Source text - English Pursuant to an agreement between the Bank as the Administrator of the Fund (the Fund) and the Employer as Recipient of the grant(s) being administered by the Bank, the Employer intends using part of the proceeds of the grant(s) for eligible payments under the Contract. Payment by the Bank will be made on behalf of the Employer and only at the request of the Employer and upon approval by the Bank in accordance with the terms and conditions of the Grant Agreement entered by and between the Bank and the Employer on conditions of the Project funding and will be subject in all respects to the terms and conditions of that agreement. The proceeds of the Bank’s grant(s) will not be used for payments to persons or entities or for any import of goods if such payment or import is prohibited by a decision of the United Nations Security Council taken under Chapter VII of the Charter of the United Nations. Except as the Bank may specifically otherwise agree, no other party other than the Employer shall derive any rights from the agreement or have any rights to the proceeds of the grant(s).
Translation - Slovak Podľa dohody medzi bankou ako správcom fondu (fond) a objednávateľom ako príjemcom grantu, ktorý spravuje banka, má objednávateľ v úmysle použiť časť prostriedkov z grantu na oprávnené platby podľa zmluvy. Platba bankou sa bude realizovať v mene objednávateľa a len na požiadanie objednávateľa a po schválení bankou v súlade so zmluvnými podmienkami Grantovej dohody, ktorú medzi sebou uzavreli banka a objednávateľ o podmienkach financovania projektu, a bude vo všetkých ohľadoch podliehať zmluvným podmienkam tejto dohody. Prostriedky z grantu od banky sa nepoužijú na platby fyzickým ani právnickým osobám ani na akýkoľvek dovoz vybavenia, ak je takáto platba alebo dovoz zakázaný na základe rozhodnutia Bezpečnostnej rady OSN podľa kapitoly VII Charty Spojených národov. Okrem prípadov, keď banka bude súhlasiť s iným postupom, žiadna iná strana okrem objednávateľa nesmie nadobudnúť žiadne práva z tejto dohody ani právo na prostriedky z grantu.

Source text - English Lockout activities
Lockout refers to the activities established for:
• control of hazardous energy (high temperature, high pressure...),
• complete electrical and I&C Lockout and disconnection of electrical, systems, I&C systems, diagnostic system, etc.,
• complete mechanical Lockout and discharge of process and operational media of technological systems, as described by the Occupational Safety and Health Acts (OSHA), regulations and European Directives and guidelines.
To achieve system lockout, all energy sources will be switched off or disconnected, and the accumulated energy will be released, retained or dissipated. Each energy source will be equipped with a lock or a lockout device to prevent accidental start-up. It also refers to application of warning tags to energy sources to ensure the equipment is not restarted until service or other specific activity has been completed. Tags may be used with locks to provide for enhanced safety.
In order to perform dismantling activities in the scope of the Project, it will be necessary to secure systems, power off cables, etc. Some of these tasks will be as follows, with the respective definition of responsibilities/performance of implementation:
• Identification of systems and/or components, which have to be kept in operation without any modifications by the Employer. The corresponding tagging will be performed by the Contractor.
• The Employer shall identify those systems/components, which shall remain in operation and those, which are required to be separated/rerouted/relocated to other technological system/building and for which the Contractor shall elaborate the documentation in accordance with Employer’s guidelines and instructions including the basic directive RS/PD/ZSM Management of operational and technical documentation.
• The Employer shall identify and the Contractor shall tag those systems/components, which shall not remain in operation, i.e. which are to be dismantled. Their identification (tagging) shall be performed by the Contractor.
• The Employer shall identify those systems/components, which have to be secured (put in a safe mode) previously to the dismantling of other systems/components; and tagging implementation by the Contractor.
• The Contractor shall elaborate, under control of the Employer, the required documentation to secure the systems/work orders.
• In case any system/component shall be separated/rerouted/relocated as part of the lockout activities, the Contractor shall elaborate the necessary Detailed Design . The Contractor shall provide support during the approval process of the Detailed Design by the supervision bodies, as necessary.
• The Contractor shall elaborate the Detailed Design (design for dismantling works) necessary for the systems/components, which will not be kept in operation. The Contractor shall provide support during the approval process of the Detailed Design by the supervision bodies, as necessary.
