A chance meeting
A serpent sat in the branch of a tree. There, he saw a caterpillar that had just hatched from an egg. It was brand new to the world and it was very hungry. The snake called out to the caterpillar, saying, “Hello, cousin. I see you were just born. What a lot of problems you are facing! Are you feeling alright?”
The caterpillar looked at the snake and said, “I feel fine, friend. Better than fine. I’m a bit hungry, though, so if you don’t mind, I’ll snack on this leaf while we talk.”
“I don’t mind at all,” responded the snake. “In fact, I think it’s quite brave of you to eat, given what that will do to you.”
“I don’t understand,” said the caterpillar. “What will eating do to me?”
“Oh my! It’s awful,” replied the viper. “You’ll grow so fast that your skin won’t be able to keep up. It’ll split open and you’ll have to grow another. It looks very painful.”
The caterpillar was afraid. No creature on Earth wants to feel pain, and the serpent certainly sounded sure of himself. In between mouthfuls of food, he seriously thought about not eating.
“My, my, but you have a hearty appetite!” exclaimed the snake. “You’ve already eaten the leaf on which you were born. I’m worried that the tree you live in won’t be large enough to satisfy your great appetite.”
“I hate to worry you so,” replied the caterpillar, “but I must eat in order to become that which I’m destined to be.”
“Ahhh,” said the serpent. “You have a destiny. What, pray tell, is your destiny, worm?”
“I’m not an ordinary worm. I am to be a butterfly someday,” explained the caterpillar.
“Oh, you’re certainly not ordinary if you’re actually a butterfly. But what makes you so sure that you’re not just a worm? I tell you, cousin, you look like an ordinary worm to me.”
“I just know that I am what I say I am,” said the caterpillar with confidence.
“I don’t know for sure, but I’ve been around awhile. I think perhaps you’re confused. Butterflies look nothing like you. They aren’t worms. They have wings. You’re not a soaring thing. You’re a maggot,” the snake said.
The caterpillar was confused. After all, he had just been born and the snake seemed very sure of himself. Maybe the snake was right. Maybe he wasn’t who he thought he was.
“If I were you, I’d get down from this tree and explore the world a little. Maybe you are a butterfly, but right now, you look like a worm. I think you’d enjoy taking a look around,” said the serpent, perhaps helpfully.
“I’m still pretty small,” replied the caterpillar. “I think I’ll stay where I am until I get bigger.”
“Suit yourself,” replied the snake. “I’ll see you around, worm.”
Growth and molting
The little caterpillar ate all day and most of the night. He noticed that his skin was feeling very tight. “This is just like the snake predicted,” thought the worm. “What if he is right about everything?”
The next day, the snake returned to the tree.
“Your skin seems tight today. Does it hurt much?” he inquired.
“Yes, it does,” admitted the worm.
“Well, stop eating and climb down from your tree. The world will be much less painful for you if you take my advice,” said the snake.
“I think I’ll stay where I am for today,” said the caterpillar, though he was tempted to listen to the snake. Eating was getting tiresome and, yet, he felt compelled to continue.
“Suit yourself,” the snake replied yet again. “Perhaps by tomorrow you’ll have changed your mind.”
When the snake returned the following day, the caterpillar had molted.
“Are you feeling OK, worm? To tell you the truth, you look unwell. Your skin is shiny and your face is distorted,” said the serpent.
“I’m holding my breath,” squeaked the worm. “I must do that to make room for myself inside my own skin. I’m stretching it out so I can stay in it.”
“That sounds difficult. Have you considered not eating? I know I’ve mentioned that before, but perhaps now you will listen to me. Come down from your tree and see the world. I’ll help you. I’ve been here a very long time,” explained the serpent.
“No thanks,” replied the caterpillar. “Breaking through my skin has made me very tired. I must rest today because I don’t think I’m finished yet.”
“Indeed you aren’t, if you are who you say you are,” reasoned the snake. “But, if you’re not who you think you are, you’re wasting your life up in this branch.”
“I’ll let you know tomorrow,” replied the worm.
