Die Implantation eines permanenten Herzschrittmachers sollte erst nach einer eingehender Diagnostik erfolgen. Der Eingriff ist keine Not-Operation und daher gut planbar. Nach einer entsprechenden Hautdesinfektion des Operationsgebietes und einer lokalen Anästhesie wird ein 5-6 cm langer Hautschnitt unterhalb des Schlüsselbeines links oder rechts (Sulcus deltoideo-pectoralis) durchgeführt. Danach kann mit der Präparation der Armvene (Vena cephalica) begonnen werden, was in ca. 90% der Fälle auch gelingt. Mittels eines nichtresorbierbaren Nahtmaterials wird eine Tabaksbeutelnaht an der Vene angelegt und diese danach eröffnet. Über diese Öffnung werden die Schrittmacherelektroden eingeführt und unter Röntgenkontrolle im rechten Vorhof und/oder in der rechten Herzkammer (in Abhängigkeit von der medizinischen Indikation) positioniert. Nach der Fixierung der Elektroden wird eine Tasche für den eigentlichen Schrittmacher präpariert. Diese kann sowohl unter der Haut (subkutan) als auch unter dem Brustmuskel (subpectoral) angelegt werden. In wenigen Fällen kann der Schrittmacher auch im Bauchraum (Abdomen) platziert werden.
Bei der Verwendung von myokardialen Elektroden werden diese direkt auf das Herz aufgenäht. Dazu ist eine kleine Eröffnung des Brustkorbes (Thorakotomie) links im 4. oder 5. Zwischenrippenraum notwendig. Die Operation kann nur unter Vollnarkose durchgeführt werden. Zur Zeit befinden sich diese
Schrittmacherelektroden noch in der Studie und werden auch in Zukunft nur bei Patienten mit schwerer Herzinsuffizienz angewendet.
Nach Beendigung der Operation wird die Haut durch eine Naht verschlossen und steril verbunden. In der Regel kann der Patient schon kurze Zeit später mobilisiert werden.
Translation - Romanian Tehnica implantării pacemaker-ului
Implantarea unui pacemaker permanent ar trebui să aibă loc numai după o diagnosticare detaliată. Intervenţia nu este o operaţie de urgenţă şi de aceea este uşor de planificat. După dezinfecţia cutanată corespunzătoare a regiunii operatorii şi anestezia locală, se practică o incizie cutanată de 5-6 cm lungime sub clavicula stângă sau dreaptă (sulcus deltoideo-pectoralis/şanţul deltoido-pectoral). Apoi se poate începe prepararea venei braţului (vena cephalica), ceea ce se şi reuşeşte în circa 90% din cazuri. Cu ajutorul unui material de sutură nerezorbabil se efectuează sutura “în pungă de tutun” la nivelul venei, care apoi se deschide. Prin acest orificiu se introduc electrozii pacemaker-ului, care se plasează sub control radiografic în atriul şi/sau ventriculul drept (în funcţie de indicaţia medicală). După fixarea electrozilor, se prepară un buzunar pentru pacemaker-ul propriu-zis. Acesta se poate plasa atât sub piele (subcutan) cât şi sub muşchiul pectoral (subpectoral). În puţine cazuri, pacemaker-ul se poate plasa şi în cavitatea abdominală (abdomen).
În cazul folosirii electrozilor miocardici, aceştia se vor sutura direct pe cord. Pentru aceasta este necesară o mică deschizătură a cutiei toracice (toracotomie) în stânga, în spaţiul intercostal IV sau V. Operaţia se poate efectua doar în narcoză. Deocamdată aceşti electrozi de pacemaker se află încă în faza de studiu şi vor fi aplicaţi şi în viitor numai la pacienţii cu insuficiență cardiacă gravă.
După terminarea operaţiei se suturează pielea şi se pansează steril. De regulă, pacientul poate fi mobilizat deja la scurt timp după aceea.

Source text - English HbA1c is formed by the non-enzymatic glycation of the N-terminus of the b-chain of hemoglobin A.
The level of HbA1c is proportional to the level of glucose in the blood and is widely accepted as an indicator of the mean daily blood glucose concentration over the preceding two months.
Recent studies have shown that the regular measurement of HbA1c leads to changes in diabetes treatment and improvement of metabolic control as indicated by a lowering of HbA1c values.
Principles of the Procedure
The concentration of A1c and the concentration of total hemoglobin are measured and the ratio is reported.
The automated method (A1c and tHb_2) uses 4 reagents: A1c Denaturant Reagent (A1c DENAT), Total Hemoglobin Reagent (tHb_2), A1c Agglutinator Reagent (A1c R1) and A1c Antibody Reagent (A1c R2).
In an automated pretreatment step, the whole blood sample is mixed with the A1c Denaturant Reagent.
The red blood cells are lysed and the hemoglobin chain is hydrolyzed by the protease present in the reagent.
For the measurement of total hemoglobin, the Total Hemoglobin Reagent (tHb_2) is used.
The method is based on the conversion of all hemoglobin derivatives into alkaline hematin in an alkaline solution of a nonionic detergent as described in the original procedure of Wolf, Lang, and .
A latex agglutination inhibition method is used for the measurement of specific A1c.
A second protease in the R1 reagent further hydrolyzes the HbA1c sample to a glycated pentapeptide, which competes with the agglutinator (synthetic polymer containing multiple copies of the immunoreactive portion of A1c) for the anti-HbA1c antibody, thereby reducing the rate of agglutination.
A concentration curve is obtained by monitoring the change in scattered light as a change of absorbance.
The actual change in absorbance is inversely proportional to the concentration of A1c in the sample.

Nivelul de HbA1c este proporţional cu nivelul glicemiei şi este larg acceptat ca un indicator al glicemiei medii zilnice din cursul ultimelor două luni dinaintea testării.
Studii recente au arătat că determinarea regulată a HbA1c duce la modificări ale tratamentului diabetului şi la îmbunătăţirea controlului metabolic, fapt indicat de scăderea valorilor HbA1c.
Principii de procedură
Se măsoară concentraţia de A1c şi concentraţia hemoglobinei totale şi se notează raportul dintre ele.

Pentru măsurarea hemoglobinei totale se foloseşte reactivul hemoglobinei totale (tHb_2).
Metoda se bazează pe transformarea tuturor derivaţilor de hemoglobină în hematină alcalină într-o soluţie alcalină de detergent anionic, aşa cum se descrie în procedura originală a lui Wolf, Lang şi .

Deşi cele mai cunoscute modificări ale rezonanţei sunt cele datorate VPI pe fondul despicăturilor palatine, în practica logopedică sunt adesea întâlnite o serie de alte situaţii în care VPI este însoţită de o simptomatologie diversă cu repercusiuni asupra calităţii şi inteligibilităţii pronunţiei şi a vorbirii. În cele ce urmează prezentăm un caz prin intermediul căruia stabilim relaţia dintre examinarea complexă şi programul de intervenţie recuperatorie.

Although the best-known changes in resonation are owed to VPI in conjunction with cleft palate, in speech therapy we often encounter many other situations in which VPI is accompanied by a diverse symptomatology with repercussions on the quality and intelligibility of pronunciation and speech. In the following we present a case by means of which we would like to establish the relationship between the complex examination and the rehabilitation intervention program.
