Module Description
The TMC-250 Cardiac Trigger Module is a CT gating module that provides ECG and Respiration synchronization for the purpose of motion artifact removal in the generation of cardiac and other physiological images using CT systems. The module will interface to the patient for ECG “R” wave detection utilizing a proprietary 4 lead patient cable and lead set. The module will interface to the patient for respiration breath detection through a proprietary pillow like pneumatic device attached to the chest by a strap.
The TMC-250 is designed for operation within the CT imaging room for “in the gantry table” installation, and to communicate with the CT system computing resources through a serial communications link. The TMC-250 requires less than 5 watts from a +8 to +24V medical grade power supply.
The TMC-250 communicates three simultaneous ECG vectors, a respiration waveform, an ECG trigger and a respiration trigger to the CT system computing resource.

Tenofovir Disoproxil Fumarate: Long-term oral carcinogenicity studies of tenofovir disoproxil fumarate in mice and rats were carried out at exposures up to approximately 16 times (mice) and 5 times (rats) those observed in humans at the therapeutic dose for HIV 1 infection. At the high dose in female mice, liver adenomas were increased at exposures 16 times that in humans. In rats, the study was negative for carcinogenic findings at exposures up to 5 times that observed in humans at the therapeutic dose.
Tenofovir disoproxil fumarate was mutagenic in the in vitro mouse lymphoma assay and negative in an in vitro bacterial mutagenicity test (Ames test). In an in vivo mouse micronucleus assay, tenofovir disoproxil fumarate was negative when administered to male mice.

Activation and Conjugation for Monovalent Pneumococcal Polysaccharide CRM197 Conjugate Serotype 4
The process development work performed for Monovalent Pneumococcal Polysaccharide CRM197 Conjugate Serotype 4 demonstrates that the manufacturing processes for clinical batches are comparable to those used for process validation, and the processes produce comparable MBC. Process changes and scale up during 13vPnC MBC Serotype 4 clinical and commercial manufacturing were justified by demonstrating comparability of CQA and the associated dependent characteristics data.
There were no significant process changes to the activation process during the various phases of clinical development. The activated pneumococcal polysaccharide intermediates produced for CTM batches was produced in Medford Building 101 using the licensed XYZ commercial process.
Serotype 4 MBC utilized in 13vPnC Phase 1 and 2 and some Phase 3 clinical trials was produced in Medford Building 101 using the licensed XYZ commercial process. Phase 3 material was also produced in Medford Building 101, however during the Phase 3 program, and prior to process validation, the following changes to the Serotype 4 Conjugation process were made:
• Increase in activated pnemococcal polysaccharide input to scale up the conjugate process
• Implementation of a polypropylene rod as a reconstitution device to ensure wetting of the lyophilized material
• The reconstitution temperature was increased from 23 ± 2°C to 37 ± 2°C to facilitate reconstitution of the co-lyophilized saccharide.
• Use of phosphate buffer instead of WFI to prepare the Sodium cyanoborohydride matrix to permit shorter aging time for Sodium cyanoborohydride to remove residual borohydride
• The used of 5 mM Succinate/0.9% NaCl in the MBC final matrix instead of 0.9% NaCl to impart buffering capacity and to harmonize the matrix with the formulated drug product
• The use of disposable polyethylene flexible containers for dispensing of MBC by a closed system operation rather than stainless steel containers
These batches met all release specifications in place at the time when the batches were released and exhibited representative characteristics for the CQA and associated dependent characteristics.