Wrapped up in his destiny
The following day, the snake came back to the tree again, but the worm was nowhere to be found. The snake called out, “Worm! Where have you gone? Did you decide to take my advice and climb down?”
“I’m here,” came a familiar voice.
“Where? I don’t see you,” the snake said.
“Here, just above where I was yesterday,” said the voice.
“I see you now. You’re making a shroud for yourself! Do you expect to die?” inquired the serpent.
The caterpillar was hanging upside down and had partially covered himself in silk.
“This is no shroud, friend, though I am uncertain exactly what it is,” the worm responded.
“Oh cousin, you must listen to me. You’re dying! The silk that binds you is your undoing. Detach yourself and come down from there. The world is yours, I swear to you, if you come with me.” At this point, the serpent was almost begging.
“You’re my only friend, snake, but I can’t do what you ask. It’s in my nature to follow the path that I’ve found myself on. Please forgive me for not taking your excellent offer, but I can’t stop what has started. It’s my destiny to fulfill,” said the caterpillar.
“You’re a fool! I’m trying to save you!” said the serpent adamantly.
Silk cocoon - We all have wings fiction
“Perhaps I have to die as a worm in order to live as a butterfly,” was the response. As the caterpillar said this, the last of the silk encapsulated him and he was gone.
Inside, he could feel himself dissolving. Maybe the snake had been right. It seemed that he’d built his own sarcophagus of silk! He was in pain. The very elements that made him were shifting, and it was brutal. He cried out to the One, but there was no answer. “Help me. I’m here,” he pleaded, but the pain continued. His organs, skin and eyes disappeared. He was blind and raw and afraid.
The caterpillar remained in his crucible for two days. On the third day, he saw light. He stretched and pushed until he found himself on the same branch on which he was born. The snake was waiting.
A beautiful butterfly
“Hello, butterfly,” said the snake.
The worm, now a butterfly, shook his wings and spread them out to dry. They were beautiful stained-glass masterpieces that caught the sun’s rays and suggested glory.
“I see you, my friend,” said the butterfly to the snake. “Thank you so much for all of your help.”
“You took none of my advice, so I think I wasn’t of any help at all,” replied the snake.
“Without the doubt that you gave me, I would never have really known who I am,” replied the beautiful, shining and delicate creature.
“You’re lucky in the world, butterfly. Some of us are born on our bellies and we die on our bellies. We eat the dust of life and are despised for our position. Yet, I’m the shadow that informs you of light. I’m the darkness without which there’s no dawn. I am what I am every bit as much as you and yet there are no wings awaiting me,” said the serpent.
“We’re all winged, serpent, just at different times. You’ve served your purpose admirably and you’re exactly as you say you are. I’m living proof that all butterflies were once worms, just as all saints were once sinners. We’re all winged, my friend. Yours are on the inside,” said the butterfly.
“God bless you,” said the snake.
“God bless you,” said the butterfly.

Um encontro casual
Uma serpente estava sentada no ramo de uma árvore. Ali, viu uma lagarta que tinha acabado de chocar de um ovo. Acabava de chegar ao mundo e estava esfomeada. A cobra chamou a lagarta, dizendo: "Olá, prima. Vejo que acabou de nascer. Que quantidade de problemas que você enfrenta! Sente-se bem?
A lagarta olhou para a cobra e disse: "Estou bem, amiga. Mais que bem. Mas tenho um pouco de fome, portanto se não se importa, vou mordiscando esta folha enquanto falamos.
"Não me importo nada", respondeu a cobra. "De facto, acho que é muito corajoso da sua parte comer, dado o que isso lhe vai fazer."
"Não compreendo", disse a lagarta. "O que é que comer me vai fazer?"
"Oh, meu Deus! É horrível", respondeu a víbora. "Vai crescer tão depressa que a sua pele não será capaz de acompanhar o crescimento. A pele vai estalar e vai ter que produzir outra. Parece ser muito doloroso.
A lagarta teve medo. Nenhuma criatura na Terra quer sentir dor, e a serpente certamente parecia segura de si própria. Entre cada dentada, pensou seriamente em não comer.
