Effets du taping McConnell et du Kinesio-Taping sur la douleur chez les patients souffrant d’un syndrome fémoro-patellaire : revue systématique de la littérature

Contexte
Différentes techniques sont à la disposition du kinésithérapeute dans la prise en charge des syndromes fémoro-patellaires (SFP). Le taping propose une solution à l’efficacité controversée. Le choix d’intégrer cet outil à son arsenal thérapeutique doit passer par une démarche d’évaluation pour ne pas succomber aux effets de mode.
Objectif
Évaluer et comparer les bénéfices du Kinesio-Taping (KT) et du taping McConnell (MT) sur la douleur dans le cadre du syndrome fémoro-patellaire.
Méthode
Des articles évaluant chaque type de taping ont été recherchés via MEDLINE, Cochrane Library, CINAHL, Science Direct, SPORTDiscus et PEDro. Les études devaient évaluer la douleur et respecter des critères d’inclusion précis. Leur qualité méthodologique a été évaluée par l’échelle PEDro, et leur résultats ont été extraits et analysés.
Résultats
Dix-neuf articles respectent les critères d’inclusion et de qualité. Le MT apparaît comme un meilleur choix dans la recherche d’un effet antalgique, à condition que la correction soit adaptée à la mesure de l’alignement de la patella du sujet. Le KT présente des résultats positifs sur la douleur lors de comparaisons avant et après son application. Ces résultats sont en revanche trop peu significatifs quand il est comparé à un groupe contrôle. Les études disponibles sont globalement de qualité assez faible et les mécanismes d’action de chacune des techniques restent peu démontrés.
Conclusion
Le taping McConnell permet une diminution des phénomènes douloureux chez les patients présentant un SFP. Le Kinesio-Taping souffre d’un manque d’études de bonne qualité pour permettre d’arriver aux mêmes conclusions. La douleur ne constitue cependant qu’un critère parmi d’autres dans le choix du kinésithérapeute.
Introduction
Le syndrome fémoro-patellaire (SFP) est fréquemment rencontré en rééducation, et touche principalement l’adolescent et le jeune adulte. Il s’agit de la pathologie du genou générant le plus de consultations en médecine générale ainsi qu’en médecine du sport, où elle se développe volontiers chez les coureurs [1]. Les patients atteints de SFP décrivent des douleurs diffuses en regard de l’articulation fémoro-patellaire, aggravées lors d’activités telles que la montée et la descente d’escaliers, la course, ainsi que lors de la position assise prolongée.
Le mécanisme de cette pathologie est complexe et multifactoriel. Il se traduirait notamment par un mauvais engagement de la patella dans la trochlée fémorale, augmentant la pression sur les facettes articulaires latérales [2] ; [3]. Ce défaut d’engagement peut résulter d’une association de différents facteurs. Une augmentation de l’angle Q du genou favorise la traction latérale de la patella par les structures actives et passives qui l’entourent [2]. Le déséquilibre musculaire entre vaste médial (VM) et vaste latéral (VL) est lui aussi souvent mis en cause. Ce déséquilibre peut se traduire soit par une diminution de la force du VM, soit par une augmentation de son délai d’activation [4]. Les troubles de la statique des articulations sus- et sous-jacentes peuvent être également recherchés, en particulier un valgus de cheville prononcé ou la tendance à l’adduction et à la rotation interne excessive de la hanche [3]. Des réserves sont à émettre sur ces interprétations. Des défauts d’alignement ne sont pas systématiquement retrouvés chez les patients souffrant d’un SFP, mais ces mesures évaluées en statique ne nous renseignent pas assez sur la cinétique de la patella lors du mouvement [2].
Le SFP touchant particulièrement les sujets jeunes et sportifs, on retrouve fréquemment l’utilisation du taping dans la prise en charge de ces patients. Deux d’entre eux sont communément rencontrés dans l’arsenal thérapeutique à disposition du kinésithérapeute. Le taping décrit par Jenny McConnell et le Kinesio-Taping décrit par Kenzo Kase.
