Source text - English Toxic megacolon. Toxic dilatation (megacolon) is defined as total or segmental non-obstructive dilatation of the colon >= 5.5 cm associated with systemic toxicity. Although its true incidence has not been reported, approximately 5 % of patients with acute, severe colitis admitted to hospital will have toxic dilatation. Risk factors include hypokalaemia, hypomagnesaemia, bowel preparation, and the use of anti-diarrhoeal therapy. Earlier diagnosis of severe colitis, more intensive medical management and earlier surgery have reduced the incidence and mortality of toxic megaolon complicating UC. In addition to iv hydrocortisone, empirical treatment with oral vancomycin should be considered until stool is confirmed negative for Clostridium difficile toxin. Nasogastric suction cannot be expected to decompress the colon and is unnecessary. The classic knee-elbow position may relieve dinstension, but is generally impracticable. An opinion from an experienced colorectal surgeon is required on the day of admission. It should be made clear to all that there is a limited window of opportunity for medical treatment to work and that if there is no improvement early colectomy will be necessary.
Translation - Slovak Toxický megakolón. Toxická dilatácia (megakolón) je definovaná ako celková alebo segmentálna neobštrukčná dilatácia hrubého čreva (minimálne 5,5 cm) spojená so systémovou toxicitou.56 Hoci jej skutočný výskyt sa neuvádzal, približne
5 % hospitalizovaných pacientov s akútnou ťažkou kolitídou bude mať toxickú dilatáciu.56 Medzi rizikové faktory patria hypokaliémia, hypomagnezémia, preparácia čreva a terapia proti hnačke.56 Skoršie diagnostikovanie ťažkej kolitídy, intenzívnejšia liečba a skorší chirurgický zákrok znížili výskyt a úmrtnosť na toxický megakolón komplikujúci ulceróznu kolitídu. Okrem intravenózne podávaného hydrokortizónu sa má zvážiť empirická liečba s perorálnym podávaním vancomycínu až kým sa nepotvrdí, že v stolici nie je prítomný toxín produkovaný baktériou Clostridium difficile. Nemožno očakávať, že nazogastrické odsávanie zníži tlak v čreve a nie je potrebné. Klasická poloha koleno-lakeť môže rozšírenie zmierniť,120, je však všeobecne neuskutočniteľná. V deň prijatia pacienta je potrebné, aby svoj názor povedal skúsený kolorektálny chirurg. Všetkým treba vysvetliť, že existuje obmedzená príležitosť na liečbu a ak sa stav pacienta nezlepší, bude potrebná včasná kolektómia.