"Meu Deus, mas que apetite saudável", exclamou a cobra. "Já comeu a folha na qual nasceu. Preocupo-me que a árvore na qual vive não será suficientemente grande para satisfazer esse apetite."
"Detesto preocupá-la tanto", respondeu a lagarta, "mas tenho que comer para me tornar no que estou destinada a ser."
"Ah", disse a serpente. "Você tem um destino. E diga, qual é o seu destino, minhoca??”
"Não sou uma minhoca comum. Um dia serei borboleta", explicou a lagarta.
"Oh, você certamente não é comum se é realmente uma borboleta. Mas o que a faz ter tanta certeza de que não é apenas uma minhoca? Digo-lhe, prima, você parece-me uma minhoca comum.
"Só sei que sou o que digo que sou", disse a lagarta com confiança..
"Não tenho a certeza, mas já aqui estou há uns tempos. Acho que talvez esteja confusa. As borboletas não se parecem nada consigo. Não são minhocas. Têm asas. Você não voa. Você é uma larva", disse a cobra.
A lagarta estava confusa. Afinal, tinha acabado de nascer e a cobra parecia muito segura segura de si própria. Talvez a cobra tivesse razão. Talvez ela não fosse quem pensava que era.
"Se eu fosse a si, descia desta árvore e explorava um pouco o mundo. Talvez seja uma borboleta, mas por agora, parece uma minhoca. Acho que iria gostar de dar uma olhada ao seu redor", disse a serpente, talvez prestável.
"Ainda sou muito pequena", respondeu a lagarta. "Acho que vou ficar onde estou até ser maior."
"Como queira", respondeu a cobra. "Vemo-nos por aí, minhoca."
Crescimento e metamorfose
A lagartinha comeu durante todo o dia e durante a maior parte da noite. Notou que a sua pele se sentia muito apertada. "Isto é mesmo como a cobra previu", pensou a lagarta. "E se ela tiver razão em relação a tudo?"
No dia seguinte, a cobra voltou à árvore.
"A sua pele parece apertada hoje. Dói muito?", perguntou.
"Sim, dói", admitiu a lagarta.
"Então, pare de comer e desça da árvore. O mundo será muito menos doloroso para si se aceitar o meu conselho", disse a cobra.
"Acho que por hoje vou ficar onde estou", disse a lagarta, embora ela tivesse sido tentada a ouvir a cobra. Tornava-se cansativo comer e, ainda assim, a lagarta sentia-se obrigada a continuar.
"Como queira", respondeu outra vez a cobra. "Talvez amanhã tenha mudado de ideias."
Quando a cobra voltou no dia seguinte, a lagarta tinha trocado de pele.
"Sente-se bem, minhoca? Para ser sincera, está com mau aspecto. A sua pele está brilhante e a sua cara está distorcida", disse a serpente.
"Estou a suster a respiração", chiou a lagarta. "Tem que ser, para alargar a minha pele. Estou a esticá-la para que eu possa caber dentro dela.
"Isso parece difícil. Já pensou em não comer? Sei que já lhe tinha sugerido isso, mas talvez agora me dê ouvidos. Desça da sua árvore e veja o mundo. Eu ajudo-a. Já ando por aqui há muito tempo", explicou a serpente.
"Não obrigada", respondeu a lagarta. "Romper a minha pele deixou-me muito cansada. Hoje tenho que descansar, porque acho que ainda não acabei.
"Na verdade, não acabou, se você é quem diz que é", argumentou a cobra. "Mas, se você não é quem pensa, está a desperdiçar a sua vida neste ramo."
"Amanhã eu digo-lhe", respondeu a lagarta.
Embrulhada no seu destino
No dia seguinte, a cobra voltou novamente para a árvore, mas não encontrou a lagarta em lado nenhum. A cobra gritou: "Minhoca! Para onde foi? Decidiu seguir o meu conselho e descer?
"Estou aqui", disse uma voz familiar.
"Onde? Não a vejo", disse a cobra.
"Aqui, um pouco acima de onde eu estava ontem", disse a voz.
"Já a vejo. Está a criar uma mortalha para si mesma! Espera morrer?", perguntou a serpente.
