Malandraki Ohio University, Ohio, USA AND Areti Okalidou University of Macedonia, Thessaloniki, Greece Abstract The subject of this study is a 10-year-old non-verbal Greek boy, C.Z., who has been diagnosed with both bilateral sensorineural profound hearing loss and autism. The Picture Exchange Communication System (PECS), with some modifications and extensions was applied to C.Z. for a 4-month intensive intervention period. His original communication and behavioral status as well as the PECS application process are being presented along with the communicative, language and psychosocial outcomes following the entire intervention program. In addition, follow-up data is presented six months post. INTRODUCTION Little research has focused on the coexistence of hearing loss and autism in children (Jure, Rapin & Tuchman, 1991, Gillberg & Steffenburg, 1993). Epidemiological studies reveal that this co-morbidity is not as infrequent as it is probably thought. As Konstantareas and Homatidis (1987) report, auditory peculiarities and abnormalities in autistic children have often been noted, yet no systematic and reliable data have been collected on the frequency of hearing loss or peripheral ear problems in the autistic population. Generally, it is reported that hearing problems are much more frequent in people with neurological and developmental disorders, (van Schrojenstein Lantman-de Valk, 1997). In particular, Jure, Rapin and Tuchman (1991) found that 61 out of 1150 hearing impaired children covered the criteria for the diagnosis of autism, a percentage equal to 5,3% of the hearing-impaired population. In the most recent study of Rosenhall et al (1999) in Sweden, 199 autistic children were examined audiologically. The results indicated that 7,9% of these children were found to exhibit mild to moderate hearing loss and 3,5% had severe or profound hearing losses. Based on the above studies, it can be concluded that the incidence of hearing loss in the autistic children is higher than the one that can be found in normal populations. The same conclusion stands when incidence is examined from opposite end, that is the occurrence of autism in hearing-impaired children. Despite the increased co-morbidity of autism and hearing loss, there has been a scarcity of studies using complete audiometrical battery tests, i.e. combined measures of pure tone audiometry and tympanometry, to assess the hearing status of autistic children (Smith, Miller, Stewart, Walter & Mc Connell, 1988). This limitation is warranted because hearing assessment is much more difficult in the combined cases of autism and hearing impairment, as it is for all multiply disabled children (Jure, Rapin & Tuchman, 1991). On one hand, no in-depth hearing examinations are carried out on individuals that present severe autistic behaviors (Hayes & Gordon, 1977), and on the other hand, in many combined cases, the diagnosis of hearing loss comes much earlier than the diagnosis of autism (Jure, Rapin & Tuchman, 1991). Nevertheless, it is obvious that when both conditions occur, the communicative and language problems are even more severe as compared to any of the two disorders in isolation (Jure, Rapin & Tuchman, 1991; Rosenhall et al, 1999). Although for each of the two impairments, autism and hearing loss, many specialized intervention methods are being widely used, others with more and others with less success, no research elements are currently known concerning therapy for the combined cases of autism and hearing impairment. As Jure, Rapin and Tuchman (1991) state, there are no school programs specifically designed for autistic hearing-impaired children. In their study, they suggest that the ideal solution for these children would be their placement in a special class of a school for the hard-of-hearing with a behavioral intervention program combined with sign language training. Hence, a promising avenue on the end of training communication in the case of autistic-deaf children may be combining a non-verbal, alternative communication approach with the principles of behavioral analysis. Alternative Communication Systems have been commonly used to promote the communication skills of non-verbal autistic children. The American Speech-Language-Hearing Association defines Alternative Augmentative Communication as an area of clinical practice that attempts to compensate (temporarily or permanently) for the disability of people with severe expressive communication disorders, (1989). However, the communication challenge for both verbal and non-verbal behavioral programs (Carr, Binkoff, Kologinsky & Eddy, 1978; Charlop, Shreibman& Thibodeau, 1985; Lovaas, 1977) still is to assist autistic children in developing functional communication and spontaneous language (oral or sign) use. Most importantly, the acquisition of functional communication is considered a primary prognostic factor for long-term communication outcomes (Bondy & Frost, 1994). A method that targets functional communication early on via non-verbal means is the Picture Exchange Communication System (PECS) that was firstly developed in the Delaware Autistic Program by Lori Frost and Andrew Bondy, (Frost & Bondy, 1994). This system is the PECS is characterized by “the combination of principles from applied behavior analysis and guidelines from the area of alternative and augmentative communication” and it constitutes “a rapidly acquired, self-initiating communication system”, (Bondy & Frost, 1998). At first, PECS was designed for young children with autism, but afterwards it was also applied with success to people with other developmental disorders across the age range (Marriner, 1993, Liddle, 2001). The basic idea of PECS is to provide to individuals who have limited communication abilities a system by which they will interact with others in order to make their needs and desires known (Frost and Bondy, 1994). PECS users learn to approach a communicative partner and exchange with him/her the picture of a desired item with the item itself. By doing this, they learn to initiate spontaneous communication within a social context and to participate in the cause-effect act of communication (Frost and Bondy, 1994), an act which is fundamental to the communication process altogether. Although there are limited reports on the application of PECS in the literature, its few documented implementations in individuals with autistic spectrum disorders and other developmental disabilities, result in very encouraging communicative and social outcomes (Bondy & Peterson, 1990; Bondy & Frost, 1994; 2001; Charlop-Christy, Carpenter, Le, LeBlanc & Kellet, 2002; Finnegan, Mclaughlin, Ryan, Wachowiak, Swanson, & Bondy, 1987; Liddle, 2001; Simon, Whitehair & Toll, 1996). Bondy and Frost (1994) indicated that 85 autistic children acquired functional communication via PECS. Bondy and Peterson (1990) reported that 59% of autistic children who used PECS for more than one year managed to acquire oral language as their sole means of communication. Finnegan et al (1987) indicated that the application of PECS resulted in decreases of problem behaviors and increases in appropriate social behavior. Furthermore, Charlop et al. (2002) provided empirical evidence for the positive outcomes obtained by PECS in the acquisition of speech and communicative-social behaviors and, also, in the reduction of problem behavior. The clinical case reported in the present study concerns a multi-handicapped child diagnosed with both autism and profound hearing loss. Since the child has never been aided and was non-verbal, PECS was chosen as the alternative communication system appropriate for this child’s intervention program on communication training. The present study constitutes the first documented trial of the application of PECS in the combined case of autism and profound hearing loss and also the only known documented incidence of the PECS application in Greece. It occurred within the framework of the child’s biannual communication intervention program at school. The overall goal was to establish functional communication for this child who could not communicate his needs and feelings by any verbal or non-verbal means. The study’s goal was to assess the efficacy of PECS as a communication method in the case of a school-age, autistic-deaf child. METHOD CASE HISTORY The subject was a ten-year-old Greek boy, C.Z., who had been diagnosed with both congenital profound hearing loss and autism. An IQ assessment was not administered due to the lack of nonverbal standardized tests in this area for the Greek population. The study was approved by the Research Committee of the Technological and Educational Institute of Patras and by the National Foundation of Deaf and Hard of Hearing Individuals of Southwestern Greece and consent was given by C.Z.’s parents. A complete battery of audiological assessments, i.e. pure-tone audiometry, tympanometry and auditory brainstem responses (ABRs), was performed at the Department of Audiology and Neurootology of the Greek Public Children Hospital “Agia Sophia”, Athens, Greece, and established the diagnosis of profound bilateral sensorineural hearing loss. The diagnosis of autism was made by the psychologist -- can they make diagnosis of autism??-- of the National Foundation of Deaf and Hard of Hearing Individuals of Southwestern Greece. The family history of C.Z. revealed that his parents are both deaf and communicate with each other through the Greek Sign Language but his nineteen-year-old sister is hearing and normally functioning. ASSESSMENT Behavioral observations In order to obtain informal, direct information on the child’s behavior and psychosocial condition, a one-week observation was agreed upon. The observation lasted 7 days (almost 6 hours per day) and occurred in the place where C.Z. spent most of his time during the day, that is at the boarding house. The two trainers of the program performed the behavioral observation independently or in unison?]