INTRODUCTION
You have been invited to take part in a research study to look at the effect of different doses of an experimental new treatment for high blood pressure (hypertension). Before you decide to take part in this study, you should read this sheet carefully. The sheet will provide you with information about the new drug you are being asked to take and the study procedures you will be involved in.
There are other treatments available to treat your high blood pressure, which your doctor will discuss with you. If you decide not to participate in this study your doctor will advise alternative treatment.
Your doctor will discuss this information with you and if you have any questions, please feel free to ask. You will also be provided with a copy of this sheet.
THE EXPERIMENTAL DRUG
The experimental drug is spirapril.
Spirapril is a well known antihypertensive drug in Japan, where it was granted a marketing authorisation in 1995.
Several studies have already been performed with this drug; about 2, 000 patients and healthy volunteers have already received it.
ABOUT THE STUDY
The aim of the study is to confirm the clinically useful dose-range as well as the optimal dose of spirapril to be administered to patients with high blood pressure.
Therefore we will conduct a large multicentre trial to compare the antihypertensive efficacy and safety of four different doses of spirapril (10 mg twice a day, 20 mg twice a day, 40 mg twice a day and 20 mg once a day) against placebo (non pharmacologically active molecule). 330 patients will be enrolled in the study in about 15-20 Spanish and Italian centers.
The study is divided into two phases. The first phase is called washout / run-in period. During this period lasting two weeks, you will take two capsules a day of placebo (one in the morning and one in the evening). If you are already taking medication for your high blood pressure, then your doctor will ask you to stop your current treatment for the whole time that you are involved in the study.
At the end of the washout run-in period, if your blood pressure is within the limits required by the protocol, you will enter the double blind phase and you will be randomly assigned to one of the four doses of spirapril or to placebo treatment. You will take one capsule in the morning after breakfast (taken from the bottle with a red cap) and one capsule in the evening after dinner (taken from the bottle with a white cap) with a little liquid. The duration of therapy will be 56 days. The day of the visit you are not to take the study drug, which will be administered in the presence of the investigator.
During the double blind period neither you nor your doctor will know which treatment you are getting. In an emergency, however, the doctor can easily find out what your treatment is.
Throughout the study it is important that you try not to take any other medicine(s) and to tell the doctor if you have any illness. If, for any reason, you need to stop taking the new drug or you need other medication then you must tell the doctor immediately.
WHAT IS INVOLVED?
If you agree to take part in the study, you will be asked to sign a consent form and to attend the hospital for up to 7 study visits (a visit every 7 days during the washout run-in period and every 14 days during the double-blind treatment period). Whilst you are taking part in the study, you will be asked not to begin a diet (such as slimming diet), not to add salt to your food or to eat salty food. You will also be asked not to smoke (e.g. cigarettes, cigars & pipes) or drink caffeine-containing beverages (e.g. tea & coffee) before each study visit and not to drink grapefruit juice at breakfast or before and after drug intake.
At the first visit, your medical history will be recorded. A physical examination will be performed three times (at the first visit, at the end of the washout run-in period, at the end of the study) and an electrocardiogram (ECG) at visits 1 and 7 (or at final visit). Your blood pressure and heart rate will be measured at each visit. A blood sample will be drawn at the first visit and at the end of the study. On this occasion a urine sample will be collected.
A subgroup of patients (about 25% of the randomized patients) will undergo 24 hour ambulatory blood pressure monitoring (ABPM) at the end of the washout run-in period and at the end of the study. This procedure enables the measurement of your blood pressure every 20 minutes during all the day and the night. If you accept this procedure, you have to come to the site two additional times to apply and remove the monitoring device: one day before the end of the washout run-in period and one day before the end of the treatment period. ABPM is not an invasive examination.
WHAT ARE THE RISKS?
It is possible that your blood pressure may not be controlled well enough by the study drug. If your blood pressure does become too high or too low, then your doctor will remove you from the study and offer you another treatment.
