English to Croatian: Abdominal Aortic Surgery

Haemodynamic investigations were conducted in 9 patients during aortic surgery for Leriche's syndrome. Most of these patients had ischaemic heart disease without cardiac failure. Fluid loading was carried out before and during aortic clamping. It was controlled according to the optimal pulmonary wedge pressure determined the day before surgery. Only six of the nine patients received nitroglycerin (NTG: 0.2 mg/kg/min) throughout the operation. Before clamping, the cardiac index improved in patients treated with NTG. During clamping, the patients not treated with NTG showed a drop in their cardiac index, an increase in peripheral resistance and in left ventricular work. After declamping, there were no haemodynamic differences between the two groups. Two patients not treated with NTG developed renal failure post-operatively; one patient developed cardiac failure requiring a dopamine infusion during surgery. In this series, the association of optimal volume loading with a per-operative infusion of 0.2 mg/kg/min of NTG gave good haemodynamic stability.

Hemodinamska ispitivanja provedena su na devet pacijenata tijekom operacije aorte zbog Lericheovog sindroma. Većina tih pacijenata imala je ishemijsku bolest bez zatajenja srca. Prije i tijekom stezanja aorte izvršena je nadoknada tekućine, kontrolirana prema optimalnom zaglavnom tlaku određenom dan prije zahvata. Samo je šest od devet pacijenata primilo nitroglicerin (NTG: 0,2 mg/kg/min) tijekom operacije. Prije stezanja, kod pacijenata koji su liječeni NTG-om, srčani se indeks poboljšao. Tijekom stezanja, pacijenti koji nisu liječeni NTG-om pokazali su pad srčanog indeksa, povećanje perifernog otpora i rada lijevog ventrikula. Nakon stezanja, među dvjema grupama nije bilo razlike u hemodinamici. Kod dvaju pacijenata koji nisu liječeni NTG-om došlo je do postoperativnog zatajenja bubrega; kod jednog je pacijenta došlo do zatajenja srca i potrebe za infuzijom dopamina tijekom zahvata. U ovoj seriji ispitivanja, povezanost optimalnog volumnog opterećenja s predoperativnom infuzijom NTG-a od 0,2 mg/kg/min pruža dobru hemodinamsku stabilnost.

Spektralna analiza površinskih valova (SASW) je relativno nova nedestruktivna seizmička metoda razvijena radi određivanja elastičnih modula raznih materijala u području vrlo malih deformacija, te promjene tih modula s dubinom.
Metoda se zasniva na disperzivnim karakteristikama Rayleighevih valova uvažavajući činjenicu da se površinski Rayleighevi valovi različitih valnih duljina, odnosno frekvencija šire do relativno različitih dubina. Tako se valovi niže frekvencije, pa prema tome veće valne duljine šire dublje u medij nego valovi visoke frekvencije, odnosno male valne duljine. U uslojenom mediju brzina širenja vala na
površini ovisi o frekvenciji, odnosno valnoj duljini vala. Ta promjena brzine širenja vala na površini s valnom duljinom naziva se disperzija vala i u uskoj je povezanosti s krutosnim karakteristikama uslojenog medija kroz koji val prolazi.
Suštinu postupka predstavlja generiranje i mjerenje površinskih Rayleighevih valova. Rayleihgyevi valovi generiraju se mehaničkim vertikalnim impulsom na površini terena. Vertikalni senzori, geofoni i akcelerometri, postavljaju se na unaprijed definiranim razmacima i mjere brzinu i akceleraciju prolazećeg vala. Fourierovom analizom primljeni signal se transformira iz vremenske u frekventnu
domenu i na transformiranom signalu provodi se daljnja spektralna analiza. Korištenjem spektralnih funkcija faze cross power spektra i coherence određuju se disperzivne karakteristike ulaznog vala.
Povratnom analizom se, iskorištavajući disperzivne karakteristike Reighlyevog vala, mogu dobiti krutosne karakteristike uslojenog medija.
Kako metoda podrazumijeva generiranje i mjerenje površinskih valova, cijeli postupak se odvija na površini terena te metoda spada u grupu nerazornih seizmičkih mjerenja. Metoda je potencijalno vrlo pouzdana i ne zahtijeva skupa bušenja, a rješava neke od temeljnih problema površinske refrakcije, jer može detektirati mekši sloj koji se nalazi ispod krućeg sloja. Sam postupak je brz, razmjerno jeftin, a razvojem mjerne tehnike se potencijalno može ubrzati do automatizma.

Spectral Analysis of Surface Waves (SASW) is a relatively new non-destructive seismic method developed for the purpose of determining both elastic modules of different materials in the area of rather minor deformation and the change of these modules caused by difference in depth.
The method is based on dispersive properties of Rayleigh waves, taking into account the fact that surface Rayleigh waves with different wavelengths i.e. different frequencies propagate to relatively different depths. Namely, waves with lower frequency, and thus with longer wavelength, spread deeper into the medium than waves with higher frequency, i.e. shorter wavelength. In a layered medium, the wave velocity at the surface depends on its frequency, i.e. its wavelength. This change of wave velocity at the surface caused by the change in wavelength is known as the dispersion of a wave and it is in close connection with the stiffness of a layered medium through which a wave passes.
The essence of the process is the generation and measurement of surface Rayleigh waves. Rayleigh waves are generated by a mechanical vertical impulse on the ground surface. Vertical sensors, geophones and accelerometers, which are placed at predefined distances, measure the velocity and acceleration of the passing wave. Fourier's analysis transforms the received signal from the time domain to the frequency domain, which is followed by further spectral analysis of the transformed signal. The dispersive properties of an input wave are determined by using the spectral functions of coherence and cross power spectrum phase.
Using dispersive properties of Rayleigh wave, the response analysis can show stiffness properties of a layered medium.
Since the method requires the generation and measurement of surface waves, the whole process takes place on the ground surface, thus the method can be considered as non-destructive seismic measurement. This is a potentially highly reliable method, which does not require expensive drilling, and, due to the fact that it can detect a softer layer below a stiffer one, it solves some of the main problems of the surface refraction.The process itself is fast, comparatively inexpensive, and the development of measuring technique can potentially speed it up until it reaches the point of automatism.
