarize the endpoints as unilateral disease (only one eye/ear still diseased), bilateral (both eyes/ ears still diseased) and cured (neither eye/ ear is diseased).
A third approach would be to use an analytical approach that treats the information from both eyes (ears) as bivariate data and incorporates the correlation between the paired observations as part of the analysis.

Source text - English Like most first efforts in programming languages, Fortran was seriously flawed, both in details of the language design and more importantly in the lack of support for modern concepts of data and module structuring.
As Backus himself said in retrospect:
We did not regard language design as a difficult problem, merely a simple prelude to the real problem: designing a compiler which could preduce efficient programs.
Nevertheless, the advantages of the abstraction quickly won over most programmers: quicker and more reliable development, and less machine dependence since register and machine instructions are abstracted away.
Because most early computing was on scientific problems, Fortran became the standard language in science and engineering, and is only now being replaced by other languages.
Fortran has undergone extensibe modernization (1966, 1977, 1990) to adapt it to the requirements of modern software development.
C was developed by Dennis Ritchie of Bell Laboratories in the early 1970's as an implementation language for the UNIX operating system.
Operating systems were traditionally written in assembly language because high-level languages were considered inefficient, C abstracts away the details of assembly language programming by offering structured control statements and data structures (arrays and records), while at the same time it retains all the flexibility of low-level programming in assembly language (pointers and bit-level operations).
Since UNIX was readily available to universities, and since it is written in a portable language rather than in raw assembly language, it quickly became the system of choice in academic and research institutions.
When new computers and applications moved from these institutions to the commercial marketplace, they too UNIX and C with them.
C is designed to be close to assembly language so it is extremely flexible, the problem is that this flexibility makes it extremely easy to write programs with abscure bugs because unsafe constructs are not checked by the compiler as they would be in Pascal.
C is a sharp tool when used expertly on small programs, but can cause serious trouble when used on large software systems develped by teams of varying ability.
We will point out many of the dangers of constructs in C and show how to avoid major pitfalls.
The C language was standardized in 1989 by the American National Standards Institute (ANSI); essentially the same standaard was adopted by the International Standards Organization (ISO) a year later.

Source text - English Nothing says state sovereignty like currency. We put pictures of presidents, monarchs, and prime ministers on our banknotes.
Currency has become fundamentally linked to our notion of national economies, national power, and even national identity.
Could a currency break this traditional bond with the nationstate? Could digital technology go as far as replacing banks or governments as arbiters of trust and create a new protocol for doing business around the world?
Bitcoin, a new transnational currency released in the midst of the financial crisis in 2008–2009, offers a case study for the future of currency as the code-ification of money intensifies. Bitcoin is a “digital currency”—a currency that is stored in code and traded online. It is also a “cryptocurrency,” a term that is often used interchangeably with “digital currency” but signifies that the currency uses cryptographic methods in an attempt to make it secure.
Bitcoin has become the world’s first cryptocurrency to gain widespread use. Although there are dozens of cryptocurrencies, it is currently the largest and most influential. At first glance, Bitcoin looks kind of like PayPal in that it offers a way to pay for goods online, with no physical interaction needed. As of the 2014 holiday season, some 21,000 merchants accepted bitcoins, including household names like Victoria’s Secret, Amazon, eBay, and Kmart. At first glance, there also looks to be an investment dimension to Bitcoin. It has the properties of a speculative asset, with its value going up and down with huge gyrations. But there is much more to it.
Bitcoin has encapsulated many of the contradictions and possibilities of digital currencies in a world still largely defined by national economies and governments. Its origin comes from ideological communities deeply skeptical of governments, traditional financial institutions, and “fiat currency” (money that draws its value from government law). Bitcoin has developed a new community around an online currency trying to circumvent these established institutions.
A fiat currency fundamentally depends on trust. People must have a shared belief that the currency is ultimately worth something. The word fiat means “it shall be”: a pronouncement from on high that the currency has value. Bitcoin came about both as a result of declining trust in the traditional financial system during the financial crisis and because of its technological advance in creating a trustable mechanism for monetary exchange online.
On October 31, 2008, a research paper, “Bitcoin: A Peer-to-Peer Electronic Cash System,” was published on a cryptography listserv by a mysterious author identified as “Satoshi Nakamoto” who has kept his/her (their?) identity unknown. It called for the creation of the world’s “first decentralized digital currency.” Satoshi Nakamoto condemned state-based currencies:

I've probably made 500 knives,
but the unknown of what we're gonna cut in this competition,
it's pretty intimidating.
I've been a blade sports competitor for a while.
I've done some crazy stuff in my life.

utterly bad ass knife competition ever created.
Welcome to "Forged In Fire: Knife or Death."
Many of you have forged your own blades,
others had them made to their own specifications.
We've got katanas,
competition choppers, and
traditional bowie knives, among others.
You and your blade will be tested in a series of
timed challenges that
focus on precision,
technique, strength
blade sharpness, and durability.
It is my honor to introduce to you Special Forces Green Beret Tu Lam,
a martial arts master, and weapons expert.
If your techniques are on point,
if you choose the right angles,
have efficient movement,
you can win this competition.
Ladies and gentlemen,
the first course is
Knife Fight.
The Knife Fight course is a true test of speed and power.
The first obstacle you'll face:Trail Blazer.
You must cut through six wooden dowels,
each releasing a lit torch.
When the torches fall, igniting the steel pits,
it's time to move on to the next obstacle,
Stick and Move.
You must stab, slice, and cut through three separate containers to release enough contents
so that the attached counterweights reach the ground
before the containers rise to high to hit, and
before you cross that line,
or you will be penalized.
Next up is Ice Pick
You must chop into a 3-foot block of ice,
puncture the plastic pipe, and release the red liquid
Then you move on to Lifeline,
a sudden-death obstacle.
You must fully slice through three hanging objects with one swing each
Fail to slice through the thickest part,your run time ends, and
you're not allowed to go on any further in the course
If you make it this far,
your last obstacle is Curtain Call,
where you must cut completely through the wall of meat,
and did the sheet of metal.
Your time ends when you plunge your blade through the tree stump.
That course is frightening.
There's ice blocks, and there's wood boxes.
I'm gonna put some good strikes on it,
but I sure hope my blade doesn't break.
Fish, chicken, holy hell!
This is a competitor's dream.
Every cut I like to do is here.
All obstacles you leave unfinished will incur a 25-second penalty.
After every competitor finishes and
penalties are added up,
the two that make it the farthest the fastest will move on to
second, even more grueling course
challenge:Dead Run
The winner earns a cash prize, and
a spot in the grand champion showdown at the end of the season
to vie for a $20,000 cash prize
and the title of "Knife Or Death" champion.
We station our very own blade expert,

He's gonna give your blade one final once-over
before we give you the green light to tackle Knife Fight.
If he finds any problems with your blade,
he can disqualify you from this competition.

It's been a part of my spirit my whole life.
That was hand forged to the specifications of my body.
You can't take a sharpened blade out of Japan.
They're actually considered national treasures,
so I was lucky enough to have found a Japanese smiththat actually lived in China,
and they were able to send it to me here in the United States.
Yeah, this is a beautiful blade,
but it does seem to have a little bit of wiggle.
This can affect the performance of the blade.
I've to do a little bit of testing, make sure it's safe and handles well before you're allowed on the course with it.

I'm really excited to see Karl. He is the modern-day samurai.
The katana began to develop when the Mongolians invaded Japan.
A sharper blade was needed to pierce the Mongol warrior's leather armor.
Let's see how it cuts through a box and gravel.
His blade, by far, is the largest blade in today's competition.
You have 28 inches of cutting power.
He can wield this thing with one hand,
or he can double up with both hands.

Several X-rays were taken of Michael Jackson. They showed he had osteoarthritis in some of his fingers and his spine."

The vertebrae in the spine are cushioned by areas of cartilage. In osteoarthritis, the cartilage degenerates the bones are left unprotected. This places pressure on the joints and in extreme circumstances the bones can grind together."
"SHEPHERD:
This is a condition that would cause him some pain and stiffness, and might limit his movements."

Osteoarthritis could explain why he was struggling with movement but it couldn’t explain why he was struggling to breathe."
"SHEPHERD:
Most of his organs are in good condition for his age. The exception is his lungs and they were like a battlefield. The lining of the airways showed a lot of damage and both lungs showed widespread inflammation and quite extensive scarring. For Michael Jackson, it would have made it harder for him to exert himself and he would have fatigued more easily. The next question I would want to ask is what’s caused all this damage?"

