First, under "Amparo", you have "Predios, Labores y Operaciones - PLO". The amount is $113,340,000. Then for "Daño Emergente y Lucro Cesante", the column "% de Cobertura" gives "10% del PLO", and the amount is $11,334,000, which is 10% of the first figure. And so on. It is quite clear that "% del PLO" in your document must refer to the percentage coverage of "Predios, Labores y Operaciones".

As for how to translate it, I think your suggestion is also correct. "Premises, work and operations" was the accepted answer in this previous question:

You don't find references to "premises, work and operations" cover in English, but "premises and operations" is a standard type of liability cover:

  -> Thanks, Phil. I wondered about that. I'm not quite sure whether anything is lost if you do, so I think I would probably leave it in to be on the safe side, but clearly "premises and operations" is the standard expression in English.