Le taping McConnell (MT) est apparu au début des années 1980, et comprend l’examen de la position de la patella et sa correction par un tape rigide [5]. Son auteur décrit trois types de positions pathologiques de la patella : Une composante de « glide » (translation latérale), une composante de « tilt » (inclinaison), ainsi qu’une composante correspondant à la rotation de la patella dans son plan frontal. La correction de ces défauts d’alignement par la pose d’un tape implique une première bande de protection cutanée en regard de la face antérieure du genou, suivi par une bande adhésive non élastique appliquée en regard de la patella. Le thérapeute y applique une tension dans le sens de la correction désirée ( Fig. 1).
Le Kinesio-Taping (KT) a été créé dans les années 1970 par Kenzo Kase, et utilise des bandes adhésives élastiques dont l’orientation des fibres permettrait d’avoir un impact sur l’activité et l’extensibilité musculaire, la proprioception et la circulation sanguine et lymphatique [6]. La traction exercée dépend de l’effet recherché, et provoqueraient des stimuli des mécanorécepteurs sous-cutanés, ainsi qu’une « libération » et une réorientation des structures fasciales sous-jacentes. L’engouement pour cette technique est relativement récent, d’où un nombre limité d’études, rarement comparables entre elles du fait des variations des techniques de pose et des pathologies visées par celles-ci. Dans le cadre du SFP, son application est fréquemment retrouvée en regard du quadriceps (Fig. 2).

Face aux différentes techniques à sa disposition, le masseur-kinésithérapeute se doit d’adopter une démarche scientifique pour faire un choix basé sur l’efficacité et non sur les « modes » que peuvent créer l’industrie de la santé. Cette démarche d’« Evidence Based Practice » (EBP) se distingue ainsi d’une pratique empirique basée uniquement sur les expériences du thérapeute ou sur ses croyances [7]. L’objectif de cette revue est d’explorer la littérature de façon méthodologique et scientifique pour justifier l’utilisation du MT ou du KT chez des patients souffrants d’un syndrome fémoro-patellaire. Des revues semblables ont déjà étudié séparément chacune de ces techniques, mais aucune n’a visé à les confronter [8]; [9]; [10]; [11] ; [12]. Les deux techniques ayant des modes d’actions différents, nous avons choisi d’utiliser la douleur comme variable de comparaison.
Méthode
Stratégie de recherche bibliographique
Des études évaluant les effets de chaque type de taping individuellement ou en les comparant entre eux ont été recherchées dans des bases de données électroniques. La recherche via MEDLINE, Cochrane Library, CINAHL, Science Direct et SPORTDiscus a été réalisée en utilisant une combinaison de mots clés : « (knee* OR patell*) AND (tape OR taping OR kinesio* OR mcconnell) AND pain ». Les recherches sur la base de donnée PEDro s’est faite en cherchant le terme « patell* » dans les rubriques « taping », « pain » et « knee ». La dernière recherche est datée du 24 août 2015. Afin d’éviter les biais de publication, des experts ont été contactés par e-mail pour recenser des articles non publiés ou issus de la littérature grise. Une recherche complémentaire d’articles a été réalisée dans un second temps via les bibliographies ou les citations présentes dans la littérature.