. The following behaviors were noted: • Short duration of eye contact, less than 5 seconds. • Stereotypical movements, especially with hands, such as clapping. • A stereotypical walk from the depth of the main corridor to the kitchen, three times and over per half hour. • Severe obsession to sit on a particular chair, located in an isolated part of the children’s playroom, on which he didn’t let anyone else sit. • Daily refusal in entering and playing in the children’s playroom. Communication Assessment A) Communication skills In order to gauge C.Z.’ s ability of expressing his needs and feelings, a questionnaire was developed and was distributed to C.Z.’ s teacher and caregivers at the boarding house. It is listed in Appendix 1. A response analysis of the questionnaire revealed that C.Z. communicated rarely and only when he wanted to ask for something (usually something edible). Specifically, C.Z. indicated his needs a) by going towards the thing he wants – (more often) b) by autistic leading, that is.. (what do you mean?) c) by a sign which means “come” d) by screaming or moving his head (more rare) Overall, C.Z. had great difficulty communicating his needs to his teacher and caregivers efficiently by either verbal or nonverbal means. The only reported communicative act was the hand sign which meant “come” and was, however, rarely used. By this sign he “invited” someone to do something for him or to give him something, without indicating anything else and without making eye contact. He did not use Greek Sign Language, despite a) his exposure to this language since his birth, as his parents communicate through Greek Sign Language, and b) his enrollment in a school and boarding house for the Hard-of-Hearing and Deaf Children for the last three years. B) Language Comprehension An informal evaluation of his comprehension abilities was made by the first trainer. The second trainer served as an additional observer for the purpose of validating C.Z.’ s responses. C.Z. did not generally follow one-step commands that were given in any communication mode, that is verbally, in writing or in sign language. By exception, he followed two commands, i.e. “sit down” and “stand up” which were given by one trainer via a hand gesture. Furthermore, he did not respond to his name either auditorily or in written language, or visuo-auditorily nor by fingerspelling. However, C.Z. exhibited comprehension of some written words as indicated below. (was there a second observer actually collecting data? Do you have inter-observer agreement data? Describe the second observer’s duties in greater detail ) C) Written language skills As reported by his teachers, C.Z. had already developed some writing skills at school, thus writing was further exploited. Examination of his notebooks and an informal discussion with his teacher and caregivers revealed that C.Z. had a relatively sizable receptive vocabulary; as he was able to recognize (are we sure he possessed receptive vocabulary of 150 words?? Then why do we list only about 40? How do we know they were 150? Did you test it/ How/ Describe procedure) and copy almost 150 written words. Informal examination of C.Z.’ s expressive vocabulary was made via an elicitation procedure which was familiar to C.Z., as it was being used by his tutor in class tasks. The first trainer and C.Z.’ s tutor took turns in modeling the desired responses, i.e. writing and fingerspelling, upon presentation of colorful cards. Subsequently, they prompted C.Z. to respond accordingly upon subsequent presentations of stimuli cards. C.Z. had no problem following this task as this was a familiar class procedure for him. Results indicated that C.Z. a) could write and fingerspell the following parts of the body: eye, hair, mouth, nose, ears, leg, hands, neck (elicited), b) could write and fingerspell 10 basic colors: red, black, blue, white, green, purple, brown, light blue, yellow and orange (elicited), c) could write and fingerspell the following animals: cat, horse, goat, chicken, bird, and dog (elicited), and d) he could write and fingerspell the following fruits: orange, apple, lemon, strawberry, banana and pear (elicited). Reportedly, however, he had only once exhibited spontaneous writing: at 8 years old he wrote the correct date of that particular day on the black board of his classroom. Despite that, he had never used writing as a communication mean, i.e. to request objects or to express feelings. In sum, C.Z.’ s written language skills mainly involved copying and understanding written words and these skills were later on utilized by the researchers in his intervention plan. Cognitive skills assessment Matching and Sequencing Skills The matching tasks (matching same objects, same pictures, objects with pictures, pictures with objects, colors with pictures) were all completed successfully by elicitation. In sequencing tasks (numbers and sequence cards) he had difficulty following the relevant instructions and he did not complete them successfully Assessment Impressions & Recommendations for Intervention Overall C.Z.’s autistic features did not allow him to communicate functionally with other people, deaf or hearing, even though he had been exposed to sign language both in school and at home since birth. Thus, the goal of the intervention was to establish a functional communication system which would enable him to express his needs and feelings. For that purpose, PECS was chosen and a few adaptations and changes were made by the researchers in order to adopt PECS procedures for the case of an autistic child with superimposed profound hearing loss. SETTINGS, TRAINERS AND MATERIALS Settings The settings in which the different phases of PECS were applied included almost all of the common places of the boarding house, a speech therapy room and his classroom at school. The therapy room initially used for the implementation of the first phases of PECS was a 10ft x 16ft room with one table and three chairs. Two of the chairs were placed next to each other-for the second trainer and the child-and the third chair was facing them from the opposite side of the table-for the main trainer. In subsequent phases of the program, sessions were being held in a variety of places which represent the actual living environments of the child, namely, the common room, the playroom, the dining room, the bedroom, the study room of the boarding house as well as a few other bigger classrooms of the school. The bigger classrooms enabled the increase in distance between the child, the trainers and PECS materials, which was a program requirement in advanced phases of PECS. Trainers The main trainer was a senior undergraduate student of the Dept. of Speech and Language Therapy of T.E.I., Patras, Greece (also an author of the present study), and the second trainer was a kindergarten teacher. Six additional trainers participated in the advanced phases of the Program, i.e. five C.Z.’s caregivers at the boarding house and his teacher. Materials Before the beginning of the intervention trainers devoted a few days in preparing and constructing the necessary material, i.e. the pictures and the cards that were constructed according to the requirements of PECS (Frost and Bondy, 1994). The initial type of the cards used was black and white pictures with thick black line drawing and all the pictures had the same size. An example of the pictures used is given in Fig.1. Figure1: Initial type of pictures All of these pictures were designed by hand by a semiprofessional designer and then scanned and processed by the researchers with the Software Program Graphic Converter. From the second phase and on, a three-ring binder was introduced as the communication book where all pictures and later word-cards were attached with small case letters. Velcro was used to attach the pictures, the word-cards, a sentence strip and cards with the phrases “I want”, “This is” and “Hello” in Greek Language to the communication book. The picture cards were replaced by word-cards of the same size in the first phase and in subsequent phases these word-cards were furthermore reduced approximately in half the size. During the 4th phase many half-size cards were added to C.Z.’s communication book and were organized in six categories. Table 1 presents the items that were included in each category during this phase. Useful Items Glass, plate, spoon, fork, straw. *Since it had been found during the evaluation process that C.Z. recognized the colors, trainers included them in his book. STUDY DESIGN Data pertaining to the child’s communication profile before and after the intervention program was recorded. In addition, gross observations were being noted regarding the behavioral and social patterns evidenced before and after the intervention. Following the establishment of baselines regarding the child’s communicative and psychosocial behaviors during the one-week observation and the three-day evaluation period, the training of PECS began. The data recording was made by two trainers in conjunction and at subsequent phases of PECS data recording was made by the first trainer and the one of C.Z.’ s caregivers who was on duty at the time. The main intervention program was highly intensive and lasted approximately four months (from 28/11/2000 to 19/3/2001). During the intensive treatment period 3 to 5 sessions of 30 to 45 minutes were conducted each week. Additionally, during the last one and a half month, the trainers were in C.Z.’s environment (boarding house and school) for eight to ten hours daily, so that C.Z. could acquire the communication system in his daily environment and activities. A follow-up, less intensive intervention took place by a single trainer for another four months. This trainer did not participate in the main program but received in-service training for two (?) weeks. (more?) INTERVENTION PROGRAM Reinforcements assessment Before the actual intervention started, an assessment of potential reinforcers was performed in order for the most desirable items to be chosen for the particular individual. Three sessions were devoted on this task of making successful choices of toys and edibles. The procedure strictly followed the principles and guidelines of PECS training Manual (Frost & Bondy, 1994) and constituted a multiple-without-replacement assessment (DeLeon, Iwata & Roscoe, 1997). For the category of toys, a total of eight items were chosen, in order of child’s preference: car, play-mobile, truck, play-do, colored pencils, magic markers, teddy bear and ball. The edibles though were difficult