• Issuance of the documents for execution of works and tag-out activities by the Employer in cooperation with the Contractor (tasks, work orders, securing orders, orders for works on HV, VHV, EHV electric wirings, fire orders, orders for works under conditions of the increased radiation risk) in accordance with procedure of company JAVYS BZ/KB/SM-06 Organising of safe work.
Translation - Slovak Zaistenie (lock-out)
Zaistenie sa vzťahuje na činnosti zavedené na:
• riadenie nebezpečnej energie (vysoká teplota, vysoký tlak, atď.),
• kompletné elektrické zaistenie a zaistenie SKR a odpojenie elektrických systémov a systémov SKR, diagnostického systému atď.,
• kompletné mechanické zaistenie a vypustenie technologických a prevádzkových médií z technologických systémov, ako je popísané v zákonoch a prepisoch o bezpečnosti a ochrane zdravia pri práci, v nariadeniach a smerniciach Európskej únie.
Pri zaisťovaní systému sa vypnú alebo odpoja všetky zdroje energie a naakumulovaná energia sa uvoľní, zadrží alebo rozptýli. Každý zdroj energie bude vybavený zámkom alebo zaisťovacím zariadením, aby sa zabránilo náhodnému spusteniu. Týka sa to aj použitia výstražných štítkov na zdrojoch energie, aby sa zabezpečilo, že sa zariadenie nereštartuje, pokiaľ sa nedokončí servis alebo iná špecifická činnosť. Na dosiahnutie väčšej bezpečnosti sa môžu použiť štítky so zámkami.
Aby bolo možné vykonávať demontážne činnosti v rozsahu projektu, bude potrebné zabezpečiť systémy, odpojiť káble a pod. Niektoré z týchto úloh budú s príslušným definovaním zodpovedností/realizovania činností nasledovné:
• Určenie systémov a/alebo komponentov, ktoré treba ponechať v prevádzke bez akýchkoľvek zmien Objednávateľom. Príslušné štítkovanie vykoná Zhotoviteľ.
• Objednávateľ určí tie systémy/komponenty, ktoré treba ponechať v prevádzke a kde sa vyžaduje oddelenie/pretrasovanie/premiestnenie do iného technologického systému/objektu a pre ktoré musí Zhotoviteľ vypracovať dokumentáciu podľa smerníc a návodov Objednávateľa vrátane základnej smernice RS/PD/ZSM Riadenie prevádzkovej a technickej dokumentácie. Príslušné štítkovanie vykoná Zhotoviteľ.
• Objednávateľ určí tie systémy/komponenty, ktoré sa neponechajú v prevádzke, teda sa demontujú. Ich označenie (štítkovanie) urobí Zhotoviteľ.
• Objednávateľ určí tie systémy/komponenty, ktoré sa musia zabezpečiť (dať do bezpečného režimu) ešte pred demontážou iných systémov/komponentov a ich štítkovanie Zhotoviteľom.
• Zhotoviteľ vypracuje pod dohľadom Objednávateľa požadovanú dokumentáciu na zabezpečenie systémov/pracovných príkazov.
• Ak sa musí ktorýkoľvek systém/komponent oddeliť/pretrasovať/premiestniť ako súčasť zaisťovacích činností, Zhotoviteľ musí vypracovať potrebný realizačný projekt . Zhotoviteľ musí podľa potreby poskytnúť podporu počas procesu schvaľovania realizačného projektu dozornými orgánmi.
• Zhotoviteľ vypracuje realizačný projekt (projekt demontážnych prác) potrebného pre systémy/komponenty, ktoré nezostanú v prevádzke. Zhotoviteľ musí podľa potreby poskytnúť podporu počas procesu schvaľovania realizačného projektu dozornými orgánmi.
• Vystavenie dokumentov na výkon a zaistenie prác Objednávateľom v spolupráci so Zhotoviteľom (úlohy, pracovné príkazy, zaisťovacie príkazy, príkazy na práce na elektrických vedeniach VN, VVN a ZVN, požiarne príkazy, príkazy na práce v podmienkach zvýšeného radiačného rizika) podľa smernice JAVYS BZ/KB/SM-06 Organizácia bezpečnej práce