Known side effects of spirapril are tiredness, headache, dizziness, cough, palpitations. All these side-effects are usually mild and disappear spontaneously as soon as treatment is stopped.
During the study, women who are able to have children will have to use hormonal contraception (e.g. the Pill) or an intra –uterine device (IUD) to avoid pregnancy. They must also have a negative pregnancy test at the start of the study. This is because the effects of the study drug on the foetus are unknown and may be harmful.
WHAT ARE THE BENEFITS?
By taking part in the study, you will benefit from regular blood pressure monitoring, which should improve the chances of controlling your high blood pressure well. The results of this study may give useful information about the treatment of your illness, the effectiveness of this type of drug and may also help in the development of a new treatment for high blood pressure.
CONFIDENTIALITY
Only authorised people (e.g. those directly concerned with the study, and representatives of the sponsor and/or regulatory authorities) are allowed to see your medical records. Any study-related information that identifies you will remain confidential and will not be made publicly available. All data collected from the centre will be anonymous.
COMPENSATION
Your participation in the trial is totally on a voluntary basis; any refusal of yours will in no way jeopardize your relationship with the investigator and any healthcare benefits to which you are entitled. Should any new information about the drug or the study in which you are taking part become available after the signature of this informed consent, it will be communicated to you by the investigator, as appropriate. You are free to withdraw your consent at any time.
On the other hand, you may be excluded from the trial at any time, even against your will,
if the investigator deems it necessary for your health or for proper conduct
of the trial.
The study was approved by competent Authorities, and will be conducted in accordance with the regulations in force concerning clinical research.
All visits and procedures (including laboratory tests) required for the study are free of charge.

FARMACO USATO NELLO STUDIO (SPERIMENTALE)
Il farmaco usato nello studio è spirapril.
Spirapril è un farmaco antipertensivo ben noto in Giappone, dove nel 1995 è stata concessa l’autorizzazione all’immissione in commercio.
Sono già stati eseguiti diversi studi su questo farmaco. Circa 2.000 pazienti e volontari sani hanno già ricevuto il farmaco.
INFORMAZIONI SULLO STUDIO
Lo scopo dello studio consiste nel confermare l'intervallo di dosi clinicamente utile, nonché la dose di spirapril ottimale, da somministrare a pazienti con pressione del sangue alta.
Pertanto, verrà condotto uno studio multicentrico di vasta portata per confrontare l’efficacia e la sicurezza antipertensiva di quattro dosi diverse di spirapril (10 mg due volte al giorno, 20 mg due volte al giorno, 40 mg due volte al giorno e 20 mg una volta al giorno) rispetto al placebo (molecola non farmacologicamente attiva). Nello studio saranno arruolati 330 pazienti in circa 15-20 centri situati in Spagna e in Italia.
Lo studio è costituito da due fasi. La prima fase viene chiamata periodo di washout/run-in. In questo periodo che durerà due settimane, Lei assumerà due capsule al giorno di placebo (una la mattina e una la sera). Se sta già assumendo farmaci per la pressione del sangue alta, il Suo medico Le chiederà di interrompere il trattamento in corso, per tutto il periodo in cui Lei sarà coinvolto nello studio.
Al termine del periodo di washout/run-in, se la Sua pressione del sangue rientrerà nei limiti richiesti dal protocollo, Lei farà ingresso nella fase in doppio cieco e sarà assegnato in modo casuale a una delle quattro dosi di spirapril o al trattamento con placebo. Assumerà una capsula la mattina dopo colazione (presa dal flacone con il tappo rosso) e una capsula la sera dopo cena (presa dal flacone con il tappo bianco), con un po' d'acqua. La terapia durerà 56 giorni. Il giorno della visita non deve assumere il farmaco in studio, che sarà somministrato in presenza del medico responsabile dello studio.
Durante il periodo in doppio cieco, né Lei né il Suo medico sarete a conoscenza del trattamento che Lei sta ricevendo. In caso di emergenza, tuttavia, il medico è in grado di scoprire facilmente a quale trattamento Lei è stato assegnato.
