A study in mice and human tissue suggests that astrocytes harbor HIV.
The HIV virus can take refuge in the brain even when treated with antiretroviral therapies, only to later infect other organs in the body if that treatment is stopped, a new study in mice and human tissue suggests.
Untreated HIV, the virus that causes AIDS, cripples the immune system and leaves the body vulnerable to life-threatening illness. Combination antiretroviral therapy, or cART, can significantly lower concentrations of the virus in the body, to the point that the pathogen can become undetectable, symptoms largely disappear and the treated person is no longer infectious to others. But cART must be taken daily, and if treatment pauses, the virus may reemerge from hidden sanctuaries in the body.
The new study, published June 11 in the journal PLOS Pathogens, suggests that one of these hideouts is in brain cells called astrocytes. Astrocytes constitute roughly 60% of the total cells in the human brain, according to the report, and in an infected person, the study authors estimate that between 1% and 3% of these cells could harbor HIV.
"Even 1% could be significant as a reservoir, as a sanctuary site, for the virus," said study author Lena Al-Harthi, a professor and chair in the Department of Microbial Pathogens and Immunity at Rush University Medical Center in Chicago. "If we're going to try to find an HIV cure, you can't neglect the role of the brain as a reservoir."
Al-Harthi and her colleagues drew their conclusions from a mouse model of HIV injected with human cells, as well as examinations of postmortem human brain tissue. While both experiments provide insight into the role of astrocytes in HIV infection, more work must be done to nail down exactly how the virus takes hold in human patients, an expert told Live Science.
"Animal models can tell us quite a bit. They're not humans, but they can inform us quite a bit," said Dr. Lishomwa Ndhlovu, a professor of immunology in medicine at Weill Cornell Medicine, who was not involved in the study. If astrocytes can act as a reservoir for HIV in human infection, and that virus can exit the brain and trigger infection elsewhere, as the mouse study indicates, "we do need to figure out how to eliminate the virus from these compartments" to devise a successful cure, he said.
Lying in wait
Astrocytes, named for their star-like shape, come in a variety of subtypes and play critical roles in the central nervous system, according to BrainFacts.org, a public information initiative run in part by the Society for Neuroscience. The cells help deliver nutrients to neurons, or the brain cells that transmit electrical signals, and they can spur or subdue inflammatory reactions in the brain. Astrocytes also shape and maintain the wiring of the central nervous system and fortify the blood-brain barrier, a border of tissue separating circulating blood from brain cells.
Scientists knew that the HIV virus infiltrates the brain during infection, as infected people can develop dementia and other cognitive deficits.

HIV, virus penyebab AIDS, jika tidak ditangani dapat melumpuhkan sistem imun dan membuat tubuh rentan terhadap penyakit yang mengancam nyawa. Menurut jurnal tersebut, sekitar 60% dari keseluruhan sel di otak manusia terdiri dari astrosit dan para peneliti memperkirakan pada orang yang terinfeksi HIV 1% hingga 3% diantaranya dapat menyimpan virus.
"Bahkan 1% adalah angka yang bermakna sebagai tempat berlindung virus tersebut," ucap peneliti Lena Al-Harthi, profesor dan ketua Departemen Patogen Mikrobial dan Imunitas, Rush University Medical Center di Chicago. "Jika kita ingin menemukan obat untuk HIV, kita tidak bisa mengabaikan peran otak sebagai penyimpan virus."