for him to choose. He couldn’t choose anything in two sessions, most likely because he had been chastised by caregivers in the past for grabbing food. In view of the apparent difficulty, C.Z.’ s caregivers were asked to provide their opinions regarding his favorite edibles. C.Z.’ s reportedly preferred items were used by the trainers as reinforcers, plus water, which was added because of its vital importance in life. A total of nine edibles have been chosen which were banana, cookie, chocolate, chips, apple, pear, coca-cola, orange juice and water. PECS Description The training followed the guidelines described in the treatment protocol of PECS Training Manual (Frost & Bondy, 1994). According to this Protocol, PECS consists of six main phases and one additional phase. The 1st phase consists of three stages and the goal of this phase is for the person to learn to pick a single picture (which lie in front of him) and give it to one trainer, who then gives the person the desired item which lies opposite to the picture (Frost & Bondy, 1994) [mention stages]. The 2nd phase also has three stages and introduces the idea of the communication board. Now the picture is attached to a communication board and the trainer, as well as the communication board, gradually move away so that the person can learn to go towards the communication board and subsequently towards the communicative partner no matter where they are. A variety of trainers are necessary to generalize to different communicative partners (Frost & Bondy, 1994) [mention the stages]. The 3rd phase involves an increase of the number of pictures and requires that the trainee discriminates among the several pictures and is able to use all of them appropriately during the communication exchange. It has three stages as well (Frost & Bondy, 1994). The 4th phase introduces sentence structure building with the “I want” card (why said picture?). The goal of this phase is for the child to be able to request many items using a three-word phrase. Now the exchange task includes the following steps: picking up the “I want” card, putting it on a sentence strip, picking up the card with the written word of the desired item, putting it on the sentence strip next to the “I want” card and finally removing the strip from the communication board and giving it to a communicative partner. Whenever the child gives the strip to the trainer, the trainer must turn it around and tell loudly what it says. Gradually the trainer can reduce the number of words he/she says, so that the child can say the rest of the words completing the sentence (Frost & Bondy, 1994b) [mention somewhere how did C.Z. do on that/ Did he complete the trainer’s sentence ever/ By which mode, sign or etc. Also manton in stages]. The 5th phase of PECS is different from the 4th only in that now the child has to answer the question “What do you want?” by forming phrases with the “I want” picture and the each time desired item card, (Frost & Bondy, 1994). [Stages?] The 6th phase goes beyond the requesting task and begins to teach responsive and spontaneous commenting. It has many stages in which questions such as “What is this?”, “What do you see?” etc separately and then in combination must be answered by the child. An extra card with the phrase “This is” is added under the “I want” card. At first the trainer asks “What is this?” while he/she holds an item and points to the “This is” card. In the next trials the pointing cue must fade. The reward for the successful trials must be something not related to the item. When this is completely acquired by the child the trainers must begin randomly asking “What do you want?” and “What is this?” for discrimination. The same process is followed for questions such as “What do you see?” etc. (Frost & Bondy, 1994). [Stages?] PECS also has an additional “phase” which includes teaching to respond to yes-no questions, more cognitive concepts etc, (Frost & Bondy, 1994). This phase was not targeted due to time limitations. PECS Modifications All the PECS phases were applied with some extensions and modifications concomitant to the child’s hearing loss and his writing facility. The alterations of PECS were the following: 1. A gradual replacement of pictures by written words, which took place during the 1st phase of PECS. At first, the pictures were presented with written words underneath them and then the pictures were completely replaced by the written words, (See indicatively Fig.2 and 3). The reason for the implementation of this extension early on was because during his evaluation C.Z. had shown increased facility with recognition of many written words. Throughout PECS training, all verbal praises, reassurances and prompts were given via nonverbal means, namely, by simultaneous usage of Greek Sign Language, intense facial expressions and intense clapping as well as praise touch (such as shoulder padding). 3. Also, at the beginning of each phase C.Z. was purposefully left free to do whatever he wanted. This allowed for the formation of new baselines prior to the onset of the training procedure at each phase. 4. During the 4th phase, the trainers’ response following C.Z.’ s return of the sentence strip to them involved more than verbalization of the sentence. They simultaneously expressed the sentence via Sign Language and with concurrent emphatic articulation while holding the strip very close to their mouth, for facilitating lip-reading. 5. In the 5th and 6th phases, written question forms replaced the verbal questions “What do you want?” and “What is this?”. These were also expressed in Sign Language and with emphatic articulation for the same reasons described above. ----stopped hre The following additional steps and innovations were also applied to facilitate program success: 1. The third stage of the 4th phase targets the spontaneous use of the communication book in everyday life and, thus, the acquisition of the whole PECS system. For this stage of the 4th phase and because of its importance and difficulty, the trainers decided to spend as many hours as possible in the boarding house –approximately 8 hours per day- for two main reasons; firstly in order to introduce the whole program in his everyday life and secondly in order to instruct his caregivers as to the exact use and functionality of the PECS program. 2. Due to time restrictions [explain in methodology!!! A limitation in methodology] and because it was considered that the 5th phase would assist in the acquisition of the third stage of the 4th phase as well, the trainers decided to begin the application of the 5th phase before the third stage of the 4th phase was acquired, that is before C.Z. learned to …what was the goal of third stage phase 4??. 3. The introduction of an additional phase, suggested by the researchers, called the “HELLO” phase, which constitutes an innovation, replaced the “additional phase” [what did they suggest in the original PECS?? mention] suggested by PECS’s creators. This extension was an further step in C.Z.’s socialization attempt. Although not initially planned due to time restrictions, the researchers decided to devote the last sessions in a new extension they made in the Program, the “Hello” phase. The concept of saying «hello» is very difficult for autistic children to acquire, (Hobson & Lee, 1998). As this concept constitutes a sign of understanding social community and its rules, it was thought of great importance to train C.Z. how to say hello through PECS’s methods. A new page was added in his communication book. This page included a card with the phrase “HELLO” in the upper left hand corner, small id photographs of people he comes across all the time, people’s names attached under the photo cards and a sentence strip. Each time someone entered the room he was in and said hello and waved at him, he should take the “HELLO” card, attach it to the sentence strip, then take the card with the appropriate name, place it on the sentence strip as well and finally take the strip, go towards the person who said hello to him, show it to this person and wave his hand back. Data Collection and Reliability To assure that the data collected were reliable, both trainers [in unison? Maybe skip completely here since you mention this below] completed the assessment forms of each session. The criterion for the trainers to move to the next PECS step was that C.Z. exhibits at least 80% success in performing each session’s trials independently (marked with ‘I’ in Table 4). These forms, an example of which is presented in Table 4, were partially adapted from PECS Training Manual, (Frost & Bondy, 1994). The trainers co-decided the results and it is rather remarkable that almost no doubting case was noticed. During the generalization of phases 4 and 5 the main trainer in cooperation with each time co-trainers (e.g. caregivers) completed the forms. What type of generalization probes were conducted) RESULTS PECS ACQUISITION AND COMMUNICATIVE RESULTS During the 4-month intensive intervention period, C.Z. managed to acquire PECS as a functional communication means [is this correct?] by which he could spontaneously fulfill his needs and desires. The communicative results are shown in short by two diagrams that represent all the phases along with the number of sessions that were required for each phase to be acquired by C.Z. These diagrams refer mainly to the rapid time of the system’s acquisition. The horizontal line represents the number of sessions or days (since only one session was given each day) and the vertical line represents the phase numbers. The bullets show the exact session-day in which each stage was acquired. For the 1st phase the diamond-bullets represent “toys” category, triangle-bullets represent “edibles” category and square-bullets represent the new gradual extension stages. In the 2nd phase the three Xs refer to the three other communicative partners who were used for the generalization of the particular phase. In phases 1, 2, 4 and 6 there is an additional 0th stage, which marks the baseline, where C.Z. was left instruction-free to do whatever he wanted in the beginning of its phase and he gave wrong responses. The phases 3 and 5 do not have a 0th stage, due to the fact that in these phases C.Z. responded correctly in the analogous situation without instruction; thus the baseline coincides with stage 1. Rename 0th stage as the null stage. Use another mark apart from the filled square for the null stage because you said that the filled square means acquired first stage. How about percent of success on various trials? Can we include this number or is it redundant since in essence you jot down only the 80%’s and over? As is shown in the first diagram (Fig. 5), the 1st phase was acquired in four sessions for the category of toys. The extension of the 1st phase (written cards instead of pictures) for the same category was acquired within two sessions. For the category of edibles the 1st phase and its extension were easily acquired in three sessions [in discussion: how fast is that compared to PECS perfomance of autistic children???]