L’evoluzione nella gestione dei soggetti con infezione da virus dell’epatite C (HCV), cui abbiamo assistito dalla scoperta del virus all’introduzione di terapie antivirali ad azione diretta (DAA), è stata notevole. Gli elevati tassi di successo terapeutico che questi farmaci hanno dimostrato, anche nei pazienti difficili, possono indurre a ritenere l’epatite cronica C un “problema risolto”, mancando solamente il vaccino per la definitiva eliminazione dell’infezione (1).
Tuttavia, la disponibilità a costi accessibili dei farmaci non basta a raggiungere l’obiettivo atteso, poiché rimane aperta la sfida dell’identificazione e dell’invio dei pazienti alle strutture deputate alle cure. Infine, vi sono alcune categorie di persone infette, come le donne in età riproduttiva o in gravidanza, in cui si associano problematiche che non sono ancora state adeguatamente comprese e affrontate.

The evolution in the management of subjects with Hepatitis C virus (HVC), which we have witnessed from the discovery of the virus to the introduction of direct-acting antiviral treatments (DAAs), have been remarkable. The high success rate of the treatment that these medications have demonstrated, even in difficult cases, can imply that hepatitis C is a “resolved problem”, with only a vaccine for the definitive elimination of the infection (1).
However, the access to affordable medications is not sufficient to reach this objective, since the identification and the referral of patients to appropriate treatment facilities remains a challenge. Lastly, there are a few categories of infected people, such as pregnant and nonpregnant women of reproductive age, which are associated with a series of problems that have not been adequately understood or addressed.

L’allarme recessione è tornato a scuotere i mercati,affondando il rendimento dei titoli di Stato statunitensi e riaccendendo il rally di oro e argento. Quest’ultimo in particolare ha rotto un’importante resistenza tecnica, per volare di oltre il 3% e superare la soglia dei 18 dollari l’oncia, spingendosi fino a 18,20 $, un record da settembre 2017
L’oro ha invece registrato un rialzo superiore all’1%, sfiorando quota 1.545 $/oncia sul mercato spot londinese, non lontano dal massimo da sei anni raggiunto lunedì (1.554,56 $).
Lo strappo è avvenuto dopo l’accentuarsi dell’inversione della curva dei rendimenti dei Treasuries, un indicatore che segnala il probabile arrivo di una recessione: il decennale è crollato sotto l’1,5% e lo spread a vantaggio della scadenza a due anni si è ampliato fino a 5,16 basis points, un record dal 2007.
L’andamento dei bond ha fatto invertire la tendenza anche ai listini azionari a Wall Street, spingendoli in terreno negativo.

The alarm of recession has come back to shake the markets plummeting the US government bond yield and is restoring the rally for gold and silver. In particular, the latter has broken an important technical strength to fly up over 3% and surpass the 18 dollar per ounce threshold, breaking the last record seen in 2017 to $18.20.
Gold has instead recorded an increase over 1%, cutting close to $1,545 per ounce on the London spot market, not far from the highest in six years reached on Monday at ($1,554.56).
The split happened after the rise of the reversal of the Treasuries’ yield curve, an indicator that marks the probable arrival of a recession: the ten-year period dropped to below 1.5% and the spread advantage for the two-year deadline expanded to 5.16 basis points, a record since 2007.
The bond performance has led to an inversion of the tendency of the main stock listings on Wall Street, driving them in negative territory. .
In addition to the tariff war, straining the sluggish data regarding the US economy: In August, consumer confidence diminished, while the price of new houses has reached the lowest level in seven years.