Commiphora molmol and Nigella sativa

Abstract: The search for an ideal dressing for diabetic foot wounds/ulcers is still ongoing. Patients’ preferences of such dressing usually relate to their local cultural background. The aim of this study was to evaluate the antibacterial effectiveness of some natural herbal medicines and honey mixtures on clinical isolates from diabetic wounds. Six mixtures consisting of honey, Commiphora molmol and Nigella sativa at different concentrations have been prepared (NP1 to NP6). All mixtures were effective against the tested bacterial isolates from three different diabetic food wound patients at 50%. NP4 and NP5 were the most effective as antibacterial for the examined seven bacterial isolates among the mixtures as they were able to kill bacteria at 25% after 24 hours of incubation. An over additive action has been noticed between honey and
herbal medicines. The effectiveness of mixtures was dose-dependent of Commiphora molmol; NP5 contained the highest amount of the later. Further research is needed to elucidate and optimize the effective combination of these natural products in clinical practice.
Diabetes is a chronic debilitating medical condition which is reaching epidemic proportions in Saudi Arabia. At least one out of five Saudis is affected and self-medication with oral natural preparations and herbs are fairly common across the country as part of complementary and alternative medicine (CAM). Foot disorders are among the most feared complications of diabetes and the ultimate endpoint of diabetic foot ulcer is amputation if not well treated. When amputation happens, it is usually associated with significant mortality, in addition to immense social, psychological and financial consequences.
Medicinal plants are used in developing countries where it is believed to have lesser side effects and lower cost. Moreover, its use depends on many factors including cultural background, education, socioeconomic factors, and nature of health problem and availability of the remedies in the local market. Honey is one of the most widely used natural products not only in Saudi Arabia but also in all Arabian countries. It has been used since ancient times as a remedy in wound care.
Honey has potent antibacterial activity and is effective in preventing and clearing wound infections. Topical honey was shown to be effective in treating postoperative skin wounds in neonates that had failed to respond to antibiotic therapy. It has been demonstrated in many studies that the antibacterial effects of honey are attributed to its high osmolarity, low pH, hydrogen peroxide content, and presence of other uncharacterized compounds. In Saudi Arabia, some herbs, natural products and CAM preparations are occasionally added to honey or used separately by diabetics.

Abstrak: Pencarian perawatan yang ideal untuk luka/tukak diabetes di kaki masih terus dilakukan. Pasien biasanya lebih memilih perawatan yang sesuai dengan latar belakang budaya mereka. Tujuan penelitian ini adalah untuk menilai efektifitas antibakteri campuran beberapa obat herbal alami dan madu pada kultur klinis luka diabetes. Enam campuran yang terdiri dari madu, Commiphora molmol dan Nigella sativa dengan konsentrasi yang berbeda telah disiapkan (NP1 sampai NP6). Semua tipe campuran efektif melawan kultur bakteri luka diabetes yang diambil dari tiga pasien berbeda hingga 50%. NP4 dan NP5 adalah campuran yang paling efektif sebagai antibakteri pada tujuh kultur yang diamati karena mereka mampu membunuh 25% bakteri setelah 24 jam inkubasi. Aksi tambahan lain juga ditemukan pada campuran madu dan obat herbal. Efektifitas campuran ini tergantung pada dosis Commiphora molmol; NP5 mengandung Commiphora dengan jumlah tertinggi. Riset lebih mendalam dibutuhkan untuk menemukan kombinasi optimal yang paling efektif dari produk alami ini untuk penggunaan klinis.

PENDAHULUAN
Diabetes adalah kondisi medis kronis melemahkan tubuh yang telah mencapai proporsi epidemik di Arab Saudi. Setidaknya satu dari lima orang warga Arab Saudi menderita diabetes dan pengobatan mandiri dengan herbal dan preparat oral alami umum ditemukan di penjuru negeri sebagai bagian dari terapi alternatif dan komplementer (TAK). Kerusakan pada kaki adalah salah satu komplikasi diabetes yang paling ditakuti, dan satu-satunya cara untuk mengatasi tukak diabetes yang tidak tertangani dengan baik adalah dengan amputasi. Jika amputasi dilakukan, ini biasanya berkaitan dengan angka kematian yang signifikan, belum lagi dampak sosial, psikologis, dan ekonomi yang cukup berat.
Tumbuhan obat digunakan di negara berkembang karena dipercaya memiliki efek samping yang lebih sedikit dan biaya yang lebih murah. Selain itu, penggunaannya juga dipengaruhi oleh banyak faktor termasuk latar belakang budaya, pendidikan, faktor sosioekonomi dan akar masalah kesehatan, serta ketersediaan obat tersebut di pasar lokal. Madu telah digunakan sejak zaman dahulu kala sebagai obat dalam perawatan luka.
Madu memiliki aktivitas antibakteri yang kuat dan efektif dalam mencegah dan membersihkan infeksi luka. Madu topikal terbukti efektif merawat luka pasca-operasi pada neonatus yang sebelumnya gagal merespon pengobatan dengan antibiotik. Banyak penelitian telah menunjukkan bahwa efek antibakteri madu dikaitkan dengan osmolaritasnya yang tinggi, pH yang rendah, kandungan hidrogen peroksida, dan keberadaan senyawa tidak terkarakterisasi lainnya. Di Arab Saudi, beberapa tumbuhan herbal, produk alami dan TAK ditambahkan ke dalam madu atau digunakan secara terpisah oleh penderita diabetes.

