Source text - English In clinical and research contexts, observation acquires considerable importance, transforming itself from a simple and unscientific method into a flexible, structured and reliable one. With the progress of studies on children, for example, the use of "child biographies" has been abandoned, to give space to systematic observation and experimentation. As a matter of fact, over time naturalistic observations have left room for standardized observations. One could argue, in the words of Venuti, that observation "[...] acts as a bridge between an explanatory model of reference and a subsequent process of analysis and interpretation of data". To observe, therefore, it is no longer enough to look at a group of children interacting or recorded video of the behavior between mother and child, as observation must always refer to a theoretical and explanatory model, which will guide the researcher to formulate hypotheses and analyze the data. What to observe, how, where and when to do it, will depend on the purpose of the research and the theoretical approach of reference.
There are different types of observations:
• psychoanalytic observation, which has psychoanalysis as its theoretical model;
• piagetian observation, which has as its theoretical model the psychology of development;
• ethological observation, which has ethology as its theoretical model.

Tehát a megfigyeléshez immár nem elég pusztán az, ha interakció közben nézünk egy gyerekcsoportot, vagy egy anya és gyermeke közti viselkedésről készült felvételt, mivel a megfigyelésnek mindig egy elméleti és magyarázati modell vonatkozásában kell történnie, amely segíti a kutatót hipotézisek megfogalmazásában és az adatelemzésben. Az, hogy mit, hogyan, mikor és hol figyelünk meg, a kutatás céljától és a referenciaként szolgáló elméleti megközelítéstől függ.
Különböző típusú megfigyelések léteznek:
• pszichoanalitikus megfigyelés, amelynek elméleti modellje a pszichoanalízis;

Source text - Hungarian Egy kapcsolódó tanulmányunkban (Baji és szerzőtársai, 2012) a háztartási egészségügyi kiadások regresszivitását vizsgáltuk a 2005 és 2008 közötti időszakban (amely a gyógyszerre és gyógyászati segédeszközökre költött kiadásokat, térítési díjakat és a hálapénzt foglalja magában). A hálapénz az összes egészségügyre fordított kiadások csupán 4–9 százalékát tette ki a vizsgált időszakban, ami a háztartások összes bevételének körülbelül 0,2-0,3 százaléka volt. (Viszonyításképpen: a háztartások legtöbbet gyógyszerekre és gyógyászati segédeszközre költöttek, ez tette ki a háztartások egészségügyre fordított kiadásainak 78–85 százalékát.) Az éves hálapénzkiadás regresszív volt, vagyis az alsó jövedelmi ötödökbe tartozó, szegényebb háztartások a jövedelmük magasabb százalékát fordították hálapénzkiadásra, mint a gazdagabb, felső jövedelmi ötödbe tartozó háztartások. Translation - English In one of our related studies (Baji et al, 2012) we examined the regressivity of household healthcare expenditure in the period between 2005-2008 (which is comprised of the expenditures spent on medications and medical aids, usage fees, and informal payments). Informal payments totalled only 4-9 percent of healthcare-related expenditure in the period examined, which was approximately 0.2-0.3 percent of the total income of households. (In comparison: households spent the largest amount on medications and medical aids; these comprised 78-85 percent of the healthcare-related expenditure of households.) The annual informal payment expenditure was regressive, that is, poorer households belonging to the bottom income fifths spent a higher percentage of their income on informal payments than did wealthier households belonging to the top income fifth. Consequently, informal payments meant a greater burden for poorer households. However, it also emerged that in 2007, through the (temporary) introduction of the “visit fee”, informal payments became proportionate to the income. This may have been for two reasons: poorer households either paid less informal payments, or did not even visit doctors.

Source text - Hungarian Azért, hogy kiszűrjük néhány megfigyelt tényező hatását az egészségügyi szolgáltatások igénybevételének gyakoriságára, valamint egyes betegségek előfordulási valószínűségére, regressziós modelleket becsültünk. A felnőtt népességet és az egészségügyi szakdolgozókat a legalább 24 éves népesség almintáján hasonlítottuk össze. Külön modellekben vizsgáltuk az aktív és volt orvosokat és egyéb diplomásokat az adatoknak erre a három csoportra szűkített almintáján. A különböző egészségügyi szolgáltatások igénybevételének éves gyakoriságát OLS regressziókkal vizsgáltuk, az egyes krónikus, illetve akut betegségek valószínűségét pedig kétkimenetes probit modellekkel. E modellekben a függő változók azt mutatták, hogy az illető szenved-e a megfigyelt krónikus betegségben (vagyis, hogy évente legalább négyszer kiváltotta-e a betegség kezeléséhez használt gyógyszercsoport gyógyszereit). Emellett olyan modellt is lefuttattunk, mely annak valószínűségét vizsgálta, hogy az egyénnek volt-e szívinfarktusa a vizsgált időszakban.
Translation - English In order to remove the effect of a few observed factors on the frequency of healthcare service use, and on the occurrence likelihood of certain diseases, we have estimated regression models. We compared the adult population and specialised healthcare workers using the subsample of the population of 24 years of age or older. We have examined active and former physicians and those with other tertiary educational attainment in separate models, using a subsample of data narrowed down to these three groups. We have examined the annual frequency of the use of the various healthcare services through OLS regressions, and we examined the likelihood of certain chronic and acute diseases through binary outcome probit models. The dependent variables in these models showed whether the individual suffers from the observed chronic illness (that is, whether they have purchased medications belonging to the medication group that is used for treating the given disease at least four times per year). Additionally, we have also run a model that examined the likelihood of whether the individual had a myocardial infarction during the period examined.

