O modelo de Donabedian pressupõe a existência de três fatores essenciais na avaliação da qualidade – estrutura, processo e resultados – e, eventualmente, uma relação causal entre eles. Como tal, interessa saber se os três fatores se correlacionam no mesmo sentido, em sentidos distintos ou se, por outro lado, não se correlacionam de todo. Para exemplificarmos esta questão, a perceção que os enfermeiros têm dos aspetos estruturais pode ser globalmente positiva, mas a mesma atitude favorável poderá não ser estendida aos fatores de processo e resultados. As estruturas podem ser ótimas mas os processos e os resultados podem deixar muito a desejar. Se, no entanto, a perceção positiva se estende aos três fatores, esses devem correlacionar-se positivamente.
Se cada um dos fatores indica a qualidade, da perspetiva de gestão, a avaliação positiva dos três fatores e a consequente correlação estatística positiva entre eles constitui um resultado positivo. É preciso esclarecer que o termo “modelo”, neste caso, constitui um construto simples. Normalmente, este termo refere-se, não apenas à composição dos fenómenos em estudo, mas também à forma como estes se interrelacionam. Os modelos mais sofisticados permitem a previsão do comportamento de um fenómeno baseado nas informações obtidas e assentam na forma precisa como as relações entre os dados são modeladas . Não é isso que o modelo Donabediano pretende.

A Donabedian model assumes the existence of three essential factors in assessing quality – structure, process and outcome – and possibly a causal relationship between them. Assuming they exist, it is important to know if the three factors are correlated in the same direction, in opposing directions, or if they do not correlate at all. To exemplify this, the perception that nurses have of structural aspects may be generally positive but the same favourable attitude may not stretch to processes and outcomes. The structures may be seen as fine but processes and outcomes may leave much to be desired. If, however, the positive perception extends to all three factors, then they should all be positively correlated.
If each factor indicates quality, from the perspective of management, the positive assessment of the three factors and the consequent positive statistical correlation between them is a positive result. We should clarify that the term “model” in this case, is a simple construct. Usually it refers not only to the composition of the phenomena under study, but also to how these are interrelated. More sophisticated models allow us to predict the behaviour of a phenomenon based on the information obtained and are based on the modelling of precise relationships between the data. This is not what a Donabedian model attempts to do.

I have been living in Portugal since 1975 and teaching at university level since 1980. I have recently retired from the teaching profession so I will be able to dedicate my time to translating, proofreading and editing. There is a lot of pressure on people working in higher education to get papers published. That is not so easy to do especially if you are not used to writing academic papers or if your command of English is not as good as you would like it to be. Over the years I have gained a lot of experience in a number of diverse subjects of an academic nature. I left school with GCEs in English, History and Geography and studied Sociology at polytechnic before the polytechnics became integrated into the university system. In Portugal, I studied Urban and Regional Planning for my master's degree while teaching English as a Foreign Language. I passed the Institute of Linguists (now Chartered Institute of Linguists) Final exam in Portuguese in 1978 and obtained a Diploma in the Teaching of English as a Foreign Language (Royal Society of Arts) in the same year.
Since 1990, I have lectured in Statistics, Quantitative and Qualitative Methods, Anthropology and Sociology. I have authored a number of academic publications.
