En este sentido, habiendo sido claramente informado, mediante la firma del presente documento, manifiesto de forma libre, voluntaria y consecuente que opto por el aseguramiento privado con la indicada compañía aseguradora, en lugar de la Tarjeta Sanitaria Europea (TSE), o cualesquiera otros seguros públicos o privados. El coste de esta asistencia será asumido por mi seguro/mutua y en su defecto yo mismo en los términos establecidos por XXX.
Asimismo autorizo a XXX a acceder a la información necesaria para realizar los citados trámites, siempre de conformidad con la Ley Orgánica 15/1999 de 13 de Diciembre, de Protección de Datos de Carácter Personal y el real decreto 1720/2007 de 21 de Diciembre por el que se aprueba el reglamento de Medidas de Seguridad de Ficheros automatizados que contengan datos de carácter personal.

In diesem Sinne wurde ich dementsprechend informiert und erkläre freiwillig mit meiner Unterschrift, dass ich privat durch angegebene Krankenversicherung behandelt werden will, anstatt über die Europäische Krankenversicherungskarte bzw. andere Krankenversicherungen privater als auch gesetzlicher Natur. Die Kosten für die Behandlung werden von meiner Versicherung/ gesetzlichen Versicherung oder von mir innerhalb der Bestimmungen von Global Medical Care S.L. übernommen.
Desgleichen bevollmächtige ich GLOBAL MEDICAL CARE S.L. zum Zugang zu jener Information, die für angesprochene Bearbeitung notwendig ist, stets im Einverständnis mit dem Grundgesetz 15/1999 vom 13.