Source text - English The effect of psychological and behavioral interventions on the hypothalamic-pituitary-adrenal axis (HPA axis) has also been tested in functional somatic syndromes or disorders. Two studies in chronic fatigue syndrome (CFS) and fibromyalgia (FM) observed that cognitive behavioral treatment alone or in combination with exercise therapy resulted in restoration of cortisol levels. It should be noted that neither study included a control group. Moreover, although these two studies indicated that hypocortisolism in FSDs is reversible by treatment, they did not specifically examine whether these alterations were due to reducing adverse behavioral consequences, such as sleep disturbances, physical inactivity, or pain experience. Cortisol levels may also have a prognostic value, since it has been demonstrated that lower daily cortisol output and a flattened diurnal rhythm predict a poor response to cognitive behavioral therapy in CFS. Interestingly, it has also been reported that CFS patients who respond less well to cognitive behavioral therapy are the ones who are more persistently physically inactive. However, the question whether non-responders with hypocortisolism in this study might represent a physically inactive group has not been assessed and remains to be answered. Preliminary evidence for physical inactivity as a cause of hypocortisolism rather than a confounder comes from a small study performed in 18 regularly exercising healthy adults. Subjects were asked to discontinue their regular aerobics lessons for one week. The subset of healthy subjects that developed symptoms of pain and fatigue after exercise deprivation was characterized by lower cortisol levels at baseline. Authors of this study speculated that the subset of healthy individuals with lower cortisol levels unknowingly exercise regularly to augment the function of HPA axis and thus suppress symptoms.
Translation - Hungarian Pszichológiai és viselkedési intervenciók a hipotalamusz-hipofízis-mellékvese tengelyre gyakorolt hatását is vizsgálták a funkcionális szomatikus zavarokkal kapcsolatban. Két, a krónikus fáradtság szindrómát és a fibromialgiát vizsgáló kutatás azt figyelte meg, hogy a kognitív-viselkedési kezelés önmagában vagy testmozgásos terápiával kombinálva a kortizolszint helyreállását eredményezte. Ám az megjegyzendő, hogy egyik kutatásban sem volt kontrollcsoport. Illetve, ugyan ez a két tanulmány azt jelezte, hogy a funkcionális szomatikus zavarok során fellépő hipokortizolizmus kezeléssel visszafordítható, azt nem vizsgálták meg, hogy ezek a módosulások a káros viselkedési következmények - pl. alvási nehézségek, mozgásszegény életmód vagy fájdalom tapasztalása - csökkenése okán jöttek-e létre. A kortizolszintnek lehet prognosztikus értéke is, mivel kutatások bemutatták, hogy az alacsonyabb napi kortizolkibocsátás és a lapított napi ritmus a kognitív viselkedésterápiára adott elégtelen választ jelzik előre a krónikus fáradtság szindróma esetén. Érdekes módon arról is érkeztek jelentések, hogy azok a krónikus fáradtság szindróma páciensek reagálnak kevésbé jól a kognitív viselkedésterápiára, akiknél tartósabb és jellemzőbb a mozgásszegény életmód. Ugyanakkor azt a kérdést, hogy a hipokortizolizmussal élő nem-reagálók ebben a kutatásban esetleg egy mozgásszegény életmódot élő csoport-e, nem vizsgálták: ez tehát még megválaszolandó. Előzetes bizonyíték arra, hogy a mozgásszegény életmód a hipokortizolizmus oka lehet, nem pedig zavaró tényező, egy 18 rendszeres testmozgást végző, egészséges felnőtt részvételével végzett kutatásban található. Az alanyokat megkérték, hogy egy hétre függesszék fel a rendszeres aerobic óráikat. Az egészséges alanyok azon részhalmazát, akik a testmozgásmegvonásra fájdalom és fáradtság tüneteivel reagáltak, már alapállapotként is alacsonyabb kortizolszint jellemezte. Ennek a kutatásnak a szerzői azt feltételezték, hogy az alacsonyabb kortizolszintű egészséges személyekből álló részhalmaz tagjai tudtukon kívül azért végeznek rendszeres testmozgást, hogy megerősítsék a hipotalamusz-hipofízis-mellékvese tengely működését, és ezzel elnyomják a tüneteket.