Nel corso dell’intero studio, è importante che non assuma un qualsiasi altro farmaco/eventuali altri farmaci e che riferisca al medico se soffre di una qualsiasi malattia. Se, per una qualunque ragione, deve interrompere l’assunzione del nuovo farmaco o ha bisogno di assumere altri farmaci, è necessario comunicarlo immediatamente al medico.
COSA COMPORTA LA PARTECIPAZIONE ALLO STUDIO?
Se accetta di partecipare allo studio, Le verrà chiesto di firmare un modulo di consenso e di recarsi in ospedale per un numero massimo di 7 visite di controllo (una visita ogni 7 giorni durante il periodo di washout/run-in e ogni 14 giorni durante il periodo di trattamento in doppio cieco). Mentre prenderà parte allo studio, Le verrà chiesto di non iniziare alcuna dieta (ad esempio una dieta dimagrante), di non aggiungere sale agli alimenti, né di mangiare cibo salato. Le sarà anche chiesto di non fumare (p.es. sigarette, sigari e pipe), né di bere bevande contenenti caffeina (p.es. tè e caffè) prima di ogni visita di controllo e nemmeno di bere succo di pompelmo a colazione, o prima e dopo avere assunto il farmaco.
Alla prima visita, sarà registrata la Sua storia medica. Lei sarà sottoposto a una visita medica (esame obiettivo) per ben tre volte (alla prima visita, alla fine del periodo di washout/run-in, alla fine dello studio) e a un elettrocardiogramma (ECG) alle visite 1 e 7 (o alla visita finale). Durante ogni visita, Le saranno misurate pressione del sangue e frequenza cardiaca. Nel corso della prima visita e alla fine dello studio Le sarà prelevato un campione di sangue. In questa circostanza sarà eseguita anche un’analisi delle urine.
Un sottogruppo di pazienti (circa il 25% dei pazienti assegnati in modo casuale) sarà sottoposto a un monitoraggio ambulatoriale della pressione arteriosa nelle 24 ore (ABPM), alla fine del periodo di washout/run-in e alla fine dello studio. Questa procedura consente di misurare la Sua pressione del sangue ogni 20 minuti durante tutta la giornata e la notte. Se accetta questa procedura, deve recarsi presso il centro dello studio altre due volte, per applicare e togliere il dispositivo di monitoraggio: un giorno prima della fine del periodo di washout/run-in e un giorno prima della fine del periodo di trattamento. L’ABPM non è un esame invasivo.
QUALI SONO I RISCHI?
È possibile che la Sua pressione del sangue non venga tenuta sufficientemente sotto controllo dal farmaco in studio. Se la Sua pressione del sangue si alza o si abbassa troppo, il Suo medico La ritirerà dallo studio e Le offrirà un altro trattamento.
Gli effetti collaterali noti di spirapril sono: stanchezza, mal di testa, capogiri, tosse e palpitazioni. Di solito, tutti questi effetti collaterali sono lievi e spariscono spontaneamente, non appena il trattamento viene interrotto.
Durante lo studio, le donne in età fertile dovranno usare un contraccettivo ormonale (p.es. la pillola) o un dispositivo intrauterino (IUD) per evitare la gravidanza. Devono inoltre avere un test di gravidanza negativo all’inizio dello studio. Questo perché gli effetti del farmaco in studio sul feto non sono noti e possono essere dannosi.
QUALI SONO I BENEFICI?
Partecipando allo studio, Lei beneficerà di un regolare monitoraggio della pressione del sangue, il che dovrebbe fornirle maggiori possibilità di tenere adeguatamente sotto controllo la Sua pressione del sangue alta. I risultati di questo studio possono fornire informazioni utili sul trattamento della Sua malattia, sull’efficacia di questo tipo di farmaco e possono anche contribuire allo sviluppo di un nuovo trattamento per la pressione del sangue alta.
