The general objective of this programme project is to increase knowledge on the pathogenesis and metabolic consequences of obesity by integrating research in 5 different but interrelated areas. Firstly, we wish to assess the impact of amount and cellular phenotype of ectopic pericardial fat on coronary atherosclerosis in patients undergoing elective cardiac surgery, and relate the findings to their vascular and metabolic status. Secondly, we hope to provide a test for the hypothesis that the beneficial impact of physical training on metabolic state and vascular function is mediated by the effect of nitric oxide on mitochondrial biogenesis and function (which has been shown to be defective in obesity). Thirdly, we are keen to test whether satellite cells in skeletal muscle are responsible for ectopic fat deposition in obesity by examining the molecular factors that drive their differentiation into adipocytes. Moreover, we wish to investigate a new population of PDGFalpha+ mesenchymal progenitor cells that has been identified and characterised in muscle interstitium. Fourthly, we shall continue to investigate transdifferentiation, of brown into white but also of white into brown adipocytes. This follows from the recently revised notion, that brown fat may be more abundant in adult man than previously thought and that this may have functional and therapeutical implications. Also, we have demonstrated that fat infiltration of blood-derived monocytes with phagocytic capacity (macrophages) is not simply the response to adipocytokine-mediated attraction but a ‘foreign body’ reaction triggered by the death of obese adipocytes. The enhanced propensity of hypertrophic adipocytes to undergo apoptosis thereby triggering a true phagocytic response is a paradigm shift to explain the subclinical inflammation characteristic of obesity/T2DM. Finally, we hope to shed some light on the role of the gut in obesity/T2DM. The incretin effect is impaired in both obesity (and T2DM) and measuring the reversal of this defect following major weight loss will provide clinically meaningful information. More broadly, by measuring site-specific insulin resistance (liver, peripheral tissues and adipose tissue) and ß-cell function in subjects spanning a range of BMI from normal to morbid obesity, we will have the background against which to gauge the metabolic effects of 3 bariatric operations (RYGB, IISG, BPD) with increasing ‘hindgut’ and decreasing ‘foregut’ characteristics. The question, whether or not weight loss is sufficient to explain the metabolic improvements and which operation adds non-weight-dependent effects, will be conclusively answered. The outcome of this concerted effort is to gain a better understanding of how different processes, ranging from mitochondrial biogenesis to cell transdifferentiation to inflammation to gut interference, converge into the development of ectopic fat with pathogenic potential.

L’obiettivo generale di questo progetto è quello di incrementare la conoscenza della patogenesi e delle conseguenze metaboliche dell’obesità integrando la ricerca all’interno di 5 aree scientifiche diverse ma interrelate.
In primo luogo, vorremmo valutare l’impatto di quantità e fenotipo cellulare del grasso ectopico pericardico sull’aterosclerosi coronarica nei pazienti sottoposti a chirurgia cardiaca elettiva, e mettere in relazione questi risultati con il loro stato vascolare e metabolico. In secondo luogo, speriamo di fornire un test per l’ipotesi che l’impatto benefico dell’esercizio fisico sullo stato metabolico e la funzione vascolare è mediata dall’effetto dell’ossido nitrico sulla biogenesi e funzione mitocondriale (è stato dimostrato che quest’ultima è alterata nell’obesità). Terzo, cercheremo di capire se le cellule satellite del muscolo scheletrico siano responsabili del deposito di grasso ectopico, esaminando i fattori molecolari che guidano la loro differenziazione in adipociti. Nell’ambito di questa ricerca vorremmo anche indagare una nuova popolazione di cellule PDGFalpha+ mesencimali identificate e caratterizzate nell’interstizio muscolare. Quarto, continueremo ad indagare la transdifferenzzazione degli adipociti da bruni a bianchi, ma anche da bianchi a bruni. Questa ricerca nasce dalla nozione recentemente riesaminata che il grasso bruno sia più abbondante nell’essere umano adulto di quanto si credesse e che questo possa avere implicazioni funzionali e terapeutiche. Inoltre, abbiamo dimostrato che l’infiltrazione grassa di monociti derivati dal sangue con capacità fagocitaria (macrofagi) non è semplicemente la risposta all’attrazione adipocitochine-mediata, ma una reazione da ‘corpo estraneo’ scatenato dalla morte degli adipociti obesi. L’aumento della tendenza degli adipociti ipertrofici di andare incontro ad apoptosi scatenando quindi una reale risposta fagocitaria è un paradigma per spiegare l’infiammazione subclinica caratteristica dell’obesità/diabete di tipo 2. Infine, ci proponiamo di chiarire il ruolo dell’intestino nell’obesità/diabete di tipo 2. L’effetto incretinico è alterato sia nell’obesità che nel diabete di tipo 2, e attraverso la misurazione del miglioramento di questa alterazione a seguito di una importante perdita di peso, potremo essere in grado di fornire informazioni clinicamente significative. In senso più ampio, misurando l’insulino-resistenza sito-specifica (fegato, tessuti periferici e tessuto adiposo) e la funzio b-cellulare in soggetti con ampio range di BMI da normali a obesi gravi, otterremo un background sul quale misurare gli effetti metabolici di 3 tipi di chirurgia bariatrica (RYGB, IISG, BPD) che determinano un aumento delle caratteristiche di hindgut e una diminuzione di quelle di foregut. La questione se la perdita di peso sia sufficiente a spiegare il miglioramento metabolico e quale operazione aggiunge effetti che non dipendono dal peso, verrà definitivamente chiarita. Il risultato finale di questo impegno congiunto è di ottenere una conoscenza migliore su come processi diversi, che vanno dalla biogenesi mitocondriale alla transdifferenzzazione cellulare, all’infiammazione, e all’interferenza dell’intestino, convergono nello sviluppo del grasso ectopico a potenziale patogeno.

Bio

I am an Italy-based English to Italian translator.

After 20 years of translating mainly as part of my job as an assistant to a medical doctor, in 2015 I decided that translating could become my fulltime job. I enrolled in a Postgraduate course in ENG>ITA Specialist Translation (Universities of Bari, Genoa and Pisa) and have graduated in January 2016.

While I have been living in Italy since I was 13, my English knowledge comes from an English-speaking parent and attending English-speaking schools.

In the last years, I have mainly been working in the technology field translating a wide range of different kinds of documents, such as motorcycle manuals, kitchen procedures, sport apps, photocamera manuals and brochures, waterworks website. Recently I have been enjoying translating press releases for an American politician.

In the medicine field I have translated scientific papers, congress abstracts, grant applications, EU Research Projects, pharmaceutical company contracts, clinical trial protocols, consent forms, SPCs, package leaflets, instrument manuals as well as medicine related correspondence mainly in the fields of diabetes, metabolism, hypertension, obesity, endocrinology.

In the tourism field I have translated tourist guides and brochures as well as tourism and hotel websites mainly relating to Tuscany, where I live.

I have also done editing and translating in the environment field for the EU Environment LIFE Programmes.

The Postgraduate degree in ENG>ITA Specialist Translation has broadened my translation knowledge in particular in the fields of biomedicine, technology and environment. I have a Bachelor’s degree in Cinema Music and Theatre from the University of Pisa. In this field I have translated subtitles and art related documents.