. Totally the 1st Phase with its extension for both categories lasted 9 sessions. The 2nd phase was a bit longer than the 1st one. In the same diagram it is obvious that eight sessions were necessary so that this phase could be acquired by C.Z. The generalization of communication partners lasted three sessions, one session for each one of three new communicative partners. This Phase was completed in 11 sessions. The 3rd phase was completed in three sessions. In particular the first stage was acquired in one session and the third stage in two. The second stage is the correspondence check stage that was applied during all these three sessions. The second diagram, (Fig. 6), shows the course of acquisition for the rest four phases. In particular the two first stages of phase 4 [describe the stages shortly ] were acquired in three sessions, but the third stage, which in essence constitutes the spontaneous use of the Program, was first acquired in twenty days. As already mentioned the spontaneous use of the communication book was a very difficult task that needed much time and that is why trainers began to spend as many hours as possible in the boarding house. The 5th phase with the question “What do you want?” was completed in one session, but its spontaneous use was first acquired in about fifteen days [is that the 3rd stage? Describe what he could and could not do in stages with stage description]. The beginning of the completed acquisition of phases 4 and 5 constitutes also the beginning of the acquisition of the PECS system as a means of communication. As it has already been mentioned, the application of phase 5 began before phase 4 was completely finished. As it is shown in the diagram, phase 5 was first acquired one day before phase 4 did. The additional phase [6th?] in which the “HELLO” task was applied was not fully acquired by C.Z. but within the last two sessions he could understand what to do, so as to say “hello” to someone, although he never acquired this phase spontaneously [ which stages did he reach? Which stages did he miss?]. [ I do not understand the statement compared to the graph.. No stage is mentioned but you still use filled square –try to plot things in a uniform way. Another instance is where you use 0th stage plus 1st stage whereas in the previous graph you only used stage 1st ]. Figure 6: Progress of acquisition of 4, 5, 6 and additional “HELLO” phases To conclude, in a very small period of time – 4 months of intensive intervention - C.Z. managed to acquire an absolutely functional communication system. Thus, while before the application of PECS he had a very limited and non-functional communication profile, after the entire intervention he had a much higher communication level, which was socially completely functional. The following Before-After Table (Table 6) presents these results all together. EMERGENCE OF SIGN LANGUAGE AND VERBALIZATIONS As it has already been stated, from the 4th phase and afterwards the trainers began to present all the cards to C.Z. while simultaneously making their sign in Greek Sign Language. Furthermore, during the whole intervention the trainers were using sign language along with other means. C.Z. is a child that was exposed to Greek Sign Language from his birth. His parents are both deaf and communicate with each other through signing. He has also been attending a boarding school for the deaf and he has been living in a boarding house with deaf children for three years. Despite all this exposure to sign language, he had never been able to use any signs. During the generalization of phases 4 and 5, C.Z. started to use some of the signs the trainers had shown him with the cards. In particular, the first signs he made were the signs for water, toilet, food and pencil, which he made during the time he matched the cards on the sentence strip and prepared to give it to one of the trainers [ at which phase and stage did this occur? Please Mention]. Another quite remarkable observation, which was systematically observed, was the onset of vocalizations. C.Z. vocalized during the generalization of phases 4 and 5, at exactly the moment he was choosing the cards to put them on the sentence strip. These vocalizations seemed like an effort that he made to read each word aloud. Until then no similar vocalizations had been observed, although his ability to recognize words existed for the last two years. This newly acquired behavior was concomitant to PECS usage. PSYCHOSOCIAL AND BEHAVIORAL RESULTS The acquisition of a communication system was found to have a positive impact to C.Z.’s psychosocial functioning. In the beginning of the intervention, C.Z. psychosocial condition was characterized by many autistic and antisocial behaviors. Following intervention and specifically since trainers began to apply the system in his everyday environment, a very big change in C.Z. was noticed. This change was gradual and became more impressive day by day. Table 7 shows exactly how each of his initial observed behaviors had changed following the PECS intervention. He developed a positive affect, having a smiling face and many times he hugged others suddenly. Continuous denial to enter and play in the children’s playroom. Since the moment PECS was applied in their playroom he entered to it daily, thus beginning to interact with other children in playing tasks. Another important outcome was the change of other children’s behavior towards C.Z. According to his caregivers, the other children always treated C.Z. very positively and even protectively, but after the application of the PECS, their behavior became even more positive. In particular, they began to play with him more often and to hug and kiss him rather often as well. Many times they tried to be his trainers, with great success, by taking the card with the written question “What do you want?” and asking him in the same way his trainers did. This whole change in other children’s behavior showed that they had started to appreciate C.Z. much more, because now they saw that he was able to communicate through a means (I think this is a wrong expression), which was really interesting to them. Maybe the most critical psychosocial impact of the whole intervention was the modification of C.Z.’s behavior towards his mother. Whenever his mother had come to take him home, he had never expressed any feeling such as great expectation for this event. Even when he faced her after not seeing her for a long period of time, he reacted almost indifferently. For the first time, a few weeks before the intervention was completed, he exhibited new behaviors towards his mother when she came to take him home. At first he was very anxious upon her anticipation, and then, when his mother came, he quickly grabbed his bag and ran towards her. He was smiling and, as his mother reported that it was the first time she had seen him so happy. In general, she assured the trainers that his behavior was improved at home as well. DURING AND AFTER 6-MONTHS (you do not have follow-up data. We refer to follow-up when intervention has stopped and we aim to evaluate whether the child continues to perform at intervention levels after the removal of intervention) For the next four months there was a follow-up intervention, in which one trainer continued to see C.Z. twice a week. The goals for this follow-up intervention included continuation of the additional “HELLO” phase and addition of more cards-concepts in his communication book. In this follow-up period C.Z. continued to have an excellent performance in using PECS spontaneously either as a request or as a comment tool. However, the «HELLO» phase was never acquired spontaneously by him, although it was being taught during all this period. For the last two months C.Z was not using his book at all, due to summer vacations. Six months after the intensive intervention period, a two-hour observation was arranged in the boarding house. The points that were recorded prove that the entire intervention was completely effective for this particular child. Specifically, the following behaviors were recorded by the trainers during the two-hour observation: C.Z. spontaneously requested 5 items and skills by using his communication book. He also spontaneously requested “toilet” and “water” through Greek Sign Language. He understood two orders that were given to him through the Greek Sign Language, (to wash his hands and to bring his bag). He responded correctly in alternating written questions “What do you want?” and “What is this?”