M: how long you have been working in this specialty?
R: 30 years
M: how does the patient with prostate cancer know he have a disease in the beginning?
R: most of the patients presents with a senile enlarged prostate
M: okay
R: dysuria, burning urination, blood presence in urination, and old aged group. So I run scans and tests, CT scans now are available, if we got suspected, we make biopsy. We do free and total PSA, which also gives us a good indicator of the prostate
M: does the patient expect he has prostate cancer from their symptoms or they think it's minor thing?
R: they think it's a minor thing, even with the presence of blood, the take it simply
M: are there any patients who don’t have symptoms at all, and discover it accidentally?
R: yes, in some cases after performing analysis of the prostate after prostatectomy we discover a carcinoma inside it.

R: this represents 25-20% of the patients
M: they do this operation not because of cancer?
R: yes they do it for other causes.
M: okay, how many percent of the patient comes to you referred from other specialties, and how many comes to see you directly?
R: 20-30% of the cases are referred from other specialties
M: from what specialties?
R: internal medicine then andrology in order.
M: at what stage usually the referred patients come to you?
R: stage 2 usually. We have 3 stages, mild-moderate-severe, and they usually come to me at a moderate stage
M: what about the patients who come to you directly? At what stage?
R: hmm maybe the first stage
M: how frequently you follow up with your patients?
R: at the beginning, there is weekly follow up, then monthly. But the follow-up frequency increases at the winter
M: okay
R: Because it's a seasonal disease from 1/11 till 1/5 due to the congestion. In the summer there is no congestion so he feels better in the summer
M: does the follow-up frequency differ from patient to patient like metastatic or nonmetastatic hormonal sensitive or castrate resistant?
R: sure, most of hormonal cases are managed by the Oncology Institute, not us. We only diagnose then we give medical treatment
M: so you treat hormonal sensitive only, not castrate-resistant?
R: mostly
M: does the frequency of follow up differ from metastatic to nonmetastatic?
R: yes
M: how?
R: in nonmetastatic patients we must do close observation to avoid the patient to turn to metastatic. And in metastatic patients we must do close observation to avoid visceral metastasis like in the liver or the lung
M: okay
R: but there is a good thing regarding prostate cancer, the prostate cancer most of the time is surrounded by a fibrosis so, it has slow spread rate. However it's the major source for more than 60% of tumors in male body
M: okay, what is the total number of cancer patients you saw last month, regardless the type of cancer?
R: around 3-4 whether in my hospital or clinic.
