Abstract
Purpose of Review
Diet is a pillar of type 2 diabetes mellitus (T2DM) management. Intermittent fasting (IF) is postulated as a novel approach, able to improve glucose control and potentially capable of reversing some of the pathophysiological alterations of this condition. In this review, the molecular and clinical evidence of diets based on intermittent energy restriction (IER) in laboratory animal models and subjects with type 2 diabetes is discussed. The mechanisms through which IF are thought to improve glucose homeostasis and reverse β cell failure are also reviewed.
Recent Findings
Studies derived from murine models suggest that IER is associated with improvements in β cell function and insulin resistance. Two main mechanisms have been demonstrated, one derived from the autophagy-lysosome pathway and, the other from an increase in neurogenin3 (Ngn3) levels (a marker for endocrine progenitor cells like β cells during development). Notably, IER also promotes reconstruction of gut microbiota. In mice, all effects were independent of weight loss. By contrast, in human studies, outcomes are widely attributable to weight loss. The more consistent results are reductions in body weight, visceral fat, and glucose and insulin levels. Increases in HDL cholesterol levels are also frequently reported. The decrease in insulin levels observed in humans is in opposition with the increase reported in mice, suggesting that the main mechanism in humans is an improvement in peripheral insulin action.
Summary Recommending diets based on intermittent fasting in humans is based on the promising results found in animal models where an improvement in β cell function has been recorded. β cell function after IF has not been assessed in human subjects with T2DM. This review provides information regarding different protocols for the implementation of IF in diabetic persons and also provides important safety advice in order to avoid adverse effects. Clinical studies do not show an increased risk of hypoglycemia, and a recent case series reported reversal of T2DM.

O jejum intermitente (JI) é postulado como uma nova abordagem, capaz de melhorar o controle glicêmico e potencialmente capaz de reverter algumas das alterações fisiopatológicas dessa condição. Nesta revisão, são discutidas as evidências moleculares e clínicas de dietas baseadas em restrição intermitente de energia (RIE) em modelos de animais de laboratório e indivíduos com diabetes tipo 2. Os mecanismos pelos quais se pensa que o JI melhora a homeostase da glicose e a reversão da falência das células β também são revisados.
Achados Recentes
Estudos derivados de modelos murinos sugerem que a RIE está associada a melhorias na função das células β e na resistência à insulina. Dois mecanismos principais foram demonstrados, um derivado da via autofagia-lisossomo e outro de um aumento nos níveis de neurogenina3 (Ngn3) (um marcador para células progenitoras endócrinas, como células β, durante o desenvolvimento). Notavelmente, a RIE também promove a reconstrução da microbiota intestinal. Nos camundongos, todos os efeitos foram independentes da perda de peso. Por outro lado, nos estudos em humanos, os resultados são amplamente atribuíveis à perda de peso. Source text - Portuguese Este estudo trata sobre a proposta de lei complementar número duzentos e setenta e sete do ano de dois mil e oito, que trata sobre a implementação do Imposto Sobre Grandes Fortunas. Abordar-se-á sobre a eficácia no combate à pobreza, redistribuição de renda e eventuais problemas arrecadatórios e sociais que o mesmo pode trazer para o país. A linha de pesquisa utilizada está dentro da área de ética, sustentabilidade e administração pública. Para confecção do mesmo, utilizou-se de material doutrinário, legislativo e documental, analisando e comparando as opiniões e dados fornecidos para expor a situação e analisar quanto ao tamanho do problema estudado. O objetivo é analisar as diferentes correntes de pensamento que constroem a doutrina em relação à proposta de imposto sobre grandes fortunas, na forma da proposta de lei complementar. O imposto busca combater a desigualdade através de um sistema que cobre de quem mais possui renda para transmitir, via estatal, a quem menos possui. Porém, é estudado que isso pode acarretar em um desestímulo à produção interna brasileira e, consequentemente, causar fuga de investimentos e capitais do território nacional, o que agravaria a crise econômica vigente. Para tanto, o imposto não deve ser opressor ao ponto de causar esse tipo de problema financeiro, e também não deve ser brando com quem mais possui, impossibilitando a ação estatal no combate a desigualdade.

