Source text - English HISTORY OF PRESENT ILLNESS:
Ahmad is a 51-year-old gentleman, not known to have any chronic illness, smoker, presented to our clinic with 1
vear history of right tongue ulcer associated with pain and 10 kg weight loss, no compressive symptoms. On
examination he had a right lateral oral tongue ulcer about 4 by 3 cm with palpable mass that cross the midline
tongue, right level II palpable neck lymph node about 3 cm. CT head and neck and CAP and PET scan done for
him and showed right infiltrative tongue mass with invasion to the floor of mouth and sublingual space and right
level LL and III lymph nodes largest 2.7 cm with extracapsular spread and invasion of the sternocleidomastoid
muscle, no definitive distant mets. The patient brought slides of tongue biopsy done at security forces hospital and
reported in our hospital as invasive squamous cell carcinoma. Tumor board held for him and staged as T4 N2 MO
tongue SCC and planned for surgery. Patient underwent subtotal glossectomy + right radical neck dissection + left
supraomohyoid neck dissection + ALT free flap + tracheotomy on 1/8/2017 with all negative frozen sections.
Patient had a stable perioperative course and post op tumor board on 21/8/2017 confirmed the stage and planned
him for concurrent chemoradiation (CCRT). Patient now admitted with corked tracheostomy tube and on NGT
feeding for swallowing assessment and home care supply arrangement.
Translation - Arabic يبلغ أحمد من العمر 51 عاما، لم يعرف أنه مصاب بأمراض مزمنة،وهو مدخن، حيث قدم إلى عيادتنا بعد سنة من إصابتة قرحة اللسان اليمنى مترافقة مع ألم و خسران 10 كيلوغرامات من الوزن، لا توجد أعراض ضاغطة، أثناء الفحص تبين أن لديه قرحة لسانية بالجانب الايمن من الفم بحوالي 4x3سم مع كتلة واضحة في خط وسط اللسان ، في المستوى الأيمن الثاني العقدة الليمفاوية للرقبة واضحة حوالي 3 سم. تم إجراء فحص ل CTالرأس والرقبة و CAP و PET، وأظهرت الفحوصات وجود كتلة إرتشاحية لسانية غائرة باتجاه قاع الفم والفجوة تحت اللسان , و العقد اللمفاوية في المستوي الأيمن الثاني والثالث أكبر 2.7 سم مع انتشار خارج المحفظة إغارة العضلات القصية الترقوية الخشائية، لامسافة نهائية واضحة. جلب المريض الشرائح من خزعة اللسان بعملية أجراها في مستشفى قوات الأمن وجاء إلى المستشفى الخاص بنا بصفته مصاب بسرطان خلايا حرشفية غزوية. انعقد المجلس الخاص بالاورام وأدرج المريض بأنه مصاب ب T4N2M0باللسان وSCC وقرروا ا لعملية جراحية. خضع المريض لعملية استئصال لسان غير تام + تشريح جذري لطرف الرقبة الأيمن + تشريح العضلة الكتفية اللامية لطرف الرقبة الأيسر + ALT سديلة حرة+ القصبة الهوائية في 1 \ 8 \ 2017 مع جميع المقاطع المجمدة السلبية.
تلقى المريض معالجة ثابتة أنثاء الفترة المحيطة بالجراحة وانعقد المجلس في تاريخ 21 \ 8 \ 2017 حيث أكد أن المرحلة التالية هي الإشعاع الكيميائي المتزامن (CCRT). يجب الاعتراف أن المريض مع أنبوب فغر الرغامى وعلى تغذية NGT لتقييم البلع وترتيب تقديم الرعاية المنزلية.

Source text - English 14. For purposes of this Agreement, “Confidential Information” means any and all information, whether in oral, written, graphic or electronic form, provided by the Disclosing Party to the Receiving Party, including but not limited to, the business affairs, customers, clients or suppliers of the other Party or the terms and conditions of this Agreement, or any information it has received or obtained relating to the other Party as a result of negotiating or entering into this Agreement.
15. A party may disclose, or permit the disclosure of information which would otherwise be confidential if that it is disclosed to the extent required by any applicable law, court order or any stock exchange, securities exchange, regulatory or other governmental authority or tax authority, provided that (i) such disclosing Party gives as much prior written notice of such disclosure as possible to the other Party; and (ii) that such
disclosing Party takes into account the reasonable requests of the other Party in relation to the form, timing, and content of such disclosure.
Translation - Arabic 14. في سبيل تحقيق أغراض هذه الاتفاقية ، يعني مصطلح "المعلومات السرية" أي وجميع المعلومات ، سواء كانت شفهية أو خطية أو بيانية أو إلكترونية ، والمقدمة من قبل الطرف المفصح إلى الطرف المتلقي ، بما في ذلك على سبيل المثال لا الحصر, الشؤون التجارية أو بيانات عن العملاء أو موردي الطرف الآخر أو شروط وأحكام هذه الاتفاقية، أو أي معلومات تلقاها أو حصل عليها تتعلق بالطرف الآخر نتيجة للتفاوض أو إبرام هذه الاتفاقية.