RISERVATEZZA
Hanno il permesso di accedere alla Sua documentazione medica solo le persone autorizzate (p.es. coloro che sono coinvolti direttamente nello studio, i rappresentanti dello sponsor e/o delle autorità regolatorie). Qualsiasi informazione relativa allo studio che riveli la Sua identità, rimarrà riservata e non sarà resa pubblica. Tutti i dati raccolti dal centro dello studio rimarranno anonimi.
COMPENSO
La Sua partecipazione allo studio è su base del tutto volontaria. Qualora eventuali nuove informazioni sul farmaco o sullo studio, a cui Lei sta partecipando, siano rese disponibili successivamente alla sottoscrizione del presente consenso informato, il medico dello studio provvederà a comunicargliele, se necessario. Lei è libero di ritirare il Suo consenso in ogni momento.
D’altro canto, Lei può essere escluso dallo studio in un qualsiasi momento, persino contro la Sua volontà, qualora il medico dello studio lo ritenga indispensabile per la Sua salute o per l’adeguata conduzione dello studio.

DETERMINATION OF SAMPLE SIZE
The primary efficacy variable is the change in DBP in sitting position at the end of the treatment (Day + 56) compared to baseline (visit 3, Day 0).
When the sample size in each of the 5 groups is 54, a one-way analysis of variance will have 80% power to detect at the 0.05 level a difference between active treatment and placebo of 4 mmHg. Previous clinical experience with Spirapril indicated that the expected rate of drop-out during the run-in period is about 10%, so it is estimated that 335 patients will be enrolled to randomize 300 patients. The Dropout rate during the double-blind phase is also expected to be about 10%, resulting in 270 patients completing the study.
The estimate of the standard deviation is based upon data from previous study conducted by the sponsor or available in the scientific literature. The average standard deviation for changes in DBP for patients treated with spirapril was 7.5 mmHg.
POPULATIONS FOR ANALYSIS
The primary analysis of change in sitting DBP from Day 0 to Day + 56 will be performed on the intention-to-treat (ITT) population and the per protocol (PP) population. The ITT population is the target population. Any differing results between this and the PP population will be discussed and reviewed. The secondary efficacy variables will be analysed using the ITT population only. All safety summaries will be performed using the safety population.
The ITT population is defined as all randomised patients receiving at least one dose of double-blind study medication and who have at least one post-baseline efficacy assessment. If patients do not complete eight weeks of double-blind treatment, the last available BP readings will be carried forward to Day + 56 for the purpose of the ITT analyses (LOCF). The PP population is defined as all patients in the ITT population, satisfying all inclusion and exclusion criteria and with no major protocol violations. A major violation is defined as a deviation likely to significantly affect treatment efficacy. These violations will be determined before breaking the blind. The safety population is defined as all randomised patients receiving at least one dose of study medication.
ANALYTICAL MEASURES
Summary statistics will be presented for demographic data and baseline characteristics; no formal statistical analyses will be performed on these parameters.
Within treatment comparisons will be performed calculating the 95% C.I. for the changes in DBP to Day +56 from baseline.
An analysis of covariance will be performed, with baseline DBP as covariate, the treatment, centre and interaction centre-by-treatment as factors. A preliminary test for the centre-by-treatment interaction will be performed at 0.10 significance level. If the centre-by-treatment is not significant, the model without interactions will be used. Significant factors and interactions will be investigated further. The change in SBP will be analysed using similar statistical techniques.

La popolazione ITT è la popolazione target. Saranno discussi e riesaminati eventuali risultati discordanti tra la popolazione ITT e la popolazione PP. Le variabili secondarie di efficacia saranno analizzate usando solo la popolazione ITT. Tutti i sunti sul profilo di sicurezza saranno eseguiti avvalendosi solo della popolazione utilizzata ai fini degli studi di sicurezza.