Translation - English This study deals with the proposed complementary law number two hundred and seventy-seven in the year two thousand and eight, which deals with the implementation of the Wealth Tax. It will focus on the effectiveness in combating poverty, income redistribution and eventual tax and social problems that it can bring to the country. The line of research used is within the area of ethics, sustainability and public administration. For its production, doctrinal, legislative and documentary material was used, analyzing and comparing the opinions and data provided to expose the situation and analyze the size of the problem studied. The objective is to analyze the different currents of thought that build the doctrine in relation to the proposed wealth tax, in the form of the proposed complementary law. The tax seeks to combat inequality through a system that charges those who have more income to transmit, through the state, to those who have less. However, it is studied that this may result in a disincentive to Brazilian domestic production and, consequently, cause a flight of investments and capital from the national territory, which would aggravate the current economic crisis. To this end, the tax should not be so oppressive as to cause this type of financial problem, nor should it be mild to those who own the most, making state action to combat inequality impossible.

Source text - Portuguese O grande impacto econômico e social que a indústria da construção civil tem sobre o Brasil, e a própria demanda atual, encaminham a necessidade de discutir sobre seus processos de organização produtiva e tecnológica, incluindo conceitos de qualidade relacionados ao ambiente e a sustentabilidade. A alternativa já utilizada em grande escala por países desenvolvidos é a construção a seco com estrutura constituída por perfis de aço formados a frio, conhecida como Light Steel Framing. Trata-se de um sistema construtivo de concepção racional e industrializado, rápido e sustentável. Desse modo, este trabalho apresenta a viabilidade da competitividade no mercado de um método construtivo sustentável em formas orçamentárias, evidenciando para meios comparativos os pontos positivos e negativos de ambos sistemas, e levando em conta ainda a inserção de inovações em meio cultural e social. Utilizando mecanismos orçamentários como o SINAPI, um projeto arquitetônico de uma casa unifamiliar em Steel Frame foi adaptado a uma estrutura convencional e orçado de acordo com a estipulação da quantidade de seus componentes. O orçamento na construção a seco, já fornecido por uma empresa do ramo, foi comparado com a tabela orçamentária realizada no método construtivo convencional. Com isso, pode-se concluir que embora industrializada, a construção em Light Steel Framing apresenta valores mais altos, podendo ser motivados pelo desconhecimento do mercado local, ou situações em que as empresas detém a fabricação e o fornecimento do material ou que, simplesmente, a construção industrializada passa a valer a pena a partir de uma escala que a região ainda não comporta.

Translation - English The great economic and social impact that the construction industry has on Brazil, and the current demand itself, lead to the need to discuss its processes of productive and technological organization, including concepts of quality related to the environment and sustainability. The alternative already used on a large scale by developed countries is dry construction with a structure consisting of cold-formed steel profiles, known as Light Steel Framing. It is a constructive system of rational and industrialized design, fast and sustainable. Thus, this work presents the feasibility of competitiveness in the market for a sustainable construction method in terms of its budget, showing for comparative means the positive and negative points of both systems, and also taking into account the insertion of innovations in cultural and social environments. Using budgetary mechanisms such as SINAPI, an architectural project for a single-family house in Steel Frame was adapted to a conventional structure and budgeted according to the stipulation of the quantity of its components. The dry construction budget, already provided by a company in the industry, was compared with the budget table made using the conventional construction method. With this, it can be concluded that although industrialized, the construction in Light Steel Framing presents higher values, which may be motivated by the ignorance of the local market, or situations in which the companies stop the manufacture and supply of the material or that, simply, industrialized construction becomes worthwhile on a scale that the region does not yet support.
