I2R-MC-BIDB: A Comparison of LY2605541 versus Insulin Glargine as Basal Insulin Treatment in Combination with Oral Anti Hyperglycemia Medications in Insulin-Naïve Patients with Type 2 Diabetes Mellitus: An Open-Label, Randomized, 52-week Study
Introduction
You are invited to take part voluntarily in a research study of a study drug known as LY2605541 which is being tested as a treatment for Type 2 diabetes mellitus. During the study, you will continue to take your current oral glucose lowering medications at the same dose that you are now using along with LY2605541 or insulin glargine. If you take more than 3 oral antidiabetes medicines, your doctor will decide which 3 drugs should be maintained. Like any other medication, taking a combination of these drugs may have raised risks and discomforts. Ask your doctor about any risks that may be associated with combination medication.
Eli Lilly and Company is sponsoring this study. Before agreeing to participate in this research study, it is important that you read and understand this form. It describes the purpose, procedures, benefits, risks, discomforts and precautions of the study. It also describes the alternative procedures that are available to you and your right to withdraw from the study at any time. If you participate in the study, you will receive a copy of this form to keep for your records.
Your participation in this study is expected to last up to approximately between 58 and 60 weeks. Up to 663 patients will be participating in this study.
Purpose of the Study
The primary purpose of your participation in this study is to help answer the following research question(s), and not to provide you treatment for your condition:
• Whether LY2605541 can help patients with type 2 diabetes mellitus as starter insulin.
• How LY2605541 compares to insulin glargine in terms of efficacy.
• The safety of LY2605541 and any side effects you might have when you take it.
Qualifications to Participate
The doctor in charge of this study or a member of the study staff has discussed with you the requirements for participation in this study.
You can take part in this study if:
• You are 20 years old or more.
• You have had type 2 diabetes mellitus for 1 year or more.
• You have not been treated with insulin at any time for more than 4 continuous weeks in the past 2 years.
• You have been taking at least 2 types of stable dose oral blood glucose lowering medicines for the past 3 months before study entry.
• You are willing to take insulin shots and monitor blood glucose during the study by yourself or with the help of a caregiver.
• You have access to a telephone and a refrigerator facility.
• You have used a reliable birth control measure for past the 6 weeks before screening and are willing to continue using birth control measures for up to 2 weeks after the last dose of study drug.

Low Blood Sugar (Hypoglycemia)
Hypoglycemia is defined as low blood sugar (usually less than 70 mg/dL (3.9 mmo/L)). People who take certain diabetes medications are at greater risk for hypoglycemia when they skip or delay meals.
Symptoms of low blood sugar include:
If your blood sugar drops below 70 mg/dL (3.9 mmo/L), you may feel shaky, lightheaded, and confused. You may also have cold, clammy hands and/or a headache.
Treating Low Blood Sugar
• Consume a source of simple sugar, such as 4 to 6 ounces of juice or a sugared soft drink, or 2 to 3 sugar cubes, to allow a rapid rise in blood sugar. Avoid drinking or eating more than this amount. If you do, you will get more calories than you need and your blood sugar levels will end up too high.
• Check your blood sugar level approximately 10 to 15 minutes after ingestion of simple sugar.
• Once your blood sugar reaches an acceptable level, eat a snack containing protein and carbohydrate to maintain blood sugar if your next scheduled meal or snack is more than an hour away.

Great pecuniary success has come to a few metropolitan newspaper; owners, moderate success to many owners in other cities ; but the number of successful owners is very small compared with the thousands, in number, of journalists who are working for salary only the men who represent the journalism of the day.
It is difficult to compare the rewards of journalism with those of any other business or profession. If we consider the pecuniary rewards the comparison certainly must be unfavorable. Let us see:
Many successful lawyers have incomes from fifty thousand dollars upward, a year. Many physicians and many surgeons make fifty thousand dollars or more by the practice of their profession. There are oculists and artists who make thirty thousand plus. Our prize operatic singers have soared to two hundred thousand. The presidents of banks, railroad companies, insurance companies, steel companies, copper companies men who have achieved high success in their business commonly enough have salaries of from fifty thousand to one hundred thousand dollars a year and every opportunity to double the sum if they choose to live up to their privileges. These are the prizes of the calling to the most successful men in it; and in a way they measure the success of the men who have won them. But there are few prizes in the newspaper business. Nothing like these big salaries is paid to the men who achieve supreme journalistic success. In New York City for instance and New York is the best newspaper city in the world, pays the biggest salaries, and offers the best journalistic advantages and chances possibly ten editors have twenty-five thousand dollars a year. One brilliant editor has much more than this sum for the reason that his contract with the owner made when the sheet’s circulation was small was Hased on the number of papers to be sold. The circulation increased to phenomenal figures and so did the editor's pay. Of the seven thousand newspaper editors and writers in New York City, a number not exceeding twenty have salaries of more than twenty thousand dollars; yet those who have achieved genuine success in the business success that is relatively as great as that of the bank presidents and professional men mentioned above may be numbered by the hundred. Newspaper salaries are very much larger than they were forty years ago, double as much in some departments, yet, despite this, the pecuniary rewards have no comparison with those of many other professions or businesses.
