Heading on a Forensic Examiner´s form. The document is from Panama, it´s attached to the death certificate.
The form sates particulars of the deceased, place and cause of death, and it is certified by the forensic examiner.
The "'único de parte" portion is giving me a headache.

Flavio, I agree with you. "Standard" sounds a lot better than "single". It's a shame I didn't realise earlier. Still, better late than never I suppose.
