You are invited to participate in a research study investigating COVID-19 infection. Please read this form and ask any questions you may have before agreeing to be in the study.
Purpose: The purpose of this study is to try to understand why COVID-19 infections cause this excess immune reaction, and try to find ways to treat it.
Procedures: Participation in this study will involve completion of a medical questionnaire survey, blood draw, and nasal or throat swabs, and the option to participate in a follow-up study 30-90 days later.
Risks: Risks of this study are minimal. Some of the foreseeable risks or discomforts of your participation include drawing blood, which may cause temporary discomfort or the needle stick may hurt. There is a small risk of bruising and fainting, and a rare risk of infection. Nasal and throat swabs can be uncomfortable and cause tearing eyes or runny nose, or a gagging reflex, but the sensation is only temporary.
Benefits: You will not directly benefit from this study. If you agree to be re-contacted, you will be invited (along with your family) to join the researchers at a conference at the end of the study where we will present our results and provide information and guidance that may help improve the health for COVID patients. You will also be able to share your experience and ask questions that could be of benefit to you and your community. Your participation has the potential to help the medical and scientific communities better understand the role of COVID-19 related immune responses.
Alternatives: The alternative is not to participate in the study.
Compensation: You will not be compensated for this part of the study. However, if you participate in the optional follow-up study, you will receive a $25 visa gift card at the follow-up blood draw to thank you for your participation, and an additional $25 visa gift card for an optional lung function test that can be conducted at the same visit.
Voluntary Participation: Your participation in this study is voluntary. You can decide to participate or not to participate, or to withdraw from it at any point without penalty or loss of benefits to which you are otherwise entitled to or already have.