A lagarta estava pendurada de cabeça para baixo e tinha-se coberto parcialmente de seda.
"Não é uma mortalha, amiga, embora eu não tenha a certeza exatamente o que é", respondeu a minhoca.
"Ó prima, você tem que me ouvir. Está a morrer! A seda que a prende é a sua desgraça. Solte-se e desça daí. O mundo é seu, prometo-lhe, se vier comigo. A serpente praticamente implorava a este ponto.
"Você é a minha única amiga, cobra, mas não posso fazer o que me pede. Faz parte da minha natureza seguir o caminho no qual me encontro. Por favor, perdoe-me por não aceitar a sua excelente oferta, mas não posso parar o que comecei. É o meu destino", disse a lagarta.
"É uma tola! Estou a tentar salvá-la!", disse a serpente fervorosamente.
"Talvez eu tenha que morrer como uma minhoca para viver como uma borboleta", foi a resposta. Assim que a lagarta disse isto, o resto da seda a envolveu e ela desapareceu.
Lá dentro, sentia-se dissolver. Talvez a cobra tivesse razão. Talvez tivesse construído o seu próprio sarcófago de seda! Tinha dores. Os elementos que a fizeram deslocavam-se, e era brutal. Ela suplicou ao Único, mas não houve resposta. "Ajude-me. Estou aqui", implorou, mas a dor continuou. Os seus órgãos, pele e olhos desapareceram. Ficou cega, ferida e com medo.
A lagarta permaneceu na sua provação durante dois dias. No terceiro dia, viu a luz. Esticou e empurrou até se encontrar no mesmo ramo em que nasceu. A cobra estava à espera.
Uma linda borboleta
"Olá, borboleta", disse a cobra.
A lagarta, agora uma borboleta, sacudiu as asas e abriu-as para as secar. Eram belas obras-primas de vitrais que refletiam os raios de sol e sugeriam glória.
"Estou a vê-la, minha amiga", disse a borboleta para a cobra. "Muito obrigado por toda a sua ajuda."
"Não aceitou nenhum dos meus conselhos, portanto acho que não ajudei nada", respondeu a cobra.
"Sem dúvida que sim, ou eu nunca saberia realmente quem sou", respondeu a bela, brilhante e delicada criatura.
"Você tem sorte no mundo, borboleta. Alguns de nós nascemos a rastejar e morremos a rastejar. Comemos a poeira da vida e somos desprezados pela nossa posição. No entanto, eu sou a sombra que a informa da luz. Sou a escuridão sem a qual não há amanhecer. Sou o que sou, tanto quanto você, mas não tenho asas à minha espera", disse a serpente.

That mortal man should feed upon the creature that feeds his lamp, and, like Stubb, eat him by his own light, as you may say; this seems so outlandish a thing that one must needs go a little into the history and philosophy of it.
It is upon record, that three centuries ago the tongue of the Right Whale was esteemed a great delicacy in France, and commanded large prices there. Also, that in Henry VIIIth’s time, a certain cook of the court obtained a handsome reward for inventing an admirable sauce to be eaten with barbecued porpoises, which, you remember, are a species of whale. Porpoises, indeed, are to this day considered fine eating. The meat is made into balls about the size of billiard balls, and being well seasoned and spiced might be taken for turtle-balls or veal balls. The old monks of Dunfermline were very fond of them. They had a great porpoise grant from the crown.
The fact is, that among his hunters at least, the whale would by all hands be considered a noble dish, were there not so much of him; but when you come to sit down before a meat-pie nearly one hundred feet long, it takes away your appetite. Only the most unprejudiced of men like Stubb, nowadays partake of cooked whales; but the Esquimaux are not so fastidious. We all know how they live upon whales, and have rare old vintages of prime old train oil. Zogranda, one of their most famous doctors, recommends strips of blubber for infants, as being exceedingly juicy and nourishing. And this reminds me that certain Englishmen, who long ago were accidentally left in Greenland by a whaling vessel—that these men actually lived for several months on the mouldy scraps of whales which had been left ashore after trying out the blubber. Among the Dutch whalemen these scraps are called “fritters”; which, indeed, they greatly resemble, being brown and crisp, and smelling something like old Amsterdam housewives’ dough-nuts or oly-cooks, when fresh. They have such an eatable look that the most self-denying stranger can hardly keep his hands off.

