Source text - Serbian Код наше пацијенткиње први симптоми клостридијалне инфекције јавили су се након хоспитализације и примене комбиноване антибиотске терапије.
Истраживања потврђују да су комо- рбидитетна стања и дуготрајна примена антибиотика, посебно оних широког спектра дејства, најзначајнији фактори ризика за развој инфекције (6).
Пораст учесталости инфекције повезује се и с појавом вирулентих сојева (најчешће сој 027) (7, 8).
Међутим, у нашем случају није било могуће одредити сој (риботип) Clostridium difficile PCR методом из техничких разлога (ниједна референтнa установа у Србији није у могућности да детектује риботипове).
Дијагноза токсичног мегаколона постављена је на основу клиничке слике, радиографског и СТ налаза.
Када је у питању радиографски налаз, подаци из клиничких студија су контрадикторни, неки користе > 5 сm као граничну вредност (9), док друге студије наводе дистензију попречне осе дебелог црева преко 6 сm (10).
У нашем случају била су задовољена оба критеријума.
Осим повећања пречника колона, код пацијената са токсичним мегаколоном на СТ налазу се уочава и периколична масна инфилтрација, задебљање зида дебелог црева, одсуство или поремећај хаустралних набора, присуство слободне течности (асцита) (9, 10).
Translation - English The first symptoms of clostridial infection in our patient occurred after hospitalization and the use of combined antibiotic therapy.
Studies confirm that comorbidities and long-term use of antibiotics, especially those with a broad-spectrum, are the most significant risk factors for the development of infection (6).
The increase in the incidence of infection is also associated with the occurrence of virulent strains (usually strain 027) (7, 8).
However, in our case it was not possible to determine the strain (ribotype) Clostridium difficile using PCR method due to technical reasons (no reference institution in Serbia is able to detect ribotypes).
The diagnosis of toxic megacolon was based on the clinical picture, radiographic and CT findings.
When it comes to radiographic findings, data from clinical studies are contradictory, some use > 5 cm as a limit value (9), while other studies suggest a distension across the transverse axis of the colon of more than 6 cm (10).
In our case, both criteria were met.
In addition to the increase of diameter of the colon, the CT findings of patients with toxic megacolon also show pericolic fat infiltration, thickening of the colon wall, absence or disorder of haustral folds, presence of free fluid (ascites) (9, 10).