. His caregivers commented that he used many more signs of the Greek Sign Language to communicate. [new signs or old ones taught by the PECS program??] He often smiled and was very affectionate with his trainers. He had even more reduced stereotypical movements with hands. [some quantification?] Most of the time he was on the couches with the other children and only once he went towards his “isolated chair” and he did not sit on it. DISCUSSION The primary goal of this study was to establish a functional communication system for a 10-year-old boy diagnosed with autism and profound hearing loss, who by this age could not communicate functionally. The communicative results described above show that this goal was completed with success. Within a period of 4 months of intensive intervention, C.Z. managed to acquire a functional communication tool, i.e. PECS communication book. Additionally, the whole intervention led to very positive changes in C.Z.’s psychosocial development. During the ending period of PECS’s C.Z. was much happier and started to become socialized. This very significant change underlines the importance of communication and its critical role in the development of socialization skills. C.Z. could now make his desires and needs known and that made him enthusiastic. Also, something remarkable as well, is the reduction of autistic behaviors that was noticed. After the intervention, C.Z. permanently reduced the frequency of stereotypical movements, started to play and interact with other children, and ceased having an obsession with a particular chair. [Erased stuff] PECS constitutes a program that targets communication, but also its methodical behavioral principles target autistic behaviors as well. The reduction of autistic behaviors in C.Z. shows the effectiveness of PECS in the rehabilitation of the autistic syndrome. The positive impact of PECS application on behavior management problems and autistic behaviors of individuals has been well documented by the creators of PECS, (Bondy & Frost, 1998), as well as by Finnegan et al (1987) and by the most autistic behavior Charlop-Christy et al (2002). [maybe here you should mention more on the above studies.. making a comparison with data or outcomes of your study.]. Pertaining to language skills, he started acquiring some Greek Sign Language concepts for the first time in his life, and six months later this ability was [erased] broadened. This acquisition can be paralleled with the acquisition of verbal language concepts by hearing autistic children after the application of PECS, (Bondy & Frost, 2001). This means that, like many of these children begin to speak and express themselves in their language by connecting the verbal word with the picture, C.Z. began to “speak” in his language, the Greek Sign Language, by connecting each sign with the written word. Thus, PECS in the case of C.Z. functioned as the means to acquire sign language. Moreover, the fact that he began to make some vocalizations that he had never made before, leads to the conclusion that the particular intervention Program, although it does not target verbal language, it encourages it and it is by no means an obstacle for its acquisition. The above result throws support to the acknowledged claim [findings can stay if you want] of PECS creators that this system can facilitate speech production even without directly targeting it, (Bondy & Frost, 1998, Bondy & Frost, 2001, Charlop-Christy et al, 2002). It is interesting to note that C.Z.’s communication development encompassed both language and speech areas in a rudimentary way, despite his nonverbal orientation. The additional “HELLO” phase is an innovative idea that needs further exploration. The researchers believe that it would be worth testing its acquisition in deaf, autistic or autistic-deaf individuals since it offers a further step to sociability. First, greetings are a very basic element of socialization, and second, their exact purpose makes their acquisition very difficult by autistic people. In particular, Hobson and Lee (1998) found that autistic children are much less likely to greet verbally or with gestures in comparison with children with mental retardation. Thus, this new suggested phase may lead them into understanding critical social rules. One limitation of this study includes the scoring of child’s attempts in each session, which was conducted by both trainers in coordination [maybe in unison?] and not separately. [erased]. However, C.Z’s new profile was commented highly by all the staff members of his boarding school who knew him for _ years and verified that this was dramatic change. Another limitation, due to the fact that the major focus of the present study was the change of the communication profile of C.Z., was that only descriptive data on his behavioral and social patterns were collected at two ends, the beginning and the end of the intervention. More quantifiable and continuous data within the intervention period might have given us more information on how and when his behavior began changing. The present study confirms the effectiveness of Picture Exchange Communication System, with a few adaptations in a combined case of autism and profound hearing loss. It also proves the impact of communication in everyone’s social life and confirms the findings of PECS’s creators that their system encourages the emergence of both speech and language production. Since this study reports only a single case, it is imperative that more research should be conducted in the future on the combined cases of autism and hearing impairment and mostly on their rehabilitation in order to validate the above findings. Επιπρόσθετα, παρουσιάζονται ακόλουθα δεδομένα έξι μηνών μετά. ΕΙΣΑΓΩΓΗ Λίγες έρευνες έχουν εστιάσει στην συνύπαρξη της βαρηκοΐας και του αυτισμού στα παιδιά. (Jure, Rapin & Tuchman, 1991, Gillberg & Steffenburg, 1993). Επιδημιολογικές μελέτες αποκαλύπτουν πως αυτή η συνυπάρχουσα νοσηρότητα δεν είναι τόσο σπάνια όσο πιθανώς νομίζεται. Όπως αναφέρουν οι Κωνστανταρέα και Χωματίδης (1989), οι ακουστικές ιδιαιτερότητες και ανωμαλίες έχουν συχνά καταγραφεί σε αυτιστικά παιδιά, όμως δεν έχουν συλλεχθεί συστηματικά και αξιόπιστα δεδομένα σχετικά με τη συχνότητα της βαρηκοΐας ή περιφερικών ακουστικών προβλημάτων στον αυτιστικό πληθυσμό. Γενικά, αναφέρεται ότι προβλήματα ακοής είναι πολύ πιο συχνά σε άτομα με νευρολογικές και αναπτυξιακές διαταραχές (von Schrojenstein Lautman – deValk, 1987). Ειδικότερα, οι Jure, Rapin και Tuchman (1991) βρήκαν ότι 61 από 1150 βαρήκοα παιδιά ανταποκρίνονταν στα κριτήρια για τη διάγνωση του αυτισμού, ποσοστό ίσο με το 5,3% του βαρήκοου πληθυσμού. Στην πιο πρόσφατη έρευνα των Rosenhall et al (1999) στη Σουηδία, 199 αυτιστικά παιδιά εξετάστηκαν ακουολογικά. Τα αποτελέσματα κατέδειξαν ότι το 7,9% αυτών των παιδιών βρέθηκαν να έχουν ελαφρά προς μέτρια βαρηκοΐα και το 35% είχαν σοβαρή ή βαριά βαρηκοΐα. Με βάση τις παραπάνω μελέτες, μπορεί να συμπεράνει κανείς ότι η συχνότητα εμφάνισης βαρηκοΐας στα αυτιστικά παιδιά είναι μεγαλύτερη απ’ αυτήν που βρίσκει κανείς σε φυσιολογικούς πληθυσμούς. Το ίδιο συμπέρασμα ισχύει όταν η συχνότητα εμφάνισης εξετάζεται από το αντίθετο άκρο, δηλαδή την εμφάνιση αυτισμού σε βαρήκοα παιδιά. Παρά την αυξημένη συνυπάρχουσα νοσηρότητα του αυτισμού και της βαρηκοΐας, υπήρξαν ελάχιστες μελέτες που χρησιμοποιούν πλήρη ακουομετρικά τεστ συστοιχίας, τουτέστιν συνδυασμένα μέτρα ακουομετρίας καθαρού τόνου και τυμπανομετρίας, για να αξιολογήσουν την κατάσταση ακοής των αυτιστικών παιδιών (Smith, Miller, Stewart, Walter & McConell, 1988). Αυτός ο περιορισμός είναι δικαιολογημένος γιατί η αξιολόγηση της ακοής είναι πολύ πιο δύσκολη στις συνδυαστικές περιπτώσεις αυτισμού και ακουστικής βλάβης, όπως ισχύει για όλα τα παιδιά με πολλαπλές αναπηρίες (Jure, Rapin & Tuchman, 1991). Από τη μια πλευρά δεν διεξάγονται ακουστικές εξετάσεις σε βάθος σε άτομα που παρουσιάζουν σοβαρές αυτιστικές συμπεριφορές (Hayes & Gordon, 1977) και από την άλλη σε πολλές συνδυαστικές περιπτώσεις η διάγνωση της βαρηκοΐας προηγείται κατά πολύ της διάγνωσης του αυτισμού (Jure, Rapin & Tuchman, 1991). Παρ’ όλα αυτά, είναι προφανές ότι όταν ισχύουν και οι δύο όροι, τότε τα επικοινωνιακά και γλωσσικά προβλήματα είναι ακόμη πιο βαριά σε σύγκριση με αυτά οποιασδήποτε από τις δύο διαταραχές μεμονωμένα (Jure, Rapin & Tuchman, 1991; Rosenhall et al, 1999). Παρότι και για τις δύο βλάβες, τον αυτισμό και την βαρηκοΐα, χρησιμοποιούνται ευρέως πολλές εξειδικευμένες μέθοδοι παρέμβασης, άλλες με περισσότερη και άλλες με λιγότερη επιτυχία, ερευνητικά δεδομένα δεν είναι γνωστά επί του παρόντος σχετικά με τη θεραπεία των συνδυαστικών περιπτώσεων αυτισμού και ακουστικής βλάβης. Όπως διαπιστώνουν οι Jure, Rapin και Tuchman (1999), δεν υπάρχουν σχολικά προγράμματα ειδικά σχεδιασμένα για αυτιστικά παιδιά με ακουστική βλάβη. Στη μελέτη τους, προτείνουν ότι η ιδανική λύση γι’ αυτά τα παιδιά θα ήταν η ένταξή τους σε μία ειδική τάξη σχολείου βαρήκοων με ένα συμπεριφορικό πρόγραμμα παρέμβασης συνδυασμένο με εκμάθηση νοηματικής γλώσσας. Ως εκ τούτου, μια υποσχόμενη λεωφόρος στην υπηρεσία της εκπαιδευτικής επικοινωνίας στην περίπτωση των αυτιστικών – κωφών παιδιών μπορεί να συνδυάζει μια μη λεκτική, εναλλακτική επικοινωνιακή προσέγγιση με τις αρχές της ανάλυσης συμπεριφοράς. Εναλλακτικά Επικοινωνιακά Συστήματα χρησιμοποιήθηκαν συχνά για να προάγουν τις επικοινωνιακές δεξιότητες των μη-λεκτικών αυτιστικών παιδιών. Η Αμερικανική Ένωση Λόγου – Γλώσσας – Ακοής (American Speech – Language – Hearing Association) ορίζει την Εναλλακτική Αυξητική Επικοινωνία (Alternative Augmentative Communication) ως μια περιοχή κλινικής πράξης που επιχειρεί να αντισταθμίσει (προσωρινά ή μόνιμα) την αναπηρία ατόμων με βαριές εκφραστικές επικοινωνιακές διαταραχές (1989). Ωστόσο, η επικοινωνιακή πρόκληση τόσο για λεκτικά όσο και για μη-λεκτικά προγράμματα συμπεριφοράς (Carr, Binkoff, Kologinsky & Eddy, 1978; Charlop, Shreibman & Thibodeau, 1985; Lovaas, 1977) εξακολουθεί να είναι να επικουρήσει τα αυτιστικά παιδιά στην ανάπτυξη λειτουργικής επικοινωνίας και αυθόρμητης γλωσσικής (προφορικής ή νοηματικής) χρήσης. Ύψιστης σημασίας, η απόκτηση της λειτουργικής επικοινωνίας θεωρείται ένας στοιχειώδης προγνωστικός παράγοντας για μακροχρόνια επικοινωνιακά αποτελέσματα (Bondy & Frost, 1994). Μια μέθοδος που στοχεύει στη λειτουργική επικοινωνία από νωρίς μέσω μη-λεκτικών μέσων είναι το Picture Exchange Communication Systems (PECS) (Επικοινωνιακό Σύστημα Ανταλλαγής Εικόνων) που αναπτύχθηκε αρχικά στο Πρόγραμμα Αυτισμού του Delaware από την Lori Frost και τον Andrew Bondy (Frost & Bondy, 1994). Το σύστημα PECS χαρακτηρίζεται από «το συνδυασμό αρχών από την εφαρμοσμένη ανάλυση συμπεριφοράς και προδιαγραφών από την περιοχή της εναλλακτικής και της αυξητικής επικοινωνίας» και συνιστά «ένα ταχύτατα αποκτώμενο αυτοδιδασκόμενο επικοινωνιακό σύστημα» (Bondy & Frost, 1999). Αρχικά, το PECS σχεδιάστηκε για μικρά παιδιά με αυτισμό, αλλά αργότερα εφαρμόστηκε, επίσης, με επιτυχία σε άτομα με άλλες αναπτυξιακές διαταραχές σε όλο το φάσμα των ηλικιών (Marriner, 1993, Liddle, 2001). Η βασική ιδέα του PECS είναι να παρέχει στα άτομα που έχουν περιορισμένες επικοινωνιακές ικανότητες ένα σύστημα με το οποίο θα