Sélection des articles
Seuls les articles sous forme d’études contrôlées et d’études croisées sont éligibles dans ce protocole de sélection, les cas cliniques et autres rapports de congrès présentant peu de niveaux de preuves. Des contraintes linguistiques ont été appliquées, et n’ont été retenus que les articles rédigés en français, en anglais ou en espagnol. Cette première étape de sélection des articles a été réalisée sur lecture du titre et du résumé. La deuxième étape de sélection s’est appuyée sur une lecture complète. Elle a pour objectif de ne conserver que les études répondant à des critères d’inclusion précis. Les critères suivants ont été imposés :
population : Sujets adultes avec diagnostic de SFP ou de douleur antérieure de genou sans lésions structurelles manifestes (gonarthrose, tendinopathie, antécédents d’entorse ou de luxation, antécédents chirurgicaux…). Les études évaluant uniquement des sujets sains sont exclues ;
• critère d’évaluation : Douleur mesurée en tant que critère principal ou secondaire. L’outil d’évaluation de la douleur doit fournir une mesure quantitative, à l’aide par exemple d’une échelle visuelle analogique (EVA) ou d’une échelle numérique (EN) ;
• interventions :
∘ pour les études évaluant le KT, c’est l’application en regard de la patella et du quadriceps qui a été retenue. Aucune technique de pose propre au SFP n’a fait état d’un consensus. Les variations dans les techniques de pose ont donc été acceptées pour éviter une inclusion trop restrictive, tant que celles-ci présentait une application en regard du quadriceps,
∘ pour les études évaluant le taping type McConnell, la correction systématique par un glide médial sans évaluation préalable de la position de la patella a été acceptée. Cette simplification est souvent retrouvée dans la littérature, mais les données seront évaluées séparément du protocole standard par soucis de qualité méthodologique.
Évaluation de la qualité des articles
L’échelle PEDro a été utilisée pour l’évaluation de la qualité méthodologique des articles respectant les critères d’inclusion. Il s’agit d’une échelle de 11 critères permettant de juger la validité interne d’une étude et de mettre en évidence les éventuels biais présents dans celle-ci. Les items recherchés concernent notamment l’éligibilité des sujets, la randomisation, la similarité des groupes en début d’étude, le caractère « aveugle » des sujets, des thérapeutes et des examinateurs, le suivi, ou encore la comparaison des groupes. Pour chaque critère respecté, un point est accordé et une note totale sur 10 est attribuée (le premier item ne rentre pas dans le calcul final). Plus le score obtenu est élevé, plus l’étude est de bonne qualité.
Nous avons choisi de conserver les articles présentant un score modéré (4 à 5) à fort (supérieur à 5), et de ne pas retenir les articles bénéficiant d’un score inférieur à 4. Les résultats des articles retenus ont été extraits, interprétés et classés sous forme de tableaux. Pour faciliter leur analyse, ils ont été divisés en trois groupes, selon qu’ils traitent du Kinesio-Taping, du taping McConnell, ou du taping McConnell modifié (avec une correction en glide médial systématique).
Résultats
Résultats de la recherche
Le détail des résultats de la recherche est disponible en Tableau I. La recherche dans les bases de données a fourni 1280 résultats (la présence de nombreux doublons entre les bases de données n’a pas pu être comptabilisée à ce stade et explique ce chiffre élevé). Il est intéressant de noter qu’aucun article en français n’a été retrouvé.

Après sélection préliminaire basée sur le titre et le résumé, 87 articles ont été retenus, soit 40 après retrait des doublons. 4 articles supplémentaires ont été intégrés via la littérature grise ou suite à leur présence dans les bibliographies des articles précédents. La lecture des 44 articles a permis l’exclusion de 17 d’entre eux par non respect des critères d’éligibilité :

Au final, 27 articles ont été retenus pour l’évaluation de leur validité interne.
Qualité méthodologique des études.
Les 27 articles ont été évalués et présentent un score moyen de 4,40, avec un écart-type de 1,72 (Tableau II). 19 d’entre elles ont présenté un score PEDro supérieur ou égal à 4 et ont intégré la liste définitive (Fig. 3). À noter qu’aucune étude ne valide le critère « thérapeute en aveugle », puisque l’application des deux types de tape nécessite une connaissance de la technique de pose. Seulement deux études valident le critère d’assignation secrète [13] ; [14]. Globalement, les résultats obtenus mettent en évidence un faible nombre d’études présentant une qualité méthodologique satisfaisante. La validité externe, non évaluée par l’échelle PEDro, souffre de la faible taille des échantillons étudiés. Onze études ont une population inférieure ou égale à 20 patients. L’échantillon le plus élevé est retrouvé dans l’étude de Clark, avec 81 sujets [15]. Ces échantillons sont dans certaines études peu représentatives représentatifs. Quatre articles n’étudient que des sujets femmes [6]; [16]; [17] ; [4], 1 évalue une population uniquement militaire [18], et 2 études imposent un critère d’activité physique minimum dans la sélection de leurs patients [16] ; [19].

