English to Persian (Farsi): Meningococcal C-Conjugate Vaccine Consent Form
Source text - English Meningococcal C-Conjugate Vaccine Consent Form
As of January 2005, Meningococcal C-conjugate vaccine is publicly funded for children 12 years of age (generally in Grade 7) and youth aged 15 to 19 years of age (born 1985 to 1990 inclusive). Although this program is voluntary, immunization against this very serious disease is strongly recommended.
What is Invasive Meningococcal Disease (IMD)?
• IMD is a serious disease caused by the bacteria Neisseria_meningitides.
• It can cause meningitis (swelling of the lining of the brain and spinal cord) or meningococcemia (a serious infection of the blood).
• Every year in Canada there are about 400 cases, and up to 10% result in death.
• Serogroup C is one specific strain of bacteria that can cause IMD and can result in severe long-term health complications or death. This strain accounts for
30-50% of all cases in Canada.
• Serogroup C has been almost exclusively responsible for localized clusters or outbreaks of IMD in schools and communities.
• People with IMD may have fever, severe headache, a stiff neck, nausea, vomiting and sometimes a rash.
How is IMD spread?
• The bacteria that cause IMD can live in the body, in particular at the back of the nose or throat, without causing illness.
• Up to 10% of the population carries the bacteria at any time, however, most people never develop active disease.
• The bacteria is spread by coughing, sneezing, kissing, sharing eating utensils, food, drinks (e.g. straws, cans, water bottles), cigarettes, lipsticks, or other contact where saliva is passed from one person to another.
How can I prevent IMD (Serogroup C)?
• Get the Meningococcal C-conjugate vaccine.
• Do not share eating utensils, drink cans etc.
• Cover your nose/mouth with a tissue when you cough or sneeze.
• Wash your hands!

• Facts about Meningococcal C-conjugate vaccine:
• The vaccine has been shown to provide 90-97% protection against meningococcal disease caused by serogroup C.
• The vaccine is recommend for those in the “high risk” groups – children under 5 years of age, adolescents and people of all ages with certain medical conditions.
• This is not a live vaccine – there is no danger of getting meningitis from the vaccine.
• Only one injection is required for those over one year of age. A booster dose is not thought to be necessary at this time, but studies are ongoing to look into protection lasting more than 5 years.
• Meningococcal C-conjugate vaccine can be given safely with other vaccines, such as the Hepatitis B vaccine.
Does this vaccine have side effects?
• Side effects are usually mild and go away within 1 to 2 days.
• Fewer than half of those who get the vaccine may experience minor side effects such as some soreness, redness or swelling at the needle site.
• Applying ice to the immunization site and/or using acetaminophen can help minimize these reactions.
• Occasionally, headache, decreased appetite, diarrhea, vomiting and/or slight fever may result.
• Serious reactions are very rare and may include difficulty breathing, swelling of the face or mouth, hives or fever over 39ºC.
• If serious side effects occur see your doctor or go to an emergency department.