αλληλεπιδρούν με τους άλλους έτσι ώστε να γνωστοποιούν τις ανάγκες και τις επιθυμίες τους (Frost & Bondy, 1994). Οι χρήστες του PECS μαθαίνουν να πλησιάζουν έναν επικοινωνιακό σύντροφο και να ανταλλάσσουν με αυτόν/αυτήν την εικόνα ενός επιθυμητού αντικειμένου με το ίδιο το αντικείμενο. Κάνοντας κάτι τέτοιο, μαθαίνουν να εκκινούν αυθόρμητη επικοινωνία μέσα σε ένα κοινωνικό πλαίσιο, και να συμμετέχουν στην επικοινωνιακή πράξη αιτίας – αποτελέσματος (Frost & Bondy, 1994) η οποία είναι θεμελιώδης στην επικοινωνιακή διαδικασία συνολικά. Παρότι υπάρχουν περιορισμένες αναφορές στην εφαρμογή του PECS στη βιβλιογραφία, οι λιγοστές τεκμηριωμένες εφαρμογές σε άτομα με αυτιστικές διαταραχές, καταλήγουν σε πολύ ενθαρρυντικά επικοινωνιακά και κοινωνικά αποτελέσματα (Bondy & Peterson, 1990; Bondy & Frost, 1994; 2001; Charlop-Christy, Carpenter, Le, LeBlanc & Kellet, 2002; Finnegan, Mclaughlin, Ryan, Wachowiak, Swanson & Bondy, 1987; Liddle, 2001; Simon, Whitehair & Toll, 1996). Οι Bondy και Frost (1994) έδειξαν ότι 85 αυτιστικά παιδιά κατέκτησαν τη λειτουργική επικοινωνία μέσω του PECS. Οι Bondy και Peterson (1990) ανέφεραν ότι το 59% των αυτιστικών παιδιών που χρησιμοποίησαν το PECS για περισσότερο από ένα χρόνο κατάφεραν να αποκτήσουν την προφορική γλώσσα ως το μοναδικό τρόπο επικοινωνίας. Οι Finnegan et al (1987) κατέδειξαν ότι η εφαρμογή του PECS είχε σαν αποτέλεσμα μειώσεις των προβληματικών συμπεριφορών και αυξήσεις στην αρμόζουσα κοινωνική συμπεριφορά. Επιπρόσθετα, οι Charlop et al (2002) παρείχαν εμπειρικά αποδεικτικά στοιχεία για τα θετικά αποτελέσματα που κατακτήθηκαν με το PECS στην απόκτηση του λόγου και των επικοινωνιακών – κοινωνικών συμπεριφορών και, επίσης, στη μείωση της προβληματικής συμπεριφοράς. Η κλινική περίπτωση που αναφέρεται στην παρούσα μελέτη αφορά ένα παιδί με πολλαπλές αναπηρίες που του διαγνώσθηκε αυτισμός και βαριά βαρηκοΐα. Καθώς το παιδί δεν έλαβε ποτέ καμία θεραπευτική υποστήριξη και ήταν μη λεκτικό το PECS επιλέχθηκε ως το εναλλακτικό επικοινωνιακό σύστημα που θα ήταν κατάλληλο ως πρόγραμμα παρέμβασης αυτού του παιδιού στην επικοινωνιακή εκπαίδευση. Η παρούσα μελέτη αποτελεί την πρώτη τεκμηριωμένη δοκιμή της εφαρμογής του PECS στην συνδυαστική περίπτωση αυτισμού και βαριάς βαρηκοΐας και επίσης τη μόνη γνωστή τεκμηριωμένη περίσταση εφαρμογής του PECS στην Ελλάδα. Έλαβε χώρα μέσα στο πλαίσιο του διετούς προγράμματος επικοινωνιακής παρέμβασης στο σχολείο. Ο συνολικός στόχος ήταν να εδραιώσουμε τη λειτουργική επικοινωνία σ’ αυτό το παιδί που δεν μπορούσε να εξωτερικεύσει τις ανάγκες του και τα συναισθήματά του με κανένα λεκτικό ή μη λεκτικό μέσο. Ο στόχος της μελέτης ήταν να αξιολογήσει την αποτελεσματικότητα του PECS ως μιας επικοινωνιακής μεθόδου στην περίπτωση ενός αυτιστικού – κωφού παιδιού σχολικής ηλικίας. ΜΕΘΟΔΟΣ ΙΣΤΟΡΙΚΟ ΠΕΡΙΠΤΩΣΗΣ Το υποκείμενο ήταν ένα δεκάχρονο αγόρι, ελληνικής καταγωγής, ο Χ.Ζ. που του διαγνώστηκε συγγενής σοβαρή βαρηκοΐα και αυτισμός. Δεν του έγινε τεστ ευφυίας IQ εξαιτίας της έλλειψης μη λεκτικών σταθμισμένων τεστ σε αυτήν την περιοχή για τον ελληνικό πληθυσμό. Η μελέτη εγκρίθηκε από την Επιτροπή Ερευνών του Τεχνολογικού Εκπαιδευτικού Ιδρύματος Πάτρας και από το Εθνικό Ίδρυμα Κωφών και Βαρήκοων Ατόμων Νοτιοδυτικής Ελλάδας και δόθηκε συγκατάθεση από τους γονείς του Χ.Ζ. Μια πλήρης συστοιχία ακουολογικών αξιολογήσεων, τουτέστιν ακουομετρία καθαρού τόνου, τυμπανομετρία και ακουστικές αποκρίσεις εγκεφαλικού στελέχους (ABRs), πραγματοποιήθηκε στο Τμήμα Ακουολογίας και Νευροωτολογίας του Ελληνικού Δημόσιου Παιδικού Νοσοκομείου «Αγία Σοφία» Αθήνα, Ελλάδα και εδραίωσε τη διάγνωση της βαριάς αμφίπλευρης αισθητηριονευρικής βαρηκοΐας. Η διάγνωση του αυτισμού έγινε από τη ψυχολόγο [μπορούν να κάνουν διάγνωση του αυτισμού;] του Εθνικού Ιδρύματος των Κωφών και των Βαρήκοων Ατόμων της Νοτιοδυτικής Ελλάδας. Το οικογενειακό ιστορικό του Χ.Ζ. αποκάλυψε ότι οι γονείς του είναι και οι δύο κωφοί και επικοινωνούν ο ένας με τον άλλον μέσω της Ελληνικής Νοηματικής Γλώσσας αλλά η δεκαεννιάχρονη αδερφή του ακούει και συμπεριφέρεται φυσιολογικά. ΑΞΙΟΛΟΓΗΣΗ Παρατηρήσεις συμπεριφοράς Για να μπορέσουμε να αποκτήσουμε ανεπίσημα, ευθεία πληροφόρηση αναφορικά με τη συμπεριφορά του παιδιού και την ψυχοκοινωνική του κατάσταση, συμφωνήθηκε παρατήρηση μιας εβδομάδας. Η παρατήρηση διήρκεσε 7 ημέρες (σχεδόν 6 ώρες την ημέρα) και έλαβε χώρα στο μέρος που Χ.Ζ. περνούσε τις περισσότερες ώρες του κατά τη διάρκεια της ημέρας, δηλαδή το οικοτροφείο. Οι δύο εκπαιδευτές του προγράμματος εκτέλεσαν την παρατήρηση συμπεριφοράς [ανεξάρτητα ή και οι δύο μαζί;]. Καταγράφηκαν οι παρακάτω συμπεριφορές: • Σύντομη διάρκεια οπτικής επαφής, λιγότερο από 5 δευτερόλεπτα. • Στερεότυπες κινήσεις, ιδιαίτερα με τα χέρια, όπως παλαμάκια. • Στερεότυπος περίπατος από το βάθος του κυρίως διαδρόμου προς την κουζίνα, τρεις φορές και παραπάνω για μισή ώρα. • Σοβαρή εμμονή να καθίσει σε μια συγκεκριμένη καρέκλα, τοποθετημένη σε ένα απομονωμένο μέρος του παιδικού δωματίου παιχνιδιών, στην οποία δεν άφηνε να καθίσει κανένας άλλος. • Καθημερινή άρνηση να μπει και να παίξει στο παιδικό δωμάτιο παιχνιδιών. Αξιολόγηση Επικοινωνίας Α) Επικοινωνιακές δεξιότητες Για να μετρήσουμε την ικανότητα του Χ.Ζ. να εκφράσει τις ανάγκες και τα συναισθήματά του, κατασκευάσαμε ένα ερωτηματολόγιο και το διανείμαμε στη δασκάλα του Χ.Ζ. και στις βρεφονηπιοκόμους στο οικοτροφείο. Περιλαμβάνεται στο Παράρτημα 1. Μια ανάλυση των αποκρίσεων του ερωτηματολογίου αποκάλυψε ότι ο Χ.Ζ. επικοινωνούσε σπάνια και μόνο όταν ήθελε να ζητήσει κάτι (συνήθως κάτι φαγώσιμο). Ειδικότερα ο Χ.Ζ. υποδείκνυε τις ανάγκες του: α) πηγαίνοντας κοντά στο πράγμα που θέλει (πιο συχνά) β) με αυτιστική πορεία/καθοδήγηση γ) με ένα νόημα που σημαίνει «έλα» δ) ουρλιάζοντας ή κουνώντας το κεφάλι του (πιο σπάνια) Γενικά, ο Χ.Ζ. είχε μεγάλη δυσκολία να εξωτερικεύσει τις ανάγκες του στη δασκάλα του και στις βρεφονηπιοκόμους επαρκώς είτε με λεκτικά είτε με μη λεκτικά μέσα. Η μοναδική καταγεγραμμένη επικοινωνιακή πράξη ήταν το νόημα με το χέρι που σήμαινε «έλα» και το οποίο από την άλλη μεριά, χρησιμοποιούνταν σπάνια. Με αυτό το νόημα «προσκαλούσε» κάποιον να κάνει κάτι γι’ αυτόν ή να του δώσει κάτι, χωρίς να υποδεικνύει κάτι άλλο ή χωρίς να έχει οπτική επαφή. Δεν χρησιμοποιούσε την Ελληνική Νοηματική Γλώσσα παρά: α) την έκθεσή του σε αυτή τη γλώσσα από την γέννησή του, καθώς οι γονείς του επικοινωνούσαν μέσω της Ελληνικής Νοηματικής Γλώσσας και β) την εγγραφή του σε σχολείο με οικοτροφείο για τα Βαρήκοα και Κωφά Παιδιά τα τελευταία τρία χρόνια. Β) Γλωσσική Κατανόηση Μια ανεπίσημη εκτίμηση των αντιληπτικών του ικανοτήτων έγινε από την πρώτη εκπαιδεύτρια. Η δεύτερη εκπαιδεύτρια χρησίμευσε ως ένας επιπρόσθετος παρατηρητής με σκοπό να επικυρώσει τις αποκρίσεις του Χ.Ζ. Ο Χ.Ζ. δεν υπάκουσε/εκτέλεσε γενικά εντολές ενός βήματος που του δόθηκαν με οποιοδήποτε επικοινωνιακό τρόπο, δηλαδή λεκτικά, γραπτά ή σε νοηματική γλώσσα. Κατ’ εξαίρεση, εκτέλεσε δύο εντολές, τουτέστιν «κάθησε κάτω» και «σήκω επάνω» που του δόθηκαν από έναν εκπαιδευτή με μια χειρονομία. Ακόμη, δεν αποκρίθηκε στο όνομά του είτε ακουστικά, είτε σε γραπτή γλώσσα, είτε οπτικοακουστικά ούτε με δακτυλοσυλλαβισμό κωφαλάλων. Ωστόσο, ο Χ.Ζ. παρουσίασε κατανόηση κάποιων γραπτών λέξεων όπως φαίνεται παρακάτω (υπήρχε δεύτερος παρατηρητής που να συνέλεγε δεδομένα; Έχετε συμφωνία δεδομένων διαπαρατηρητή. Περίγραψε τα καθήκοντα του δεύτερου παρατηρητή με μεγαλύτερη λεπτομέρεια). Γ) Δεξιότητες γραπτού λόγου Όπως ανέφεραν οι δάσκαλοί του, ο Χ.Ζ. είχε ήδη αναπτύξει κάποιες γραπτές δεξιότητες στο σχολείο, κι έτσι το γράψιμο αξιοποιήθηκε περαιτέρω.Μια εξέταση των τετραδίων του και μια ανεπίσημη συζήτηση με την δασκάλα του και τις νηπιαγωγούς αποκάλυψαν ότι ο Χ.Ζ. είχε ένα σχετικά ευμέγεθες λεξιλόγιο πρόσληψης καθώς μπορούσε να αναγνωρίσει [είμαστε σίγουροι ότι κατείχε λεξιλόγιο πρόσληψης 150 λέξεων; Τότε γιατί καταγράψαμε μόνο περίπου 40; Πως ξέρουμε ότι ήταν 150; Το εξετάσατε; /Πώς;/ Περιγράψτε τη διαδικασία] και να αντιγράψει σχεδόν 150 γραπτές λέξεις. Ανεπίσημη εξέταση του εκφραστικού λεξιλογίου του Χ.Ζ. έγινε μέσω μιας μαιευτικής διαδικασίας η οποία ήταν οικεία στον Χ.Ζ., καθώς χρησιμοποιούνταν από τον δάσκαλό του στα σχολικά καθήκοντα. Ο πρώτος εκπαιδευτής και ο δάσκαλος του Χ.Ζ. εναλλάσσονταν στη διάπλαση των επιθυμητών αποκρίσεων, τουτέστιν γραπτά και με δακτυλοσυλλαβισμό κωφαλάλων, κατόπιν παρουσίασης χρωματιστών καρτελών. Κατόπιν, υποκίνησαν τον Χ.Ζ. να απαντήσει ανάλογα σε ακόλουθες επιδείξεις καρτελών με ερεθίσματα. Ο Χ.Ζ. δεν είχε κανένα πρόβλημα να εκτελέσει αυτήν την εργασία καθώς αυτή ήταν μια γνωστή διαδικασία της τάξης γι’ αυτόν. Τα αποτελέσματα έδειξαν ότι ο Χ.Ζ. α) μπορούσε να γράψει και να δακτυλοορθογραφήσει τα παρακάτω μέρη του σώματος: μάτι, μαλλιά, στόμα, μύτη, αυτιά, πόδι, χέρια, λαιμός (του εκμαιεύτηκε), β) μπορούσε να γράψει και να δακτυλοορθογραφήσει 10 βασικά χρώματα: κόκκινο, μαύρο, μπλε, άσπρο, πράσινο, βυσσινί, καφές, γαλάζιο, κίτρινο και πορτοκαλί (του εκμαιεύτηκε), γ) μπορούσε να γράψει και να δακτυλοορθογραφήσει τα παρακάτω ζώα: γάτα, άλογο, κατσίκα, κοτόπουλο, πουλί και σκύλος (του εκμαιεύτηκε), και δ) μπορούσε να γράψει και να δακτυλοορθογραφήσει τα παρακάτω φρούτα: πορτοκάλι, μήλο, λεμόνι, φράουλα, μπανάνα και αχλάδι (του εκμαιεύτηκε). Ωστόσο, καταγραμμένα, μόνο μια φορά επέδειξε αυθόρμητο γράψιμο: όταν ήταν 8 χρονών έγραψε στον πίνακα της τάξης του τη σωστή ημερομηνία εκείνης της συγκεκριμένης ημέρας. Παρά ταύτα, δεν χρησιμοποίησε ποτέ το γράψιμο ως μέσο επικοινωνίας, δηλαδή για να ζητήσει αντικείμενα ή να εκφράσει συναισθήματα. Συνοψίζοντας, οι γραπτές γλωσσικές δεξιότητες του Χ.Ζ. κυρίως συμπεριλάμβαναν αντιγραφή και κατανόηση γραπτών λέξεων και αυτές τις δεξιότητες χρησιμοποίησαν αργότερα οι ερευνητές στο πρόγραμμα παρέμβασής του. Αξιολόγηση Γνωστικών Δεξιοτήτων Δεξιότητες Αντιστοίχησης και Ακολουθίας Οι εργασίες αντιστοίχησης (ταίριασε τα ίδια αντικείμενα, τις ίδιες εικόνες, αντικείμενα με εικόνες, εικόνες με αντικείμενα, χρώματα με εικόνες) συμπληρώθηκαν όλες επιτυχώς με εκμαίευση [περίγραψε τη διαδικασία εκμαίευσης πολύ σύντομα]. Στις εργασίες ακολουθίας (αριθμούς και καρτέλες διαδοχής) είχε δυσκολία να ακολουθήσει τις σχετικές οδηγίες και δεν τις ολοκλήρωσε επιτυχώς. Εντυπώσεις Αξιολόγησης και Υποδείξεις για παρέμβαση Συνολικά τα αυτιστικά γνωρίσματα του Χ.