A possible explanation is written all over Jackson’s face. In his mid-20s he developed a symmetrical red rash around his cheeks, a classic symptom of another rare skin disease, discoid lupus, which causes hair to fall out anywhere on the body and can damage the lips."
"SHEPHERD:
We can see from the autopsy that Jackson had dark tattoos on his eyebrows and there were tattoos on the eyelids like eyeliner, and he’d also got tattoos in pink around his lips."

Lupus would also explain Jackson’s habit of using umbrellas. The disease leaves skin sensitive to sunlight and it doesn’t only affect the skin. It can wreak havoc across the whole body."
"SHEPHERD:
Lupus is an auto-immune disease and these diseases occur when the body attacks parts of itself. "
"V/O:
White blood cells usually attack foreign invaders in the body. But in lupus these cells strike at the body itself, including the lungs, leaving them heavily damaged. "
"SHEPHERD:
They’re chronic diseases with episodes of remission and recurrence. Their recurrence can be triggered by a number of things: stress, exertion and lack of sleep. "

But is there an alternative explanation for Jackson’s lung damage? For twenty-five years, an increasingly unstable and drug-dependent Jackson had been visiting the clinic of dermatologist Dr Arnold Klein. According to those who worked there, these visits had become less and less about his skin."

On every visit he’d receive at least one injection. The syringe was full of demarol, a powerful medical version of heroin."

Usually the effects were pretty quick. He might pass out, he might be just tired, he might be loopy."

If he was with the kids, that would be a little bit of a distraction for him. The kids would usually sit in Dr Klein’s office. They’d be running and screaming and joking, and it would just, it would be a lot of fun until Michael got really out of it."

Source text - English We have now traced how food production arose in a few centers, and how it spread at unequal rates from there to other areas. Those geographic differences constitute important ultimate answers to Yali's question about why different peoples ended up with disparate degrees of power and affluence. However, food production itself is not a proximate cause. In a one-on-one fight, a naked farmer would have no advantage over a naked hunter-gatherer.
Instead, one part of explanation for farmer power lies in the much denser populations that food production could support: ten naked farmers certainly would have an advantage over one naked hunter-gatherer in a fight. The other part is that neither farmers nor hunter-gatherers are naked, at least not figuratively. Farmers tend to brethe our nastier germs, to own better weapons and armor, to own more-powerful technology in general, and to live under centralized governments with literate elites better able to wage wars of conquest. Hence the next four chapters will explore how the ultimate cause of food production led to the proximate causes of germs, literacy, technology, and centralized government.
The links connecting, livestock and crops to germs were unforgettably illustrated for me by a hospital cause about which I learned through a physician friend. When my friend was an inexperienced young doctor, he was called into a hospital room to deal with a married couple stressed-out by a mysterious illness. It did not help that the couple was also having difficulty communicatng with each other, and with my friend. The husband was a small, timid man, sick with pneumonia caused by an unidentified microbe, and with only limited command of the English language. Acting as traslator was his beautiful wife, worried about her husband's condition and freighteded by the unfamiliar hospital environment. My friend was also stressed-out from a long week of hospital environment. This incident sounds bizrrely one-of-a-kind and of no possible broader significance. In fact, it illustrates an enormous subject of great importance: human diseases of animal origins. Very few of us love sheep in the carnal sense that this patient did. But most of us platonically love our pet animals, such as our dogs and cats. As a society, we certainly appear to have an inordinate fondness of sheep and other livestock, to judge from the vast numbers of them that we keep. 이 사건은 기괴하고 이례적이긴 하지만 역학으로 번질 가능성은 없는 듯하다. 그래도 ‘동물에서 원인한 인간 질병‘이라는 엄청나게 의미심장한 주제를 예시하는 사건임에는 틀림없다. 이 환자처럼 성욕 감각으로 양을 사랑하는 사람은 거의 없다. 사람들은 대부분 개와 고양이 같은 애완동물을 정서적으로 사랑한다. 사회 전체가 보유하는 양이나 가축의 수효로만 판단하더라도 분명 인간은 이런 가축들을 엄청 좋아하는 것처럼 보인다. 예를 들어, 호구조사 당시 전체 인구 1708만 5400명이 보유한 양이 1억 6160만 마리나 되었으니 오스트레일리아인의 양사랑은 가히 짐작할 만하다.
