Hospital Treatment: 04 to 06 March 2005

Diagnosis:
Atrial fibrillation with Tachyarrhythmia absoluta, now sinus rhythm
Struma cervicalis nodosa
Hypercholsterinemia

History:
The patient noticed a sudden pain in his shoulder during washing his feet. The doctor found a fast heart rate and a low blood pressure, so that the patient was referred to hospital.

Clinical findings:
Heart rate initially 170/min, irregular. No heart murmurs. Lungs normal. Blood pressure in admission 160/100 mmHg.

Further examinations:
ECG: Tachyarrhythmia absoluta, heart rate initially 170/min. No indication for an acute ischemia.
Laboratory: slightly elevated liver enzymes. No elevated heart enzymes. CRP normal.
Echocardiographia: slight left ventricular hypertrophy.
Sonogram of thyroid: one node left lobe (6 x 7 mm), one node right lobe (6 x 8 mm).
Stress-ECG: no sings of a stress-dependent ischemia.
The thyroid hormones are being examined. The results are expected in about 2 weeks.

Therapy and Development:
After treatment with digoxine, beta blockers and anticoagulation spontaneous conversion into sinus rhythm. Follow up examinations of ECG and laboratory were normal. The patient felt well without any further symptoms.

Recommendations:
Medication: Beloc Zok (metoprolol) 1-0-0
Simvastatin as before

Control of cholesterol and blood pressure. Szintigram of thyroid and follow up-ECGs in Italy. The patient should be released from duty for the next two weeks. he should be referred back to Italy by MEDEVAC and is able to fly without any special monitoring equipment.

Mentre si lavava i piedi, il paziente ha avvertito un improvviso dolore alla spalla sinistra. Il medico ha constatato un ritmo cardiaco accellerato e ipotensione arteriosa, pertanto il paziente è stato ricoverato in ospedale.

Rilevamenti clinici:
Ritmo cardiaco iniziale 170/min, aritmico. Nessun soffio cardiaco. Polmoni nella norma. Pressione del sangue al ricovero 160/100 mmHg.

Ulteriori esami:
ECG: Tachiaritmia, ritmo cardiaco iniziale 170/min. Nessun segno di ischemia acuta.
Laboratorio: Enzimi epatici lievemente elevati. Enzimi cardiaci nella norma. CRP normale.
Ecocardiografia: lieve ipertrofia ventricolare sinistra.
Ecografia della tiroide: un nodulo nel lobo sinistro (6 x 7 mm), un nodulo nel lobo destro (6 x 8 mm).
ECG sotto sforzo: nessun segno di ischemia da sforzo.
L’esame degli ormoni tiroidei è in svolgimento. I risultati sono attesi tra 2 settimane circa.

Terapia ed Evoluzione:
Dopo trattamento con digossina, beta bloccanti e anticoagulanti passaggio al ritmo sinusale. Successivi esami ECG e di laboratorio, normali. Il paziente si è sentito bene senza avvertire ulteriori sintomi.

Raccomantazioni:
Medicazioni: Beloc Zok (metoprololo) 1-0-0
Simvastatina come sopra

Controllare colesterolo e pressione del sangue. Una volta in Italia, scintigrafia tiroidea e tracciati ECG di controllo. Il paziente dovrebbe mantenersi a riposo per le prossime due settimane. Dovrebbe essere riportato a casa tramite MEDEVAC senza necessità di particolari apparecchiature per il monitoraggio