La popolazione ITT viene definita come l’insieme di tutti i pazienti assegnati per randomizzazione ad almeno una dose del farmaco in studio nella fase in doppio cieco e che presentano almeno una valutazione d’efficacia effettuata dopo i valori basali. Se i pazienti non completano le otto settimane di trattamento in doppio cieco, le ultime letture disponibili sulla PA saranno riportate al giorno +56, allo scopo di analizzare la popolazione ITT (LOCF) (Last Observation Carried Forward - Considerazione dell’ultima osservazione effettuata). Si definisce popolazione PP l’insieme di tutti i pazienti della popolazione ITT, che soddisfano ogni criterio di inclusione e di esclusione e senza alcuna violazione grave del protocollo. Si definisce violazione grave una deviazione che probabilmente incide in modo significativo sull’efficacia del trattamento. Tali violazioni saranno stabilite prima dell’apertura del cieco. Si definisce popolazione utilizzata ai fini degli studi di sicurezza, l’insieme di tutti i pazienti assegnati per randomizzazione ad almeno una dose del farmaco in studio.
MISURE ANALITICHE
Saranno presentate statistiche sommarie che riguardano i dati demografici e le caratteristiche basali. Su tali parametri non sarà eseguita alcuna analisi statistica formale.
Nel trattamento si effettueranno confronti, calcolando l’intervallo di confidenza (IC) al 95% per le variazioni della PAD fino al giorno +56, rispetto al basale.
Sarà condotta un’analisi della covarianza, usando la PAD basale come covariata e il trattamento, il centro e l’interazione centro per trattamento (centre-by-treatment) come fattori. Sarà eseguito un test preliminare per l’interazione centro per trattamento al livello di significatività di 0,10. Se il fattore centro per trattamento non è significativo, si userà il modello senza alcuna interazione. Verranno inoltre valutati fattori e interazioni significativi. La variazione della PAS sarà analizzata con l’impiego di tecniche statistiche analoghe.
I pazienti verranno inoltre classificati in pazienti responder o non-responder PAD. La percentuale di pazienti responder sarà confrontata attraverso gruppi di trattamento con l’ausilio del Test Esatto di Fisher. I pazienti responder PAS verranno analizzati mediante tecniche statistiche simili.
L'analisi dell'ABPM sarà effettuata sui pazienti che presenteranno un ABPM valido prima e dopo la randomizzazione. Per ciascun paziente è necessario calcolare la PAD e la PAS medie diurne, notturne e ambulatoriali complessive, mettendo a confronto le differenze tra le visite 3 e 7. L’analisi si baserà su un modello lineare con trattamento e centro come fattori e il valore rilevato al basale (visita 3) come covariata.
L’indice di omogeneità sarà calcolato dopo 8 settimane di trattamento per PAS e PAD. Tale indice si otterrà dividendo la media delle variazioni di PAD e PAS nelle 24 ore (trattamento basale) per la corrispondente deviazione standard. Verrà analizzato usando un modello ANOVA.
Il rapporto valle-picco sarà calcolato utilizzando la mediana dei valori sia di PAS sia di PAD alla 23a e 24a ora dopo la somministrazione mattutina e la mediana dei valori sia di PAS sia di PAD tra la 2a e 8a ora dopo la somministrazione mattutina.
ANALISI DI SICUREZZA PIANIFICATE
Il numero di pazienti che segnalano eventi avversi e le tipologie di evento avverso saranno riassunti per gruppo di trattamento.
Verranno prodotte tabelle di scostamento (shift tables) per i dati di laboratorio, valutando le variazioni dallo screening (visita 2) fino alla fine del trattamento (giorno +56). La frequenza cardiaca e le valutazioni di sicurezza della pressione arteriosa verranno riassunte per gruppo di trattamento, usando la statistica descrittiva. Il numero di pazienti con un ECG, che nel corso dello studio varia da normale ad anomalo, sarà riassunto per gruppo di trattamento.
Tutti gli altri dati di sicurezza verranno sintetizzati in modo appropriato.
Non sarà effettuata alcuna analisi statistica formale sui dati di sicurezza.