Une vue d’ensemble des articles retenus met en avant un nombre très important d’études évaluant le taping McConnell par rapport au Kinesio-Taping. Cette différence vient du fait que cette dernière est une technique qui s’est développée plus récemment, et que ses champs d’application sont beaucoup plus larges que le simple traitement du SFP. Au total, ce sont donc 6 études concernant le KT qui ont été analysées.
Les études évaluant le taping McConnell par une simple correction en glide médial ont été regroupées dans un groupe distinct, pour permettre de pondérer les conclusions selon la présence ou non d’une évaluation préalable de l’alignement de la patella. Ainsi, 8 articles concernent le taping par simple médialisation de la patella, et 7 articles étudient le taping McConnell tel qu’il a été originalement décrit, avec correction en fonction de la position de la patella.

Abstract
Context
Different techniques are available to physiotherapists in the management of patellofemoral pain syndrome (PFPS). As a proposed treatment, the effectiveness of taping is a controversial one. When choosing whether or not to integrate this technique into their therapeutic arsenal, the physoptherapist must take an evaluative approach to avoid succumbing to the pitfalls of fashionable therapies.
Objective
To evaluate and compare the benefits of Kinesio-Tape (KT) and McConnell Tape (MT) when treating pain associated with Patellofemoral pain syndrome.
Method
A range of articles evaluating each type of taping has been studied from MEDLINE, Cochrane Library, CINAHL, Science Direct, SPORTDiscus and PEDro. The studies should evaluate pain and respect and maintain the precise inclusion criteria. Their methodological quality have been evaluated by the PEDro scale and their results have been extracted and analysed.
Results
The 19 articles used met the inclusion and quality criteria. McConnell Tape seemed to be a better choice in the study for analgesic effect, provided that the correction is adapted to the specific patella alignment of the subject. Kinesio-tape presented positive results for pain-relief when comparing before and after its application. These results are however not significant enough when compared to a control group. Studies available worldwide are not of a high enough quality and the mechanisms of actions for each technique remain poorly demonstrated.
Conclusion
McConnell taping caused a reduction of painful phenomena among people presenting with PFPS. Kinesio-taping suffers from a lack of quality research which prevents the determination of similar conclusions. Pain only represents one of a number of criteria considered in the physiotherapist’s decision-making process.
Introduction
Patellofemoral pain syndrome (PFPS) is a common issue requiring rehabilitation. It typically affects teenagers and young adults. This particular knee problem causes the most consultations in general practice as well as in sports medicine, where it readily presents among runners. Patients suffering with PFPS describe anterior knee pain, aggravated by activities such as climbing or descending stairs, jogging as well as prolonged periods of sitting.
The mechanics of this condition are complex and multifactorial. The condition presents primarily as a poor connection of the kneecap within the femoral trochlea, which increases pressure on the lateral facet joints. Such a defect can result in a variety of consequences. An increase in the Q angle of the patella promotes lateral traction by the active and passive structures that surround it. A muscular imbalance between the Vastus Medialis Oblique (VMO) and the Vastus Lateralis (VL) is also often the cause. We can explain this imbalance as a decrease in power of the VMO, due to a delay in the muscle’s activation. Issues with the static stabilisers above and below must also be analysed, particularly in cases of excessive ankle valgus or those with a tendency to adduction and excessive internal hip rotation. There are reservations about these interpretations. We did not exclusively find problems of malalignment in patients suffering from PFPS. Furthermore, the static measures evaluated do not provide much information in terms of movement on the kinetics of the patella.
