Is there an official way to translate this sentence? It is a doctor's declaration in a hospital discharge letter. I would appreciate any help translating it.

agree  Ron Armon: Although the ending is a bit off - I would change "as an oath given by me" to "as a testimony given by me under oath"

agree  Eynnat: Although it doesn't say 'duly' signed by me - that particular 'din' refers back to the first 'din', i.e. din ze ke'din ze ;-) What affidavit? It doesn't say tatzhir anywhere.
