Perkadox can be found in Ullmann's Encyclopedia of Industrial Chemistry
(http://interscience.wiley.com/ullmanns) and is manufactured by Akzo Nobel,
however since there are many formulations and preparations of this organic
peroxide (used as a starter for polymerization reactions) you'd best
contact Akzo Nobel directly.for perkadox 16-w40.

The CAS registry number is 15520-11-3
w-40 means 40% suspension in water.