PFPS often presents in the young and in subjects who partake in sport. The use of taping is common in the management of such patients. Two such examples are commonly adopted in the therapeutic arsenal available to a physiotherapist.
McConnell taping (MT) appeared at the start of the 1980s and was used alongside an examination of the position of the patella to correct any malalignment via the use of a rigid tape. The original author described three pathologic positions of the patella: An element of “glide” (lateral movement), an element of “tilt” (tilting), as well as an element corresponding to the rotation of the patella in its frontal plane. In order to correct such malalignments, the tape is applied with an initial protective strip facing the anterior surface of the knee. This is followed by a non-elastic adhesive band applied onto the patella. The therapy consists of applying tension in the direction of the desired corrective measure. (Appendix.1)
Kinesio-taping (KT) was created in the 1970s by Kenzo Kase. The use of the tape incorporates adhesive elastic bands placed in such a position that the fibres affect the muscle activity, extensibility, proprioception, blood and lymphatic circulation. The force exerted depends on the desired effect and will provoke the stimulation of subcutaneous mechano-receptors as well as facial release and mobilisation. Enthusiasm for this technique is relatively recent. It comes from a limited number of studies which are incomparable due to variations in pathologies and techniques implemented. For treating PFPS, the tape is often applied to the quadriceps. (Appendix 2)
The physiotherapist has several techniques at their disposal. It is essential for them to adopt a scientific approach in making their decision, one based on efficacy and not on “trends” within the health industry. “Evidence Based Practice” (EBP) is different from empirically based practice which is based purely on the experiences or beliefs of a physiotherapist [7]. This review intends to methodologically and scientifically explore the literature in an attempt to justify the use of MT and/ or KT among people suffering from Patellofemoral Pain Syndrome. The following relevant reviews study the techniques separately and in direct contrast to each other [8]; [9]; [10]; [11]; [12]... Given that the two techniques have different modes of action, we have chosen to use pain as the comparative variable.
Method
Bibliography compilation strategy
Several studies evaluating the effects of each individual type of taping and some comparing the two types have been chosen from various electronic collections. Searches via MEDLINE, the Cochrane Library, CINAHL, Science Direct and SPORTDiscus have been incorporated searching with combinations of key words: “(knee* OR patell*) AND (tape OR taping OR kinesio* OR mcconnell) AND pain”. Searches using the PEDro database used the term “patell*” under the heading “taping”, “pain” and “knee”. The most recent search is dated the 24th of August 2015. To avoid publication bias, we contacted several experts by email to identify articles that have not been published or items of grey literature.
Article Selection Criteria
Only items in the form of controlled or crossover studies are eligible according to the selection protocol. Too little evidence was visible in clinical cases and other congress reports. We applied linguistic constraints and only used articles in French, English and Spanish. We achieved this initial stage of article selection by looking at the title and the summary. The second stage of selection involved a complete reading of the article.
We observed the following criteria:
Population: All subjects are adults with a diagnosis of PFPS or anterior knee pain with no visible structural damage (gonarthrosis, tendinopathy, a history of sprain or dislocation damage, surgical history…). Studies evaluating purely healthy subjects were excluded;
Evaluation criteria: We measured pain in line with the first and second criteria. The evaluation method for the pain had to be based on quantitative measures, with the help of the Visual analogue scale (VAS) or the Numerical scale (NS) for example;
Interventions:
- In the studies evaluating KT, we retained applications of the tape to both the patella and the quadriceps. There is no consensus on the specific application technique for PFPS. We accepted variations in the application techniques to avoid overly restrictive inclusion criteria, as long as the application is to the quadriceps.