Source text - English Tertiary irrigation and Drainage System
From the TOR Deliverables for the Detailed Engineering Design Reports, it is required to include: c) Detailed engineering design of Irrigation including Tertiary and drainage canal and related structure.
Relative to this issue, the Design of Tertiary Irrigation and Drainage System is a very huge task for a very short period of time. Based on personal experiences, the scope of works for this magnitude requires years to finish the task just for the detailed design of the Tertiary Irrigation and Drainage canal and appurtenant structures. The Tertiary design works will be in parallel with the detailed design of the main Irrigation and Drainage system.
The design of the secondary canal requires details of off-takes. The off-take location, the highest elevation of the field and slope of the ground will be the bases also in the design of water surface elevation of the secondary canal where the off-take of the Tertiary canal shall be placed.
In this presumption, The Design Engineer cannot do the design works at the same time for the main system and the tertiary system. Hence additional Irrigation and Drainage Design Engineers are required to do the work on time.
The Tertiary Block
The size of Tertiary Block is 50 ha. And has 4 to 5 Tertiary Units of 10 ha to 12.5 ha each unit. The proposed Walimpong Irrigation Scheme has no technical irrigation scheme. The proposed area of development is 27,000 ha. Developing the tertiary block for Walimpong Irrigation scheme will be approximately 540 tertiary blocks. The Tertiary block is composed of 1 Tertiary canal, 4 or 5 Quaternary Canal, 1 Collector Drain (dk) and 4 to 5 farm drains (dt). It also needs 1 box and 4 to 5 quaternary boxes. A tertiary block will also need Farm Roads.
Preparation of Tertiary Block Layout
1. Prepare Preliminary Layout – prepare draft layout in the Topographic survey with the correct scale
2. Checking of Preliminary Layout – verify and check draft layout in the field if there is time
3. Finalization of layout – prepare the final scheme layout
To summarize, the detailed design works needed for a tertiary block will be:
1. The Tertiary Block boundaries will be marked on the Scheme layout after the main irrigation and drainage system has been drawn or set;
2. The Tertiary blocks shall be maximum of 50 ha;
3. 1 Tertiary Canal of 800 m -1000 m length;
4. 4 or 5 quaternary canal of 250 m to 300 m length
5. 1 Collector Drain (dk) of 800m – 1000 m length
6. 4 or 5 Farm Drain (dt) of 250 m to 300 m length
7. 1 offtake structure
8. 2 to 3 Tertiary Boxes
9. 4 Quaternary Boxes
10. 4 to 5 Farm cross drain structures
11. 4 to 5 Farm Road crossing structures

Luas lahan yang diusulkan adalah 27.000 ha. Petak Tersier yang dibutuhkan untuk Skema Irigasi Walimpong diperkirakan sebanyak 540 petak tersier. Petak Tersier terdiri dari 1 saluran tersier, 4 sampai 5 saluran kuarter, 1 saluran pengumpul (dk) dan 4 sampai 5 drainase lapang (dt). Petak tersier memerlukan 1 boks tersier dan 4 sampai 5 boks kuarter. Petak tersier juga membutuhkan jalan pertanian.