Translation - English Initially, what had saddened Affonso the most was the view from the terrace – which, surely, used to extend to the ocean. But the houses built all around in the last few years had covered the splendid horizon. Nowadays, separated by a stretch of road, only a narrow strip of water and hill could be seen between two five-storey buildings, the entirety of the landscape in front of Ramalhete. Nevertheless, Affonso ended up finding an intimate charm in it. It was like a marine canvas, framed by white stone slabs, suspended from the blue sky across the terrace, depicting, in its infinite varieties of light and color, the brief episodes of a peaceful river life: sometimes the sail from a Trafaria boat elegantly eloping a busy life; other times a galley wearing all its cloth sails, taking advantage of a leisurely breeze in a crimson afternoon; or a luxury liner’s melancholy settling down, locked up and ready for the waves, here in a moment and gone in the next, as if devoured by the unsettling sea; or the dark shadow of an English battleship in the golden dust of quiet naps, for days... O trapiche se destaca na brancura do areal, que conserva as marcas dos passos dos capitães da areia, que já se recolheram. Ao longe, a fraca luz da lanterna da Porta do Mar, botequim de marítimos, parece agonizar. Passa um vento frio que levanta a areia e torna difíceis os passos do negro João Grande, que se recolhe. Vai curvado pelo vento como a vela de um barco. É alto, o mais alto do bando, e o mais forte também, negro de carapinha baixa e músculos retesados, embora tenha apenas treze anos, dos quais quatro passados na mais absoluta liberdade, correndo as ruas da Bahia com os capitães da areia. Restantes aspetos sem outras alterações tendo apreciação o seio longitudinal superior.
Sinusite maxilar crónica direita com quisto de retenção submucoso.
Apreciação normal das amígdalas cerebelosas. Normal expressão da sela turca.
Não há lesões ocupando espaço nem coleções hemáticas intracranianas.
Absence of focal lesions in the brain parenchyma aside from a small pineal gland cyst which does not cause compression over the corpora quadrigemina nor the aqueduct of Sylvius.
Brain parenchyma shows no alterations. Diffusion study without alterations. MRV study reveals homolateral hypoplasia of the right lateral sinus, sigmoid sinus and jugular vein, particularly on its posterior segment which couldn't be observed, demonstrated Pacchionian granulation on the lateral sinus transition as well as an also narrowed sigmoid sinus, without evidence of venous thrombosis, but showing some segmental stenosis on the left lateral sinus and the filiform segment of the right lateral/sigmoid sinus.
Other aspects without alterations, particularly the superior sagittal sinus.
Chronic right maxillary sinusitis with mucous retention cyst.
Normal evaluation of the cerebellar tonsils. Sella turcica appears normal.
There are no space-occupying lesions nor intracranial blood pooling.

A ecoestrutura tiróideia é ligeira e difusamente heterogénea, a integrar analiticamente, destacando-se no lobo direito dois nódulos, um no terço inferior posterior, sólido, quase isoecogénico com icrocalcificação, medindo 10 x 7 mm e outro, em situação externa na vertente anterior,
predominantemente quístico não puro, septado, com 19 x 9 mm, propondo-se correlação com PAAF.
Coexistem outros nódulos hipoecogénicos/quistos dispersos por ambos os lobos com diâmetros máximos de 2mm.
Ausência de adenopatias peri tiroideias.

Thyroid echotexture is light and diffusely heterogeneous, in keeping with analysis, showing two nodules on the right lobe, one of them on the posterior inferior third, measuring 10 x 7 mm, solid and nearly isoechogenic with microcalcification. The second nodule on the anterior slope in external position is mainly cystic, non-pure and septated, measuring 19 x 9 mm, suggested correlation with fine needle biopsy.
Presence of other hypoechogenic nodules/cysts scattered between both lobes, with maximum diameters of 2mm.