- In the studies evaluating the McConnell-style taping, we accepted the systematic correction via the use of medial glide without prior evaluation of the position of the patella. We often found this simplification in the literature but we will evaluate the information separately to the standard protocol in order to maintain methodological rigour.
Evaluating the quality of the articles/Article quality evaluation
In order to evaluate the methodological quality of articles with respect to the inclusion criteria, we used the PEDro scale. The scale consists of 11 criteria, which facilitate the judgement of the intrinsic validity of a study and highlight possible biases present within them. The criteria assessed primarily focuses on the items’ eligibility, their randomness, the similarity of groups at the start of the study, the “blind” nature of the subject, the therapists and examiners, the monitoring systems in place and even the comparisons made between groups. Each criterion is attributed a mark out of 10 (the first is not considered in the final calculation).The higher the score, the higher the quality of the study.
We chose to use the articles that presented a moderate (4 to 5) to strong (>5) score and not to use the articles that received a score lower than 4. The results from the articles have been extracted, interpreted and classified in the form of a table. In order to analyse them, we divided them into three groups. Those that use Kinesio-taping, those that use McConnell taping and those that use a modified version of the McConnel taping. (With a systematic correction of medial glide)
Results
Research Results
Details of the research results are available in Table I (Appendix 3). Research conducted in databases produced 1280 results (The presence of duplicate results between databases prevented total compatibility at this stage and is the reason for this elevated figure). It is interesting to note that we did not find any articles in French.
After the preliminary selection process based on title and abstract, we retained 87 articles and removed 40 duplicates. We integrated 4 supplementary articles as grey literature or following their inclusion in bibliographies of the preceding articles. A further reading of the 44 articles lead to the exclusion of 17 of them, which did not match the eligibility criteria:
• 2 did not match the linguistic criteria (articles in Chinese and Turkish)
• 3 were not available in their complete versions

Ultimately, we retained 27 articles for evaluation of internal validity.
Methodological Quality of the studies.
The 27 articles that we evaluated presented an average score of 4.4 with a standard deviation of 1.72 (Table II). 19 of them achieved a PEDro score of equal to or greater than 4 and have been integrated into the definitive list (Fig.3). It is important to note that not every study achieved the criteria of a “blind therapist”, because the application of the two types of tape require understanding of the administration techniques. Only two studies achieved the anonymity criteria [13], [14]. The international set of results showed a low number of the studies had a satisfactory quality of methodology. External validity, which the PEDro scale does not evaluate, suffers from there being too few sample studies. Eleven studies were made up of less than or equal to 20 patients. The largest sample, with 81 patients was in the Clark study. These samples are less representative in some of the studies. Four articles only involved female patients [6]; [16]; [17]; [4], One assessed a purely military group [18], and two studies required a minimum level of physical activity in selection of their patients [16]; [19].

An overview of the articles revealed that a large number of the studies evaluated McConnell taping as opposed to Kinesio-taping. This could be because the latter technique was developed more recently and that its applications are much wider than simply that of treating PFPS. Six studies in total studying KT have been analysed.
We separated the studies evaluating the use of McConnell taping for creating a medial glide correction and organised them in a different group. This is to allow us to draw conclusions regardless of whether or not preliminary evaluation of patella alignment was conducted. In addition, eight articles discuss taping for patella medialisation and seven articles use McConnell taping as originally intended by correcting the position of the patella.
Kinesio-Tape

Six studies including a total number of 172 participants evaluated the use of KT. We grouped the samples between 20 and 49 patients, with differences in the chronicity of PFPS. Three of the six studies present evidence of a significant pain improvement compared to one placebo group or one group without tape [20], [21] and [22].
Campolo found evidence of a reduction in pain when climbing stairs in comparison to the group without tape (p= 0.0339) [22], but not when compared to the group which used it for a correction of medial glide. With the group using the placebo tape, Freedman [21] found a significant improvement (p= 0.025) where Aytar [6] did not (p = 0.932). The improvement found by Freedman was not significant during the squat.