Ζ. δεν του επέτρεψαν να επικοινωνήσει λειτουργικά με άλλους ανθρώπους, κωφούς ή ακούοντες, παρότι είχε εκτεθεί στην νοηματική γλώσσα και στο σχολείο και στο σπίτι από την γέννησή του. Έτσι, ο στόχος της παρέμβασης ήταν να εδραιώσει ένα λειτουργικό επικοινωνιακό σύστημα που θα τον καθιστούσε ικανό να εκφράσει τις ανάγκες και τα αισθήματά του. Για αυτό το σκοπό, επιλέχθηκε το PECS και έγιναν κάποιες προσαρμογές και αλλαγές από τους ερευνητές έτσι ώστε να υιοθετηθούν οι διαδικασίες του PECS για την περίπτωση ενός αυτιστικού παιδιού με επιπρόσθετη βαρηκοΐα βαριάς μορφής. ΧΩΡΟΙ, ΕΚΠΑΙΔΕΥΤΕΣ ΚΑΙ ΥΛΙΚΑ Χώροι Οι χώροι στους οποίους εφαρμόστηκαν οι διάφορες φάσεις του PECS συμπεριλάμβαναν όλους τους κοινόχρηστους χώρους του οικοτροφείου, την αίθουσα λογοθεραπείας και την αίθουσά του στο σχολείο. Η αίθουσα θεραπείας που αρχικά χρησιμοποιήθηκε για την εφαρμογή των πρώτων φάσεων του PECS ήταν ένα δωμάτιο 3 × 4 μέτρα (10 × 16 πόδια) με ένα τραπέζι και τρεις καρέκλες. Δύο από τις καρέκλες τοποθετήθηκαν η μια δίπλα στην άλλη – για τον δεύτερο εκπαιδευτή και το παιδί – και η τρίτη καρέκλα τις αντίκριζε από την αντίθετη πλευρά του τραπεζιού – για τον κύριο εκπαιδευτή. Σε ακόλουθες φάσεις του προγράμματος, διεξάγονταν συνεδρίες σε μια ποικιλία τόπων που αντιπροσωπεύουν τα πραγματικά περιβάλλοντα διαβίωσης του παιδιού, δηλαδή την αίθουσα των μαθητών, την αίθουσα του παιχνιδιού, την τραπεζαρία, το υπνοδωμάτιο, το αναγνωστήριο του οικοτροφείου καθώς και μερικές άλλες μεγαλύτερες αίθουσες του σχολείου. Οι μεγαλύτερες αίθουσες καθιστούσαν ικανή την αύξηση της απόστασης ανάμεσα στο παιδί, τους εκπαιδευτές και το υλικό του PECS. Εκπαιδευτές Η κύρια εκπαιδεύτρια ήταν μία τελειόφοιτη προπτυχιακή φοιτήτρια του Τμήματος Θεραπείας Λόγου και Γλώσσας του ΑΤΕΙ Πάτρας, Ελλάδα (και συγγραφέας της παρούσας μελέτης) και ο δεύτερος εκπαιδευτής ήταν νηπιαγωγός. Έξι επιπλέον εκπαιδευτές συμμετείχαν στις προχωρημένες φάσεις του Προγράμματος, τουτέστιν πέντε βρεφονηπιοκόμοι του Χ.Ζ. στο οικοτροφείο και η δασκάλα του. Υλικά Πριν από την έναρξη της παρέμβασης οι εκπαιδευτές αφιέρωσαν μερικές μέρες για την προετοιμασία και κατασκευή του απαραίτητου υλικού, δηλαδή τις εικόνες και τις καρτέλες που κατασκευάστηκαν με βάση τις προδιαγραφές του PECS (Frost and Bondy, 1994). Η αρχική μορφή των καρτελών που χρησιμοποιήθηκαν ήταν ασπρόμαυρες εικόνες με σχεδίαση χοντρής μαύρης γραμμής και όλες οι εικόνες είχαν το ίδιο μέγεθος. Ένα παράδειγμα των εικόνων που χρησιμοποιήθηκαν δίνεται στο Σχήμα 1. Σχήμα 1: Αρχική μορφή εικόνων Όλες αυτές οι εικόνες σχεδιάστηκαν στο χέρι από έναν ημιεπαγγελματία σχεδιαστή και κατόπιν σαρώθηκαν και επεξεργάστηκαν από τους ερευνητές με το πρόγραμμα software Graphic Converter. Από τη δεύτερη φάση και έπειτα, εισήχθηκε ένα ντοσιέ με τρεις κρίκους ως το επικοινωνιακό βιβλίο στο οποίο επισυνάπτονταν όλες οι εικόνες και αργότερα οι καρτέλες λέξεων με μικρά γράμματα. Χρησιμοποιήθηκε βελούδο με κόπιτσες (velcro) για να προσκολληθούν οι εικόνες, οι κάρτες λέξεων, μια λουρίδα πρότασης και κάρτες με τις φράσεις «θέλω», «Αυτό είναι» και «Γεια» στην Ελληνική Γλώσσα στο βιβλίο επικοινωνίας. Οι καρτέλες εικόνων αντικαταστάθηκαν με καρτέλες λέξεων του ίδιου μεγέθους στην πρώτη φάση και σε ακόλουθες φάσεις οι καρτέλες λέξεων μειώθηκαν περαιτέρω περίπου στο μισό μέγεθος. Κατά τη διάρκεια της 4ης φάσης πολλές καρτέλες μισού μεγέθους προστέθηκαν στο βιβλίο επικοινωνίας του Χ.Ζ. και οργανώθηκαν σε έξι κατηγορίες. Ο Πίνακας 1 παρουσιάζει τα αντικείμενα που συμπεριλήφθηκαν στην κάθε κατηγορία κατά τη διάρκεια αυτής της φάσης. ΣΧΕΔΙΟ ΜΕΛΕΤΗΣ Καταγράφηκαν δεδομένα που χαρακτηρίζουν το επικοινωνιακό προφίλ του παιδιού πριν και μετά το πρόγραμμα παρέμβασης. Επιπλέον, σημειώνονταν χοντρικές παρατηρήσεις αναφορικά με τα συμπεριφορικά και κοινωνικά πρότυπα που ήταν προφανή πριν και μετά την παρέμβαση. Ακολουθώντας την καθιέρωση των βασικών κριτηρίων αναφορικά με τις επικοινωνιακές και ψυχοκοινωνικές συμπεριφορές του παιδιού κατά τη διάρκεια της παρατήρησης της μιας εβδομάδας και της τριήμερης περιόδου αξιολόγησης, ξεκίνησε η εκπαίδευση στο PECS. Η καταγραφή των δεδομένων έγινε από δύο εκπαιδευτές σε συζυγία και σε ακόλουθες φάσεις του PECS η καταγραφή των δεδομένων έγινε από έναν εκπαιδευτή και τον ένα βρεφονηπιοκόμο του Χ.Ζ.που ήταν σε υπηρεσία εκείνη την ώρα [περίγραψε περισσότερο τη διαδικασία βαθμολόγησης ανάμεσα στους δύο εκπαιδευτές και πως κατέγραψε το μέτρο/βασικό κριτήριο σε κάθε στάδιο/έγραψες γι’ αυτό κάπου ήδη, αλλά πρέπει να το γράψεις εδώ]. Το κύριο πρόγραμμα παρέμβασης ήταν ιδιαίτερα εντατικό και διήρκεσε περίπου τέσσερις μήνες (από 28/11/2000 ως 19/03/2201). Κατά τη διάρκεια της περιόδου εντατικής θεραπείας κάθε βδομάδα διεξαγόταν 3 με 5 συνεδρίες των 30 με 45 λεπτών. Επιπλέον, κατά τη διάρκεια του τελευταίου ενάμισι μήνα, οι εκπαιδευτές βρίσκονταν στο περιβάλλον του Χ.Ζ. (οικοτροφείο και σχολείο) για οκτώ με δέκα ώρες ημερησίως, έτσι ώστε ο Χ.Ζ. να μπορέσει να κατακτήσει το επικοινωνιακό σύστημα στο καθημερινό του περιβάλλον και τις δραστηριότητές του. Μια ακόλουθη, λιγότερο εντατική παρέμβαση έλαβε χώρα από ένα μεμονωμένο εκπαιδευτή για τέσσερις επιπλέον μήνες. Ο εκπαιδευτής δεν συμμετείχε στο κύριο πρόγραμμα αλλά έλαβε επαγγελματική εκπαίδευση δύο (;) εβδομάδων (περισσότερο;). ΠΡΟΓΡΑΜΜΑ ΠΑΡΕΜΒΑΣΗΣ Αξιολόγηση επιβραβεύσεων Προτού ξεκινήσει η πραγματική παρέμβαση πραγματοποιήθηκε μια αξιολόγηση των δυνητικών επιβραβευτών έτσι ώστε να επιλεγούν τα πλέον επιθυμητά αντικείμενα για το συγκεκριμένο άτομο. Τρεις συνεδρίες αφιερώθηκαν σ’ αυτήν την εργασία για να κάνουμε επιτυχείς επιλογές παιχνιδιών και φαγώσιμων. Η διαδικασία ακολούθησε αυστηρά τις αρχές και τις οδηγίες του Εγχειριδίου εκπαίδευσης στο PECS (Frost & Bondy, 1994) και συνιστούσε μια πολλαπλή αναντικατάστατη αξιολόγηση (Deleon, Iwata & Roscoe, 1997). Για την κατηγορία των παιχνιδιών, ένα σύνολο οκτώ αντικειμένων επιλέχθηκαν, κατά σειρά προτίμησης του παιδιού: αυτοκίνητο, πλέϊμομπίλ, φορτηγό, πλαστελίνη, χρωματιστά μολύβια, φωσφορίζοντες μαρκαδόροι, αρκουδάκι και μπάλα. Όμως τα φαγώσιμα του ήταν δύσκολο να τα επιλέξει. Δεν μπορούσε να διαλέξει τίποτε για δύο συνεδρίες, πιθανότερα γιατί τον είχαν τιμωρήσει οι βρεφονηπιοκόμοι στο παρελθόν γιατί άρπαζε φαγητό. Ενόψει της εμφανούς δυσκολίας, ζητήθηκε από τις βρεφονηπιοκόμους του Χ.Ζ. να παρέχουν τις γνώμες τους, σχετικά με τα αγαπημένα του τρόφιμα. Τα προτιμώμενα αντικείμενα που αναφέρθηκαν χρησιμοποιήθηκαν από τους εκπαιδευτές ως επιβραβευτές, συν το νερό, που προστέθηκε εξαιτίας της ζωτικής σημασίας του. Ένα σύνολο εννιά φαγώσιμων επιλέχθηκαν τα οποία ήταν μπανάνα, μπισκότο, σοκολάτα, πατατάκια, μήλο, αχλάδι, κόκα – κόλα, χυμός πορτοκάλι και νερό. Περιγραφή του PECS Η εκπαίδευση ακολούθησε τις κατευθυντήριες γραμμές που περιγράφονται στο πρωτόκολλο θεραπείας του Εγχειριδίου Εκπαίδευσης του PECS (Frost & Bondy, 1994). Σύμφωνα με αυτό το πρωτόκολλο, το PECS συνίσταται σε έξι κύριες φάσεις και μία επιπλέον φάση. Η 1η φάση αποτελείται από τρία στάδια και ο στόχος αυτής της φάσης είναι για το άτομο να μάθει να παίρνει μια εικόνα (που βρίσκεται μπροστά του) και να τη δίνει στον εκπαιδευτή, που κατόπιν δίνει στο άτομο το επιθυμητό αντικείμενο που βρίσκεται απέναντι από την εικόνα (Frost & Bondy, 1994) [ανέφερε τα στάδια]. Η 2η φάση έχει επίσης τρία στάδια και εισάγει την ιδέα του επικοινωνιακού πίνακα. Τώρα η εικόνα επικολλάται σε έναν επικοινωνιακό πίνακα και ο εκπαιδευτής, καθώς και ο επικοινωνιακός πίνακας, βαθμιαία απομακρύνονται έτσι ώστε το άτομο να μπορεί να μάθει να πηγαίνει προς τον επικοινωνιακό σύντροφο όπου κι αν βρίσκεται. Ένα σύνολο εκπαιδευτών είναι αναγκαίο για να μετατραπούν χάριν απλούστευσης σε διαφορετικούς επικοινωνιακούς συντρόφους (Frost & Bondy, 1994) [ανέφερε τα στάδια]. Η 3η φάση περιλαμβάνει μια αύξηση του αριθμού των εικόνων και απαιτεί ο εκπαιδευόμενος να διακρίνει ανάμεσα στις διάφορες εικόνες και να είναι ικανός να τις χρησιμοποιήσει όλες κατάλληλα κατά την επικοινωνιακή ανταλλαγή. Έχει, επίσης, τρία στάδια (Frost & Bondy, 1994). Η 4η φάση εισάγει δόμηση προτάσεων με την καρτέλα «Εγώ θέλω» (γιατί είπες εικόνα;). Ο στόχος αυτής της φάσης είναι το παιδί να είναι ικανό να ζητήσει πολλά αντικείμενα φτιάχνοντας μια φράση τριών λέξεων. Τώρα η εργασία ανταλλαγής περιλαμβάνει τα παρακάτω βήματα: να πάρει την καρτέλα «Εγώ θέλω», να τη βάλει σε μια λουρίδα πρότασης, να διαλέξει την καρτέλα με τη γραμμένη λέξη του επιθυμητού αντικειμένου, να τη βάλει πάνω στην λουρίδα πρότασης δίπλα στην καρτέλα «Εγώ θέλω» και τέλος να αφαιρέσει την λουρίδα από τον πίνακα επικοινωνίας και να τη δώσει σε έναν επικοινωνιακό σύντροφο. Όποτε το παιδί δίνει την ταινία στον εκπαιδευτή, ο εκπαιδευτής πρέπει να τη γυρίζει ανάποδα και να λέει φωναχτά τι λέει. Βαθμιαία ο εκπαιδευτής μπορεί να μειώσει τον αριθμό των λέξεων που αυτός/αυτή λέει, έτσι ώστε το παιδί να μπορεί να λέει τις υπόλοιπες λέξεις συμπληρώνοντας την πρόταση (Frost & Bondy, 1994b) [ανέφερε κάπου πως τα πήγε ο Χ.Ζ. σ’ αυτό/Συμπλήρωσε ποτέ την πρόταση του εκπαιδευτή/Με ποια μορφή, νόημα ή κ.λ.π. Επίσης, ανέφερε τα στάδια]. Η 5η φάση του PECS είναι διαφορετική από την 4η μόνο στο ότι το παιδί πρέπει να απαντήσει στην ερώτηση «Τι θέλεις;» σχηματίζοντας φράσεις με την καρτέλα «Εγώ θέλω» και το επιθυμητό αντικείμενο κάθε φοράς (Frost & Bondy, 1994) [Στάδια]. Η 6η φάση ξεπερνάει την απαιτούμενη εργασία και ξεκινάει να διδάσκει τον απαντητικό και αυθόρμητο σχολιασμό. Έχει πολλά στάδια στα οποία ερωτήσεις όπως «Τι είναι αυτό;», «Τι βλέπεις;» κλπ. πρέπει να απαντηθούν από το παιδί χωριστά και μετά σε συνδυασμό. Μια επιπλέον καρτέλα με τη φράση «Αυτό είναι» προστίθεται κάτω από την καρτέλα «Εγώ θέλω». Αρχικά ο εκπαιδευτής ρωτάει «Τι είναι αυτό;» ενώ αυτός/αυτή κρατάει ένα αντικείμενο και δείχνει προς την καρτέλα «Αυτό είναι». Στις επόμενες δοκιμές το επιδεικνυόμενο σύνθημα πρέπει να ξεθωριάσει. Η επιβράβευση για τις επιτυχημένες δοκιμές πρέπει να είναι κάτι που δεν σχετίζεται με το αντικείμενο. Όταν αυτό το κατακτήσει πλήρως το παιδί οι εκπαιδευτές θα πρέπει να αρχίσουν να ρωτάνε τυχαία «Τι θέλεις;» και «Τι είναι αυτό;» για να τα διακρίνει. Η ίδια διαδικασία ακολουθείται για ερωτήσεις όπως «Τι βλέπεις;» κλπ. (Frost & Bondy, 1994) [Στάδια]. Το PECS έχει μια επιπρόσθετη «φάση» που περιλαμβάνει τη διδασκαλία της απόκρισης σε ερωτήσεις ναι και όχι, πιο γνωστικές έννοιες κλπ. (Frost & Bondy, 1994). Δεν στοχεύσαμε σ’ αυτή τη φάση εξαιτίας χρονικών περιορισμών. Τροποποιήσεις του PECS Όλες οι φάσεις του PECS εφαρμόστηκαν με κάποιες επεκτάσεις και τροποποιήσεις επακόλουθες της βαρηκοΐας του παιδιού και της γραπτής του ικανότητας. Οι τροποποιήσεις του PECS ήταν οι ακόλουθες: 1. Μια σταδιακή αντικατάσταση των εικόνων με γραπτές λέξεις, που έλαβε χώρα κατά την 1η φάση του PECS. Αρχικά οι εικόνες παρουσιάζονταν με γραπτές λέξεις κάτω απ’ αυτές και κατόπιν οι εικόνες αντικαθίσταντο πλήρως από τις γραπτές λέξεις (Βλέπε ενδεικτικά Σχ. 2 και 3). Η αιτία για την εφαρμογή αυτής της επέκτασης στην αρχή ήταν γιατί κατά την αποτίμησή του, ο Χ.Ζ. είχε επιδείξει αυξημένη ευχέρεια στην αναγνώριση πολλών γραπτών λέξεων. Σε όλη τη διάρκεια της εκπαίδευσης στο PECS, όλοι οι λεκτικοί έπαινοι, οι επιβεβαιώσεις και οι παρακινήσεις δίνονταν με μη λεκτικά μέσα, με ταυτόχρονη χρήση της Ελληνικής Νοηματικής Γλώσσας, έντονες εκφράσεις του προσώπου κι έντονο χειροκρότημα καθώς και επαινετικό άγγιγμα (όπως φιλικό χτύπημα στον ώμο). 3. Επίσης, στην έναρξη κάθε φάσης ο Χ.Ζ. αφήνονταν σκόπιμα ελεύθερος να κάνει οτιδήποτε ήθελε. Αυτό επέτρεπε τη διαμόρφωση, το σχεδιασμό καινούργιων βασικών κριτηρίων πριν το ξεκίνημα της εκπαιδευτικής διαδικασίας σε κάθε φάση. 4. Κατά τη διάρκεια της 4ης φάσης, η απόκριση του εκπαιδευτή που έπονταν της επιστροφής από τον Χ.Ζ. της λουρίδας της πρότασης απαιτούσε κάτι περισσότερο από λεκτική έκφραση της πρότασης. Ταυτόχρονα εξέφραζαν την πρόταση μέσω της Νοηματικής Γλώσσας και με ταυτόχρονη εμφατική άρθρωση ενώ κρατούσαν την ταινία πολύ κοντά στο στόμα του, για να διευκολύνουν τη χειλεοανάγνωση. 5. Στις 5η και 6η φάσεις, γραπτές ερωτηματικές λέξεις αντικατέστησαν τις λεκτικές ερωτήσεις «Τι θέλεις;» και «Τι είναι αυτό;». Αυτές εκφράστηκαν επίσης στην Νοηματική Γλώσσα και με εμφατική άρθρωση για τους ίδιους λόγους που περιγράφηκαν πιο σπάνια. Τα παρακάτω επιπρόσθετα βήματα και νεωτερισμοί εφαρμόστηκαν επίσης για να διευκολύνουν την επιτυχία του προγράμματος: 1. Το τρίτο στάδιο της 4ης φάσης στοχεύει στην αυθόρμητη χρήση του επικοινωνιακού βιβλίου στην καθημερινή ζωή και έτσι στην εκμάθηση ολόκληρου του συστήματος PECS. Γι’ αυτό το στάδιο της 4ης φάσης και εξαιτίας της σπουδαιότητας και της δυσκολίας του, οι εκπαιδευτές αποφάσισαν να περνούν όσο περισσότερες ώρες ήταν δυνατό στο οικοτροφείο – σχεδόν 8 ώρες την ημέρα – για δύο κυρίως λόγους, πρώτον για να εισάγουν ολόκληρο το πρόγραμμα στην καθημερινή του ζωή και δεύτερον για να διδάξουν στις βρεφονηπιοκόμους την ακριβή χρήση και τη λειτουργικότητα του προγράμματος PECS. 2. Εξαιτίας των χρονικών περιορισμών [εξήγησέ το στη μεθοδολογία!!! ένας περιορισμός στη Μεθοδολογία] κι επειδή θεωρήθηκε ότι η 5η φάση θα βοηθούσε στην κατάκτηση του τρίτου σταδίου της 4ης φάσης, επίσης, οι εκπαιδευτές αποφάσισαν να ξεκινήσουν την εφαρμογή της 5ης φάσης πριν το τρίτο στάδιο της 4ης φάσης να κατακτηθεί, δηλαδή προτού ο Χ.Ζ. να μάθει ποιος ήταν ο στόχος του τρίτου σταδίου της φάσης 4; 3. Η εισαγωγή μιας επιπλέον φάσης, που προτάθηκε από τους ερευνητές, που ονομάστηκε η φάση «ΓΕΙΑ», που αποτελεί νεωτερισμό, αντικατέστησε την «επιπλέον φάση» [τι πρότειναν αυτοί στο αυθεντικό αρχικό PECS; ανέφερε] που πρότειναν οι δημιουργοί του PECS. Αυτή η επέκταση αποτέλεσε ένα περαιτέρω βήμα στην προσπάθεια κοινωνικοποίησης του Χ.Ζ. Παρότι δεν είχε αρχικά σχεδιασθεί εξαιτίας χρονικών περιορισμών, οι ερευνητές αποφάσισαν να αφιερώσουν τις τελευταίες συνεδρίες σε μία καινούργια επέκταση που έκαναν στο Πρόγραμμα, τη φάση «ΓΕΙΑ». Η έννοια του να πεις «Γεια» είναι πολύ δύσκολη να την μάθουν τα αυτιστικά παιδιά (Hobson & Lee, 1998). Καθώς αυτή η έννοια συνιστά ένα σημάδι κατανόησης της ανθρώπινης κοινότητας και των κανόνων της, θεωρήθηκε μέγιστης σημασίας να εκπαιδεύσουμε τον Χ.Ζ. πως να λέει γεια μέσω των μεθόδων του PECS. Μια καινούργια σελίδα προστέθηκε στο επικοινωνιακό του βιβλίο. Αυτή η εικόνα περιλάμβανε μια καρτέλα με τη φράση «ΓΕΙΑ» στην επάνω αριστερή γωνία, μικρές φωτογραφίες ταυτότητας ανθρώπων που συναντάει συνέχεια, τα ονόματα αυτών των ανθρώπων κολλημένα κάτω από τις φωτογραφίες καρτέλες και μια λουρίδα πρότασης. Κάθε φορά που κάποιος έμπαινε στην αίθουσα που αυτός βρίσκονταν και του έλεγε γεια και τον χαιρετούσε, αυτός θα έπρεπε να πάρει την καρτέλα «HELLO», να την κολλήσει στην ταινία πρότασης, μετά να πάρει την καρτέλα με το κατάλληλο όνομα, να το βάλει κι αυτό στην ταινία πρότασης και τελικά να πάρει την ταινία να πάει προς τον άνθρωπο που του είπε γεια, να του την δείξει και να τον αντιχαιρετήσει. Συλλογή Δεδομένων και Αξιοπιστία Για να διασφαλίσουν ότι τα συλλεγμένα δεδομένα ήταν αξιόπιστα, και οι δύο εκπαιδευτές [μαζί; Ίσως θα πρέπει να το παραλήψεις τελείως εδώ καθώς το αναφέρεις παρακάτω] συμπλήρωσαν τα έντυπα αξιολόγησης για κάθε συνεδρία. Το κριτήριο για τους εκπαιδευτές να προχωρήσουν στο επόμενο βήμα του PECS ήταν να επιδείξει ο Χ.Ζ. τουλάχιστον 80% επιτυχία στην εκτέλεση των δοκιμών κάθε συνεδρίας ανεξάρτητα (σημειώνεται με «Α» στον Πίνακα 4). Αυτά τα έντυπα, ένα παράδειγμα των οποίων παρουσιάζεται στον Πίνακα 4, προσαρμόστηκαν τμηματικά από το Εγχειρίδιο Εκπαίδευσης του PECS, (Frost & Bondy, 1994). Οι εκπαιδευτές συναποφάσισαν τα αποτελέσματα και είναι μάλλον αξιοσημείωτο ότι δεν παρατηρήθηκε σχεδόν καμία περίπτωση αμφισβήτησης. Κατά τη διάρκεια της γενίκευσης των φάσεων 4 και 5 ο κύριος εκπαιδευτής σε συνεργασία με τους κάθε φορά συνεκπαιδευτές (π.χ. βρεφονηπιοκόμους) συμπλήρωσε τα έντυπα. ΠΙΝΑΚΑΣ 4. [αν αυτό είναι κάτι που χρησιμοποιεί το PECS σε παρακαλώ βάλτο στο παράρτημα] Έντυπο αξιολόγησης για την πρώτη θεραπευτική συνεδρία για την 1η εικόνα, δηλαδή το αυτοκίνητο. ΑΠΟΤΕΛΕΣΜΑΤΑ Η ΕΚΜΑΘΗΣΗ ΤΟΥ PECS ΚΑΙ ΤΑ ΕΠΙΚΟΙΝΩΝΙΑΚΑ ΑΠΟΤΕΛΕΣΜΑΤΑ Κατά την 4μηνη εντατική περίοδο παρέμβασης, ο Χ.Ζ. κατάφερε να μάθει το PECS ως ένα μέσο λειτουργικής επικοινωνίας [αυτό είναι σωστό;] με το οποίο θα μπορούσε να καλύπτει αυθόρμητα τις ανάγκες και τις επιθυμίες του. Τα επικοινωνιακά αποτελέσματα δείχνονται εν συντομία με δύο διαγράμματα που αναπαριστούν όλες τις φάσεις μαζί με τον αριθμό συνεδριών που απαιτούνταν για κάθε φάση που έπρεπε να κατακτήσει ο Χ.Ζ. Αυτά τα διαγράμματα αναφέρονται κυρίως στην εκμάθηση του συστήματος στη συγκεκριμένη χρονική στιγμή. Η οριζόντια γραμμή αναπαριστά τον αριθμό των συνεδριών ή τις ημέρες (καθώς κάθε μέρα γινόταν μία συνεδρία) και η κάθε γραμμή αναπαριστά τον αριθμό των φάσεων. Οι κουκίδες δείχνουν την ακριβή συνεδρία – ημέρα στην οποία κάθε στάδιο κατακτήθηκε. Για την 1η φάση οι ρόμβοι παριστάνουν την κατηγορία «παιχνίδια», τα τρίγωνα παριστάνουν την κατηγορία «φαγώσιμα» και τα τετράγωνα παριστάνουν τα καινούργια στάδια βαθμιαίας επέκτασης. Στη 2η φάση τα τρία Χ αναφέρονται στους άλλους τρεις επικοινωνιακούς συντρόφους που χρησιμοποιήθηκαν για τη γενίκευση της συγκεκριμένης φάσης. Στις φάσεις 1, 2, 4 και 6 υπάρχει ένα επιπλέον Οο στάδιο, το οποίο σηματοδοτεί το όριο (βασικό κριτήριο), όπου ο Χ.Ζ. είχε αφεθεί χωρίς οδηγίες να κάνει ότι ήθελε στο ξεκίνημα της κάθε φάσης και έδωσε λανθασμένες απαντήσεις. Οι φάσεις 3 και 5 δεν έχουν Οο στάδιο, εξαιτίας του γεγονότος ότι σ’ αυτές τις φάσεις ο Χ.Ζ. απάντησε ορθά στην αντίστοιχη περίσταση χωρίς οδηγίες• έτσι το όριο συμπίπτει με το στάδιο 1. Αριθμός συνεδρίας Σχήμα 5:Πρόοδος της εκμάθησης των φάσεων 1, 2, 3 σχόλια πάνω στο γράφημα: γράψε παιχνίδια – ε για να υποδείξεις την εικόνα• έστρεψε την προέκταση της 1ης φάσης και εικόνα & φαγητό και γράψε παιχνίδια – ε λ (για εικόνα και λέξη) και παιχνίδια – λ (μόνο για λέξη). Μετονόμασε το Οο στάδιο σε στάδιο μηδέν γιατί είπες ότι το γεμισμένο τετράγωνο σημαίνει ότι το πρώτο στάδιο έχει κατακτηθεί. Τι θα έλεγες για ποσοστό επιτυχίας στις διάφορες δοκιμές; Μπορούμε να συμπεριλάβουμε αυτόν τον αριθμό ή είναι περιττός εφόσον κατ’ ουσία κατέγραψες πρόχειρα μόνο το 80% και πάνω; Καθώς φαίνεται στο πρώτο διάγραμμα (Σχ. 5), η 1η φάση κατακτήθηκε σε τέσσερις συνεδρίες για την κατηγορία των παιχνιδιών. Για την κατηγορία των φαγώσιμων η 1η φάση και η επέκτασή της κατακτήθηκαν εύκολα σε τρεις συνεδρίες [προς συζήτηση: πόσο γρήγορο είναι αυτό σε σύγκριση με την επίδοση του PECS για τα αυτιστικά στάδια;;;]. Συνολικά η 1η φάση με την επέκτασή της και για τις δύο κατηγορίες διήρκεσε 9 συνεδρίες. Η 2η φάση ήταν λίγο μεγαλύτερη χρονικά από την πρώτη. Bio

I was born in Thessaloniki, Greece and lived there for the biggest part of my life. My professional field is Translation & Interpreting, and Foreign Language Education.
I started learning English at the age of two by watching popular American TV series and soap operas and listening to English and American pop music. Later on my parents sent me to a private school where English was part of the curriculum since kindergarten. I did my first "interpreting" at the age of eight when I translated my parents to an American couple who were on holidays at the island of Thassos. Since then I attained all the series of certificates, I graduated the English department of Thessaloniki and I obtained the Diploma In Translation from the IoL and a Msc in Translatology.
At the age of twelve, first grade in Greek Junior High School, I started learning my second foreign language, which was French, in which I excelled and I obtained all the series of French certificates until I got a certificate in which I opted for Translation.
Immediately after that I realized my big passion for learning Italian, which I learned so fast, as if I spoke the language in a "previous lifetime". I have obtained all the series of certificates in no time at all and I am used by the Greek Courts as an official translator - interpreter.
I have always admired the spirit of discipline and order of the German people so that has been the reason why I always wanted to learn the German language. The fact that I saw a dream in German and I couldn't understand anything, motivated me to start learning German on the beautiful island of Crete.
My passion for learning foreign languages and communicating with people from other countries has never ceased and lead me to my fifth foreign language, Russian. An insight of Russian culture and literature is something that I always wanted to have.
Studying & teaching foreign languages and cultures, in combination with Translating - Interpreting in various scientific fields, has satisfied my natural curiosity for knowledge, research and traveling. Nowadays, apart from the usual load work, I am working on a PhD in Translation Of Linguistics and Telecommunication.
